1.A Single-Arm Clinical Trial of Fuyuan Tiansui Decoction in the Treatment of Cerebellar Ataxia Syndrome

[Objective]To observe the efficacy of Fuyuan Tiansui Decoction in the treatment of cerebellar ataxia syndrome(CAS).[Methods]A total of 29 patients with CAS treated at Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine outpatient clinics from February to October 2024 were enrolled.All participants received Fuyuan Tiansui Decoction treatment for 12 weeks.Before and after treatment,the changes of International Cooperative Ataxia Scale(ICARS),Scale for the Assessment and Rating of Ataxia(SARA),Activities of Daily Living(ADL)score and traditional Chinese medicine(TCM)syndrome scale were compared.[Results]A total of 29 patients were enrolled,with 27 completing the 12-week treatment course(2 withdrawals).Post-treatment analysis revealed an ICARS score improvement rate of 55.6%and a stabilization rate of 25.9%.The ICARS score was 35.85±12.90 before treatment and 34.33±13.10 after treatment(P<0.05).The SARA score and TCM syndrome scale score were significantly decreased(P<0.05).During the treatment,the adverse reaction rate was 11.1%,including 3 cases of diarrhea.[Conclusion]Fuyuan Tiansui Decoction can relieve the ataxia symptoms and the syndrome of kidney deficiency and marrow deficiency in patients with CAS.
2.Analysis of death characteristics and disease burden of lung cancer in China from 2004 to 2021

Objective:To explore the death characteristics and disease burden of lung cancer in Chinese population from 2004 to 2021.Methods:A cross sectional study was conducted. The demographic data and lung cancer mortality surveillance data from 605 surveillance areas in the Chinese mortality surveillance dataset from 2004 to 2021 were used to calculate the crude death rate (CDR), potential years of life lost, average potential years of life lost, work years of potential life lost, and average work years of potential life lost for lung cancer in the population. Joinpoint model was used, and the trend of lung cancer mortality rates in different characteristic populations in the surveillance area was analyzed using average annual percentage change (AAPC) and annual percentage change (APC).Results:The CDR of lung cancer in the surveillance population of China from 2004 to 2021 was 42.94/100 000, with males having a higher CDR than females (58.94/100 000 vs. 26.92/100 000); the overall population, male and female lung cancer CDR showed an upward trend (AAPC were 2.70%, 3.20% and 1.85%, respectively), with a decreasing trend in the total population in 2004 and 2005 (APC = -1.04%), an increasing trend from 2006 to 2014 (APC = 4.46%), and an increasing trend from 2015 to 2021 (APC = 1.41%); the overall increase rate of lung cancer CDR in rural registered residence population (AAPC = 3.70%) was higher than that in urban registered residence population (AAPC = 1.30%). From 2004 to 2021, the age specific lung cancer CDR of the population in surveillance areas of China increased with age, reaching a peak of 437.77/100 000 in the population aged 85 years and above. From 2004 to 2021, the potential life loss caused by lung cancer in surveillance areas of China was 8 532 790.08 person years, with an average potential life loss of 6.55 years. Among them, females had a higher potential life loss than males (8.70 years vs. 4.30 years); the average potential work loss caused by lung cancer in males was 7.02 years, while in females it was 6.76 years. Conclusions:From 2004 to 2021, the overall mortality rate of lung cancer in China shows an upward trend, and the burden of lung cancer disease is relatively heavy. Male and elderly populations need to be given special attention.

