Sarcopenia, characterized by reduced muscle mass, decreased strength, and impaired function, can lead to adverse outcomes, such as frailty, falls, fractures and disability, and subsequently lead to decreased quality of life, increased risk of complications and mortality and elevated healthcare costs. With the accelerating process of global aging, the prevalence of sarcopenia has significantly risen, posing a major public health threat and socioeconomic burden to populations worldwide. To more effectively implement medical nutrition management and exercise intervention for patients with sarcopenia, the Chinese Clinical Nutritionist Center of Chinese Medical Doctor Association and the Nutrition Oncology Branch and Geriatric Nutrition Branch of the Chinese Nutrition Society, in collaboration with the Nutrition and Food Safety Branch of Chinese Geriatrics Society, have developed the Expert Consensus on Medical Nutrition Management and Exercise Intervention for Sarcopenia (2024) based on the latest research evidence, clinical practice experiences and clinical nutrition guidelines. This consensus serves as a reference for clinical diagnosis and treatment of sarcopenia. The consensus emphasizes that the diagnosis and treatment of sarcopenia should focus on elderly population and also encompass patients with related chronic diseases including diabetes, inflammatory bowel disease, cancer, cognitive dysfunction and Parkinson's disease. Multi-modal lifestyle interventions centered on nutrition and exercise remain the preferred strategy for the prevention and treatment of sarcopenia.

Objective:To investigate the "burnout-engagement continuum" status of postgraduate supervisors of the Professional Master's Program in Clinical Medicine and to identify key populations.Methods:From October to November 2023, an anonymous online questionnaire survey was conducted to determine the "burnout-engagement continuum" status of postgraduate supervisors of the Professional Master's Program in Clinical Medicine. The scores of different dimensions of job burnout are expressed as mean±standard deviation, and the distribution of supervisors of different status is presented in frequency and percentage. All data analyses were performed in R 4.3.1. Distribution of the five profiles among supervisors of different status was compared using the chi-square test and compared pairwise using the Bonferroni method.Results:A total of 2 664 valid questionnaires were collected. The scores of emotional exhaustion, cynicism and personal accomplishment were（2.46±0.95),（1.74±0.89), and（4.23±0.90), respectively. The prevalence of job engagement, inefficacy, overexertion, disengagement, and burnout among the supervisors were 42.76%（1 139/2 664), 43.54%（1 160/2 664), 33.03%（880/2 664), 14.71%（392/2 664), and 13.66%（364/2 664), respectively. In terms of demographics, the distribution of the "burnout-engagement continuum" was significantly different by age, sex, marital status, and highest education level（ P<0.05). In terms of work-related characteristics, the distribution of the "burnout-engagement continuum" was significantly different by income, professional title, administrative position, and working hours on weekdays and holidays（ P<0.05). The distribution of engagement, inefficacy, and overexertion was significantly different between the eastern and western regions（ P<0.05). Conclusions:There is a high percentage of individuals with burnout among postgraduate supervisors of the Professional Master's Program in Clinical Medicine, and inefficacy is a common negative state. Supervisors who work in the western regions, are under 45 years of age, are male, are married, have a doctorate degree, hold associate senior professional title, have lower relative income, are not holding administrative duties, and work more than 10 hours on workdays and more than 4 hours on holidays are key populations that require increased attention. The psychological state of supervisors can be improved by providing them with more resources, work benefits, and incentives.

Objective:To analyze the scores and differences of various aspects of public health graduate education management in China, and to provide suggestions for universities to enhance graduate education management.Methods:Based on the data from the 2021 National Survey on Education Satisfaction and Job Evaluation of Medical Graduate Supervisors, we described the public health graduate education management in China from the five perspectives of student enrollment, curriculum, practice, research, and school management. The t-test and analysis of variance were used to analyze the differences in individual characteristics. Multiple linear regression was used to analyze factors influencing the scores of public health graduate education management. Results:Supervisors gave the highest score to curriculum and teaching (mean score, 4.00±0.77), followed by management system and input (mean score, 3.97±0.87), research conditions and achievements (mean score, 3.93±0.80), practical education conditions and effects (mean score, 3.80±0.91), and discipline attractiveness and enrollment quality (mean score, 3.70±0.84). Supervisors who supervised professional degrees, doctoral students, and located in the eastern region gave relatively high scores to public health graduate education, and those with associate professor titles gave relatively low scores.Conclusions:The quality of students recruited for public health graduate education needs to be improved, curriculum design and teaching method need to be adjusted, practice conditions need to be improved, research funding is insufficient, and the construction of academic communication platforms needs to be improved. The relevant universities/colleges need to strengthen the management and increase the investment to promote the high-quality development of public health graduate education.

