1.Immunodynamic changes in a mouse model of malignant pleural effusion

Background:
Malignant pleural effusion (MPE), a common complication of advanced cancers, is associated with poor prognosis and reduced quality of life. Although host–tumor interactions are known to drive MPE development, the associated immune dynamics during disease progression remain unclear. Using a Lewis lung carcinoma-induced MPE model in C57BL/6JNidfc mice, we systematically evaluated general parameters and immune cell changes at two-day intervals throughout disease progression.
Results:
The day of Lewis lung carcinoma cell injection into the pleural space was designated as day 0. By day 10 post-injection (p.i.), MPE-bearing mice exhibited ~ 10% body weight loss, marking the experimental endpoint. Pleural tumor mass and pleural effusion volume were minimal up to day 4 p.i. but increased sharply from day 6 onward.CD45⁺ immune cell counts rose over time, and days 6, 8, and 10 p.i. marked key stages of MPE progression. On day 6, B cells, T cells, and natural killer cells, but not macrophages and neutrophils, increased significantly compared to earlier timepoints. By day 8, all immune cell subsets except T cells exceeded day 6 levels, and at day 10, natural killer cell numbers declined while others continued to increase. Besides, the numbers of CD8⁺ T cells, Th1 cells, regulatory T cells, and M2 macrophages progressively increased from day 6 to 10. Based on these data, days 6 and 10 were defined as early and advanced MPE stages, respectively, with distinct immune phenotypes. In advanced MPE, CD8⁺ T cells displayed reduced IFN-γ, TNF-α, Granzyme B, Perforin, FasL, and Ki-67, but upregulated PD-1 and CTLA-4 relative to early stage. Similarly, Th1 cells showed decreased IFN-γ, TNF-α, and IL-2 production along with reduced Ki-67 expression. Advanced-stage M2 macrophages exhibited lower MHC-II levels and impaired phagocytosis, but higher PD-L1 and IL-10 production, while neutrophils showed reduced TNF-α release and phagocytic activity.
Conclusions
Our findings characterize the temporal immune dynamics associated with MPE progression in a mouse model, revealing a transition from an early immunostimulatory state to a late immunosuppressive state. This study enhances our understanding of MPE immunopathogenesis and provides a foundation for developing precise, stagespecific therapeutic strategies.
2.A cross-sectional study on the screening-detected rate of scoliosis among primary and secondary school students in Jiangqiao Town in Jiading District of Shanghai in 2025

ObjectiveTo investigate the detection status of scoliosis and its influencing factors among primary and secondary school students in Jiangqiao Town in Jiading District of Shanghai, thereby providing evidence for the prevention and management of scoliosis in children and adolescents. MethodsA stratified cluster random sampling was conducted in March 2025. Three primary schools and two secondary schools in Jiangqiao Town were randomly selected; within each school, 3‒4 entire classes were randomly chosen. Screening was performed in accordance with the criteria specified in GB/T 16133‒2014 Screening for Abnormal Spinal Curvature in Children and Adolescents, and a self-designed structured questionnaire was administered to conduct face-to-face surveys (Cronbach’s α=0.730). ResultsA total of 772 valid questionnaires were collected on-site, representing a 100% response rate. The sample consisted of 413 males (53.50%) and 359 females (46.50%), including 385 primary school students (49.87%) and 387 secondary school students (50.13%). The detection rate of scoliosis in primary and secondary school students was 12.05%. The detection rates were 8.72% for males and 15.88% for females, with the female rate being significantly higher than the male rate (χ²=9.29, P=0.002). For primary and secondary school students, the detection rates were 6.75% and 17.31%, respectively, with the secondary school student rate being significantly higher (χ²=20.31, P<0.001). The results of univariate analysis revealed that students with sufficient sleep and standard sitting postures during reading/writing had a significantly lower scoliosis detection rate compared to those with insufficient sleep and non-standard sitting postures (χ²=5.90, P=0.015 and χ²=5.09, P=0.024). Multivariate logistic regression analyses revealed that the detection rate of scoliosis was significantly higher in females than in males (OR=1.846, 95%CI: 1.156‒2.984), and significantly higher in secondary school students than in primary school students (OR=2.722, 95%CI: 1.656‒4.472). ConclusionThe scoliosis detection rate among primary and secondary school students in Jiangqiao Town in Jiading District is relatively high. Females and secondary school students are key populations requiring targeted prevention and control efforts. It is imperative to strengthen health education on scoliosis, promote early screening and diagnosis, and implement timely targeted interventions.
3.Analysis of Medical Personnel's Satisfaction with Medical Service Prices and Influencing Factors

