1.Phenotypic heterogeneity and management strategies for two brothers with XIAP deficiency syndrome.

OBJECTIVE:
To summarize the clinical features and management of two brothers affected with X-linked inhibitor of apoptosis protein (XIAP) deficiency.
METHODS:
This study retrospectively analyzed the clinical presentations, treatment, and follow-up of two brothers with XIAP deficiency diagnosed at Shanghai Children's Hospital in 2020, and summarized similar cases recorded in databases such as PubMed, Wanfang, Chinese Medical Association Journals, and WIP from January 2006 to November 2024. This study was approved by the Medical Ethics Committee of our hospital (Ethics No.: 2025R128-E01).
RESULTS:
Patient 1 was the younger brother, who presented at 8 years of age with growth retardation, folliculitis, erythema nodosum, and perineal abscess. Sequencing revealed that he has carried a hemizygous c.566T>C (p.Leu189Pro) variant of the XIAP gene, which was inherited from his mother. He was allergic to infliximab treatment and underwent allogeneic stem cell transplantation (HSCT) in January 2021. During a follow-up of 3 years and 10 months post-transplantation, he showed no gastrointestinal symptoms and had a good outcome. Patient 2 was the elder brother, who presented at 10 years and 6 months of age with growth retardation, rash, and anal fistula. Genetic testing revealed the same variant. He was treated with oral azathioprine but did not have regular follow-ups. At 14-years-and-6-months of age, he had developed severe gastrointestinal infection and hemophagocytic lymphohistiocytosis, which was alleviated after treatment with antibiotics, glucocorticoids, immunoglobulin, and rituximab. He is currently being prepared for HSCT. A total of 13 publications were retrieved, which involved 64 patients from 23 families, with 23 different variants identified. The main clinical manifestations included splenomegaly (34 cases, 53.1%), hemophagocytic lymphohistiocytosis (27 cases, 42.2%), and inflammatory bowel disease or colitis (20 cases, 31.8%). There were significant phenotypic differences among patients from the same family. Thirteen patients (20.3%) underwent HSCT, with a survival rate of 61.5%.
CONCLUSION
For male children with early onset, poor treatment response, especially those with unexplained splenomegaly and IBD-like symptoms, early genetic testing is recommended. HSCT is a safe and effective treatment for XIAP deficiency. For patients with developmental delay, early onset, and severe IBD phenotype, early transplantation is recommended.

2.Effects of sleep deprivation on lactic acid levels and cognitive function in mice

Objective:To explore the potential effects of sleep deprivation on lactic acid metabolism and cognitive function in mice.Methods:Twelve SPF-grade BALB/c mice were randomly divided into a normal control group and a sleep deprivation group using a random number table, with 6 mice in each group. The horizontal platform method was used to establish a mouse sleep deprivation model. The Morris water maze was employed to assess cognitive ability, including initial quadrant residence time, platform crossing times, escape latency, and total swimming distance. Serum lactic acid levels and inflammatory factors such as interleukin (IL)-1β, IL-6, and tumor necrosis factor (TNF)-α were also detected.Results:There was no significant difference in body weight between the two groups before and after sleep deprivation (all P>0.05). Compared with the control group, the sleep deprivation group showed prolonged residence time in the initial quadrant, significantly shorter residence time in the target quadrant, and fewer platform crossings (all P<0.05). Escape latency and total swimming distance in the sleep deprivation group were significantly longer than those in the control group (all P<0.01). Serum lactic acid levels and inflammatory factors IL-6, IL-1β, and TNF-α in the sleep deprivation group were significantly higher than those in the control group (all P<0.05). Conclusions:Sleep deprivation impairs cognitive ability in mice, accompanied by significant increases in lactic acid and inflammatory factors.
3.Effects of Wenyang Jiedu Tongluo Recipe on a mouse model of diabetic nephropathy based on macrophage recruitment and polarization

