1.Minimally invasive management of spontaneous renal vascular rupture:a report of two cases

Objective To investigate the diagnosis and treatment of 2 cases with spontaneous renal vascular rupture, and discuss the possible causes and treatment methods of this disease. Methods The clinical data and treatment methods of the 2 patients with spontaneous renal vascular rupture treated with renal artery embolization in Universal Global Xi'an Beihuan Hospital were analyzed. Relevant literature was reviewed to summarize the possible causes of this condition. Results The patients had no obvious history of trauma and no obvious abnormal coagulation function, and sought treatment due to sudden lumbar and abdominal pain. Laboratory tests revealed significantly decreased hemoglobin level, and elevated blood glucose (27.8, 27.6mmol/L). Abdominal computed tomography (CT) indicated perirenal hematoma and active bleeding. The bleeding arteries were located at the terminal branches of the anterior renal artery. Precise embolization was performed using CT angiography and selective renal artery angiography plus embolization, achieving an immediate hemostasis success rate of 100%. Postoperative hemoglobin level stabilized, and follow-up CT scans showed significant absorption of the perirenal hematoma without significant deterioration of renal function. Based on literature review and case analysis, the possible cause of spontaneous renal vascular rupture in these two patients was renal vascular sclerosis due to long-term diabetes. Other common etiologies of this condition included renal tumor, vascular malformation, and hypertension. Conclusion Spontaneous renal vascular rupture is rare. Renal artery embolization, as aminimally invasive treatment, can effectively control bleeding and protect renal function. The specific pathogenesis of this condition still requires further investigation. Long-term diabetes mellitus with poor blood glucose control may be associated factors, but more evidence is needed.
2.Clinical efficacy of TIPS combined with main splenic artery embolization in the treatment of portal hypertension upper gastrointestinal bleeding complicated with extensive portal vein thrombosis

Objective:To analyze the efficacy of transjugular intrahepatic portosystemic shunt (TIPS) combined with main splenic artery embolization in the treatment of patients with portal hypertension upper gastrointestinal bleeding complicated with extensive portal vein thrombosis (PVT).Methods:This study was a prospective, single-center, open-label, single-arm clinical trial. In the first phase, 81 patients with portal hypertension upper gastrointestinal bleeding who were admitted to Beijing Shijitan Hospital, Capital Medical University from January 2018 to December 2018 were consecutively enrolled, including 57 males and 24 females, with the age of (51.3±10.4) years. During TIPS surgery, the pressure of the portal vein before and after the balloon blocking the splenic artery was measured to clarify the contribution of the splenic artery to portal hypertension. In the second stage, from January 2019 to December 2022, 104 patients with portal hypertension upper gastrointestinal bleeding complicated with extensive PVT were re-enrolled, including 71 males and 33 females, with the age of (50.9±12.5) years. TIPS combined with main splenic artery embolization was performed, and portal vein pressure was measured before and after embolization. Follow up on the postoperative esophageal and gastric varices of the patients in the second stage.Results:The portal vein pressures before and after the first stage of balloon occlusion of the splenic artery were (35.2±8.4) mmHg (1 mmHg=0.133 kPa) and (24.2±6.3) mmHg, respectively. The pressure after occlusion was lower than that before occlusion, and the difference was statistically significant ( t=10.54, P<0.001). The portal vein pressures before and after the second stage embolization were (36.1±9.5) mmHg and (21.1±4.7) mmHg respectively. The pressure after embolization was lower than that before embolization, and the difference was statistically significant ( t=13.47, P<0.001). In the second stage, among the 104 patients, the proportion of those whose varicose veins disappeared or improved 6 months after the operation was 43.3%(45/104) and 51.0%(53/104), respectively. There were no patients with aggravation or rebleeding due to rupture. One year later, 8 patients (7.7%) had aggravated or ruptured esophageal and gastric varices with bleeding. Two years later, 12 patients (11.5%) had aggravated or bleeding. Conclusion:TIPS combined with main splenic artery embolization can effectively reduce the portal vein pressure in patients with portal hypertension upper gastrointestinal bleeding complicated with extensive PVT, improve the degree of esophageal and gastric varices, and reduce the risk of gastrointestinal bleeding.
3.Padi2 knockout exacerbates depressive-like behaviors in socially isolated mice

