1.Comparative Study on Clinical Outcomes of Posterior Endoscopic Cervical Foraminotomy under Local Anesthesia with Conscious Sedation and General Anesthesia

Objective:
: Posterior endoscopic cervical foraminotomy (PECF) is a minimally invasive surgical technique for treating cervical radiculopathy. Traditionally, PECF is performed under general anesthesia in the prone position, but concerns over anesthesia-related complications have led to the exploration of local anesthesia in the lateral decubitus position as an alternative. This study aims to compare the clinical outcomes, safety, and efficacy of PECF performed under local anesthesia in the lateral decubitus position versus general anesthesia in the prone position.
Methods:
: We conducted a retrospective analysis of 13 patients who underwent PECF under local anesthesia in the lateral decubitus position. The outcomes were compared with data from 357 patients across eight studies who underwent PECF under general anesthesia in the prone position. Outcomes measures included Visual analog scale (VAS) pain scores, Oswestry disability index (ODI), length of stay (LOS), minimally clinically important difference (MCID), and complications.
Results:
: Patients in the local anesthesia group demonstrated significant reductions in neck pain (VAS-N : 4.93±1.32 to 1.49±0.52, p<0.001) and arm pain (VAS-A : 8.69±0.75 to 1.85±1.46, p<0.001), achieving a mean pain reduction of 78.8%. These improvements were comparable to the general anesthesia group (VAS-N : 4.80 to 1.28; VAS-A : 6.71 to 1.23). Functional outcomes improved significantly in both groups, with ODI scores improving from 54.76% to 9.82% locally and from 39.92% to 9.62% in the general group. Although LOS was slightly longer for the local anesthesia group (5.85±3.20 vs. 4.81±2.17 days, p=0.18), post-procedure monitoring time was significantly shorter (3.2 vs. 7.4 hours, p<0.001). The local anesthesia group reported zero complications (0%; 95% confidence interval [CI], 0–22.8%) compared to an 8.68% complication rate (95% CI, 5.8–11.6%) in the general anesthesia cohort (p=0.612).
Conclusion
: PECF under local anesthesia in the lateral decubitus position provides comparably effective pain relief and functional improvement comparable to general anesthesia, though the difference in complication rates was not statistically significant and requires larger studies for confirmation. This technique may be particularly advantageous for patients at higher risk for anesthesia-related complications. Further research is warranted to validate these findings in larger, prospective studies.

The Oriental stork (Ciconia boyciana), an endangered species, faces significant threats despite extensive conservation efforts aimed at their reintroduction and long-term survival in South Korea. Understanding the primary causes of mortality and identifying predominant pathological lesions are crucial for enhancing these efforts. This study aimed to investigate the causes of unexplained mortality in Oriental storks between 2019 and 2023 through comprehensive postmortem examinations. Twelve storks, including both wild and captive birds, classified as “unknown“ deaths, were examined. The results revealed that parasitic infections, particularly in wild storks, were frequently observed (41.7%). Additionally, gastrointestinal obstruction due to the ingestion of foreign materials was identified in a wild stork, highlighting the impact of environmental pollution. Despite the prevalence of inflammatory lesions in liver (58.3%), no cases of hepatitis were confirmed, except for one case of liver rupture in a wild stork. Notably, three nestling death cases from a single litter of captive storks suggested potential parental infanticide or lack of parental care. The overall results highlight the importance of addressing parasitic infections and environmental risks to improve the survival of wild Oriental storks, and emphasize the need for effective management in breeding facilities. Continuous monitoring and pathological evaluations for mortality cases are essential for understanding disease trends and developing adaptive conservation strategies to ensure the long-term preservation of this endangered species.
3.Bacterial profiling of head lice isolated from the Orang Asli: A first report in Malaysia

This study was carried out to determine from bacterial profiling to the bacterial
profiles of head lice among the Orang Asli communities. The head lice were collected from
Orang Asli community volunteers. The surface sterilized head lice pools were subjected to
genomic DNA extraction while next generation sequencing of the 16S rRNA gene was
performed using the Illumina MiSeq platform. Six female and three male head lice identified
as Pediculus humanus capitis were collected. A total of 111 368 number of NGS sequencing
reads were recorded while another 223 bacterial taxa sequences were obtained. Symbiotic
bacteria showed the highest number of reads, with Arsenophonus and Rhodococcus sequences
being the most abundant genera in the female and male samples, respectively. The female
head lice contained a more distinct microbial diversity. Amongst the pathogenic bacterial
species sequences noted were the methicillin-resistant Staphylococcus aureus, Streptobacillus
moniliformis, Haemophilus influenzae, Bordetella pertussis and Acinetobacter baumannii.
The 16S rRNA genome sequencing revealed a number of rare and pathogenic bacterial
species within the head lice of the Orang Asli. The socio-economic practices of the community
which involved forest foraging and hunting, and their poor living conditions potentially
facilitated the transmission of zoonotic bacterial pathogens, including those found within the
head lice. Hence, there is the possibility that the head lice could serve as vectors for the
transmission of pathogenic bacteria. This study highlighted the diverse microbial community
found within the head lice’s gut of the Orang Asli, with the detection of multiple rare and
pathogenic bacteria capable of causing severe infections.