Objective To understand medical personnel's satisfaction with medical service prices and its influencing factors.Methods A random sampling method was employed to conduct a questionnaire survey among 1 863 medical personnel in public medical institutions in City D,Sichuan Province.Statistical descriptions of the collected key indicators were conducted.The survey results were analyzed using Chi-square test and binary logistic regression.Results The overall satisfaction of medical personnel with medical service prices was 33.3%,with a satisfaction rate of 56.9%for consultation category medical services prices and 42.8%for surgical category service prices.Key factors affecting satisfaction included understanding of medical service price policies,perceived importance of these policies,and patient feedback on unreasonable prices and price influence on medical behavior.Conclusion Medical staff exhibit low satisfaction with medical service prices and have insufficient awareness of related policies.Improvements in satisfaction can be achieved by enhancing medical staff's understanding and sense of identification with the policies,as well as through dynamic adjustments of prices for diagnostic and surgical services.
4.Epidemiological characteristics of dengue fever and molecular characteristics of indigenous epidemic strains in Fujian from 2019 to 2022

To study the epidemiological characteristics of dengue fever(DF)cases and the molecular characteristics of local dengue virus(DENV)in Fujian from 2019 to 2022,and provide evidence for the prevention and control of DF.Statistically analyzed the demographic and epidemiological characteristics of imported and local DF cases.The locally prevalent strains from various regions and some imported strains were sequenced to analyze homology and construct phylogenetic trees by using software MEGA(version 6.0).A total of 1 661 DF cases,including 699 imported cases and 962 local cases were reported in Fujian from 2019 to 2022.The imported cases were mainly concentrated from June to October,whereas the local cases were mainly concentrated from August to October.All 10 cities in Fujian had imported cases reported,and local cases had been reported in all regions except Longyan.The male to female ratio in the imported cases(4.07∶1)was higher than that in the local cases(0.95∶1),and the median age of the imported cases(43 years old)was lower than that of the local cases(46 years old).The imported cases mainly came from Cambodia locally prevalent strains of DENV1 all belonged to genotypeⅠ,which may originate from other provinces or Southeast Asian countries such as Cambodia.The first locally prevalent cases of DENV4 were found in Putian,and the phylogenetic trees of DENV4 E gene showed that the locally prevalent strains of DENV4 in Putian were genotypeⅡ,which may originate from Singapore.They face a high risk of local DF outbreaks caused by imported DF cases,and the local epidemic areas appear a certain degree of spreading trend.It is necessary to strengthen the multi-sectoral cooperation and improve supervision systems.Timely discovery and tracing of cases to provide a basis for scientific prevention and control of DF.
5.Regional nerve block versus general anaesthesia in elderly patients undergoing surgery for traumatic lower limb fractures