Objective To explore the impact of peptidylarginine deiminase 2(Padi2)-knockout on depressive-like behaviors in socially isolated mice.Methods Using CRISPR/Cas9 technology,a Padi2-knockout(Padi2-/-)mouse model with a C57BL/6J background was established,and the effect of Padi2 knockout was identified by genotyping and RT-qPCR detection.Six-week-old male Padi2-/- mice and wild-type C57BL/6J mice were selected and divided into normal rearing and social isolation groups,with 15 mice per group.The normal rearing group mice were housed with 5 mice per cage,and the social isolation group was housed with 1 mouse per cage,and weighed once a week.After 4 weeks,forced swimming and open field tests were conducted.After the behavioral experiments,brain tissues were taken from mice in each group,and changes in microglia in the brains were detected by immunofluorescence.Results We successfully established Padi2-/- mice.There was no difference in behavior between Padi2-/- mice and C57BL/6J mice in the normal rearing group.After social isolation,compared with C57BL/6J mice,Padi2-/- mice showed a significant increase in depressive symptoms,obvious weight gain,and a significant increase in the number of microglia in brain tissue.Conclusions Padi2 knockout exacerbated depressive-like behaviors and obesity in socially isolated mice,indicating that Padi2 is involved in the progression of depression and may be an effective target for the prevention and treatment of depression.
4.Contrast-enhanced ultrasound manifestations of nontumorous intrahepatic vascular shunts

Objective To observe contrast-enhanced ultrasound(CEUS)manifestations of nontumorous intrahepatic vascular shunts.Methods Data of 11 patients with intrahepatic vascular shunts were retrospectively analyzed,including 3 cases of intrahepatic arteriovenous shunts,3 cases of hepatic arterioportal shunts,1 case of intrahepatic arterioportal shunt complicated with intrahepatic arteriovenous shunt and 4 cases of intrahepatic portosystemic shunts.The conventional ultrasound and CEUS manifestations were observed.Results In 3 cases of intrahepatic arteriovenous shunts and 3 cases of hepatic arterioportal shunts,CEUS showed early enhancement of draining veins,early enhancement of local liver parenchyma without washout,while conventional ultrasound showed partial shunts in 2 cases with continuous enhancement showed on CEUS.The findings of 1 case of intrahepatic arterioportal shunt complicated with intrahepatic arteriovenous shunt were similar to the above two.Among 4 cases of intrahepatic portosystemic shunts,conventional ultrasound displayed all shunts,continuous enhancement of the draining hepatic vein and shunts were observed with CEUS,and transient low enhancement of local liver parenchyma were noticed in 2 cases.Conclusion Nontumorous intrahepatic vascular shunts could be divided into microscopic shunts that ultrasound could not directly display and shunts ultrasound could display.The former was characterized by early enhancement of the draining vein(portal vein or hepatic vein)on CEUS,some with early enhancement and no washout of local liver parenchyma.The latter was characterized by continuous enhancement of drainage vein and shunts,with or without transient low enhancement of local liver parenchyma.
5.From chronic pancreatitis to pancreatic cancer:Translational mechanisms,imaging assessments,preventation and treatment strategies

Pancreatic cancer is a highly lethal malignancy,among which pancreatic ductal adenocarcinoma is the most prevalent pathalogical type.The pathogenesis of pancreatic cancer associated with both genetic and non-genetic factors,but its precise etiology remained unclear.Chronic pancreatitis(CP)closely related with pancreatic cancer,which was a significant risk factor of the latter.The research progresses of mechanisms underlying the transformation from CP to pancreatic cancer,imaging assessments,along with relative preventation and treatment strategies were reviewed in this article.
6.Application and value of acoustic emission technique in joint surgery

BACKGROUND:Acoustic emission technology is one of the most reliable and perfect technologies in nondestructive testing,and is widely used in many fields such as mechanical,civil and underwater acoustics.In recent years,acoustic emission technology is initially applied in the field of biomedical engineering due to its great progress,especially in the friction evaluation of human joints and implant monitoring.OBJECTIVE:To summarize the application status,existing challenges,and potential directions of acoustic emission technology in joint surgery.METHODS:PubMed database,Web of Science database,CNKI database,and WanFang database were searched from January 1989 to March 2024 with the Chinese and English search terms"acoustics,hip replacement arthroplasties,hip prosthesis,prosthesis failure,knee osteoarthritides,knee joint,monitoring,artificial joint."A total of 2 991 articles were initially examined,and 80 articles were finally included for review analysis according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION:(1)In the field of joint surgery,acoustic emission technology is mainly used for earlier detection and diagnosis of joint-related diseases,as well as the detection of bone cracks during joint replacement surgery and postoperative prosthesis sinking,implant wear,aseptic loosening,and bone cracks.(2)The advantages of acoustic emission technology can make up for the shortcomings of traditional detection methods.It can detect arthritis early,diminish the risk of injury during surgery,monitor and reduce postoperative complications,and improve surgical safety and clinical prognosis.
7.Effects of typical persistentorganic pollutants on atherosclerosis