To address the increasing need for detecting and validating protein biomarkers in clinical specimens, mass spectrometry (MS)-based targeted proteomic techniques, including the selected reaction monitoring (SRM), parallel reaction monitoring (PRM), and massively parallel data-independent acquisition (DIA), have been developed. For optimal performance, they require the fragment ion spectra of targeted peptides as prior knowledge. In this report, we describe a MS pipe-line and spectral resource to support targeted proteomics studies for human tissue samples. To build the spectral resource, we integrated common open-source MS computational tools to assemble a freely accessible computational workflow based on Docker. We then applied the workflow to gen-erate DPHL, a comprehensive DIA pan-human library, from 1096 data-dependent acquisition (DDA) MS raw files for 16 types of cancer samples. This extensive spectral resource was then applied to a proteomic study of 17 prostate cancer (PCa) patients. Thereafter, PRM validation was applied to a larger study of 57 PCa patients and the differential expression of three proteins in prostate tumor was validated. As a second application, the DPHL spectral resource was applied to a study consisting of plasma samples from 19 diffuse large B cell lymphoma (DLBCL) patients and 18 healthy control subjects. Differentially expressed proteins between DLBCL patients and healthy control subjects were detected by DIA-MS and confirmed by PRM. The Asia-Pacific Working Group on inflammatory bowel disease (IBD) was established in Cebu, Philippines, under the auspices of the Asian Pacific Association of Gastroenterology with the goal of improving IBD care in Asia. This consensus is carried out in collaboration with Asian Organization for Crohn's and Colitis. With biologic agents and biosimilars becoming more established, it is necessary to conduct a review on existing literature and establish a consensus on when and how to introduce biologic agents and biosimilars in the conjunction with conventional treatments for ulcerative colitis (UC) and Crohn's disease (CD) in Asia. These statements also address how pharmacogenetics influence the treatments of UC and CD and provide guidance on response monitoring and strategies to restore loss of response. Finally, the review includes statements on how to manage treatment alongside possible hepatitis B and tuberculosis infections, both common in Asia. These statements have been prepared and voted upon by members of IBD workgroup employing the modified Delphi process. These statements do not intend to be all-encompassing and future revisions are likely as new data continue to emerge.

6.An Efficient and Reliable Assay for Investigating the Effects of Hypoxia/Anoxia on Drosophila.

Stroke is a leading cause of death worldwide. Up to one thousand potential drugs or interventions have been developed to treat stroke, out of which ~160 have gone on to clinical trials. However, none of them has been successful. New insights into the molecular and cellular mechanisms of ischemia-induced injury are needed for discovering new therapeutic targets. Recently, Drosophila has been used to uncover new hypoxia-related genes. In this study, we describe an efficient and reliable assay with a sophisticated apparatus for studying the effects of oxygen deprivation on flies. Using this assay, wild-type flies were exposed to an anoxic environment for varying lengths of time, then the cumulative death rate and mobility recovery were systematically analyzed. We found that anoxia for over one hour caused lethality. The cumulative death rate on day 5 after anoxia was linearly and positively correlated with the duration of anoxia, and reached 50% when the duration was 2.5 h-3 h. We also found that the mobility recovery in normoxia was slow, as the climbing ability remained largely unchanged 4 h-6 h after 2.5-h of anoxia. We suggest that 2.5 h-3 h of anoxia and 4 h-6 h of recovery before mobility analysis are appropriate for future use of the anoxia assay.

8.Comparison between elderly inpatient fallers with and without dementia.

INTRODUCTIONThis study aimed to examine the various factors associated with inpatient falls among patients with and without dementia in a hospital setting.
METHODSThis was a retrospective one-year study using data collected from Singapore General Hospital's electronic reporting system for inpatient falls.
RESULTSIn the study period, 298 patients aged ≥ 65 years fell during their hospital stay. The majority of the patients (n = 248) did not have dementia. In our study, fallers with dementia were more likely to use ambulatory aids, be visually impaired and have urinary incontinence. More patients with dementia than those without had a history of previous falls, and were placed on fall precaution with restricted freedom of movement, which at times, included restraints. However, the difference between patients who were put on restraints and those who were allowed to move freely was not statistically significant. The majority of falls in both groups occurred at the bedside. We found that fallers without dementia were more likely to fall during the morning shift, whereas fallers with dementia were more likely to fall during the night shift. Fallers with dementia were more likely to be confused at the time of the fall.
CONCLUSIONIn our study, we found that fallers with dementia were more likely to have visual impairment, have urinary incontinence, use walking aids, and to be confused and physically restrained at the time of the fall. The fallers without dementia in our study may have undiagnosed dementia.

9.Sinonasal natural killer/T-cell lymphoma presenting as pyrexia of unknown origin with nasal symptoms.

A 68-year-old Chinese man presented with an eight-month history of pyrexia of unknown origin and chronic sinusitis despite multiple courses of antibiotics. He underwent extensive investigations, including workups for infections, chronic granulomatous diseases and malignancy. Nasal biopsies were performed twice under local anaesthesia, but did not show any evidence of malignancy. Eventually, the patient was diagnosed with natural killer (NK)/T-cell lymphoma, nasal variant, based on histopathological findings from harvested deep tissue obtained via functional endoscopic sinus surgery. This study highlights that, for patients presenting with pyrexia of unknown origin and nasal symptoms, NK/T-cell lymphoma must be considered as a differential diagnosis. Generous amounts of tissue should be harvested under general anaesthesia rather than limited tissue under local anaesthesia, in order to facilitate and ensure a definitive diagnosis.
