1.Meta-analysis and Grade Evidence Evaluation of Qi-reinforcing and Blood-activating/ Stasis-expelling Chinese Patent Medicines in Treatment of Coronary Microvascular Disease

ObjectiveTo systematically evaluate the efficacy and safety of Qi-reinforcing and blood-activating/stasis-expelling Chinese patent medicines in the treatment of coronary microvascular disease (CMD). MethodsPubMed, Cochrane Library, CNKI, Wanfang Data, and VIP were searched for the randomized controlled trials (RCTs) on the treatment of CMD with Chinese patent medicines for reinforcing Qi and activating blood/expelling stasis with the time interval from inception to December 31, 2023. The primary outcome indicators included the index of microcirculatory resistance (IMR), coronary flow reserve (CFR), and corrected TIMI flow frame count (cTFC). The secondary outcome indicators included symptomatic efficacy, left ventricular ejection fraction (LVEF), hypersensitive C-reactive protein (hs-CRP), nitric oxide (NO), and adverse events. Cochrane risk-of-bias assessment tool 2.0 (RoB 2.0) and Stata 17.0 were used for literature quality evaluation and meta-analysis of the included RCTs. The Grading of Recommendations, Assessment, Development and Evaluation (GRADE) was used to evaluate the quality of evidence. ResultsA total of 36 RCTs were included in this study, involving 3 029 patients. Compared with conventional Western medicine alone, the combined use of Chinese patent medicines for reinforcing Qi and activating blood/expelling stasis and Western medicine reduced the IMR [mean difference (MD)=-5.93, 95% confidence interval (95%CI) [-8.73,-3.14], n=382, P<0.01], cTFC (MD=-9.35, 95%CI [-13.94,-4.76], n=618, P<0.01), and hs-CRP [standard mean difference (SMD)=-1.50, 95%CI [-1.90,-1.11], n=1 483, P<0.01], improved the CFR (SMD=1.14, 95%CI [0.08,2.19], n=304, P=0.03), symptomatic efficacy [relative risk (RR)=1.36, 95%CI [1.21,1.53], n=756, P<0.01], LVEF (MD=4.39, 95%CI [2.31,6.47], n=533, P<0.01), and NO (SMD=3.16, 95%CI [2.07,4.25], n=946, P<0.01) of CMD patients. In terms of safety, the combined therapy reduced the occurrence of adverse events in CMD patients (RR=0.49, 95%CI [0.29,0.82], n=591, P=0.01). GRADE showed moderate quality evidence for adverse events, low quality evidence for cTFC, symptomatic efficacy, LVEF, and NO, and very low quality evidence for IMR, CFR, and hs-CRP. ConclusionBased on microcirculatory function indicators, the combined use of Qi-reinforcing and blood-activating/stasis-expelling Chinese patent medicines and Western medicine may further improve the coronary microvascular function in CMD patients with good safety. The above conclusions remain to be verified with high-quality clinical trials.
2.Development of a prediction model based on decision tree for acute kidney injury in critically ill children and its predictive value

Objective:To establish and validate a prediction model based on least absolute shrinkage and selection operator(LASSO)regression and classification and regression tree(CART)algorithm for acute kidney injury(AKI)in PICU.Methods:The prospective derivation cohort consisted of 350 critically ill children admitted to the PICU of Children′s Hospital of Soochow University from September 2020 to January 2021.The external data set consisting of 866 critically ill children admitted to the PICU of Children′s Hospital of Soochow University from February 2021 to February 2022 was employed for the external validation.Clinical data was obtained from the electronic medical record system,including demographic characteristics,laboratory data and the pediatric risk of mortality Ⅲ(PRISM Ⅲ)score.The variables associated with AKI were identified using LASSO regression.Subsequently,a decision tree prediction model was built using the CART algorithm.The predictive value of decision tree prediction model was evaluated using the receiver operating characteristic(ROC)curve,calibration curve,and decision curve analysis.Results:Among the 350 children in the derivation cohort,107(30.6%)developed AKI during the PICU stay;and of 866 children in the external validation cohort,165(19.1%)developed AKI during the PICU stay.The LASSO regression screened 16 candidate variables for further analysis,and the decision tree model ultimately identified 4 variables more closely associated with AKI,including fold change in serum creatinine from baseline,urine volume,PRISM Ⅲ,and C-reactive protein.The decision tree model exhibited high accuracy with AUC of 0.92,0.88,and 0.86 in the training,internal validation,and external validation cohorts,respectively.The model demonstrated good calibration and clinical applicability based on the calibration curve and decision curve analysis.Conclusion:The decision tree model based on the 4 identified clinical indicators,including fold change in serum creatinine from baseline,urine volume,PRISM Ⅲ,and C-reactive protein,is effective for the early prediction of AKI.