5.Investigation on the Mechanism of Guizhi Fuling Pill in Treating Encephalopathy with"Treating Different Diseases with Same Method"Based on Network Pharmacology and Molecular Docking

[Objective]To explore the mechanism of Guizhi Fuling Pill's treating ischemic stroke(IS),subarachnoid hemorrhage(SAH),mild cognitive impairment(MCI),insomnia disorder(ID),epilepsy(EP)and vascular headaches through"treating different diseases with the same method",by using network pharmacology and molecular docking.[Methods]The active components and targets of Guizhi Fuling Pill were screened out and obtained from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP),Swiss Target Prediction database and PubChem platform.The targets of disease were searched from DrugBank,Disease Gene Network(DisGeNET),Genome Annotation Database Platform(GeneCards)and Online Mendelian Inheritance in Man(OMIM).Then the common targets of drug-disease were obtained,Cytoscape 3.9.1 software was used to construct a"drug-disease-component-target"network and protein-protein interaction(PPI)network.Gene ontology(GO)function enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis of common targets were carried out by Metascape database.AutoDock Vina software was used to verify the molecular docking of core components and key targets.[Results]After screening and deleting duplicates,155 core active ingredients of Guizhi Fuling Pill,672 related targets,196 disease targets and 78 drug-disease common targets were obtained.There were 5 core active ingredients in Guizhi Fuling Pill's"treating different diseases with the same method"for encephalopathy,which were paeonol,quercetin,paeoniflorin,cinnamaldehyde and amygdalin.There were 5 key targets obtained by PPI screening which were interleukin-6(IL6),tumor necrosis factor(TNF),insulin(INS),RAC-alpha serine/threonine-protein kinase(AKT1),interleukin-1 beta(IL1B).KEGG pathway enrichment analysis showed that the key pathways were lipid and atherosclerosis,fluid shear stress and atherosclerosis,pathways in cancer,advanced glycation end products-receptor of advanced glycation end products(AGEs-RAGE)signaling pathway in diabetic complications,Alzheimer's disease and pathways of neurodegeneration-multiple diseases.The molecular docking results showed that the key targets had good binding activity to the core active components.[Conclusion]The possible mechanism of multi-component,multi-target and multi-pathway of Guizhi Fuling Pill's treating encephalopathy through"treating different diseases with the same method"was expounded,which provided a theoretical basis and reference for further understanding the mechanism of Guizhi Fuling Pill and exploring new clinical application and experimental exploration.
6.Electroacupuncture with different waveforms for primary dysmenorrhea: A randomized controlled trial

Objective:
To observe and compare the clinical effects of different electroacupuncture waveforms on primary dysmenorrhea.
Methods:
This was a prospective, randomized, three-group, parallel-controlled trial. Participants with primary dysmenorrhea were randomly divided into dense-sparse wave, continuous wave, and discontinuous wave groups in a 1:1:1 ratio. Two lateral Ciliao (BL 32) points were used. All three groups started treatment 3–5 days before menstruation, once a day for six sessions per course of treatment, one course of treatment per menstrual cycle, and three menstrual cycles. The primary outcome measure was the proportion with an average visual analog scale (VAS) score reduction of ≥50% from baseline for dysmenorrhea in the third menstrual cycle during treatment. The secondary outcome measures included changes in dysmenorrhea VAS scores, Cox Menstrual Symptom Scale scores and the proportion of patients taking analgesic drugs.
Results:
The proportion of cases where the average VAS score for dysmenorrhea decreased by ≥50% from baseline in the third menstrual cycle was not statistically significant (P > .05). Precisely 30 min after acupuncture and regarding immediate analgesia on the most severe day of dysmenorrhea, there was a statistically significant difference in the dense-sparse wave group compared with the other two groups during the third menstrual cycle (P < .05). Additionally, there was a statistically significant difference between the dense-sparse wave and discontinuous wave groups 24 h after acupuncture (P < .05).
Conclusions
Waveform electroacupuncture can alleviate primary dysmenorrhea and its related symptoms in patients. The three groups showed similar results in terms of short- and long-term analgesic efficacy and a reduction in the number of patients taking analgesic drugs. Regarding achieving immediate analgesia, the dense-sparse wave group was slightly better than the other two groups.
7.Association between dietary retinol and prognosis of patients with esophageal squamous cell carcinoma

