Rare Occurrence of Paratesticular Rhabdomyosarcoma in Adulthood: A Case Report
Paratesticular rhabdomyosarcoma is an extremely rare malignant tumour that primarily affects pediatric patients but is uncommon in adults. Prompt diagnosis and treatment are essential to prevent metastasis and improve prognosis. A 28-year-old male was presented with a painless right scrotal swelling. Physical examination revealed a palpable mass in the right scrotal region, raising suspicion of a malignant tumour. Ultrasonography showed a hypoechoic mass originating from the right epididymis. The patient underwent a high inguinal orchidectomy, and histopathological examination confirmed the diagnosis of paratesticular rhabdomyosarcoma. Staging computed tomography revealed no evidence of metastasis. The patient received chemotherapy based on risk stratification. Paratesticular rhabdomyosarcoma carries a poor prognosis in advanced stages, highlighting the importance of early diagnosis. Surgery remains the mainstay of treatment and the role of adjuvant therapy in adults is not well established. Understanding the clinical manifestations, diagnosis and treatment of this rare malignancy can aid in improved management and outcomes
2.A clinical audit of interventional pain procedures performed as part of the newly initiated pain service in a local neurosurgical centre

Interventional Pain Procedures (IPPs) is a relatively new
treatment modality for chronic pain in Malaysia. The
Interventional Pain Service (IPS) newly set up in our
institution is led by a pain neurosurgeon and provides a
whole package of multimodal pain management including
different range of IPPs. This clinical audit is to examine the
quality of IPPs performed within the IPS in our institution
since its initiation. A total of 87 IPPs were performed on 56
chronic pain patients over 3-year duration. As high as 81.8%
of the procedures were effective and 81.5% of patients were
satisfied. Only one minor transient complication occurred
after an intradiscal procedure but none resulted in death or
permanent disability. Thus, safe and effective IPPs can be
provided as part of IPS in a local neurosurgical pain centre
to bring more comprehensive and less fragmented care for
chronic pain patients.
3.Reconstruction Options Following Wide Excision of Basal Cell Carcinoma of the Face

Basal cell carcinoma is the commonest skin malignancy diagnosed in Malaysia. Surgery is the recommended
treatment of choice with the lowest failure rate provided a clear margin is obtained. However, the defect may be too
large to be repaired primarily. Formal reconstruction using grafts and flaps should be done to achieve the optimal
aim of maintaining the integrity, function and cosmetic patient outcome. Three reconstructive methods are described
in this series to restore the facial defect following the wide local excision. The procedures described were peri-alar
crescentic advancement flap, nasolabial rotational flap and full thickness skin graft using supraclavicular skin. This
series highlights the usage of the procedures based on solid foundation and principles, without compromising the
desired outcomes for the patient.
