通过GEO(GSE276500、GSE294906、GSE218171)及TCGA数据库数据验证放疗关键基因。

Objective To investigate the application value of bedside ultrasound monitoring of quadriceps muscle changes in mechanically ventilated children in the early treatment of ICU-acquired muscle weakness (ICU-AW). Methods Eighty-two pediatric ICU patients with mechanical ventilation for>48 hours were selected. On the day of admission (D0) and on day 7 (D7), bedside ultrasound was performed to measure the thickness and cross-sectional area of the rectus femoris and vastus intermedius muscles. Patients were classified using Medical Research Council scores into control group (n = 63) and muscle weakness group (n = 19). Muscle parameters and atrophy rates were compared between groups. Diagnostic performance was assessed using receiver operating characteristic curves. Results The incidence of ICU-AW was 23.17%. At both D0 and D7, the average thickness of the rectus femoris, the average thickness of the vastus intermedius, the average area of the rectus femoris, and the average area of the vastus intermedius were significantly different between the two groups (P<0.05). The atrophy rates of the thickness of the rectus femoris and the area of the vastus intermedius were lower in the control group than in the muscle weakness group at both D0 and D7 (P<0.05). The area under the receiver operating characteristic curve for quadriceps parameters in diagnosing ICU-AW was 0.871, with a sensitivity of 89.47% and a specificity of 79.37%. Conclusion Bedside ultrasound dynamic monitoring of quadriceps changes enables early identification of ICU-AW and provides a basis for clinical intervention.

Objective To comprehensively investigate the levels of exposure and distribution characteristics of trihalomethanes (THMs) in drinking water in Guangzhou City, and evaluate the health risks of different groups of children, adolescents and adults, and to provide data and evidence for protecting human health and promoting risk control of drinking water. Methods According to the technical requirements of the "Standards for Drinking Water Quality Testing Methods" (GB/T 5750-2023), the concentration of THMs, including trichloromethane (TCM), bromodichloromethane (BDCM), dibromochloromethane (DBCM), and tribromomethane (TBM) in drinking water in Guangzhou City from 2023-2024 were detected. The health risk model recommended by USEPA was used for risk assessment. Results TCM, BDCM and DBCM were detected in the factory water and terminal water, with TCM contributing the most. There was a statistically significant difference (P<0.05) between the wet and dry seasons, and the concentration of TCM in the wet season was higher than that in the dry season. Among the multiple exposure factors, the amount of exposure through drinking water intake was much greater than that through skin absorption. The carcinogenic risk index of THMs for children, adolescents, and adults was 22.0×10-6, 12.2×10-6, and 11.4×10-6, respectively, while the non-carcinogenic risk was less than 1. Conclusion The exposure risks of THMs in children, adolescents, and adults is within an acceptable range, but monitoring needs to be strengthened, with a particular focus on children.

Objective:To investigate the vaccination rate of 23-valent pneumococcal polysaccharide vaccine (PPV23), identify determinants of vaccination willingness, and analyze barriers to vaccination among community-dwelling elderly in Xinzhuang Town of Shanghai Minhang District. Methods:This cross-sectional study was conducted in Xinzhuang Town of Shanghai Minhang District from November to December 2024. An online questionnaire was distributed via WeChat groups of neighborhood committees to community-dwelling elderly (aged≥60 years) using a convenience sampling method. The questionnaire covered basic demographic information, health status, knowledge and awareness about pneumonia and PPV23, attitudes toward PPV23 vaccination, and vaccination behavior. The χ2 test and multivariate logistic regression analysis were employed to identify factors influencing the PPV23 vaccination rate and willingness to vaccination among the elderly. Results:A total of 667 questionnaires were collected and 612 were valid (91.8%). Among 612 responders there were 223 males (36.44%) and 389 females (63.56%), and 304 (49.67%) aged 70-79 years. In all respondents, 185 (30.23%) had previously received PPV23, while 427 (69.77%) had not. Reasons for non-vaccination included unawareness of vaccine necessity (31.85%, 136/427), self-perceived good health (31.15%, 133/427), concerns about adverse reactions (21.08%, 90/427), and lack of physician recommendation (14.52%, 62/427). In all participants, 65.85%(403/612)expressed willingness for vaccination and 34.15%(209/612) did not have willingness. Factors significantly influencing vaccination willingness included negative perception of vaccine efficacy ( OR=2.750, 95% CI:1.077-7.023), concerns about adverse reactions ( OR=2.568, 95% CI:1.258-5.242), low awareness of pneumonia and PPV23 vaccine ( OR=11.608, 95% CI:4.177-32.258), perceived vaccination inconvenience ( OR=0.457, 95% CI:0.248-0.844), and belief in good health negating need ( OR=2.658, 95% CI:1.361-5.190) (all P<0.05). Conclusions:Suboptimal PPV23 vaccination rates and low willingness among community-dwelling elderly in Shanghai Xinzhuang Town are driven by knowledge gaps and perceptual barriers. Targeted interventions should prioritize physician engagement, accessible vaccination services, and public education addressing safety and efficacy concerns.

