As the core vehicle for preserving and transmitting traditional Chinese medicine（TCM） academic thought and clinical experience， the establishment of a robust quality evaluation system for TCM clinical case reports is a crucial component in the current standardization and modernization of TCM. Based on the practical experience of constructing the China Clinical Cases Library of Traditional Chinese Medicine by the China Association of Chinese Medicine， this study conducted a comprehensive analysis of critical challenges， including insufficient authenticity and unfocused evaluation criteria. It proposed a three-dimensional evaluation framework grounded in the structure-process-outcome logic， encompassing three dimensions of authenticity and standardization， characteristics and advantages， application and translational impact. This framework integrated 12 key evaluation indicators in a systematic manner. The model preserved the academic characteristics of TCM syndrome differentiation and treatment， while aligning with modern scientific research standards， achieving a balance between individualized TCM experience and standardized evaluation. Concurrently， this study provided theoretical foundations and methodological guidance for evaluating the quality of TCM clinical cases， contributing significantly to the inheritance of TCM knowledge， evidence-based practice， and the reform of talent evaluation mechanisms.

ObjectiveTo observe the effect ofShenmai injection on isolated cardiac systolic and diastolic function in myocardial infarction rats.Methods32 rats were randomly divided into a control group, a propranolol group, aShenmai + propranolol group and aShenmai group. The myocardial infarction models were made by ligating the left anterior descending coronary artery of rats .The heart was taken from myocardial infarction rat after 3 days. The indexes of Isometric tension (IT), Diastolic tension (DT), heart rate (HR), the maximum pressure ascending and descending rate of left ventricle (± LVdp/dtmax) (mmHg/s) were recorded by using Langendorff isolated heart perfusion technique at 5min before and 0.5 h, 1 h, 2 h after perfusion.ResultsCompared with the control group, the IT (2.01 ± 0.02 g, 1.52 ± 0.01 g, 1.53 ± 0.02 g), the HR (326 ± 32 time/min, 331 ± 41 time/min, 307 ± 37 time/min) and the ±LVdp/dtmax (4 012 ± 193 mmHg/s, 3 729 ± 174 mmHg/s, 3 631 ± 123 mmHg/s; 3 565 ± 202 mmHg/s, 3 632 ± 154 mmHg/s, 3 601 ± 129 mmHg/s) were apparently decreased in the propranolol group at 0.5 h, 1 h, 2 h after perfusion, and the differences were statistically significant (P<0.05). Compared with the control group, the IT (2.71 ± 0.10 g, 3.74 ± 0.04 g, 3.17 ± 0.01 g), the HR (491 ± 46 time/min, 527 ± 52 time/min, 519 ± 49 time/min) and the +LV dp/dtmax (5 690 ± 2 403 mmHg/s, 5 739 ± 219 mmHg/s, 5 825 ± 186 mmHg/s) were distinctly increased in theShenmaigroup at 0.5 h, 1 h, and 2 h after perfusion, and the differences were statistically significant (P<0.05). While the IT, DT, HR and ±LVdp/dtmax in theShenmai + propranolol group had no significant effect compared with the control group(P> 0.05).ConclusionsShenmaiinjection can improve myocardial contractility in isolated heart of myocardial infarction rats, however, propranolol can completely block the strong heart effect ofShenmaiinjection. This suggests thatShenmai injection has a beta receptor agonist effect.

ObjectiveTo evaluate the therapeutic effect of Salvia miltiorrhiza and ligustrazine combined with vinpocetine in patients with cerebral infarction.MethodsAtotal of 206 patients with cerebral infarction were randomly divided into a controlgroup and a combined treatment group according to the random number table, 103 in each group. The control group was treated with aspirin and vinpocetine, the combined treatment group received Salvia miltiorrhiza and ligustrazine injection on the basis of the control group. The serum contents of malondialdehyde (MDA) and nitric oxide (NO), as well as the serum levels of superoxide dismutase (SOD) and nitric oxide synthase (NOS) were detected before and after the treatment. The electroencephalography (EEG) amplitudes and the regional cerebral blood flow (rCBF) were recorded before and after the treatment .ResultsIn the combined treatment group, the serum content of MDA was significantly decreased (151.36 ± 11.36 mmol/Lvs.62.23 ± 4.11 mmol/L;t=74.878,P<0.01), while the serum content of NO (110.31 ± 9.51 mmol/Lvs.154.23 ± 12.21 mmol/L;t=28.801,P<0.05) and the serum levels of SOD (55.52 ± 4.47 U/mlvs.85.39 ± 7.21 U/ml;t=35.735,P<0.05) and NOS (115.21 ± 8.39 mmol/Lvs.190.12 ± 11.29 mmol/L;t=54.049,P<0.01) were significantly increased than before the treatment. The rCBF (39.39 ± 2.45 ml/100 g?minvs. 69.95 ± 4.25 ml/100 g?min;t=32.385,P<0.05) and EEG (62.41% ± 4.58%vs.97.02% ± 9.67%;t=31.012,P<0.05) in the combined treatment group were significantly increased than before the treatment. The total efficiency rate in the combined treatment group was significant higher than that in the control group (88.3% vs. 66.9%;χ2=12.343, P=0.004).ConclusionsSalvia miltiorrhiza and ligustrazine combined with vinpocetine has a beneficial therapeutic effect in patients with cerebral infarction, and it is probably associated withincreasing cerebral blood flow and enhancing antioxidant activity.