Objective:To compare the effects of regional nerve block and general anaesthesia in elderly patients undergoing traumatic lower limb fracture surgery.Methods:A total of 100 patients over 60 years old with traumatic lower extremity fracture who received surgical treatment in Yan′an People′s Hospital from October 2021 to October 2023 were prospectively selected as the study objects and divided into two groups with 50 cases in each group by random number table method. The control group was given general anesthesia, and the observation group was given regional nerve block. Both groups were given patient controlled intravenous analgesia (PCIA) after surgery. Perioperative hemodynamic indexes, surgical conditions, postoperative visual analogue scale (VAS) score, analgesia recovery rate and stress response indexes before and after surgery were compared between the two groups, and the incidence of complications were compared between the two groups.Results:The mean arterial pressure (MAP) and heart rate (HR) at the beginning of 5 min (T 1) and at the end of the operation (T 2) were decreased compared with the time point before entry (T 0), and there were no statistical differences of MAP, HR at 30 min after the operation (T 3) between the two groups ( P>0.05). The MAP and HR at T 1 and T 2 time points in the observation group were higher than those in the control group, there were statistical differences ( P<0.05). The amount of infusion in the observation group was lower than that in the control group: (443.63 ± 31.47) ml vs. (645.25 ± 45.85) ml, there was statistical difference ( P<0.01). There were no statistical differences in operation time and postoperative hospital stay between the two groups ( P>0.05). The VAS scores in the observation group at 6, 12, 24 and 48 h after surgery were lower than those in the control group: (3.21 ± 0.61) scores vs. (3.89 ± 0.74) scores, (3.45 ± 0.67) scores vs. (4.12 ± 0.78) scores, (2.57 ± 0.47) scores vs. (3.04 ± 0.64) scores, (1.96 ± 0.34) scores vs. (2.34 ± 0.45) scores; and the analgesic relief rate was lower than that in the control group: 8.00%(4/50) vs. 24.00%(12/50), there were statistical differences ( P<0.05). The levels of cortisol (Cor), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and β-endorpeptide (β-EP) at 24 h postoperative in the observation group were lower than those in the control group: (134.74 ± 28.94) ng/L vs. (158.52 ± 35.63) ng/L, (102.53 ± 15.41) μg/L vs. (128.95 ± 18.74) μg/L, (47.54 ± 5.72) ng/L vs. (59.74 ± 6.98) ng/L, (38.74 ± 4.17) μg/L vs. (45.75 ± 5.12) μg/L, there were statistical differences ( P<0.05). There was no statistical difference in complication rate between the two groups ( P>0.05). Conclusions:Regional nerve block can stabilize the intraoperative hemodynamics, reduce the stress response of the body and have a good analgesic effect in the operation of lower extremity fracture in the elderly.
6.Value of preoperative ultrasound combined with NLR,Ctn and CEA levels in predicting cervical lymph node metastasis of papillary thyroid carcinoma

Objective To investigate the predictive value of preoperative ultrasound in combination with neutrophil-to-lymphocyte ratio(NLR),calcitonin(Ctn),and carcinoembryonic antigen(CEA)for cervical lymph node metastasis in patients with papillary thyroid carcinoma.Methods A total of 103 patients diagnosed with papillary thyroid carcinoma(PTC)who were admitted to the hospital between October 2021 and October 2024 were selected as the case group.Among them,34 patients with cervical lymph node metastasis confirmed by surgical and pathological examination were assigned to the metastatic group,and 69 patients without cervical lymph node metas-tasis were assigned to the non-metastatic group.Additionally,103 patients with benign thyroid nodules admitted during the same period were enrolled as the control group.Clinical data,ultrasonographic features,and serum levels of NLR,Ctn,and CEA were compared between the metastatic and non-metastatic groups.The predictive value of ultrasonographic features and the combined detection of NLR,Ctn,and CEA levels for cervical lymph node metastasis in PTC was evaluated using receiver operating characteristic(ROC)curve analysis.Results Compared to the control group,the case group exhibited a higher proportion of patients with microcalcification and grade 3 blood flow,as well as elevated levels of NLR,Ctn,and CEA(P<0.05).Similarly,compared to the non-metastatic group,the metastatic group showed a higher proportion of patients with microcalcification and grade 3 blood flow,along with increased levels of NLR,Ctn,and CEA(P<0.05).The metastatic group tested positive,whereas the non-metastatic group tested negative.The area under the curve(AUC)for ultrasound features(micro-calcification,blood flow classification)combined with NLR,Ctn,and CEA levels in diagnosing cervical lymph node metastasis in papillary thyroid carcinoma was higher than that of individual indicators(P<0.05).Conclusions Preoperative ultrasound combined with the assessment of NLR,Ctn,and CEA levels demonstrates significant predictive value for cervical lymph node metastasis in papillary thyroid carcinoma.
7.Exploring the medication rules of GU Nai-fang,in the treatment of skin diseases based on the real world