AIM To investigate the effects of Wenyang Jiedu Tongluo Recipe(WYJDTLR)on macrophage recruitment and polarization function in a mouse model of diabetic kidney disease(DKD).METHODS 50 db/db mice were randomly divided into the model group,the valsartan group(10.29 mg/kg)and the high-dose,medium-dose and low-dose WYJDTLR groups(26.52,13.26 and 6.63 g/kg),with 10 mice in each group,in contrast to another 10 db/m mice of the blank group.After 8 weeks of administration,the mice had their levels of fasting blood glucose,24-hour urinary protein quantity(24h-UTP),serum creatinine(Scr)and blood urea nitrogen(BUN)observed;their morphological changes of renal tissues observed by HE staining;their degree of renal glycogen deposition observed by PAS staining;their degree of renal fibrosis observed by Masson staining;their levels of MCP-1 and MCF-1 in serum and TNF-α and IL-1 β in renal tissue detected by ELISA;their renal protein expressions of VCAM-1 and ICAM-1 detected by IHC and Western blot;and their renal expressions of CD86 and CD206 detected by IF.RESULTS Compared with the model group,the WYJDTLR groups displayed decreased levels of fasting blood glucose,24h-UTP,Scr and BUN(P<0.05,P<0.01);improved degree of glomerular hypertrophy,mild proliferation of mesangial cells,dilatation of renal tubular,vacuolar degeneration of renal tubular epithelial cells,deposition of glomerular glycogen,and fibrosis of renal tissues(P<0.01);decreased levels of MCP-1 and MCF-1 in serum and TNF-α and IL-1β in renal tissue(P<0.05,P<0.01);decreased renal protein expressions of VCAM-1 and ICAM-1(P<0.05,P<0.01),thus reduced the recruitment of macrophages to the kidney;decreased renal CD86 protein expression(P<0.01);and increased CD206 protein expression(P<0.01),thus inhibited M1-type polarization of macrophages and promoted M2-type polarization of macrophages.CONCLUSION WYJDTLR can delay the DKD progression in mice by reducing the occurrence of inflammatory reactions through reducing the level of macrophage recruitment factor,inhibiting the M1-type polarization,and promoting the M2-type polarization.

5.In vitro fluorescent substrate assay for the activity of leucine aminopeptidase(LAP)in Echinococcus multilocularis

This study was aimed at developing an in vitro fluorescent substrate assay for the activity of leucyl aminopeptid-ase(LAP)from Echinococcus multilocularis and comparing it with the chemical chromogenic substrate enzyme activity assay.Through the establishment of reaction conditions for the fluorescent substrate-based in vitro enzyme activity assay,we com-pared the differences between the fluorescent substrate L-Leucine-7-amido-4-methylocoumarin(Leu-AMC)and the chemical chromogenic substrate L-Leucine-4-nitroanilide(Leu-pNA)through molecular docking,inhibition rates,and precision measures.Molecular docking revealed that the fluorescent substrate Leu-AMC had higher affinity for the protein than the chemical chromogenic substrate Leu-pNA.Through analysis of the effects of varying reaction conditions on fluorescence intensi-ty,we optimized the fluorescent substrate enzyme activity assay to demonstrate favorable performance at a reaction temperature of 37℃,a pH of 9.0,a protein concentration of 800 nmol/L,and a reaction duration of 60 minutes.Leu-AMC exhibited significant and distinct responses at a 5 μmol/L substrate concentration,under varying substrate conditions.The fluo-rescent substrate assay demonstrated more significant intergroup differences than the chemical chromogenic substrate assay when various inhibitors were added.This study established a fluorescence-based enzyme activity assay for leucyl aminopeptidase from Echinococcus multilocularis by using Leu-AMC as the substrate;this method demonstrated a more significant intergroup difference and sensitivity than the chemical chromogenic substrate assay.
6.Clinical efficacy of intracardiac echocardiography guidance in transcatheter closure of latent patent fora-men ovale among divers

Objective To investigate the clinical efficacy of intracardiac echocardiography(ICE)guid-ance in transcatheter closure of latent patent foramen ovale(PFO)among divers.Methods From Jan 2023 to Nov 2024,36 young divers with PFO,who only had right-to-left shunt during Valsalva maneuver on right heart contrast echocardiography,were enrolled from No.971 Hospital of the PLA Navy.They were divided into ICE group and transthoracic echocardiography(TTE)group based on intraoperative imaging.Clinical data including baseline characteristics,echocardiographic findings,complications,procedure time,and radiation dose were recorded.Results All 36 patients(19 in TTE group and 17 in ICE group)had 100%immediate closure success rate.No vascular complications,postoperative pericardial effusion,device displacement or embolism occurred.ICE could guide wire passage through foramen ovale,significantly reducing total procedure time,fluoroscopy time,and radia-tion dose(P<0.05).Conclusion ICE enables precise closure of potent PFO in divers and decreases fluoroscopy time and radiation dose,showing significant clinical value.
7.Construction of a standardized training system for research capacity of military general practitioners and its application effect

