1.Comparison of postural errors of different immobilization methods in treatment with Cyberknife for intracranial tumors

Objective:To compare the immobilization effect of two different immobilization methods in treatment with CyberKnife for intracranial tumors.Methods:A retrospective analysis was conducted on 48 patients with intracranial tumors who received treatment with CyberKnife in the Department of Radiotherapy at Peking Union Medical College Hospital from June 2023 to July 2024.Based on the different immobilization method,patients were divided into two groups:the Double Shell Positioning System(DSPS)group(n=22)and the Qfix group(n=26).The DSPS group was immobilized using an American full-body integrated positioning frame combined with a carbon fiber bracket and DSPS film,while the Qfix group was immobilized using an Encompass board and Qfix film.All patients were treated with 6 dimensions(6D)-skull tracking technique.During treatment,images were acquired as one times per 60 seconds,which were registered with digitally reconstructed radiograph(DRR)to record and compare the overall relative errors.The data of relative displacement error during treatment was calculated as|dx|=d1-d0,and differences between the two groups were compared.The maximum displacement(|d|=dmax-dmin)was also calculated,and difference in maximum displacement between the two kinds of immobilization methods was analyzed.Results:In the comparison of the overall relative errors between the two kinds of immobilization methods,the median values in the errors of superior-inferior(SI),anterior-posterior(AP),Roll,Pitch,and Yaw directions in the Qfix group were respectively 0.174,0.309,0.150,0.147,and 0.477,which were significantly less than those(0.224,0.316,0.175,0.221,and 0.584)of DSPS group,and the differences were statistically significant(Z=-4.358,-1.698,-2.595,-6.833,-5.371,P<0.05).The difference of the relative displacement errors between two kinds of immobilization methods was not significant(P>0.05),while the median values in all directions were not zero.The comparison of the maximum displacement value between the two kinds of immobilization methods indicated the displacement amounts of Qfix group was less than those of DSPS group on SI,left-right(LR),AP,roll,pitch,and yaw directions,and the differences of them were significant(Z=-3.373,-2.525,-2.488,-3.169,-5.130,-5.166,P<0.05).Conclusion:Both immobilization methods can meet the requirements of clinical treatment.The results of comparison indicate the immobilization effect of Qfix group is best,and the combination of Encompass board and Qfix film is recommended as the immobilization method of CyberKnife-based radiotherapy for intracranial tumors.
2.Minimally invasive therapy for new-onset or residual aortic arch pathology after ascending aortic replacement

Objective:To evaluate the outcomes of minimally invasive therapy for aortic arch pathology after ascending aortic replacement.Methods:A retrospective analysis was conducted at the Department of Cardiovascular Surgery, West China Hospital of Sichuan University from 2016 to 2024. After multidisciplinary discussion, these included patients were evaluated to be at high risk for traditional open surgery. Various minimally invasive repair techniques were employed, including Ⅳb hybrid technique, physician-modified endograft and novel unibody endograft. The study outcomes were technical success, in-hospital and follow-up mortality, stroke, endoleak, and the patency of the supra-aortic vessels.Results:A total of 40 patients(32 males and 8 females) with a median age of 60 years old were included in this study. The technique success rate was 100%, with no deaths or strokes reported. The patency of the supra-aortic vessels was 100%. 10 patients underwent Type Ⅳb hybrid surgery without any endoleaks occurring. Among the 22 patients who received physician-modified endograft, endoleaks were observed in 2 cases. One of these type Ⅰc endoleaks persisted and underwent reintervention. One patient underwent femoral artery replacement due to vascular injury. For the 8 patients who received novel unibody endograft, one case required reintervention due to persistent type Ⅰc endoleaks.Conclusion:With the development of different endovascular techniques and novel branched endograft, patients with aortic arch pathology who are at high risk for redo open surgery can achieve favorable outcomes with various minimal invasive techniques. However, long-term and large-sample follow-up studies are needed for further evaluation.
3.Correlation between Gleason grade and free prostate-specific antigen,serum ferritin and uric acid levels in patients with prostate cancer

