Objective:To explore the latent categories of sleep quality in shift nurses using latent profile analysis, and analyze the characteristics and influencing factors of each category.Methods:A convenience sampling method was used to select 1 480 shift nurses from three ClassⅢ Grade A hospitals in Fuyang city from January to February 2024. Data were collected using a general information questionnaire, Pittsburgh Sleep Quality Index, Perceived Stress Scale, and the Morningness-Eveningness Questionnaire-5. Latent profile analysis was performed to identify the latent categories of sleep quality, and multivariate logistic regression analysis was used to explore the influencing factors of different sleep quality categories. In this study, a total of 1 480 questionnaires were distributed. After excluding 28 invalid questionnaires, 1 452 valid questionnaires were retrieved, the effective recovery rate of the questionnaires was 98.11%.Results:The sleep quality of 1 452 shift nurses was classified into four latent categories: 44.5% (647/1 452) were in the "sleep impairment" category, 44.6% (648/1 452) in the "good sleep" category, 4.0% (58/1 452) in the "difficulty falling asleep-low efficiency" category, and 6.8% (99/1 452) in the "sleep disorder-medication-induced sleep" category. Multivariate logistic regression analysis revealed that perceived stress, monthly night shift frequency, sleep timing, daytime napping, and years of work experience were significant influencing factors for sleep quality ( P<0.05) . Conclusions:Sleep quality among shift nurses shows heterogeneity. Nursing managers should develop tailored management strategies and implement targeted sleep hygiene interventions based on nurses' sleep quality characteristics to improve their sleep quality.

Objective To find out the willingness to choose old-age care mode and the influencing factors of the elderly in the younger age group.Methods A multi-stage stratified whole cluster random sampling method was used to conduct a questionnaire survey on the willingness of the younger elderly in Zhangye City.Results The proportion of low-aged elderly in Zhangye City who chose family old-age care was the highest,and factors such as who they relied on to solve their old-age problems,opinions on moving into old-age care institutions,knowledge of the combination of medical care and nursing care,loneliness,and the number of chronic illnesses affected the willingness of low-aged elderly to live in old-age care.Conclusion In Zhangye City,family care is the main mode of old-age care for low-aged elderly.In the future,the relevant government departments should take into account the diversified needs of the elderly and the characteristics of the influencing factors,in order to realise the precise supply of the demand for elderly care services for the elderly.

Objective:To explore the effect of time-sensitive incentive nursing on psychological resilience, quality of life, nursing satisfaction and disease cognition of postoperative patients with chronic sinusitis (CS) .Methods:From January 2019 to July 2020, the convenience sampling method was used to select 60 postoperative patients with CS who were admitted to Fuyang Hospital of Anhui Medical University as the research objects. Patients were randomly divided into the observation group and the control group, with 30 cases in each group. The control group carried out conventional nursing, and the observation group was given conventional nursing combined with time-sensitive incentive nursing. The Connor-Davidson Resilience Scale (CD-RISC) , the Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36) dimensions of mental health, general health, physical functioning and role emotional were used to compare the psychological resilience and quality of life of the two groups of patients before and after nursing. Disease cognition and nursing satisfaction of the two groups of patients after nursing were compared.Results:The scores of the CD-RISC dimensions, disease cognition, four dimensions of the SF-36, and nursing satisfaction of patients in the observation group after nursing were higher than those of the control group, and the differences were statistically significant ( P<0.01) . Conclusions:The application of time-sensitive incentive nursing in postoperative patients with CS can improve their psychological resilience, disease cognition, quality of life and nursing satisfaction.

