1.Mechanism of action of remifentanil in alleviating lung ischemia-reperfusion injury in rats by modulating HIF-1α/NLRP3 pathway to inhibit cell pyroptosis

ObjectiveTo investigate the mechanism of action of remifentanil (RMZL) in alleviating lung ischemia-reperfusion injury (LIRI) in rats by inhibiting pyroptosis through modulating hypoxia inducible factor-1α (HIF-1α)/NOD-like receptor thermal protein domain associated protein 3 (NLRP3) pathway. MethodsRats were stochastically assigned into Control group, LIRI group, RMZL low-dose group, RMZL medium-dose group, RMZL high-dose group, and RMZL high-dose+HIF-1α activator dimethyloxallyl glycine (DMOG) group, with 18 rats in each group. Rats in Control group only had their left pulmonary hilum free and did not undergo ischemia-reperfusion treatment. Except for the Control group, LIRI models were constructed in all other groups. Rats in LIRI group were intraperitoneally injected with an equal amount of physiological saline 15 minutes before constructing LIRI model; rats in Control group were intraperitoneally injected with an equal amount of physiological saline 15 minutes before freeing left pulmonary hilum; rats in other groups were intraperitoneally injected with corresponding dose of drug 15 minutes before constructing LIRI model. The wet/dry weight ratio of lungs was calculated. HE staining was used to study lung tissue pathology. Immunofluorescence staining was used to detect the relative fluorescence intensity of gasdermin D (GSDMD) and NLRP3 double positive cells in lung tissue. ELISA was used to detect interleukin-1β and IL-18 in lung tissue. Western blot was used to detect HIF-1α, NLRP3, cysteine-aspartic protease-1 (Cleaved caspase-1), and gasdermin D-N (GSDMD-N) proteins in lung tissue. ResultsCompared to the Control group, the LIRI group showed disordered alveolar structure, thickened alveolar septa, and abundant inflammatory cell infiltration in rats. The lung wet/dry weight ratio, relative fluorescence intensity of GSDMD and NLRP3 double positive cells in lung tissue, IL-1β, IL-18 levels, and HIF-1α, NLRP3, Cleaved caspase-1, and GSDMD-N proteins increased (P0.05). For the LIRI group, rats in the RMZL low, medium, and high-dose groups displayed attenuated alveolar septal thickening and reduced inflammatory cell infiltration. The lung wet/dry weight ratio, relative fluorescence intensity of GSDMD and NLRP3 double positive cells in lung tissue, IL-1β, IL-18 levels, and HIF-1α, NLRP3, Cleaved caspase-1, and GSDMD-N proteins declined, and the RMZL high-dose group showed the most prominent trend (P0.05). Compared with the RMZL high-dose group, rats in the RMZL high-dose+DMOG group exhibited thickened alveolar septa and more inflammatory cell infiltration, along with increased lung wet/dry weight ratio, relative fluorescence intensity of GSDMD and NLRP3 double positive cells in lung tissue, levels of IL-1β and IL-18, and protein expression of HIF-1α, NLRP3, Cleaved caspase-1, and GSDMD-N (P0.05). ConclusionRMZL may inhibit pyroptosis in LIRI rats by suppressing HIF-1α/NLRP3 pathway.
2.Comorbidity of myopia, and overweight and obesity among primary and middle school students in Yanqing District

