1.Impact of Mild SARS-CoV-2 Infection on Hematological Para-meters and Suitability of Apheresis Platelets Donation in Blood Donors

Objective:To investigate the effects of mild SARS-CoV-2 infection on hematological parameters of adult blood donors and the suitability of apheresis platelet donation,the changes of the hematological parameters in blood donors with mild infection of the SARS-CoV-2 Omicron variant strain were evaluated.Methods:Seventy-two blood donors with mild COVID-19 symptoms who donated consecutive apheresis platelets for 3 times from December 2022 to January 2023,42 cases among which were included in the infection-positive group,and 30 cases in the suspected infection group.Forty-two donors un-vaccinated against SARS-CoV-2,un-infected,and donated three consecutive apheresis platelets from October to November 2022 were included in the control group.The changes of blood routine testing in the positive group and the suspected infection group were retrospectively compared before(Time1)and after(Time2 and Time3)the onset of symptoms,three consecutive times(Time1,Time2,Time3)in the control group by repeated measures analysis of variance.The Bayesian discriminant method was used to establish a discriminant equation to determine whether the recent infection of SARS-CoV-2 occurred or not.Results:Simple effect of the number times of tests in the positive and suspected infection groups was significant(Finfection-positive group=6.98,P<0.001,partial η2=0.79,Fsuspected infection group=4.31,P<0.001,partial η2=0.70).The positive group and the suspected infection group had lower RBC,HCT,and HGB,and higher PLT and PCT at Time2 compared to Time1 and Time3(P<0.05).The positive group and the suspected infection group showes RDW-CV and RDW-SD at Time3 higher than Time1 and Time2(P<0.001).The simple effect of the number times of tests in the control group was not significant(F=0.96,P=0.55,partial η2=0.34).The difference of the whole blood count parameters in the control group for three times was not statistically significant(P>0.05).We established a discriminant equation to determine whether the recent infection of SARS-CoV-2 occurred or not.The equation had an eigenvalue of 0.22,a canonical correlation of 0.43(x2=27.81,P<0.001),and an analysis accuracy of 72.9%.Conclusion:The hematological indicators of RBC,HCT,HGB,PLT,PCT,RDW-CV and RDW-SD in blood donors who had infected with mild COVID-19 showed dynamic changes.The discriminant equation for whether they are infected recently with COVID-19 has a high accuracy rate.
2.Genotype Analysis of Common and Rare Thalassemia in People of Reproductive Age in Huadu District,Guangzhou

Objective:To analyze the genotypes distribution of common and rare thalassemia in people of reproductive age in Huadu district of Guangzhou,enhance the database of thalassemia.Methods:Peripheral blood samples were collected for genotype analysis in Maternity and Child Health Hospital of Huadu District from January 2016 to October 2022.Gap-PCR and Reverse dot blot hybridization were used to detect common thalassemia genotypes.DNA sequencing was performed in samples suspected of rare genotypes.Results:A total of 16 171 subjects were identified as thalassemia carriers,and the positive rate was 44.41%(16 171/36 412).The genotypes of 114 cases(0.31%)were rare.A total of 10 845 cases were identified as α-thalassemia carriers(29.78%),and--SEA/αα was the most common genotype in those people,followed by-α3.7/αα and-α4.2/αα.A total of 4 531 subjects were identified as common β-thalassemia carriers(12.44%).The most common β-thalassemia mutation in the population was β41-42/βN,followed by β654/βN and β-28/β N.A total of 681 subjects were identified as αβ thalassemia carriers(1.87%),among them--SEA/αα compounded withβ CD41-42/β N was the most common genotype.A total of 48 cases were identified as rare α-thalassemia carriers,14 types of mutations,in which Fusion gene/αα was the most common.A total of 52 cases were identified as rare β-thalassemia carriers,11 types of mutation,in which βSEA-HPFH/βN was the most common.Conclusion:The thalassemia genotypes in Huadu district are complex and diverse.We should attach great importance to the detection of rare thalassemia genotypes.
3.Genotype and Phenotype of α-Thalassemia Fusion Gene in Huadu District of Guangzhou, Guangdong Province of China.