Objective: To analyze the epidemiological history, clinical manifestations, treatment and the short-term prognosis of 31 cases of 2019 novel coronavirus(2019-nCoV) infection in children from six provinces (autonomous region) in northern China. Methods: A retrospective analysis of the epidemiological history, clinical symptoms, signs, laboratory examinations, chest imaging, treatment and the short-term prognosis of 31 cases of 2019-nCoV was conducted. The patients were diagnosed between January 25th, 2020 and February 21st, 2020 in 21 hospitals in 17 cities of six provinces(autonomous region) of Shaanxi, Gansu, Ningxia, Hebei, Henan and Shandong. Results: The age of the 31 children with 2019-nCoV infection was 7 years and 1 month (6 months -17 years). Nine cases (29%) were imported cases. Other 21 cases (68%) had contact with confirmed infected adults. One case (3%) had contact with asymptomatic returnees from Wuhan. Among the 31 children, 28 patients (90%) were family cluster cases. The clinical types were asymptomatic type in 4 cases (13%), mild type in 13 cases (42%), and common type in 14 cases (45%). No severe or critical type existed. The most common symptom was fever (n=20, 65%), including 1 case of high fever, 9 cases of moderate fever, 10 cases of low fever. Fever lasted from 1 day to 9 days. The fever of fifteen cases lasted for ≤3 d, while in other 5 cases lasted > 3 d. Other symptoms included cough (n=14, 45%), fatigue (n=3, 10%) and diarrhea (n=3, 9%). Pharyngalgia, runny nose, dizziness, headache and vomiting were rare. In the early stage, the total leukocytes count in peripheral blood decreased in 2 cases (6%), the lymphocytes count decreased in 2 cases (6%), and the platelet count increased in 2 cases (6%).Elevation of C-reactive protein (10%, 3/30), erythrocyte sedimentation rate(19%,4/21), procalcitonin(4%,1/28), liver enzyme(22%, 6/27) and muscle enzyme (15%, 4/27) occurred in different proportions. Renal function and blood glucose were normal. There were abnormal chest CT changes in 14 cases, including 9 cases with patchy ground glass opacities and nodules, mostly located in the lower lobe of both lungs near the pleural area. After receiving supportive treatment, the viral nucleic acid turned negative in 25 cases within 7-23 days. Among them, 24 children (77%) recovered and were discharged from hospital. No death occurred. Conclusions In this case series, 2019-nCoV infections in children from six provinces (autonomous region) in northern China are mainly caused by close family contact. Clinical types are asymptomatic, mild and common types. Clinical manifestations and laboratory examination results are nonspecific. Close contact history of epidemiology, nucleic acid detection and chest imaging are important bases for diagnosis. After general treatment, the short-term prognosis is good.

Objective To analyze the trend in caesarean section rate and puerpera characteristics in hospital,and provide valuable information for maternal and child health policy making and clinical practice.Methods A total of 12 041 women who delivered in the affiliated Chaohu Hospital of Anhui Medical University from October 1,2010 to September 30,2016 were selected.Based on Robson classification system,changes in the rate of caesarean delivery as well as its relationship with two-child policy and infant sex ratio were analyzed.Results The overall caesarean section rate gradually decreased from 66.9％ to 44.2％ during the past six years.Respectively,the caesarean section rate in primiparae with singleton term babies decreased to 32.1％ and the rate in multiparas without uterine scar decreased to 14.2％,and the rate in premature delivery decreased to 22.9％,the differences were significant (P＜0.01).The proportion of vaginal delivery (R1,R3),multiparas with uterine scar (R5) and twins pregnancy (R8) increased,the differences were significant (P＜0.01).The annual overall newly-born sex ratio ranged from 110:100 to 128:100.In group R1,more babies were girls,the proportion was stable,more women with premature delivery and multiparas had boy babies,but the boy babies by multiparas without uterine scar obviously decreased in the last 2 years.Conclusions Primiparae with singleton head birth,multipara without uterine scar and women with premature deliveries are the key population in the effort of reduction of caesarean section rate.The caesarean section rate and proportion were unstable in multiparas with uterine scar,breech deliveries and twin deliveries.The application of Robson classification system can improve the comparability of the surveillance data.