Objective:
To investigate the prevalence of comorbidity of myopia, and overweight and obesity among primary and middle school students in Yanqing District, Beijing Municipality, so as to provide the evidence for \prevention and control of comorbidity.
Methods:
In September from 2020 to 2024, a stratified cluster sampling method was adopted annually to select primary, junior high school, senior high school, and vocational high school students in Yanqing District as survey subjects. Myopia was screened based on Screening Protocol for Myopia in Children and Adolescents. Height an weight were measured, and overweight and obesity were determined using the Screening for overweight and Obesity in School-Age Children and Adolescents.
Results:
A total of 9 883 individuals were surveyed, including 5 219 primary school students (52.81%), 2 486 junior high school students (25.15%), and 2 178 senior high school / vocational high school students (22.04%). There were 5 154 boys (52.15%) and 4 729 girls (47.85%). From 2020 to 2024, the numbers of primary and middle school students in Yanqing District with myopia, overweight, and obesity were 4 426, 1 897, and 3 164, respectively, with detection rates of 44.78%, 19.19%, and 32.01%. The detection rates of myopia, overweight, and obesity showed an increasing trend with the elevation of school stage (all P<0.05). The detection rate of myopia was higher in girls than in boys, while the detection rate of obesity was lower in girls than in boys (all P<0.05). There were 2 588 individuals with both myopia and overweight/obesity, representing a detection rate of 26.19%, which showed an increasing trend (P<0.05), rising from 25.04% in 2020 to 34.41% in 2024. The detection rates of comorbidity of myopia, and overweight and obesity among primary school, junior high school, and senior high school / vocational high school students were 13.22%, 36.93%, and 45.00%, respectively, showing an increasing trend with the elevation of school stage (P<0.05). The difference between genders was not statistically significant (P>0.05).
Conclusions
The detection rates of comorbidity of myopia, and overweight and obesity among primary and middle school students in Yanqing District is relatively high and shows an upward trend, with students in higher school stage being the key target group for prevention and control. It is recommended to strengthen health education on the co-prevention of multiple health issues and implement differentiated interventions based on school stages.
3.Characteristics of recent infection among newly confirmed HIV-1 cases in some areas of Hubei during 2017-2022

Objective To analyze the newly reported HIV-1 infection in several prefectures of Hubei Province,and analyze its influencing factors. Methods The limiting antigen avidity enzyme immunoassay(LAg-avidity EIA,LAg) was conducted on HIV-1 positive samples confirmed by Western blot of Hubei in 2017-2022. The demographic characteristics of the newly infected samples were analyzed by χ2 test.Logistic regression model was used to analyze influencing factors of new infection rate and predict the factors associated with the HIV-1 recent infection. Results There were 403 new cases of HIV-1 from 2017 to 2022 in several prefectures of Hubei Province, of which 77 were newly infected sorted by LAg,with a new infection rate of 19.11%. The newly confirmed HIV-1 persons of whom aged ≤24 years (40.00% new infection ratio), unmarried (29.41%), college or above (31.37%), and from Voluntary counseling and testing testing(VCT) clinics (40.00%) had a higher proportion of new infections, and the difference was statistically significant. Multivariate Logistic regression analysis showed that age ≤24 years old (aOR=4.346,95%CI: 1.342-14.075) and screening from the VCT clinic (aOR=6.761,95%CI: 1.460-31.319) were more likely to be newly infected. Conclusion The proportion of new HIV infection in several prefectures of Hubei province is relatively low in recent years.Further effective publicity and intervention measures for young students and the construction of VCT clinic should be continuously promoted to achieve early diagnosis and treatment.
4.Expert consensus on the whole-process management of neovascular glaucoma associated with diabetic retinopathy

Diabetic retinopathy(DR)-related neovascular glaucoma(NVG)is a severe ocular complication of diabe-tes,characterized by a high blindness rate and complex diagnosis and treatment.Currently,there is a lack of systematic,whole-process management guidelines for DR-NVG in China.To standardize the clinical diagnosis,treatment,and health management of DR-NVG,domestic retinal disease and glaucoma experts have developed the first expert consensus on the whole-process management of neovascular glaucoma associated with diabetic retinopathy,by integrating the latest evi-dence-based medical evidence and clinical practice experience from both domestic and intemational sources.Centering on the disease characteristics of DR-NVG,this consensus proposes scientific and standardized whole-process management strategies,encompassing early screening and risk assessment,precise diagnosis and staging,multi-mode combination ther-apy,perioperative management,and long-term follow-up.It mainly covers the screening and diagnosis of high-risk popula-tions,the timing of anti-VEGF therapy and pan-retinal photocoagulation application,and indications for surgical interven-tion.The aim is to provide standardized guidance for ophthalmologists and public health workers,improve the prevention and treatment level of DR-NVG,and reduce the incidence of irreversible visual impairment.
5.Construction of a mouse model for alveolar type Ⅱ epithelial cell-specific knockout of SENP1 gene based on the Cre-loxP recombinase system