Cardiovascular disease is one of the serious health burdens in the world,and atherosclerosis is its impor-tant pathological basis.In recent years,the effect of environmental pollutants on human health has been paid more and more attention.Some new epidemiological studies and experiments have reported that environmental organic pollutants,involved in the occurrence and development of atherosclerosis,is an independent risk factor of cardiovascular diseases.Evidence shows that it is related to oxidative stress response,cell phenotype transformation,dyslipidemia,genetic changes and so on.This review summarizes available studies on the latest epidemiological studies and experiments of typical per-sistent organic pollutants such as polychlorinated biphenyls and per-and polyfluoroalkyl substances so as to provide refer-ences for future research.
8.Relationship between SIRT1 and tau protein during neuroinflammation induced by cardiopulmonary bypass in rats

Objective:To evaluate the relationship between NAD + -dependent protein deacylase sirtuin-1 (SIRT1) and tau protein during cardiopulmonary bypass (CPB)-induced neuroinflammation in rats. Method:Thirty-six healthy adult male Sprague-Dawley rats, weighing 250-300 g, were used in this study. Eighteen rats were divided into 3 groups ( n=6 each) using a random number table method: sham operation group (C1 group), CPB1 group and tau protein inhibitor methionine+ CPB group (methionine+ CPB group). Another 18 rats were divided into 3 groups ( n=6 each) using a random number table method: sham operation group (C2 group), CPB2 group and SIRT1 agonist SRT1720+ CPB group (SRT+ CPB group). Methionine+ CPB group received oral methionine 10.5 mg/kg once a day for 7 consecutive days. SRT1720 10 mg/kg was intraperitoneally injected once a day for 5 consecutive days in SRT+ CPB group. The model of CPB was developed after the end of drug administration. Rats were weaned off the bypass system after 1 h of circulatory support and sacrificed, and brain tissues were harvested for determination of the contents of tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6) and IL-4 (using enzyme-linked immunosorbent assay), expression of CD32, ARG1, SIRT1, tau protein and acetylated tau protein (ac-tau) (by Western blot), and expression of CD32, ARG1, SIRT1 and tau mRNA (by quantitative real-time polymerase chain reaction). Results:Compared with C1 group, the contents of TNF-α and IL-6 were significantly increased, the content of IL-4 was decreased, the expression of tau and CD32 protein and mRNA was up-regulated, and the expression of ARG1 protein and mRNA was down-regulated in CPB1 group ( P<0.05). Compared with CPB1 group, the contents of TNF-α and IL-6 were significantly decreased, the content of IL-4 was increased, the expression of tau and CD32 protein and mRNA was down-regulated, and the expression of ARG1 protein and mRNA was up-regulated in M+ CPB group ( P<0.05). Compared with C2 group, the contents of TNF-α and IL-6 were significantly increased, the content of IL-4 was decreased, the expression of ac-tau and tau and CD32 protein and mRNA was up-regulated, and the expression of SITR1 and ARG1 protein and mRNA was down-regulated in CPB2 group ( P<0.05). Compared with CPB2 group, the contents of TNF-α and IL-6 were significantly decreased, the content of IL-4 was increased, the expression of ac-tau and tau and CD32 protein and mRNA was down-regulated, and the expression of SITR1 and ARG1 protein and mRNA was up-regulated in SRT+ CPB group ( P<0.05). Conclusions:The down-regulated SIRT1 expression in brain tissues after cardiopulmonary bypass can inhibit deacetylation of tau protein, promote M1 polarization of microglia, inhibit M2 polarization, and ultimately induce neuroinflammation in rats.
9.Relationship between serum ferritin levels and red blood cell parameters with the risk of type 2 diabetes mellitus in medical examination individuals from Qingdao City