Objective To study the evolutionary and mutant characteristics of Ebora virus envelope glycoprotein.Methods A total of 100 Ebora virus envelope glycoproteins amino acid sequences isolated during 1976 and 2014 were collected from National Center for Biotechnology Information (NCBI).Multiple sequence alignment and phylogenetic tree analysis were performed to investigate the evolutionary and mutant characteristics of Ebora virus envelope glycoprotein.Results Glycoprotein amino acid sequences of Ebora virus isolated during 1976 and 2014 showed only 54.00％-65.00％ homology among different subtypes,while 95.00％-100.00％ homology in same subtypes.Ebola virus isolated from different regions in 2014 showed a 99.70％-100.00％ homology of glycoprotein amino acid sequences in the same subtype.The homology of glycoprotein amino acid sequences of Ebola-Zaire virus isolated from Sierra Leone in 2014 was 100.00％,but three strains of Ebola-Zaire virus isolated from Guinea showed diversity in glycoprotein amino acid sequences.Glycoprotein amino acid sequences of Ebola virus with different subtypes were on different branches of phylogenetic tree.Glycoprotein amino acid sequences of Ebola-Zaire virus isolated from Sierra Leone in 2014 were on one branch,and those of Ebola-Zaire virus isolated from other countries during 1976 and 2014 were on the another branch.Conclusions Glycoprotein amino acid sequences of Ebora virus vary with time and region.Ebola-Zaire virus isolated from different regions in 2014 may be two variants with the same origin,and hybrid phenomenon is not observed among virus of different subtypes.

OBJECTIVETo optimize the methods for genetic detection and prenatal diagnosis of Duchenne muscular dystrophy (DMD).

METHODSDenaturing high-performance liquid chromatography (DHPLC), multiplex PCR (mPCR), sequencing and other molecular techniques were used in combination for molecular diagnosis of 8 cases diagnosed as DMD.

RESULTSAmong the 8 cases, 4 have carried large deletions, 3 have point mutations, among which 6 were of de novo type. Prenatal diagnosis were offered for 5 families, the results showed that none of the fetuses had carried large deletions or point mutations. The pregnancies had continued and healthy babies were born.

CONCLUSIONCombined use of short tandem repeat, DHPLC, mPCR and sequencing can improve the detection of DMD gene mutations. By establishing and optimizing genetic and prenatal diagnostic methods, accurate genetic counseling can be provided for families affected with DMD.

OBJECTIVETo investigate the effects of prednisone on the expressions of FAK and Pyk2 in the kidneys of rats with adriamycin-induced nephritis.

METHODSThirty SD rats were randomized into normal control group, adriamycin-induced nephritic model group, and prednisone treatment group (n=10). Prednisone was administered at 10 mg/kg once daily in nephritic rats starting since the 7th day after adriamycin injection. Twenty-four-hour proteinuria was measured in the rats at different time points, and renal tissue histology was examined using transmission electron microscope. The expression levels of Pyk2, FAK and nephrin mRNA in the renal tissue were detected tested by RT-PCR, and the protein expressions of FAK, Pyk2, phosphorylated Pyk2 and phosphorylated FAK-Tyr397 were detected by Western blotting; immunohistochemistry was used for detecting nephrin protein expression in the kidney.

RESULTSCompared with the normal control group, the rats with adriamycin-induced nephritis showed significantly increased proteinuria (P<0.01), which was obviously lowered by prednisone treatment (P<0.01). Transmission electron microscopy revealed extensive fusion of the foot processes of the podocytes in the model group. Prednisone treatment promoted nephrin expression in the kidney (P<0.05). Compared with the control group, the model and prednisone treated groups showed significantly lowered nephrin mRNA expression (P<0.01) but increased FAK mRNA expression (P<0.01), but prednisone-treated group had a higher nephrin mRNA expression than the model group (P<0.05). The model group exhibited significantly increased expressions of FAK total and phosphorylated proteins, P-FAK/FAK, and P-Pyk2/Pyk2 (P<0.01), which were all lowered in the treatment group (P<0.01). Correlation analysis suggested that the expressions of FAK mRNA, FAK, pFAK, Pyk2 mRNA and pPyk2/Pyk2 were positively correlated with proteinuria (r=0.819, 0.750, 0.838, 0.762, 0.934, respectively, P<0.01).

