1.Durable complete response after discontinuation of atezolizumab-bevacizumab therapy in patients with hepatocellular carcinoma with portal vein tumor thrombosis: the first report

Hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) is associated with a dismal prognosis. Atezolizumab plus bevacizumab (atezo-bev) is the recommended palliative treatment, and approximately 10% of the patients may experience a complete response (CR), according to the mRECIST criteria. The treatment duration is until disease progression or unacceptable side effects occur. Long-term continuation can cause potential toxicities and a substantial financial burden, making early treatment discontinuation a viable option. This report describes durable CR after discontinuing atezo-bev treatment in three patients with HCC and PVTT.
2.Impact of an intraoral X-ray cone with positioning laser lights on reducing radiographic errors with the bisecting angle technique: A technical report

Purpose:
The bisecting angle technique (BAT) encounters difficulties in positioning the X-ray cone and aligningthe central beam with the tooth. To address this, a rectangular laser featuring a pointed light was integrated into theintraoral X-ray cone. This study evaluated its effectiveness in improving radiograph quality and minimizing errors.
Materials and Methods:
Fifty fifth-year Bachelor of Dental Surgery students were divided into 3 groups. Group1 (n = 16) used the paralleling technique, group 2 (n = 17) employed the conventional BAT, and group 3 (n = 17) utilized the laser-assisted BAT on mannequins. Two independent oral radiologists assessed the quality of the radiographs, categorizing the images as either diagnostically acceptable or not acceptable. Inter-group comparisons of quality and error rates were conducted using the chi-square test (significance level: P<0.05).
Results:
The paralleling technique group produced 77.5% diagnostically acceptable radiographs and 22.5% that were not diagnostically acceptable. These percentages were 65.3% and 34.7%, respectively, in the conventional BAT group and 75.3% and 24.7%, respectively, in the laser-assisted BAT group, showing results similar to the paralleling technique group. The quality of radiographs differed significantly among the groups (P<0.05). The percentage of error-free radiographs was 38.1% in the paralleling technique group, 20.6% in the conventional BAT group, and 40.0% in the laser-assisted BAT group, with these differences being statistically significant (P<0.05).
Conclusion
The device produced higher acceptability and fewer radiographic errors than the conventional BATtechnique, suggesting accurate adjustment of the X-ray cone and central beam to the desired teeth.

7.Kajian Awal Penyediaan dan Penilaian Krim Kosmeseutikal Antioksidan Berasaskan Minyak Pati Trigonella foenum-graecum

Dunia kosmetik sekarang berubah fokus kepada produk penjagaan kulit yang menggunakan ekstrak semulajadi
seperti biji halba. Tujuan kajian ini adalah untuk merumuskan krim kulit kosmeseutikal bersifat antioksidan
dengan menggunakan minyak pati biji halba (Trigonella foenum-graecum) dan seterusnya menilai pelbagai
ciri fisiokimianya termasuk ujian pemusnahan radikal bebas, penilaian organoleptik, pencirian fizikal, ujian
kestabilan dan ujian biokeselamatan. Kepekatan minyak pati 300μL/mL menunjukkan aktiviti antioksidan yang
paling tinggi secara signifikan pada 9.58% dan formulasi krim kulit dirumuskan menggunakan emulsi minyak
dalam air dengan peratusan minyak pati yang berbeza (3%, 5% dan 10%). Ujian pemusnahan radikal juga
dijalankan ke atas formulasi krim kepekatan minyak pati 3% dan ianya menunjukkan aktiviti antioksidan tertinggi
yang signifikan (p<0.05) pada 87% serta mempamerkan sifat organoleptik yang memuaskan dengan pH 4.55.
Ujian kestabilan semua formulasi telah dijalankan selama 90 hari dan hasilnya menunjukkan bahawa formulasi
yang disimpan pada suhu bilik, mempamerkan nilai pH dan kebolehsebaran yang lebih baik berbanding formulasi
yang disimpan pada 4°C. Formulasi krim juga diuji dengan ujian saringan biokeselamatan in vitro hemolisis yang
menunjukkan ketiadaan perengsa dan bahan toksik dalam krim yang dirumuskan. Ujian sensitiviti kulit semua
formulasi krim terhadap sukarelawan melaporkan tiada reaksi kulit. Hasil kajian menunjukkan minyak pati biji
halba mempunyai sifat antioksidan yang tinggi dan boleh diformulasikan dalam bentuk krim kulit kosmeseutikal
yang selamat. Walau bagaimanapun, ujian tambahan kosmeseutikal dan dermatologi diperlukan untuk penilaian
lanjut manfaat krim yang dirumuskan.

Excessive use of antibiotics is a significant public health concern with several severe implications for the development of Antibiotic Resistance and its impact on Gut Microbiota. C-reactive Protein (CRP) is non-specific, acute-phase reactant whose levels increase in response to infection or inflammation due bacterial and viral etiologies. Many studies have explored the role of CRP as a point-of-care tool to assist in decision-making and improve the efficiency of antibiotic prescribing practices among healthcare professionals in primary care settings. The main benefit of POC CRP testing is its ease of use, providing results within 2 to 3 minutes from a simple finger prick, which is optimal for an outpatient clinic. While several studies demonstrate reductions in antibiotic use with POC CRP testing, there are many controversies around those need to be resolved, including the cutoff values of CRP for antibiotic prescription, whether CRP can differentiate bacterial and viral etiologies, and whether a single CRP level can reflect the disease state. Future research needs to focus on developing uniform guidelines for interpreting CRP levels and evaluating whether serial CRP measurements, physician training, especially in primary care and rural settings, to reduce overprescribing. Ultimately, while POC CRP testing can support antibiotic stewardship, its utility must be carefully balanced with sound clinical judgment.
9.Role of D-dimer in Prognostication of Head Injury Patients: A Systematic Literature Review

Studies have suggested that patients with severe head injury, in addition to Glasgow Coma Scale (GCS), reveal high D-dimer levels which are independently associated with increased mortality. In the present review, we analyzed the literature where studies have reported the role of D-dimers as a potential biomarker in stratification and early identification of patients with TBI who are at risk of clinical deterioration, where early intervention can improve overall outcomes. After scrutinizing 246 articles, which were narrowed down to 38 (16 prospective, 9 retrospective, 2 RCTs, 2 case-control, 1 cross-sectional study). All these studies included 7,589 patients, in whom D-dimer was measured at different timings and in different ways. Most studies demonstrated significant changes that could be utilized for prognostication. However, we also observed instances where no significant changes were found. Unfortunately, direct comparisons were hindered by variations in the methods used to measure D-dimer across different outcomes. Limitations included the lack of a substantial number of RCTs, heterogeneity of available data, and the difficulty in summarization of specific cutoff points for D-dimer. Future studies, especially RCTs, should measure D-dimer levels at different times from injury for an accurate assessment of its correlation with outcomes.