ObjectiveTo explore the association between dietary retinol intake and prognosis of patients with esophageal squamous cell carcinoma. MethodsThe study enrolled 388 cases with primary esophageal squamous cell carcinoma that was pathologically diagnosed in the first affiliated hospital of Fujian medical university and the cancer hospital of Fujian medical university from July 2014 to August 2019. Chi-square test was used to determine the relationship between retinol and patients' characteristics. Kruskal-Wallis rank sum test was used to analyze the relationship between retinol and multiple nutrients. Cox proportional hazards regression model was used to explore the association between dietary retinol and esophageal squamous cell carcinoma. ResultsPatients with esophageal squamous cell carcinoma did not significantly differ in gender, age, TNM stage, tumor length or nutrient intake by different retinol intakes (P>0.05). Multivariate Cox regression analysis showed that the high retinol intake group had a better prognosis (overal survial:HR=0.279,95%CI:0.150‒0.520,P<0.001;disease-free survival:HR=0.306,95%CI:0.181‒0.516,P<0.001). ConclusionHigh dietary retinol intake may improve the prognosis of patients with esophageal squamous cell carcinoma.
8.Evaluation of corneal nerve damage in diabetic patients after panretinal photocoagulation based on the wide-field mosaic analysis of corneal subbasal nerve plexus

Objective:To explore the damage of panretinal photocoagulation (PRP) to the subbasal nerve plexus (SNP) and its related mechanisms by comparing SNP changes in wide-field mosaic between before and after PRP treatment in diabetic patients.Methods:A randomized controlled study was conducted.Fifty-seven patients (114 eyes) with type 2 diabetes mellitus and binocular diabetic retinopathy (DR) stage IV to receive PRP treatment in Shanxi Eye Hospital from April to November 2019 were enrolled.The subjects were randomly divided into horizontal-vertical laser group and vertical-horizontal laser group according to a random number table.Twenty-nine eyes from 29 patients were assigned to the horizontal-vertical laser group with the photocoagulation sequence of temporal-nasal-inferior-superior.Twenty-eight eyes from 28 patients were assigned to the vertical-horizontal laser group with the photocoagulation sequence of inferior-superior-temporal-nasal.The severer eyes of each subject were chosen as the treatment eye and the contralateral eyes were chosen as the control eye.Corneal confocal laser scanning microscopy (CCM) was performed before PRP treatment, 1 week after each photocoagulation, and 1 month after the completion of PRP treatment to collect images of the SNP over an area of 2-3 mm around the whorl-like pattern.Captured images at each time were merged into one image by using the Photoshop CC 2017 image processing software, and then the nerve fiber length (NFL) of whorl-like pattern was measured by Neuron J image analysis software.McGill pain questionnaire was used to investigate the pain of patients after each photocoagulation.The NFL changes of SNP at different time points were compared between different eyes and different photocoagulation sequence groups.The study protocol adhered to the Declaration of Helsinki and was approved by an Ethics Committee of Shanxi Eye Hospital (No.201804b). Written informed consent was obtained from each patient prior to entering the study cohort.Results:After PRP treatment, there were different degrees of neural structure loss of SNP nerve fibers in 11 treatment eyes, but there was no significant change in SNP nerve fibers in the control eyes.There were significant differences in NFL between the treatment eyes and the control eyes at various time points ( Feyes=2.020, P=0.039; Ftime=4.062, P=0.001). In the horizontal-vertical laser group, different degrees of neural structure loss on the photocoagulation side were found in SNP nerve fibers after the first and second photocoagulation.In the vertical-horizontal laser group, different degrees of neural structure loss on the photocoagulation side were found in SNP nerve fibers after the third and fourth photocoagulation.There was no significant difference in NFL of treatment eyes between the two groups ( Fgroup=0.099, P=0.754), but there was a significant difference in NFL at various time points before and after treatment ( Ftime=5.231, P<0.001). There were 9 (9/57) patients who complained of pain after PRP, which occurred at the first time of photocoagulation in 7 of them. Conclusions:SNP damage may occur after PRP in patients with DR, and SNP is prone to be damaged on the photocoagulation side when performing horizontal photocoagulation.