Objective To discuss the correlation between free prostate specific antigen(fPSA),serum ferritin(SF),blood uric acid(SUA)levels and Gleason grading in patients with prostate cancer(PCa).Methods The clinical data of 61 patients with prostate biopsy treated in 411 Hospital of Shanghai University from January to December of 2023 were retrospectively analyzed.According to the results of puncture,the patients were divided into benign prostatic hyperpla-sia(BPH)group(31 cases)and PCa group(30 cases).The levels of fPSA,SF and SUA in patients and Gleason grade in biopsy cases were analyzed.The correlation between fPSA,SF and SUA levels and Gleason grade was analyzed by the method of Spearman.And the diagnostic efficacy of fPSA,SF and SUA levels on PCa was analyzed by receiver operating characteristic curve(ROC).Results The levels of fPSA,SF and SUA in PCa group were significantly higher than those in BPH group(P<0.05).There were statistically significant differences in levels of fPSA SF and SUA in PCA patients with different Gleason grades(P<0.05).With the specificity reaching 96.08%and sensitivity reaching 94.35%,the ar-ea under the curve(AUC)of the combined fPSA,SF and SUA levels in the diagnosis of PCa was 0.982,which was higher than that of the single fPSA,SF and SUA levels(P<0.05).SF and SUA levels in PCa patients were positively correlated with Gleason grade(P<0.05),while fPSA levels were not correlated with Gleason grade(P>0.05).Conclusion The levels of SF and SUA in PCa patients are positively correlated with Gleason grade,which can be used as an important index to predict Gleason grade in PCa patients.
4.Preliminary analysis of mRNA m7G modifications in human Adenocarcinoma of esophagogastric junction.

OBJECTIVE:
To explore the potential role of mRNA m7G modification in the pathogenesis of human adenocarcinoma of esophagogastric junction (AEG).
METHODS:
Pathological tissue specimens from four AEG patients who underwent surgical treatment at the People's Hospital Affiliated to Jiangsu University between 2018 and 2019 were selected. Tumor tissues and adjacent normal tissues were collected from these patients. RNA was extracted from both tissue types and subjected to m7G methylated RNA immunoprecipitation sequencing (m7G-MeRIP-seq) to analyze the patterns of m7G modification, the characteristics of differential m7G modification sites, the differentially expressed mRNA, and the correlation between m7G modification and mRNA expression levels. Differential m7G-modified genes (MSH6, BRCA1, and SOX9) were further validated using methylated RNA immunoprecipitation quantitative PCR (MeRIP-qPCR), while the expression of METTL1 and WDR4 genes was examined by real-time quantitative PCR (RT-qPCR). This study was approved by the Medical Ethics Committee of the People's Hospital Affiliated to Jiangsu University (Ethics No. 20150083).
RESULTS:
m7G-MeRIP-seq analysis revealed that m7G modifications in both AEG and adjacent normal tissues were predominantly located in the GC-rich region surrounding the internal start codon of mRNA. Differential m7G modification sites between the two groups were closely associated with cancer-related genes. mRNA library analysis showed that differentially expressed mRNA were predominantly upregulated in AEG tissues and downregulated in adjacent normal tissues. Cross-analysis indicated that genes with hypermethylation tended to exhibit upregulated expression, while genes with hypomethylation were typically downregulated in AEG tissues. MeRIP-qPCR validation confirmed that the mRNA expression of MSH6, BRCA1, and SOX9 were significantly upregulated in AEG tissues compared to adjacent normal tissues (AEG vs. normal, P < 0.05). RT-qPCR results demonstrated that the mRNA expression levels of METTL1 and WDR4 were also upregulated in AEG tissues (AEG vs. normal, P < 0.000 5).
CONCLUSION
These findings suggest that mRNA m7G modification plays a significant role in the development of AEG. Furthermore, proteins as METTL1 and WDR4 may facilitate AEG progression by regulating mRNA m7G modification. These results provide valuable insights into the molecular mechanisms underlying AEG and may inform future therapeutic strategies for this malignancy.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.