Objective To organize and summarize the medication rules of GU Nai-fang in treating skin diseases through real-world data.Methods We collected traditional Chinese medicine prescriptions for GU Nai-fang's treatment of skin diseases from the outpatient medical record system of Shanghai Traditional Chinese Medicine Hospital to establish a database.Statistical analysis of disease types,performance,and efficacy was conducted,and association rules and systematic clustering analysis were performed using SPSS Modeler 18.0 and SPSS 26.0 software,respectively.Results A total of 5 020 patients were included,and 5 020 prescriptions were collected,involving 241 traditional Chinese medicines with a total frequency of 85 758 uses.The frequency of using heat clearing drugs,deficiency tonifying drugs,blood activating and stasis removing drugs,surface clearing drugs,and wind and dampness dispelling drugs was relatively high;most drugs tended to be cold and warm,mainly targeting the heart,lungs,and colon meridians.The top 15 Chinese medicines with the highest frequency of use were Smilacis Glabrae Rhixoma,Cortex Moutan,Radix Paeoniae Rubra,Rehmanniae Radix,Scutellariae Radix,Cynanchi Paniculati Radix et Rhizoma,Schisandrae Chinensis Fructus,Violsse Herba,Mume Fructus,Herba Pyrolae,Hedyotis Diffusae Herba,Lonicerae Japonicae Flos,Cicadae Periostracum,Bombyx Batryticatus,Radix Salviae.Association rule analysis obtained 15 high-frequency combinations of 2 traditional Chinese medicines and 3 traditional Chinese medicines.Cluster analysis resulted in 7 clustered prescriptions.Conclusion GU Nai-fang commonly used heat clearing drugs,deficiency tonifying drugs,blood activating and stasis removing drugs,surface resolving drugs,and wind and dampness dispelling drugs in the treatment of skin diseases,and Smilacis Glabrae Rhixoma,Cortex Moutan,Radix Paeoniae Rubra,Rehmanniae Radix,and Scutellariae Radix were the most frequently used drugs.

9.Incidence and influencing factors of frailty in elderly patients with hematologic malignancies: a meta-analysis

Objective:To systematically analyze the incidence and influencing factors of frailty in elderly patients with hematologic malignancies.Methods:Research on frailty in elderly patients with hematologic malignancies was retrieved from Chinese and English databases such as China National Knowledge Infrastructure, Wanfang Data, PubMed, and Web of Science. The search period was from database establishment to August 23, 2024. Two researchers screened the included studies, conducted quality assessment, and extracted data. Meta-analysis was conducted using Stata 18 and RevMan 5.4.Results:A total of seven studies were included, encompassing 19 076 elderly hematologic malignancy patients, with a frailty incidence of 59% [95% CI (0.48, 0.69) ]. Meta-analysis revealed that age [ MD=4.31, 95% CI (3.67, 4.96) ], gender [ OR=0.88, 95% CI (0.83, 0.93) ], alcohol consumption [ OR=1.67, 95% CI (1.15, 2.44) ], self-care ability [ MD=-1.79, 95% CI (-3.17, -0.41) ], anemia [ OR=6.67, 95% CI (2.94, 15.14) ], infection [ OR=1.81, 95% CI (1.16, 2.84) ], and neuropathy [ OR=2.52, 95% CI (1.38, 4.61) ] were the influencing factors of frailty in elderly patients with hematologic malignancies. Conclusions:The incidence of frailty is high in elderly patients with hematologic malignancies. Elderly patients with hematologic malignancies who are older, female, consume alcohol, have low self-care ability, anemia, infections, and neuropathy are prone to frailty. Healthcare providers can conduct early screening and intervention for high-risk populations of frailty based on risk factors to improve the quality of life for elderly hematologic malignancy patients.
