1.Pharmacognostical identification research of Inonotus obliquus

Objective To study the identification methods and traits of the Chinese medicinal materials Inonotus obliquus.Methods Samples were collected from the origin of northeast China,and source identification,molecular biology identification,trait identification,microscopic identification and HPLC assay were used to study the Inonotus obliquus,its non-pure fungous part,and counterfeit.Results Inonotus obliquus had a nodular shape,the microstructure was closely interconnected with mycelia,and the medicinal part was the sclerotium,not the"fruiting body"as stipulated in the current standards of traditional Chinese medicinal materials.Occasionally,a very small amount of tubule and subiculum could be seen in Inonotus obliquus.Tubule hole shape was circular,with a diameter of 130-190 μm,and ruptured.The pipe wall structure was special,which was composed of longitudinal arranged small compartment with diameter of 2-5-8 μm.Non-pure fungous part was the decayed host xylem infiltrated by mycelia.The contents of trametenolic acid and ergosterol in this part were significantly lower than the pure sclerotium(P<0.05).The microstructure and HPLC fingerprints of Inonotus obliquus and the counterfeit burl were clearly distinguishable from each other.Conclusion The method used is able to identify the authenticity of Inonotus obliquus and the quality of the medicinal materials.
2.Effect of Atractylodis Macrocephalae Rhizoma on Short-chain Fatty Acids and Intestinal Barrier in Mice with Slow-transit Constipation

ObjectiveTo investigate the mechanism of Atractylodis Macrocephalae Rhizoma(AMR) in the treatment of slow-transmission constipation(STC) by observing the effects of AMR on short-chain fatty acids and intestinal barries in STC mice. MethodForty-eight male KM mice were randomly divided into blank group, model group, AMR low-, medium-, high-dose groups(2.5, 5, 10 g·kg-1) and mosapride group(2.5 mg·kg-1). Except for the blank group, all groups were gavaged with loperamide suspension(5 mg·kg-1) twice daily for 14 d to construct the STC mouse model. At the same time, each drug administration group was given the corresponding drug by gavage for consecutive 14 d, the blank and model groups were gavaged with equal volume of distilled water. The effects of the treatment of AMR on body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice were observed, the pathological changes of mouse colon were observed by hematoxylin-eosin(HE) staining and periodic acid-Schiff(PAS) staining, the levels of gastrin(GAS) and motilin(MTL) in serum were detected by enzyme-linked immunosorbent assay(ELISA), gas chromatography-mass spectrometry(GC-MS) was used to detect the contents of short-chain fatty acids(SCFAs) in mouse feces, real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to determine the mRNA and protein expression levels of zonula occludens-1(ZO-1), Occludin, and Claudin-1 in the colon of mice. ResultCompared with the blank group, the body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice in the model group were significantly decreased(P<0.05, P<0.01), the arrangement of colonic tissues was disordered, and the number of goblet cells was reduced, the levels of GAS and MTL in serum were significantly decreased(P<0.01), and the levels of SCFAs in the feces were on a decreasing trend, with the contents of acetic acid, propionic acid, butyric acid, isobutyric acid and valeric acid were significantly decreased(P<0.05, P<0.01), the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colonic tissues were significantly decreased(P<0.01). The above results suggested that STC mouse model was successfully constructed. Compared with the model group, the body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice in AMP administration groups all increased significantly(P<0.05, P<0.01), the mucosal layer of the colonic tissues was structurally intact without obvious damage, and the number of goblet cells increased, serum levels of GAS and MTL were significantly increased(P<0.01), the contents of SCFAs in the feces were all on a rising trend, with the contents of acetic, propionic, butyric and isobutyric acids rising significantly(P<0.05, P<0.01), the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colonic tissues were significantly increased(P<0.05, P<0.01). ConclusionAMR is able to improve the constipation symptoms in STC mice, and its mechanism may be related to increasing the contents of SCFAs in the intestine as well as promoting the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colon.
3.Effect of Portable Oto-endoscopy System in Clinical Teaching of Otorhinolaryngology