[Objective]To summarize the theoretical framework and characteristics of traditional Chinese medicine ethics,reflecting the unique value of traditional Chinese medicine ethics within the existing knowledge system of medical ethics.[Methods]By exploring the emergence and development of traditional Chinese medicine ethics,summarizing the theoretical framework of traditional Chinese medicine ethics and exploring the main characteristics of traditional Chinese medicine ethics from the perspectives of doctor-patient,inter medical,and medical social relationships,the contemporary value and advantages of traditional Chinese medicine ethics are demonstrated.[Results]Traditional Chinese medicine ethics embodies the ethical views of valuing life and respecting death,embracing all professions,and adapting to the times through education.Compared with the ethical thinking of western medicine,it combines the humanistic connotations of traditional Chinese medicine,emphasizing interactive doctor-patient relationships,respecting collaborative doctor-doctor relationships,and emphasizing both morality and law in medical social relationships,to construct a contemporary ethical thinking system of traditional Chinese medicine.[Conclusion]The enrichment of traditional Chinese medicine ethics in contemporary medical ethics should establish a collaborative doctor-patient relationship,a collaborative doctor-doctor relationship and a medical society relationship based on moral guidance.This is of great significance for finding a way out for traditional Chinese medicine ethics,creating a new system of medical ethics with Chinese characteristics and cultivating cultural confidence.

Acromegaly is a rare disease characterized by the overproduction of growth hormone(GH)and insulin-like growth factor-1(IGF-1), which affects multiple organ systems and increases mortality risk.As the population ages, the number of elderly patients with acromegaly is rising annually, leading to more pronounced systemic complications and significantly impacting quality of life(QoL). However, there is limited data in the literature regarding the assessment of acromegaly complications in the elderly.Therefore, to enhance our understanding of acromegaly in this demographic, the present review focuses on the epidemiology, diagnosis, complication assessment, and therapeutic prognosis of acromegaly in elderly patients.

7.The role of biomarkers in the diagnosis and prediction of disease progression of IgA nephropathy

IgA nephropathy(IgAN)is one of the most prevalent forms of primary glomerulonephritis globally and a leading cause of end-stage renal disease(ESRD).The exact pathogenesis and progression mechanisms of IgAN remain unclear.Its clinical manifestations are diverse,with varying degrees of kidney involvement reflected in both clinical presentations and pathological changes.Consequently,responses to treatment and prognoses differ significantly among patients.Currently,renal needle biopsy serves as the gold standard for diagnosing IgAN;how-ever,this invasive procedure is often not well-accepted by patients,limiting its widespread clinical application and complicating disease diagnosis.Therefore,non-invasive biomarkers are crucial for assisting in the diagnosis of IgAN and evaluating the risk of disease progression.Below,we will focus on various serum and urinary biomarkers associated with IgAN at both protein and nucleic acid levels.
8.A journey map study of the health management needs of caregivers with adolescent idiopathic scoliosis

Objective:To identify the health management needs of caregivers' for adolescent patients with scoliosis based on the journey map, and provide a reference for improving the medical experience and optimizing the whole-journey care level.Methods:On the basis of literature review, combined with semi-structured interview method and group discussion method, we conducted semi-structured interview with the main caregivers of 13 adolescent scoliosis patients in Union Hospital, Tongji Medical College, Huazhong University of Science and Technology in June to October 2024, analyzed the data and drew a journey map.Results:A total of 13 respondents who met the inclusion and exclusion criteria were recruited, including 5 men and 8 women, with an age range of 32 to 45 years. Caregivers' health management needs were summarized from three dimensions: tasks, emotions, and pain points, following a timeline.Through the visualization of the journey map, the caregiver's cognitive gaps during the covert phase, information anxiety and decision-making dilemmas, need for early diagnosis and treatment, and challenges in symptom management ect. in the entire journey of health management were presented, and fifteen themes were summarized.Conclusions:The needs of caregivers for the journey of health management of postoperative adolescent idiopathic scoliosis patients are diversified, and should be demand-oriented to provide precise care support, improve medical experience, and provide ideas and methods for research on postoperative patient health management service optimization.
9.Application of a novel drainage catheter in treatment of acute high-tension breast milk cyst during lactation

Objective To develop a novel drainage catheter based on the anatomical structure of the mammary duct and establish a non-invasive treatment protocol for patients with acute high-tension breast milk cyst during lactation.Methods Ten primiparous patients with acute high-tension breast milk cyst during lactation,admitted to the postpartum clinics of three hospitals,were enrolled as study subjects.A novel drainage catheter was prepared using an epidural anesthesia catheter,and intubation was performed to unblock the obstructed mammary ducts.For cases with intubation failure,puncture as-piration was employed.Local cold compresses were applied to the breasts after treatment.Results Out of 10 patients,6 were successfully unblocked(successful group),and 4 failed(failure group).The duration of illness in the successful group was(6.00±0.82)days,which was shorter than that in the failure group[(9.50±1.80)days](P<0.05).The insertion depth of the catheter in the successful group was(6.42±0.67)cm,which was greater than that in the failure group[(4.00±0.71)cm](P<0.05).There was no statistically significant difference in cyst diameter between the two groups(P>0.05).For patients in whom unblocking failed,puncture aspiration was performed,and 3 of them experienced recurrence after the initial puncture.Ultrasound-guided re-puncture aspiration and weaning were performed.One patient with evident inflammatory symptoms was administered systemic antibiotic therapy.After treatment,7 patients resumed lactation,and follow-up results indicated that all 10 patients were cured.Conclusion Acute high-tension breast milk cyst during lactation is rela-tively rare in clinical practice.The novel drainage catheter can achieve non-invasive unblocking of distally obstructed mammary ducts in the lactiferous sinus.The comprehensive treatment protocol can improve the overall cure rate and result in a high rate of resumed lactation post-treatment.