OBJECTIVE:
To explore the carrier rate, genotype and phenotype of α-thalassemia fusion gene in Huadu district of Guangzhou, Guangdong province of China, and provide data reference for the prevention and control of thalassemia.
METHODS:
A total of 10 769 samples who were screened for thalassemia in Maternal and Child Health Hospital of Huadu District from July 2019 to November 2020 were analyzed retrospectively. Blood cell analysis and hemoglobin (Hb) electrophoresis were performed. Thalassemia genes were analyzed by gap-PCR and PCR-reverse dot blot hybridization (PCR-RDB).
RESULTS:
A total of 9 cases with α-thalassemia fusion gene were detected in 10 769 samples (0.08%). There were 7 cases with fusion gene heterozygote, 1 case with compound of α-thalassemia fusion gene and Hb G-Honolulu, 1 case with compound of α-thalassemia fusion gene and Hb QS. The MCV results of 4 samples of blood cell analysis were within the reference range, the Hb A2 value of 1 case was decreased, and there were no other abnormalities found.
CONCLUSION
The α-thalassemia fusion gene is common in Huadu district of Guangzhou, and heterozygotes are more common, and current screening methods easily lead to misdiagnosis.

4.Survey of Oncomelania hupensis in national schistosomiasis surveillance sites of Guangxi Zhuang Autonomous Region from 2015 to 2019

Objective To investigate the distribution characteristics of Oncomelania hupensis in Guangxi Zhuang Autonomous Region, so as to provide insights into the assessment of the risk of schistosomiasis transmission and the scientific formulation of the schistosomiasis surveillance strategy. Methods From 2015 to 2019, a total of 19 national schistosomiasis surveillance sites were assigned in Guangxi Zhuang Autonomous Region, including 4 fixed sites and 15 mobile sites. Snail survey was performed by means of systematic sampling in combination with environmental sampling, and the infection of Schistosoma japonicum was detected by the crushing method combined with loop-mediated isothermal amplification (LAMP) assay. Results From 2015 to 2019, snail habitats were detected at areas of 17 040 to 39 527 m2, including 6 214 m2 emerging snail habitats and 16 563 m2 re-emerging snail habitats. The overall mean density of living snails was 0.019 2 snails/0.1 m2 and the occurrence of frames with snails was 1.11% in the national schistosomiasis surveillance sites; however, no S. japonicum infection was identified in snails. The area of snail habitats increased by 121.46% in the national surveillance sites in 2019 as compared to that in 2015; however, 50.34% (Z = −0.422, P > 0.05) and 42.85% (χ2 = 130.41, P < 0.01) reductions were seen in the overall means density of living snails and the occurrence of frames with snails. All snail habitats were distributed in the 4 fixed surveillance sites, and were mainly found in ditches, paddy fields and dry lands, with weeds as the primary vegetation type. Conclusions There are still risk factors leading to re-emergent transmission of schistosomiasis in Guangxi Zhuang Autonomous Region, such as local snail spread, and the monitoring of schistosomiasis remains to be reinforced to further consolidate the achievements of schistosomiasis elimination in the region.

Introduction: The global pandemic of Corona Virus Disease
2019 (COVID-19) has led to the re-purposing of medications,
such as hydroxychloroquine and lopinavir-ritonavir in the
treatment of the earlier phase of COVID-19 before the
recognized benefit of steroids and antiviral. We aim to
explore the corrected QT (QTc) interval and ‘torsadogenic’
potential of hydroxychloroquine and lopinavir-ritonavir
utilising a combination of smartphone electrocardiogram
and 12-lead electrocardiogram monitoring.
Materials and Methods: Between 16-April-2020 to 30-April2020, patients with suspected or confirmed for COVID-19
indicated for in-patient treatment with hydroxychloroquine
with or without lopinavir-ritonavir to the Sarawak General
Hospital were monitored with KardiaMobile smartphone
electrocardiogram (AliveCor®, Mountain View, CA) or
standard 12-lead electrocardiogram. The baseline and serial
QTc intervals were monitored till the last dose of
medications or until the normalization of the QTc interval.
Results: Thirty patients were treated with
hydroxychloroquine, and 20 (66.7%) patients received a
combination of hydroxychloroquine and lopinavir-ritonavir
therapy. The maximum QTc interval was significantly
prolonged compared to baseline (434.6±28.2msec vs.
458.6±47.1msec, p=0.001). The maximum QTc interval
(456.1±45.7msec vs. 464.6±45.2msec, p=0.635) and the delta
QTc (32.6±38.5msec vs. 26.3±35.8msec, p=0.658) were not
significantly different between patients on
hydroxychloroquine or a combination of
hydroxychloroquine and lopinavir-ritonavir. Five (16.7%)
patients had QTc of 500msec or more. Four (13.3%) patients
required discontinuation of hydroxychloroquine and 3
(10.0%) patients required discontinuation of lopinavirritonavir due to QTc prolongation. However, no torsade de
pointes was observed.
Conclusions: QTc monitoring using smartphone
electrocardiogram was feasible in COVID-19 patients treated
with hydroxychloroquine with or without lopinavir-ritonavir.
The usage of hydroxychloroquine and lopinavir-ritonavir
resulted in QTc prolongation, but no torsade de pointes or
arrhythmogenic death was observed.
6.Effect of Chaihushugansan combined with fluoxetine on IL-6, IL-1β, TNF-α in postpartum depression