To explore the value of portable oto-endoscopy system in clinical teaching of otolaryngology residents. The postgraduate students serving as resident doctors in the Department of Otolaryngology of Peking Union Medical College Hospital from February to March 2022 and from February to March 2023 were selected as the research objects. Random number table method was used to divide them into experimental group and control group. The control group was first taught by theoretical explanation + electrooto-endoscopy system, and the experimental group was first taught by theoretical explanation + portable oto-endoscopy system. After one month, the two groups interchanged their teaching methodologies. The results of theoretical assessment, self-evaluation at the end of the first month of clinical learning and satisfaction with teaching effectiveness at the end of two months of clinical learning were compared between the two groups. A total of 36 residents were included in this study, with 18 in each group. After one month of clinical study, the theoretical test scores of the experimental group were significantly higher than those of the control group[(93.17±4.16) points The portable oto-endoscopy system can display the anatomy and diseases of otolaryngology more vividly and intuitively in the clinical teaching of otolaryngology, facilitate the management of clinical data, increase the learning interest of residents, fully mobilize the image thinking of medical students, and improve the post competence of residents more efficiently.

5.Liquid isolation assisted technique in radiofrequency ablation for hepatocellular carcinoma near liver surfaces

Objective:To study the safety and efficacy of liquid isolation assisted technology in ultrasound-guided percutaneous radiofrequency ablation for hepatocellular carcinoma adjacent to liver surfaces.Methods:A retrospective study was conducted on 246 patients with hepatocellular carcinoma near liver surfaces treated by radiofrequency ablation from November 2016 to November 2019 at the Department of Hepatobiliary and Pancreatic Surgery, Affiliated Tumor Hospital of Zhengzhou University. There were 182 males and 64 females, aged (56.7±9.2) years. The patients were divided into the liquid isolation group with liquid isolation assisted technology for radiofrequency ablation ( n=131) and the conventional treatment group with conventional radiofrequency ablation ( n=115). The complication, one-time complete ablation and progression-free survival rates were compared between the two groups. Results:The incidence of intraoperative complications in the liquid isolation group was 21.4% (28/131), versus 20.9% (24/115) in the conventional treatment group. There was no significant difference between the two groups ( P>0.05). The incidence of postoperative complications in the liquid isolation group was 20.6% (27/131), which was significantly lower than that in the conventional treatment group 33.9% (39/115) ( P<0.05). The incidence of postoperative pain in the liquid isolation group was 6.1% (8/131), which was significantly lower than that in the conventional treatment group 15.7% (18/115) ( P<0.05). For the 131 patients in the liquid isolation group, there were 183 tumors near to liver surfaces. For the 115 patients in the conventional treatment group, there were 142 lesions near to liver surfaces. The one-time complete ablation rate in the liquid isolation group was 94.0% (172/183), which was significantly higher than the 75.4% (107/142) in the conventional treatment group ( P<0.05). The progression-free survival rate in the liquid isolation group was significantly better than that in the conventional treatment group ( P<0.05). Conclusion:The liquid isolation technique increased the one-time complete ablation rate of liver cancer adjacent to liver surfaces, reduced the incidence of postoperative complications and improved prognosis.
6.Research advances in the diagnosis and treatment of acute-on-chronic liver failure with infection

Acute-on-chronic liver failure (ACLF) is a severe syndrome of liver diseases commonly seen in clinical practice and can lead to severe disorders and decompensation of liver synthesis, metabolism, detoxification, and biotransformation, as well as multiple organ failure and an extremely high short-term mortality rate. Infection can induce or aggravate the condition of ACLF and is an independent influencing factor for patient prognosis. This article describes the mechanism and characteristics of ACLF with infection, summarizes the common types and clinical features of infection, reviews the recommended anti-infective regimens, and emphasizes the importance of early prophylactic treatment. ACLF patients with infection tend to have critical conditions, and early diagnosis and empirical anti-infective treatment is the key to successful treatment.
7.Effects of NT-proBNP,blood lactate levels,troponin I and plasma cystatin C on the prognosis of patients undergoing cardiopulmonary bypass