BACKGROUND:Previously,a SENP1 gene-silenced human alveolar epithelial cell line was successfully constructed in vitro,and the role of SENP1 in hyperoxic lung injury was investigated at the cellular level.OBJECTIVE:To construct a mouse model of alveolar type II epithelial cell-specific knockout of SENP1 gene based on the Cre-loxP recombinase system.METHODS:SENP1flox/-mice were self-crossed to obtain SENP1flox/flox and SENP1flox/-mice;Sftpc-Cre+/+mice were crossed with wild-type mice to obtain more Sftpc-Cre+/-mice.Sftpc-Cre+/+or offspring Sftpc-Cre+/-mice were crossed with SENP1flox/-or offspring SENP1flox/flox mice to obtain SENP1flox/-Sftpc-Cre+/-double heterozygous mice.SENP1flox/-Sftpc-Cre+/-mice were then crossed with SENP1flox/flox mice to obtain SENP1flox/floxSftpc-Cre+/-mice.The genomic DNA was extracted by tail clipping and amplified by PCR.The amplified product was subjected to agarose gel electrophoresis to determine the mouse genotypes.Lung tissues of SENP1flox/flox and SENP1flox/floxSftpc-Cre+/-mice were subjected to immunofluorescence double-labelling and western blot assay to verify the knockdown effect of SENP1 gene.Heart,liver,lung and kidney tissues of SENP1flox/flox and SENP1flox/floxSftpc-Cre+/-mice were stained with hematoxylin-eosin to observe the histomorphology of each organ in the two groups of mice.RESULTS AND CONCLUSION:SENP1flox/floxSftpc-Cre+/-mice were correctly screened by agarose gel electrophoresis.Immunofluorescence double-labeling experiments showed that the mean fluorescence intensity of SENP1 was reduced in lung tissues of SENP1flox/floxSftpc-Cre+/-mice compared with that of SENP1flox/flox mice(P<0.01)and no significant co-localization of SENP1 and Sftpc was observed(P<0.01).Western blot results showed that SENP1 protein expression was reduced in lung tissues of SENP1flox/floxSftpc-Cre+/-mice compared with SENP1flox/flox mice(P<0.001).Hematoxylin-eosin staining showed no significant alterations in the histomorphology of heart,liver,lung and kidney tissues in SENP1flox/flox and SENP1flox/floxSftpc-Cre+/-mice.This study successfully constructed alveolar type II epithelial cell-specific knockout SENP1 gene mice using the Cre-loxP recombinase system,which provides a good tool for the subsequent study of the role of SENP1 gene in lung diseases such as bronchopulmonary dysplasia and idiopathic pulmonary fibrosis,in which alveolar type II epithelial cells are the main damage cells.
6.Comparison of osteogenic potential in STRO-1 positive and negative bone marrow mesenchymal stem cells

BACKGROUND:With aging,the regenerative capacity and differentiation function of bone marrow mesenchymal stem cells progressively decline,reducing bone tissue repair efficacy.Thus,identifying bone marrow mesenchymal stem cell subpopulations with enhanced osteogenic potential is of significant importance for advancing bone tissue engineering.OBJECTIVE:To evaluate the osteogenic differentiation potential differences between STRO-1 positive and negative bone marrow mesenchymal stem cells under osteogenic induction conditions.METHODS:SD rat bone marrow mesenchymal stem cells were isolated and cultured.The expression of CD29,CD45,CD90,and STRO-1 was identified via flow cytometry and immunofluorescence.Immunomagnetic cell sorting was used to separate STRO-1 positive and negative bone marrow mesenchymal stem cells.The cells of two groups were subjected to osteogenic induction for 7 and 14 days.qRT-PCR and western blotting were performed to analyze differences in osteogenesis-related gene expression(Collagen I,Runt-related transcription factor 2,osteoprotegerin,and osteocalcin)and protein levels.Alizarin red staining and alkaline phosphatase staining were used to observe calcium nodule formation.RESULTS AND CONCLUSION:Flow cytometry showed high expression levels of CD29 and CD90 and low expression of CD45,with a positive STRO-1 expression rate of 12.8%.Immunofluorescence results were consistent with those of flow cytometry.After magnetic cell sorting,STRO-1 positive cells demonstrated a higher colony formation rate than STRO-1 negative cells.On day 14,STRO-1 positive cells showed significantly higher osteogenic differentiation potential than on day 7,with significantly elevated osteogenesis-related marker levels compared to STRO-1 negative cells(P<0.01).The findings indicate that STRO-1 positive bone marrow mesenchymal stem cells exhibit significant advantages in osteogenic potential,providing a theoretical basis for their selection as ideal seed cells in bone tissue engineering.In future applications,they may represent a promising therapeutic approach for bone defect repair.