7.Relationship between postoperative revascularization efficacy and levels of VEGF,IGF-1 and TGF-β1 in joint synovial fluid in children with Perthes Disease

Objective To explore the relationship between levels of vascular endothelial growth factor(VEGF),insulin-like growth factor 1(IGF-1),and transforming growth factor-β1(TGF-β1)in the synovial fluid of children with avascular necrosis of the femoral head(also known as Perthes disease)and the efficacy of postoperative revascularization,aiming to provide a basis for subsequent diagnosis and treatment.Methods A retrospective study was conducted on 262 children with Perthes disease admitted to the Affiliated Hospital of Gansu University of Chinese Medicine from January 2023 to June 2024.Based on postoperative revascularization efficacy,patients were divided into good revascularization group(n=228)and poor revascularization group(n=34).For poor revascularization group,a 1:2 matched case-control design was used to select 68 age-matched children with hip synovitis who underwent hip joint fluid puncture as control group.Additionally,82 children with Perthes disease treated at the hospital from June 2024 to January 2025 were enrolled as a validation cohort for nomogram model verification.The expression levels of VEGF,IGF-1 and TGF-β1 in the synovial fluid of three groups were compared.Confounding biases were controlled through univariate and stratified analyses.Binary logistic regression analysis was used to identify independent factors affecting the revascularization effect.R software was utilized to draw and verify the nomogram model for predicting the postoperative revascularization effect.Results The levels of VEGF,IGF-1 and TGF-β1 in the synovial fluid of children in poor revascularization group were all higher than those in control group and good revascularization group(P<0.05).After three types of reconstructive surgeries,the levels of VEGF,IGF-1 and TGF-β1 in the synovial fluid of children with poor revascularization were all higher than those in children with good revascularization(P<0.05);however,there was no statistically significant difference in the above indicators among different surgical types(P>0.05).Binary logistic regression analysis showed that the levels of VEGF,IGF-1,and TGF-β1 in the synovial fluid were independent risk factors for poor postoperative revascularization in children with Perthes disease.The area under the ROC curve of the nomogram model established accordingly for predicting poor postoperative revascularization in children with Perthes disease was 0.875(95%CI 0.805-0.945),with a sensitivity of 0.874 and a specificity of 0.851.Moreover,the calibration curve and decision curve analysis(DCA)indicated that the model had good clinical applicability.Conclusions The increased levels of VEGF,IGF-1 and TGF-β1 in synovial fluid are associated with poor postoperative revascularization in children with Perthes disease.These three factors are expected to become prognostic indicators for children with Perthes disease.
8.Hypofractionated radiotherapy combined with transcatheter arterial chemoembolization for the treatment of metachronous hepatic oligometastasis from nasopharyngeal carcinoma:analysis of its clinical effect

Objective To explore the effectiveness and safety of hypofractionated radiotherapy(HFRT)combined with transcatheter arterial chemoembolization(TACE)for metachronous hepatic oligometastasis from nasopharyngeal carcinoma(NPC).Methods The clinical data of patients with metachronous hepatic oligometastasis from NPC,who received HFRT combined with TACE treatment at the Sichuan Provincial Cancer Hospital of China from January 2012 to October 2022,were retrospectively analyzed.The 3-year and 5-year overall survival(OS),progression-free survival(PFS),local recurrence-free survival(LRFS),no extrahepatic distant metastasis survival(EMFS),and treatment-related adverse reactions were analyzed.Results A total of 55 patients were enrolled in this study,including 36 males and 19 females,the median age at the time of occurring metachronous hepatic oligometastasis was 44(27-75)years.The 3-year and 5-year OS,PFS,LRFS,EMFS were 67.8%and 40.0%,55.8%and 30.0%,72.7%and 56.5%,63.6%and 56.5%,respectively.The subgroup analysis indicated that the treatment course of TACE ≥3 cycles could significantly improve the PFS of patients with oligometastasis.HFRT combined with TACE treatment was well tolerated by all patients,and the incidence of Grade Ⅲ-Ⅳadverse reactions was quite low.Conclusion For the treatment of metachronous hepatic oligometastasis from NPC,HFRT combined with TACE is clinically effective,besides,the patients can well tolerate the therapeutic scheme.
9.Explorations of the Clinical Pathology and Prognosis of Gastric Cancer Based on Systemic Immune Inflammation Index and Ferritin Expression