Objective To investigate the current status and experience of scientific research,attitude and demand for research training,and other research situations in military doctors with standardized training,to construct a targeted training system for research capacity of military general practitioners,and to explore the effect of the training system on the comprehensive research ability of military doctors.Methods Eighty-one military general practitioners who participated in the standardized training at The First Affiliated Hospital of Naval Medical University in 2022 were selected as research objects.A questionnaire-based survey was conducted among these doctors.SPSS 25.0 software was used to analyze the data obtained.The cultivation of research capacity included the training of basic research skills,advancement of reseach capacity,and the improvement of the assessment and incentive mechanism.Before and one year after the training,the research status,research attitude and demand,comprehensive research capacity,and mentor's evaluation for the research capacity were investigated.Results After one year of training,basic research skills and comprehensive research capacity of military general practitioners were significantly improved,mentor's evaluation was gradually improved,research achievements were significantly increased,training attitudes became more positive,and training needs changed.Conclusion The research capacity of military general practitioners has been significantly improved through the construction of training system of research capacity and continuous practice.

9.Application of Westgard Internal Quality Control Frequency on-line Calculator in Blood Cell Analyse Internal Quality Control

Objective To select the appropriate batch length and quality control procedure for the blood cell analysis project of the Women and Children's Hospital of Leshan with an online calculation tool of internal quality control frequency,and explored the measurement frequency of blood cell analysis laboratory quality control. Methods The coefficient of variation of blood cell analysis internal quality control of the Women and Children's Hospital of Leshan from April to December 2023 was collected as the imprecision of the project.The results of the laboratory participating in the 2023 National Health Commission inter-laboratory quality evaluation blood count experiment were used to evaluate the bias of each item. The allowable total error (TEa) for blood cell analysis items specified in WS/T 406-2012.Data were entered into the Westgard In-House Quality Control Frequency Calculator to calculate 10 corresponding batch lengths and different candidate statistical quality control (SQC) procedures online,including multi-rule,single-rule,and quality control concentration levels from 1 to 4.Combined with the actual situation of the laboratory,the SQC program with relatively simple quality control rules and relatively long batch length was selected. Results In blood cell analysis,13s,N=2 rule was used for WBC,HGB,MCV,MCH,MCHC,PLT,corresponding to the batch length of 1000. 13s/22s/R4s,N=2 rule was used for HCT,the corresponding batch length was 563,and the quality control frequency was 1 time per day. RBC was selected 13s/22s/R4s,N=2 rule,the corresponding batch length was 151,and the quality control frequency was 2 times a day or priority was given to improving the detection performance. Conclusion The Westgard internal quality control frequency calculator is an easy-to-use tool that allows select the right batch length and SQC program for lab,which can adjust the SQC protocol to needs.
10.Amplification effect of hearing mechanics in unilateral hearing loss.

Objective:To evaluate the effectiveness of amplification intervention with hearing aids for restoring binaural auditory function in patients with unilateral moderate to severe sensorineural hearing loss. Methods:This study selected 30 patients with normal hearing in one ear and moderate to severe sensorineural hearing loss in the other ear. They were fitted with hearing aids for the worse ear and underwent more than half a year and one year of adaptation training. The Chinese translation of the Twelve-item version of SSQ(C-SSQ12), angle identification test, speech recognition score(SRS) at different signal-to-noise ratios(SNR=5 and SNR=10) and audiometric thresholds were used to compare the results before and after hearing aid use to evaluate the effectiveness of the unilateral hearing loss intervention. Results:The results of the audiometric thresholds, C-SSQ12 scores, angle identification test, and SRS at SNR=5 and SNR=10 in the worse ear of the unilateral hearing loss patients after hearing aid use were all statistically significant compared to before hearing aid use(P<0.01). Conclusion:Amplification intervention with hearing aids has significant effects on restoring binaural auditory function in patients with unilateral moderate to severe sensorineural hearing loss.