8.Study on toxicity and mechanism of Epimedium sagittatum(Sieb.et Zucc.)Maxim based on"serum toxicology"

Aim To evaluate the toxicity and mecha-nism of Epimedium sagittatum(Sieb.et Zucc.)Maxim aqueous extract(ESMAE)on HepaRG cells based on serum toxicology.Methods MTT assay was used to detect the activity of HepaRG cells after treatment with the serum containing ESMAE from SD rats.Western blot was used to detect the effects of the serum contai-ning drug on the expression of endoplasmic reticulum stress-related proteins(PERK,eIF-2α,ATF-4,GRP78,CHOP)and pyroptosis related proteins(NL-RP1,caspase-1,cleaved caspase-1,GSDMD,GSDMD-N).MTT assay was used to detect the activity of Hep-aRG cells after treatment with the liver homogenate containing ESMAE from SD rats.Results Twenty percent serum containing drug significantly decreased the viability of HepaRG cells,with the cells exhibiting swelling,rupture,and vesicle-like pyroptosis.The ex-pression levels of endoplasmic reticulum stress-related proteins PERK,eIF-2α,ATF-4,GRP78,and CHOP significantly increased.The expression levels of pro-teins involved in the NLRP1-mediated classical pyrop-tosis pathway were significantly increased.Finally the liver homogenate containing drug decreased the cell ac-tivity,and cells exhibited swelling,rupture,and vesicle-like pyroptosis.Conclusions After administration of ESMAE,the serum containing drug and the liver ho-mogenate containing drug of rats show toxicity to Hep-aRG cells,and can induce endoplasmic reticulum stress and activate the NLRP1/caspase-1/GSDMD pyroptosis pathway in HepaRG cells.
9.Efficacy of endoscopic surgery of intracranial hematoma through frontal parallel fiber bundle approach in the treatment of hypertensive cerebral hemorrhage

Objective To investigate the application effect of endoscopic surgery of intracranial hematoma through frontal parallel fiber bundle approach in hypertensive cerebral hemorrhage(HCH).Methods 100 patients with HCH from September 2020 to September 2024 were selected and divided into the study group(transfrontal parallel fiber bundle approach neuroendoscopic-assisted intracranial hematoma evacuation,48 cases)and control group(traditional craniotomy hematoma evacuation,52 cases)according to different surgical methods.The surgical-related indicators,inflammatory indicators,neurological function[national institute of health stroke scale(NIHSS)score],modified barthel index(MBI),postoperative complications and fiber bundle protection of the two groups of patients were compared.Results Compared with the control group,the study group had a shorter operation time and less intraoperative blood loss,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the hematoma clearance rate and residual hematoma volume between the two groups(P>0.05).At 14 and 28 d after the operation,interleukin-6(IL-6)and C-reactive protein(CRP)in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Compared with the control group,the NIHSS score of the study group was lower and the MBI was higher 3 months after the operation,and the differences were statistically significant(P<0.05).There was no statistically significant difference in postoperative complications and total complications rate between the two groups of patients(P>0.05).The postoperative fiber bundle protection rate in the study group was 75.00%,significantly higher than 17.31%in the control group,and the difference was statistically significant(P<0.05).Conclusion The efficacy of transfrontal parallel fiber bundle approach neuroendoscopic-assisted intracranial hematoma evacuation in the treatment of HCH is definite.It can reduce intraoperative bleeding,inhibit postoperative inflammation,protect the brain fiber bundle of patients,improve neurological function,and enhance the ability of daily living.It is worthy of clinical promotion and application.