Objective To explore the relationship between systemic immune inflammatory index and ferritin(SF)and the clinicopathologic features and prognosis of gastric cancer(GC).Methods A retrospective analysis was conducted on 152 gastric cancer patients who underwent D2 gastrectomy at the Radiation Oncology Department of Qingbaijiang District People's Hospital in Chengdu from January 2017 to December 2020.Neutrophil count was divided by lymphocyte count to calculate Neutrophil to lymphocyte ratio(NLR).The SF level in plasma was evaluated by ELISA kit.According to the results of multivariate analysis,a new prognosis prediction system including NLR-SF score(NSS)was established.Results Multivariate Cox regression analysis showed that NLR(HR=1.053,95%CI=1.014~1.094,P=0.007),tumor grade(HR=1.944,95%CI=1.279~2.955,P=0.002)and SF(HR=1.005,95%CI=1.002~1.008,P=0.002)were independent factors affecting the prognosis of GC patients.ROC analysis showed that when the cutoff value of SF was 215.5,the AUC of predicting the death of GC patients was 0.800(95%CI=0.731~0.869),the sensitivity was 68.5%,and the specificity was 86.7%.When the cutoff value of NLR was 3.36,the AUC of predicting the death of GC patients was 0.869(95%CI=0.810~0.928),the sensitivity was 81.5%,and the specificity was 91.7%.Higher NSS was significantly correlated with the number of lymph node metastasis>3 cases,the number of deaths,the increase of neutrophil,platelet,SF,NLR and PLR levels(P<0.05),and the decrease of lymphocytes(P<0.05).Kaplan-Meier survival curve analysis showed that the higher the NSS,the shorter the survival time(χ2=75.168,P<0.001).ROC analysis showed that the AUC of NSS in predicting the death of GC patients was 0.902(95%CI=0.852~0.952),the sensitivity was 90.2%,and the specificity was 80.0%.Conclusions This study confirms that NLR and SF are independent prognostic factors of GC patients,and the NSS constructed by combining them is of great value in predicting the overall postoperative survival.
10.Porphyromonas gingivalis promotes esophageal squamous cell carcinoma progression and enhances cetuximab resistance via EGFR/GSK3β pathway induced EMT

Objective:
To investigate the regulatory role of Porphyromonas gingivalis(Pg) infection on the EGFR/GSK3β signaling axis, and its impact on epithelial-mesenchymal transition(EMT) and cetuximab(Ctx) resistance in esophageal squamous cell carcinoma(ESCC).
Methods:
Single cell RNA sequencing was employed to perform differential analysis of cellular subpopulations, identifying differentially expressed genes in ESCC tissues infected and non-infected with Pg. IHC was conducted to assess the expression of Pg and epidermal growth factor receptor(EGFR) in ESCC tissues. Western blot, RT-PCR, and IF staining were performed to evaluate EGFR expression in Pg infected ESCC cell lines KYSE70 and TE1. ESCC cells were treated with Pg and EGFR inhibitor Ctx, and divided into four groups: control(NC) group, Pg group, Ctx group, Pg+Ctx group. Cell proliferation, migration and invasion abilities were evaluated using CCK-8, plate cloning, wound healing and Transwell assay. Western blot analysis was performed to detect the expression of EMT and EGFR/GSK3β signaling pathway-associated proteins and their phosphorylation levels. Transforming growth factor-β1(TGF-β1) was used to induce EMT in ESCC cells, promoting a transition from the epithelial phenotype to mesenchymal-like phenotype. The differential effects of Ctx on these two phenotypic states were subsequently compared.
Results:
Epithelial cells were predominantly enriched in Pg-positive tissues, and Pg infection promoted the upregulation of EGFR expression in ESCC cells. Compared to the NC group, Pg treatment significantly enhanced the proliferation, invasion and migration capabili-ties of ESCC cells, and also increased chemoresistance to Ctx and reduced its antitumor efficacy. Pg induced EMT in ESCC cellsviathe EGFR/GSK3β signaling pathway. Notably, Ctx exhibited markedly weaker inhibitory effects on mesenchymal-like cells compared to epithelial ESCC cells.
Conclusion
Pg promotes ESCC cells proliferation, invasion and migration by regulating EMT through the EGFR/GSK3β signaling pathway, and enhances chemoresistance to Ctx.