Objective To observe the effect of Chaihushugansan combined with fluoxetine on interleukin -6 ( IL-6 ) , interleukin -1β( IL-1β) , tumor necrosis factor ( TNF-α) in the treatment of patients with postpartum depression.Methods 83 patients from November 2013 to July 2015 in Jiaxing Maternity and Child Health Care Hospital were randomly divided into 41 patients of observation group and 42 patients of control group.The control group were treated with fluoxetine treatment, the obsercation group received Chaihushugansan on the basis of control group.The Hamilton depression scale (HAMD) and Hamilton anxiety scale (HAMA),IL-6, IL-1β, TNF-α, high-sensitivity C-reactive protein (hs-CRP) and adverse reactions were followed up and recorded.Results The HAMD and HAMA score in observation group post-treatment were (8.31 ±2.05,9.03 ±2.08)points, which were lower than (13.96 ±2.16, 13.61 ±2.14) points in control group (P<0.05).The serum IL-6 and IL-1βlevels in observation group post-treatment were (6.59 ±3.20,5.01 ±2.83)pg/mL, which were lower than (10.64 ±3.86,9.31 ±3.42)pg/mL in control group (P<0.05).The serum TNF-αand hs-CRP levels in observation group post-treatment were ( 9.16 ±2.01 ) pg/mL, ( 3.62 ±1.06 ) mg/L, which were lower than ( 12.30 ±2.37 ) pg/mL, (5.29 ±1.14)mg/L in control group (P<0.05).The total adverse reaction rate in observation group was 7.32%, significantly lower than 23.81% in control group ( P <0.05 ) .Conclusion Chaihushugansan combined with fluoxetine has a good therapeutic effect in the treatment of postpartum depression, could significantly reduce the IL-6, IL-1β, TNF-α levels, with fewer side effects, it is better than fluoxetine alone.
7.Current Status and Characteristics of Perioperative Transfusion in Gynecologic Patients

Objective To analyze the current status and characteristics of perioperative transfusion in pa-tients undergoing gynecologic surgery and to explore the rational management of blood product use. Methods Clinical data of patients who underwent gynecologic surgery in Peking Union Medical College Hospital from Janu-ary to October in 2014 were retrospectively reviewed. Those who received perioperative transfusion were analyzed to summarize the status and characteristics of transfusion for different diseases and surgical procedures. The data of patients in 2014 were compared with those of patients in the corresponding period of 2013 . Results Altogeth-er 6203 patients underwent gynecologic surgery in Peking Union Medical College Hospital from January to October in 2014, including 286 patients who had allogeneic transfusion during perioperative period, and 80 who had au-tologous transfusion. The total number of surgery in 2014 increased by 10. 5% compared with the same period in 2013, while the allogeneic transfusion rate in 2014 (4. 6%) decreased by 27% compared with the same period in 2013 ( P=0. 001 ) . Operations for malignancies had higher transfusion rates than those for mild diseases. Cytoreductive surgery and re-cytoreductive surgery for ovarian cancer had the highest rate of transfusion (45. 3%) , and the average transfusion volume was 6. 7 U. Laparoscopy had lower transfusion rate compared with laparotomy. In radical resection of cervical cancer, approach by laparoscopy had a lower rate of transfusion than by laparotomy (6. 2% vs. 10. 9%) . Autologous transfusion was applied mainly in operations for ectopic pregnancy or internal bleeding due to rupture of corpus luteum, myomectomy and total hysterectomy, with autologous transfusion rates being 17. 0%, 5. 2%, and 2. 8%, and average transfusion volume being 749, 429, and 397 ml, respectively. Conclusions Perioperative transfusion rate in 2014 was lower than in 2013 despite more gynecologic operations in 2014 . Rational blood use in gynecologic patients can be managed by strategies including selection of appropri-ate surgery procedures, application of minimally invasive surgery, and proper use of autologous trasfusion.
8.Cellular mechanisms of emerging applications of mesenchymal stem cells