Objective:
The role of planned neoadjuvant radiotherapy or chemoradiotherapy in the non-radical resection of esophageal squamous cell carcinoma was unclear. The study aimed to evaluate their therapeutic effect and analyze the prognostic factors.
Methods:
We retrospectively analyzed the clinical data of locally advanced esophageal squamous cell carcinoma who received neoadjuvant radio therapy (33 patients) and concurrent chemoradiotherapy (119 patients) from January 2004 to December 2016 in our single-institution database.The survival rates were calculated by Kaplan-Meier method. The prognostic factors were analyzed by using Log rank test and Cox proportional hazards model.
Results:
The median follow-up was 29.8 months. One hundred and one patients survived more than 3 years. The rates of overall survival (OS) and disease-free survival (DFS) at 3 years were 63.9% and 55.6%, respectively.The rates of complete, partial and minimal pathological response of the primary tumor were 50.3%, 38.4%, 11.3%, the corresponding 3-year OS were 75.5%, 57.4%, 27.3% (
10.The value of DWI combined with DCE in the differential diagnosis of orbital lymphoma and inflammatory pseudotumor

Objective To evaluate the value of MR DWI combined with dynamic contras-t enhanced (DCE)scanning in the differential diagnosis of orbital lymphoma and inflammatory pseudotumor.Methods The clinical and imaging data of 1 5 cases with orbital lymphoma and 10 cases with inflammatory pseudotumor confirmed by surgery and pathology were analyzed retrospectively.25 patients were performed with conventional MR scanning,DWI scanning and DCE scanning.ADW4.7 software was used for DWI images and DCE images pos-t processing for analyzing the two groups of lesions in the TIC subtyping.Independent sample t test statistical analysis was used for comparing two groups of ADC values and Tmax values.The ROC curve was used for determining the statistically significant differences in measurement data of the threshold.With pathologic and immunohistochemical results as the gold standard,the accuracy,sensitivity and specificity of DWI combined with DCE in the diagnosis of orbital lymphoma and inflammatory pseudotumor were further detected.Results Among the 1 5 patients with orbital lymphoma,8 cases (8/15)occurred in the lacrimal gland area,5 cases (5/1 5)occurred in the outer cone space and 2 cases (2/15)occurred in the eyelid.Among the 10 patients with inflammatory pseudotumor,6 cases (6/10)occurred in the lacrimal gland and 4 cases (4/10)in the bulb.Among the 25 patients,9 of them had the TIC curve of outflow,6 of them had the TIC curve of platform and 10 of them had the TIC curve of inflow.The ADC value of orbital lymphoma was (0.573 ±0.1 1 6)×10-3 mm2/s,and the Tmax value was (248.2±1 59.4)ms.The ADC value of inflammatory pseudotumor was (1.186±0.244)×10-3 mm2/s,and the Tmax value was (5 5 7.6 ±98.7)ms.With the ADC=0.885×10-3 mm2/s for the threshold,the AUC was 0.9 3 6 ,with diagnostic accuracy of 9 1.3%, the sensitivity of 90.1% and specificity of 80%.With the Tmax=481.2 ms as the threshold,the AUC was 0.927,with the diagnosis accuracy rate of 88%,the sensitivity of 80%,and the specificity of 86.7%. Finally,DWI combined with DCE could reach the AUC 0.945, diagnostic accuracy of 89.5%,sensitivity of 93.3%,and specificity of 82.6%.Conclusion Conventional MRI scan combining DWI with DCE could show the histological components of the lesion of orbital tumor,which is of certain significance for the qualitative diagnosis of orbital lymphoma and inflammatory pseudotumor.