Objective:To investigate the association between serum ferritin levels, erythrocyte parameters, and the risk of developing type 2 diabetes mellitus.Methods:A total of 11 408 subjects aged 40 to 70 years who underwent physical examinations at a hospital in Qingdao from 2014 to 2015 were enrolled in the study. Data were collected through questionnaires, physical examinations, and assessments of blood biochemical indices, serum ferritin, and related erythrocyte parameters. A multivariate logistic regression model was employed to analyze the relationship of serum ferritin and red cell parameters with type 2 diabetes mellitus, while a linear regression model was utilized to examine the association between serum ferritin, red cell parameters and fasting blood glucose levels.Results:The median ( Q1, Q3) age of the subjects was 51 (45, 58) years, with 55.3% (6 305) being male. The prevalence of type 2 diabetes in women was 5.9% (300/5 103), which was lower than that in men [13.0% (817/6 305)] ( P<0.001).After adjusting for age, sex, body mass index (BMI), smoking status, alcohol consumption, and family history of diabetes, serum ferritin levels were found to be positively associated with both type 2 diabetes and fasting blood glucose levels, with OR (95% CI) and β(95% CI) values of 1.70 (1.37, 2.11) and 0.002 (0.002, 0.003), respectively. Additionally, hemoglobin, red blood cell count, and hematocrit were positively correlated with type 2 diabetes and fasting blood glucose, with OR (95% CI) values of 1.72 (1.32, 2.24), 1.91 (1.51, 2.41), and 1.52 (1.17, 1.97), and β (95% CI) values of 0.008 (0.006, 0.011), 0.365 (0.286, 0.445), and 2.543 (1.564, 3.521), respectively. Conversely, mean erythrocyte volume was negatively associated with type 2 diabetes and fasting blood glucose, with OR (95% CI) and β (95% CI) values of 0.54 (0.45, 0.66) and -0.017 (-0.023, -0.011), respectively (all P values<0.001). Conclusion:The findings indicate that serum ferritin and related erythrocyte parameters are significantly correlated with the risk of type 2 diabetes mellitus and fasting blood glucose levels.
10.Assessment of the predictive value of ultrasound imaging characteristics combined with clinical indicators for the prognosis of pancreatic ductal adenocarcinoma

Objective:To explore the value of ultrasound imaging characteristics combined with clinical indicators in assessing the prognosis of patients with pancreatic ductal adenocarcinoma (PDAC).Methods:A retrospective analysis was conducted for patients who underwent pancreatic contrast-enhanced ultrasound (CEUS) from September 2017 to October 2023 at Peking Union Medical College Hospital and were diagnosed with PDAC based on pathological findings. Various parameters were recorded, including CA19-9 levels, tumor size, location, morphologic features, echogenicity, presence of internal cystic components, dilatation of the main pancreatic duct, peripheral vascular invasion, CEUS characteristics, presence or absence of liver metastasis, and treatment methods. In April 2024, patient survival information was obtained through telephone follow-up or review of medical records. Based on the results of the cox regression model analysis, a nomogram model of the risk of death was developed. The receiver operating characteristic (ROC) curves were applied to evaluate the predictive efficacy of the model. The calibration curves were plotted to evaluate the accuracy of the model, and clinical decision curves were used to evaluate the clinical benefit of the model.Results:This study included a total of 207 patients with PDAC. As of April 2024, 71 patients were alive and 136 died, with a median survival time of 14 months (95% CI: 12 -17). Multivariate analysis confirmed that the elevated CA19-9 ( HR=1.689, 95% CI: 1.102-2.588), tumor size >4 cm ( HR=1.641, 95% CI: 1.159-2.322), taller-than-wide shapes ( HR=1.450, 95% CI: 1.019-2.065), incomplete hypo-enhancement ( HR=1.618, 95% CI: 1.100-2.380), and liver metastasis ( HR=1.687, 95% CI: 1.175-2.423) were independent risk factors for survival in patients with PDAC. A nomogram model was further constructed for 6-month, 12-month and 3-year survival of patients with PDAC. The areas under the ROC curve were 0.679, 0.705 and 0.815, respectively. The calibration curves suggested that the model was more accurate, and the clinical decision curves showed that the model had a better clinical benefit. Conclusion:The combined use of ultrasound imaging characteristics and clinical indicators could effectively predict the prognosis of PDAC patients. Specifically, tumor size >4 cm, taller-than-wide shapes, incomplete hypo-enhancement, elevated CA19-9, and the presence of liver metastasis are correlated with poorer survival outcomes. The nomogram model constructed on the basis of these factors can be used to assess the survival of patients with PDAC.