Mesenchymal stem cells (MSC) are multipotent, self-renewing cells that can be found mainly in the
bone marrow, and other post-natal organs and tissues. The ease of isolation and expansion, together
with the immunomodulatory properties and their capability to migrate to sites of infl ammation and
tumours make them a suitable candidate for therapeutic use in the clinical settings. We review here
the cellular mechanisms underlying the emerging applications of MSC in various fi elds.
9.Identification of low-risk indicators of early stage cervical cancer.

OBJECTIVETo explore the low-risk indicators of early cervical cancer.
METHODSThe medical records of 201 patients undergoing radical surgery between March 2000 and April 2011 for staging Ia2,Ib1 (tumor diameter≤2cm) cervix cancer were retrospectively reviewed, with particular focus on the pathological findings [parametrial involvement, positive margin, positive pelvic lymph node, and lymph vascular space invasion (LVSI)], treatment, and outcomes.
RESULTSThe operation duration ranged 75-330min (mean:188.87 min) and the intra-operative blood loss was approximately 100-2500 ml (mean: 583.33 ml). Pathology showed the rate of parametrial spread, positive margins, lymph node metastasis, LVSI was 0, 6.97%, 12.44%, and 17.41%. Based on the pathologic findings, the patients were classified as two groups: group A had 147 patients(73.13%) with no neoplasm or tumor diameter ≤2 cm,while group B had 54 patients (26.87%) with tumor diameter > 2 cm. The incidence of ≥ 1/2 cervical stromal invasion, LVSI, positive lymph node, underlying section of uterus involvement, and low tumor differentiation in group A and B were 20.14% vs. 85.19% (p = 0.000), 13.61% vs. 27.78%(p = 0.019), 9.52% vs. 20.37% (p=0.039), 4.82% vs. 14.81% (p=0.008), and 35.37% vs. 44.44% (p=0.025), respectively, with significant differences. Among the 163 patients who were followed up for more than 3 months, 10(6.13%) developed recurrence whereas no patient died.
CONCLUSIONSPathologic parametrial involvement in clinical stage 1a2 and 1b1 cervical cancer is uncommon. Tumor size and cervical stromal invasion can be used to identify low-risk population that are worthy of consideration for studies of less radical surgery performed in conjunction with pelvic lymphadenectomy.

10.Primary sarcoma of the ovary: clinicopathological characteristics, prognostic factors and evaluation of therapy.

BACKGROUNDThe primary ovarian sarcoma is a very rare malignancy. The objective of this study was to further investigate the clinicopathologic features and outcome in patients with primary sarcoma of the ovary.
METHODSBetween 1988 and 2007, 24 patients with primary ovarian sarcoma who underwent treatment at Peking Union Medical Hospital were reviewed retrospectively. Response to treatment, progression and overall survival were analyzed.
RESULTSPatients with ovarian sarcoma had a mean age of (54.3 ± 10.3) years, and 16 of them were postmenopausal. The most common symptom was abdominal pain, present in 14 patients. Of the 24 patients, 16 patients were pathologically diagnosed as carcinosarcoma (known as malignant mixed mesodermal tumor (MMMT)), 2 as ovarian leiomyosarcoma (LS) and 6 patients as ovarian endometrial stromal sarcoma (ESS). The patients in optimal debulking group had a median survival period of 28 months and 1-year survival rate of 71%. The patients in suboptimal debulking group had a significantly lower median survival of 6 months (P = 0.02) and 1-year survival rate of 29%. Among the patients, 23 patients received chemotherapy and most of regimens were based on platinum, 3 patients received chemoradiation. The mean number of courses of combined chemotherapy was 6.6 ± 5.0, and the response was unsatisfactory. The median survival for the entire group was 18.7 months. The one-year survival rate was 58%, and two-year survival rate only 29%.
CONCLUSIONSOvarian primary sarcoma has a poor overall prognosis. Optimal debulking surgery appears to be of prognostic significance. There is a clear need for further study to explore the role and the regimen of platinum-based chemotherapy in primary ovarian sarcoma.