Objective To explore the relationship between multiple serological indexes and cardiac prog-nosis risk of patients;To compare the four predictive value of prognosis of patients with cardiac surgery. Meth-ods A total of 127 patients treated by cardiopulmonary bypass surgery were enrolled from July 2013 to March 2015 in our department.Their baseline data were recorded.All patients were followed up for 2 years.Patients with adverse outcomes were defined as the unfavorableprognosis group during the follow-up period while the patients with nodisease progression were defined as the favorableprognosis group. Univariate analysis and Cox proportional risk model wereapplied to evaluate the effects of all factors on the patient's prognosis. Meanwhile,the prediction ability of multiple serological indexes in predicting prognosis of patients with cardiac surgery was calculated by ROC curve analysis. Results Multivariate Cox proportional hazard model showed that the △NT-proBNP(P =0.019),△plasma levels of cystatin C(P=0.037),△troponin I(P=0.028)and LVEF(P=0.045)have signifi-cant influence on the prognosis of the patients and the △NT-proBNP has the highest degree(RR = 1.598);The AUC of the NT-proBNP combined with LVEF in diagnosis of patients with cardiac surgery was 0.842,and the sen-sitivity and specificity were 75.3% and 76.2%,respectively. Conclusions The level of △NT-proBNP and LVEF have a good predictive ability for cardiac outcome in patients undergoing surgery,which is expected as a conven-tional evaluation index of cardiac surgery with cardiopulmonary bypass.
8. PBMCs microRNA-122 level correlates with virological responses to pegylated interferon alpha therapy in patients with hepatitis C genotype 1b

Objective:
To clarify the predictive power of PBMCs miR-122, as well as other clinical factors, for response to IFNα therapy in chronic HCV infected patients.
Methods:
A total of 40 patients chronically infected with HCV genotype 1b were enrolled. All the patients received pegylated interferon alpha (PEG-IFN α) in combination with ribavirin for 48 weeks. To perform the analyses, the patients were compared in terms of achieving sustained virological response (SVR) or not (NSVR) at 24th week after antiviral treatment.
Results:
SVR rate was 72.5% (29/40) and NSVR rate was 27.5% (11/40). SVR group experienced significantly lower HCV viral load, total bilirubin (TBIL), alpha fetal protein (AFP), fibroscan and laminin (LN) compared with NSVR group before treatment (
9.Genetic Diversity Analysis of Desmodium styracifolium from Different Habitats by ISSR Analysis

Objective:To assess the genetic diversity and phylogenetic relationship of Desmodium styraeifolium from fifteen regions of Guangdong and Guangxi Zhuang autonomous region. Methods:The molecular technique ISSR((inter-simple sequence repeat))was applied to investigate the genetic diversity of Desmodium styraeifolium from fifteen regions of Guangdong and Guangxi Zhuang autonomous region. The data was analyzed with Popgene 1. 32,and a cluster diagram was presented by UPGMA. Results:Totally 51 amplified fragments were obtained by 7 ISSR primers. The results analyzed by Popgene 1. 32 showed that the Shannon diversity index(I)was 0. 3,the NEI’s genetic diversity coefficient(H)was 0. 246 4,the coefficient of genetic differentiation (GST)was 0. 123 8,and the gene flow(Nm)was 3. 539 7. Conclusion:The above mentioned results exhibit that Desmodium styraeifolium from Guangdong,Guangxi and some wild herbal populations has high genetic diversity. The clustering results illustrate that the genetic distance of Desmodium styraeifolium originated from Guangdong and Guangxi is related with geographic distance.
10.Application value of DOSE Index Scoring System in Chronic Obstructive Pulmonary Disease

Objective To explore the application value of DOSE index score in the peitients with chronic obstructive pulmonary disease.Methods 122 cases of plateau COPD patients were followed up for 12 months,and we recorded and analyzed the patient's health and life.We also recorde FEV 1 and DOSE scores of the patients with COPD,and record the COPD risk events,including the number of respiratory failure and death,and the times of hospitalization,total such confinement,outpatient expenses,hospitalization expenses,mMRC,and scored in the number of exacerbations,etc.Results The DOSE index score was negatively correlated with FEV1% pred (r=0.73,P < 0.05) for 122 COPD patients,and were positively correlated with mMRC (r=085,P < 0.01),the annual number of exacerbations (r=0.71,P < 0.01),respiratory failure (r=0.65,P < 0.01),heart failure (r=0.50,P < 0.01),number of outpatient service (r=0.12,P<0.01),hospitalization time (r=0.70,P<0.01),the totalsuchconfinement (r=0.66,P<0.01),outpatient expenses (r=0.13,P> 0.13),hospitalization expenses (r=0.65,P<0.01).ROC curve was used to analyzed the cut-off point and curve area of COPD DOSE index.Conclusion The DOSE index is a simple COPD assessment tools,and is closely related to the prognosis of patients and health.
