Objective:To explore the impact of sleep duration, physical exercise, and their interactions on the risk of dyslipidemia in older adults aged ≥80 (the oldest old) in China. Methods:The study subjects were the oldest old from four rounds of Healthy Aging and Biomarkers Cohort Study (2008-2009, 2011-2012, 2014 and 2017-2018). The information about their demographic characteristics, lifestyles, physical examination results and others were collected, and fasting venous blood samples were collected from them for blood lipid testing. Competing risk model was used to analyze the causal associations of sleep duration and physical exercise with the risk for dyslipidemia. Restricted cubic spline (RCS) function was used to explore the dose-response relationship between sleep duration and the risk for dyslipidemia. Additive and multiplicative interaction model were used to explore the interaction of sleep duration and physical exercise on the risk for dyslipidemia. Results:The average age of 1 809 subjects was (93.1±7.7) years, 65.1% of them were women. The average sleep duration of the subjects was (8.0±2.5) hours/day, 28.1% of them had sleep duration for less than 7 hours/day, and 27.2% had sleep for duration more than 9 hours/day at baseline survey. During the 9-year cumulative follow-up of 6 150.6 person years (follow-up of average 3.4 years for one person), there were 304 new cases of dyslipidemia, with an incidence density of 4 942.6/100 000 person years. The results of competitive risk model analysis showed that compared with those who slept for 7-9 hours/day, the risk for dyslipidemia in oldest old with sleep duration >9 hours/day increased by 22% ( HR=1.22, 95% CI: 1.07-1.39). Compared with the oldest old having no physical exercise, the risk for dyslipidemia in the oldest old having physical exercise decreased by 33% ( HR=0.67, 95% CI: 0.57-0.78). The RCS function showed a linear positive dose-response relationship between sleep duration and the risk for hyperlipidemia. The interaction analysis showed that physical exercise and sleep duration had an antagonistic effect on the risk for hyperlipidemia. Conclusion:Physical exercise could reduce the adverse effects of prolonged sleep on blood lipids in the oldest old.

Objective:To explore the mechanism of herb-partitioned moxibustion in relieving rat intestinal inflammation by focusing on the neutrophil extracellular traps(NETs)in Crohn disease(CD)development. Methods:Rats were randomly divided into a normal group,a model group,a herb-partitioned moxibustion group, and a mesalazine group. The CD rat model was prepared with 2,4,6-trinitrobenzene sulfonic acid except for rats in the normal group.Rats in the normal group and model group did not receive any treatment but had the same fixation as the other groups.Rats in the herb-partitioned moxibustion group received herb-partitioned moxibustion at Qihai(CV6)and bilateral Tianshu(ST25).Rats in the mesalazine group received intragastric administration of mesalazine enteric-coated tablets. The general situation of rats in each group was recorded, and the histopathological changes in the colon were observed and scored by hematoxylin-eosin staining. The serum concentrations of NETs DNA(NETs-DNA),neutrophil elastase(NE)-DNA,and myeloperoxidase(MPO)-DNA were detected by ABC enzyme-linked immunosorbent assay, and the citrullinated histone 3(citH3),MPO,and NE protein and mRNA expression levels in rat colon tissue were observed by immunofluorescence and real-time quantitative polymerase chain reaction. Results:Compared with the normal group, the mucosal ulcer reached the muscularis, the epithelium was incomplete, the goblet cells decreased obviously with significant inflammatory cell infiltration in the colon;the colonic mucosa damage index(CMDI)score increased significantly(P<0.01);the serum NETs-DNA,NE-DNA,and MPO-DNA concentrations increased(P<0.05);the NE,citH3,and MPO protein and mRNA expression in the colonic tissue increased significantly in the model group(P<0.01 or P<0.05).Compared with the model group, the mucosal epithelium in the herb-partitioned moxibustion group and the mesalazine group was repaired and the goblet cells increased with a few infiltrating inflammatory cells in the colon;the CMDI score decreased(P<0.01);the serum NETs-DNA,NE-DNA,and MPO-DNA concentrations decreased(P<0.05);the NE,citH3,and MPO protein and mRNA expression in the colonic tissue was down-regulated(P<0.01 or P<0.05). Conclusion:Herb-partitioned moxibustion reduced the serum NETs complex and inhibited the protein and mRNA expression of NETs complex in the colon tissue, which may be one mechanism of herb-partitioned moxibustion in relieving colon mucosal inflammation in CD.

Asymptomatic spleen-stomach diseases refer to diseases without related symptoms and signs of abdo-minal pain, bloating, diarrhea an others in patients, but showing lesions or pathological changes discovered by modern medical techniques such as endoscopy, CT, MRI. The four examination techniques of traditional Chinese medicine (TCM) are based on symptoms and signs of patients, which are the advantage of TCM but also have certain limitations. In the context of the increasingly modernized diagnosis and treatment in TCM, it is proposed to expand the application of the four examination techniques from three aspects including microcosmic syndrome differentiation, data sharing, and artificial intelligence in asymptomatic spleen-stomach diseases, in order to achieve the goals of dynamically observing the disease process, collecting disease data in multiple dimensions, and intelligently processing disease data. This will strengthen the modern requirements of early diagnosis and treatment in TCM, and highlight the advantages of TCM in “treating disease before it arises and treating the symptoms beforehand”.