Objective To establish homogeneous immunoassay for detecting serum cardiac troponin I (cTnI) by using light induced chemiluminescent immunoassay (LiCA). Methods Polyclonal antibodies of cTnI were coated on the receptor particles, monoclonal antibodies of cTnI were biotinylated, and the donor particles were coated with streptavidin, all of which were composed of LiCA reagents. The optimal test conditions and analytical performance of the detection method were studied. Results The method was rapid, sensitive, and detection time was 17.5 min.The analytical sensitivity was 0.045 ng/mL and the functional sensitivity was 0.053 ng/mL.The recovery rate was 104.96%-108.21%;The within-run and the between-run coefficients of variation were 3.88%-5.53%and 7.60%-8.75%, respectively. The interference rates for the endogenous substances were less than 10%. The reference value of cTnI was less than 1.05 ng/mL;Results of cTnI LiCA correlated well with direct chemiluminescence detection (r2 =0.979). Conclusions This approach can be used for the quantitative detection of serum cTnI, and it is homogeneous and is free of clean separation. It provides a convenient, highly sensitive detection platform for clinical practice.

Objective To observe the curative effect of traditional Chinese medicine on the treatment of osteoarthritis of the knee with Tongbi topical ointment combined with joint cavity effusion flushing.Methods 366 patients of osteoarthritis of knee joint with effusion were collected from May 2009 to March 2013 in Hubei University of Medicine Affiliated Taihe Hospital,and randomly recruited into a control group and a treatment group by random number table method,with 188 patients in each group.The control group was treated with the joint cavity drainage and oral taking glucosamine hydrochloride capsules,the treatment group was treated with external application of Tongbi ointment based on the control group.Leukocyte osteoarthritis-1 beta (IL-1β) and hyaluronic acid (HA) were measured before and after the treatment.VAS and WOMAC osteoarthritis index were used to evaluate the effect.Results Compared with indexes before the treatment,IL-1β[(39.29±7.52) pg/ml,(28.33 ± 5.42) pg/ml] decreased significantly,HA content [(1.41 ± 0.23) μg/L,(3.34± 0.48) μg/L] increased significantly,VAS score [before treatment and 1-4weeks were (7.21±0.35),(5.47±0.34),(5.04±0.32),(4.03±0.24),(2.23±0.15)] decreased and the WOMAC index[(56.17±7.31),(46.32±6.34),(37.17± 5.31),(29.04±3.58),(15.21 ±2.37)] improved significantly in the treatment group after 4 weeks of treatment.There was a significant difference between the treatment and the control group showed (P＜0.05).Combined with the total effective rate of 98.9％ (181/183) in treatment group,the control group of 87.4％ (160/183) showed a significant difference (P＜0.01).Conclusion External application of Tongbi ointment combined with joint cavity flushing was effective in the treatment of osteoarthritis of the knee with effusion.

ObjectiveTo perform pharmacology problem-based learning (PBL) and evaluate its effects.MethodsPBL was performed for the clinical medicine class of grade 2007 and the satisfactory degree of students to teaching effects was observed with questionnaire. Results The students thought that PBL teaching had substantial contents and proper schedule and increased learning interest. Students' participating degree, mutual communication and controlling discussion procedure were fine,which reached the expected learning objective. ConclusionsThe effects of PBL teaching were excellent and most of our students could accept it.

The functional experimental teaching reform plays a very important role in raising the teaching resources,consummating the experimental teaching management system,enhancing the student to practice the ability and so on and has obtained the good effect.But it also faces some problems waiting urgently to be solved and further perfected,to which this article has made the analysis and the discussion from the management science angle.The goal lies in deepening the educational reform in functional experiment unceasingly to provide the model.

Aiming at training talents with knowledge,quality and ability and after practicing for several years,our college has showed its feature in the teaching reform of functional experiment,which can be summarized as the organ-centered teaching model with comprehensiveness,informationalization and openness.The article reflects on the forming of the model and fixes on the direction and target in the future in functional experiment teaching.