CONCLUSIONSAdriamycin increases phosphorylated FAK and Pyk2 expressions to mediate kidney injury in rats. Prednisone inhibits Pyk2 and FAK activation, decreases proteinuria, and alleviates podocyte lesions to protect the glomerular filtration barrier.

Objective To investigate the significance of Th 17 cells, Treg cells and Th17/Treg cell-associated cytokines in the development of tuberculous pleurisy through detecting the expressions of Th 17 cells and Treg cells (CD4+CD25+Foxp3+) in CD4+T cells, analyzing concentrations of IL-17, IL-23, IL-6 and TGF-βin serum of patients with tuberculous pleurisy and healthy controls and measuring levels of IL -17, IL-23, IL-6 and TGF-βin hydrothorax of patients with tuberculous pleurisy .Methods Flow cytometry was used to detect expressions of Th 17 and Treg cells in peripheral blood of patients with tuberculous pleurisy and healthy controls.ELISA method was performed for quantitative detection of concentrations of IL -17, IL-23, IL-6 and TGF-βin serum and hydrothorax .Differences and correlations between the above measured data were analyzed by the statistical software SPSS 17.0.Results Compared with the healthy control group , the expressions of Th17 cells in peripheral blood of the patients were significantly increased (1.02%±0.20%vs.0.89%±0.13%, P=0.002<0.05), while the expressions of Treg cells were significantly decreased (4.64%±0.77%vs.5.10%±0.90%, P=0.000<0.05).Correspondingly, the ratio of Th17/Treg cells (0.25±0.07) in the patients were significantly higher than that in the healthy controls (0.17±0.05, P=0.000<0.05).Concentrations of IL-17, IL-23 and IL-6 in peripheral blood and in hydrothorax of the pa-tients were (17.49±3.94) ng/L, (90.42±23.06) ng/L, (4.54±1.02) ng/L and (26.13±5.98) ng/L, (122.26±31.71) ng/L, (5.31±0.74) ng/L respectively, which were remarkably higher than the levels of IL-17 (14.45±3.81) ng/L, IL-23 (77.55±20.26) ng/L and IL-6 (4.26±0.91) ng/L in control group. In tuberculous pleurisy group , concentrations of IL-17, IL-23 and IL-6 in hydrothorax were significantly higher than those in peripheral blood with P values of 0.000, 0.000 and 0.003.There was no difference be-tween IL-6 levels in peripheral blood from patients and IL-6 levels in peripheral blood from the healthy con-trols, P=0.274.In comparison with the control group , TGF-βlevels in peripheral blood and in hydrothorax of the patients were significantly decreased (3.95 ng/L±0.79 ng/L, 3.12 ng/L±0.77 ng/L vs.3.32 ng/L ±0.80 ng/L) .In tuberculous pleurisy patients , the expression of Th17 cells in peripheral blood was nega-tively correlated with Treg cells in peripheral blood (r=-0.684, P=0.000<0.05), but was positively relat-ed to the levels of IL-17, IL-23 and IL-6 (r=0.479, 0.441, 0.326, P=0.013, 0.015, 0.017).TGF-βlevel had significantly positive correlations with Treg cells in the peripheral blood of the patients (r=0.297, P=0.024), but no significant correlation with Th17 cells was found (r=0.091, P=0.659).Conclusion Th17/Treg cell-associated cytokines might regulate the expressions of Th 17 and Treg cells and inflammatory reaction.Changes of Th17 cells, Treg cells and related cytokines might be important immunopathological mechanisms for tuberculous pleurisy .