Objective:To explore the application of cognitive interview (CI) in intercultural adaptation of the Happiness Scale for Middle-Aged Women (HAS-MW) .Methods:This was a qualitative study. In January 2022, 20 middle-aged women in community of Yanji City were selected by purposive sampling for two rounds of CI, and 10 subjects participated in each round of CI. According to the interview outline, we understood the understanding of the subjects on the HAS-MW items that were preliminarily localized in Chinese. The expert group discussed and revised the items of the scale according to the interview results. Results:In this study, a total of 2 rounds of cognitive interviews were conducted, the age of 10 respondents (P1-P10) in the first round was (49.00±3.55) , and the age of 10 respondents (P11-P20) in the second round was (51.00±3.16) . After the first round, 6 items in HAS-MW were revised due to "imperfect language, grammar and expression". The second round of CI results showed that the respondents understood the contents of the revised scale items and believed that they were consistent with the original scale. Conclusions:Through CI, we can comprehensively understand middle-aged women's understanding of the content of the HAS-MW, effectively find out the cognitive bias caused by improper language expression in the process of the scale's localization, improve the understanding of the content of the scale by the interviewees, which is conducive to intercultural adaptation of the HAS-MW and provides a reference for the revision of the scale.

The dorsal lingual epithelium, which is composed of taste buds and keratinocytes differentiated from K14+ basal cells, discriminates taste compounds and maintains the epithelial barrier. N6-methyladenosine (m6A) is the most abundant mRNA modification in eukaryotic cells. How METTL3-mediated m6A modification regulates K14+ basal cell fate during dorsal lingual epithelium formation and regeneration remains unclear. Here we show knockout of Mettl3 in K14+ cells reduced the taste buds and enhanced keratinocytes. Deletion of Mettl3 led to increased basal cell proliferation and decreased cell division in taste buds. Conditional Mettl3 knock-in mice showed little impact on taste buds or keratinization, but displayed increased proliferation of cells around taste buds in a protective manner during post-irradiation recovery. Mechanically, we revealed that the most frequent m6A modifications were enriched in Hippo and Wnt signaling, and specific peaks were observed near the stop codons of Lats1 and FZD7. Our study elucidates that METTL3 is essential for taste bud formation and could promote the quantity recovery of taste bud after radiation.

ObjectiveTo analyze the typical performance of initiating and responding behaviors of turn-taking in operational games for autistic children with low language function in special education schools and to provide a reference for intervention of turn-taking behaviors in operational games. MethodsFrom November, 2021 to January, 2022, a total of 23 autistic children with low language function (language ability ≤ three years old) in Shanghai Putuo District Qixing School were selected. Their linguistic ability was evaluated. A behavioral assessment approach was used to evaluate the behavior of initiating and responding behaviors of turn-taking in three operational games. The typical errors in initiating behaviors were summarized as difficult to initiate, untimely initiation, no response and abnormal initiation. The typical errors in responding behaviors of turn-taking in operational games were summarized as difficult to respond, untimely response, no response and abnormal response. ResultsThere was no significant differences in the performance of initiating behaviors among three types of operational games (χ2 = 11.106, P = 0.196), and there were significant differences in the performance of responding behaviors among operational games (χ2 = 26.256, P = 0.001). The initiating behaviors were postively correlated with word comprehension (r = 0.420, P < 0.05), word naming (r = 0.510, P < 0.05), and sentence imitation (r = 0.505, P < 0.05). The responding behaviors were postively correlated with word comprehension (r = 0.546, P < 0.01), word naming (r = 0.728, P < 0.01), sentence comprehension (r = 0.668, P < 0.01) and sentence imitation (r = 0.656, P < 0.01). ConclusionAutistic children with low language function showed different typical behaviors of initiating and responding behaviors of turn-taking in operational games. It is suggested that when designing training programs for turn-taking skills, targeted interventions should be made to address the typical types of errors in response and initiation turns, and individualized intervention programs should be designed to enhance children's communicative efficacy in play game and promote their language development and social participation.

Objective:To explore the surgical methods and effects of different types of pudendal thigh flaps in repairing rectovaginal fistula. Methods:From March 1996 to November 2019, totally 18 cases with rectovaginal fistula admitted to Plastic Surgery Hospital of Chinese Academy of Medical Sciences were analyzed retrospectively. The ages of the patients ranged from 17 months to 39 years with an average of 20.8 years. The bilateral pudendal thigh flap based on the anterior cutaneous branches of obturator artery can be used for larger fistula, which is larger than 1 cm in diameter and covered with serious local scars. Smaller fistula with diameter less than 1 cm can be repaired by the unilateral pudendal thigh flap pedicled with lateral branches of posterior labial artery transferred through subcutaneous tunnel. If the local tissue can be closed directly without tension, pudendal thigh fascial flap is inserted between the vaginal and rectal mucosa to act as a waterproof layer. Results:All flaps used in the 18 patients of this group survived postoperatively with fistula closure and donor site healed primarily. The size of the flaps ranged from 4 cm×2 cm to 7 cm×4 cm. The recipient sites including bilateral pudendal thigh island flaps (4 cases), unilateral pudendal thigh flaps (5 cases) and unilateral facial flaps (9 cases). No recurrence was reported during the follow-up period of 6 to 36 months. Conclusions:The application of pudendal thigh flaps can be considered as a safe and reliable procedure for the management of the middle and lower rectovaginal fistula. According to the anatomical characteristics of fistula, we could design rational flaps to meet the functional, morphological and cosmetic demands from the patients.