This consensus was developed by the Orthodontic Society of the Chinese Stomatological Association to provide a systematic, scientific, and practical guideline for informed consent in orthodontic care. Orthodontic treatment is typically lengthy, highly individualized, and involves multiple factors such as growth and development, occlusal function, and facial esthetics. Rapid technological advances and diverse risk profiles make the traditional reliance on orthodontist experience or institutional templates insufficient to ensure patients′ full understanding and autonomous decision-making. To address this, the expert panel conducted extensive reviews of domestic and international guidelines, analyzed representative dispute cases, and performed multicenter patient-clinician surveys. Using a multi-round Delphi method, the group established a standardized informed consent framework covering the initial consultation, treatment, and retention phases. The consensus emphasizes that informed consent is not only a fundamental legal and ethical requirement but also a key step in building trust, improving patient compliance, and enhancing treatment satisfaction. Orthodontists should clearly and comprehensively explain treatment plans, potential risks, uncertainties, and associated costs, while respecting the autonomy of patients or guardians, and maintain continuous communication and dynamic evaluation throughout the treatment process. The release of this consensus provides unified and authoritative guidance for clinical orthodontics, helping to standardize informed consent, enhance its transparency, safeguard patient rights, reduce medical risks, and promote high-quality, sustainable development of orthodontic practice.

Inclusion criteria: Children diagnosed with GDD according to the Diagnostic and Statistical Manual of Mental Disorders-V, ages 0.0 to 4.9 years. Exclusion criteria: children with common hearing impairment and visual impairment. The Chi-square tests were used for statistical analysis. Results:There were more boys than girls with GDD in outpatient clinics (68.2% vs. 31.8%). Among the children, the proportion of developmental delay in 5, 4, 3, and 2 domains was 31.1%, 23.4%, 22.9% and 22.6% respectively. The rate of delay in 2-3 domains was lower in boys (41.9%) than in girls (53.1%). The rate of delay in 4-5 domains was higher in boys (58.1%) than in girls (46.9%) ( χ2=352.11, P<0.001). Overall, outpatient GDD decreased with age. From 1.0-1.9 to 4.0-4.9 years of age, the proportion of children with developmental delay in 5 domains increased with age (18.2%, 36.4%, 43.9%, 52.4%). Among children aged 0.0-0.9 years, the proportion of 2 domains of developmental delay was higher (33.4%).Among children aged 1.0-1.9 years, the proportion of 2-3 domains of developmental delay was higher (30.7%). Among children aged 2.0-, 3.0-, 4.0-4.9 years, the proportion of developmental delay in 5 domains was higher (36.4%, 43.9%, 52.4%). In children with GDD, the fine motor delay occurred most frequently (85.1%), followed by social self-care (83.9%), language (79.0%), adaptation (62.3%), and gross motor (52.8%). The frequency of developmental delays in fine motor, adaptability, language, and social self-care in boys was higher than that in girls ( χ2=161.37, χ2=41.10, χ2=320.90, χ2=238.54, all P<0.001). The age groups with the highest delay incidence of gross motor, fine motor, adaptability, language, and social self-care were: 4.0-4.9 years (70.6%), 3.0-3.9 years (97.4%), 4.0-4.9 years (81.2%), 2.0-2.9 years (90.9%),2.0-2.9 years (95.4%). The proportions of fine motor delay in GDD children aged 0.0-0.9, 3.0-3.9 and 4.0-4.9 years were (74.5%, 97.4%, 96.8%) and the proportions of social self-care delay in GDD children aged 1.0- and 2.0-2.9 years were (92.1%, 95.4%). Peripheral and mild developmental delays were predominant in children with GDD. The proportion of severe language delay (6.4%) was higher than that in other fields. Conclusions:The proportion of GDD children with developmental delay in 4-5 domains was 54.5%. The most frequent domain of delay was fine motor. The frequencies of developmental delays in fine motor skills, adaptability, language, and social self-care in boys were higher than in girls. Most of the developmental delays in GDD children were marginal and mild. The rate of severe developmental delay in language was higher than in other domains.

After Hubert first reported cases of IgA nephropathy (IgAN) associated with inflammatory bowel disease (IBD) in 1984, researches further confirmed the association. Genome-wide association analyses also suggested that they may share a common genetic background. As IgAN is one of the leading causes of kidney failure and chronic kidney disease (CKD), early diagnosis and intervention can significantly improve the prognosis, and strengthening renal function follow-up and IgAN screening in IBD patients helps disease management. This article reviews the interplay between IBD and IgAN to provide reference for clinical diagnosis and treatment.