BACKGROUND:Surrounding structure of intracristal ventricular septal defects(IVSD) and membranous ventricular septal defects(MVSD)is complicated,and it is difficult to perform transcatheter closure treatment.Corresponding animal models should be established to define an optimal interventional therapy.OBJECTIVE:To prepare the animal models of IVSD and MVSD under echoeardiogram location.DESIGN:Animal modeling experiment.SETTING:Department of Cardiology,Zhongshan Hospital Affiliated to Fudan University.MATERIALS:Six healthy adult dogs,irrespective of gender,were purchased from Shanghai Experimental Animal Center.Aorta and great saphenous vein stapler-puncher(St.Jude corporation,USA)was used.The main improvement is to cut the plastic shell even,thin,or shorter in order to reduce the resistance when puncturing the"purse"in the fight ventficle and the interventficular septum.Sonos 5500 multifunction ultrasonoscope(Philips,USA)was used,and the frequency of the probe was between 2.5-3.5 MHz.METHODS:Experiments were performed at the Zhongshan Hospital Affiliated to Fudan University and Central Laboratory of Shanghai Institute of Cardiovascular Disease from April to July 2006(biosaflety level 2).After anaesthesia,6 adult dogs were fixed at decumbent position.The interventricular septum was punetured to make the preparation with the location of echocardiography(ECHO)by using the modified aorta and great saphenous vein proximal stapler-puncher.Animal intervention met the Animal Ethical Committee of Fudan University.MAIN OUTCOME MEASURES:One week after the surgery,transthoracic echocardiography examination was used to check the VSD site,the highest shunt flow rate,the pressure difference between both sides of the VSD,as well as pulmonary artenal pressure.RESULTS:The VSD models were successfully established in three survivals and included in the final result.Two models of IVSD and one model of MVSD were examined by ECHO one week after the surgery.The diameter of VSD ranges from 1.8 to 3.6 mm.The Peak Flow Rate of the VSD ranges between 1.8 and 4.0 m/s.The pressure difierence of both sides of the VSD ranges between 42 and 51 mm Hg.The pulmonary arterial systolic pressure ranges from 23 to 29 mm Hg.CoNCLUSION:Located by the ECHO,it is possible to establish animal models of the IVSD and MVSD with the modified aorta and great saphenous vein proximal stapler-puncher.

OBJECTIVETo demonstrate the clinical efficacy of micronized fenofibrate on mildly-moderately elevated LDL-C levels and reduced HDL-C levels.

METHODSDuring 1998 - 1999, 2358 patients with type IIa, IIb and IV hyperlipidemia were monitored in 16 cities in China. They were treated daily with micronized fenofibrate (micronized lipanthyl) 200 mg for 8 weeks. Lipid levels before and after the treatment were measured and analyzed.

RESULTSMicronized fenofibrate significantly increased HDL-C levels by 12.7%, the effect being inversely correlated to the baseline level of HDL-C. Out of the total patient population, a baseline level of HDL-C < 1.0 mmol/L was found in 837 patients: amongst this group, 510 patients (60.9%) were observed to have an increase in the level of HDL-C to > 1.0 mmol/L with a mean of 1.3 mmol/L, after 8-week micronized fenofibrate therapy. Furthermore, the mean LDL-C level decreased by 15.9% following an 8-week treatment of micronized fenofibrate, an effect positively correlated to the baseline level of LDL. In general, all patients tolerated the drug comfortably.

CONCLUSIONSShort-term treatment of micronized fenofibrate in patients with dyslipidemia significantly increases HDL-C level and reduces mildly-moderately elevated LDL-C level. As expected, it also reduces triglyceride levels.

ive] To observe the therapeutic effects of Xiaoji Decoction (XD) in treating intermed iate and late stages of lung cancer. [ Methods] One hundred and twelve patients with lung cancer in stage ? -? were randomly allocated to Group A, Group B and Group C. Group A ( n = 49) was treated with XD alone, Group B ( n = 33 ) was treated with various chemotherapeutic regimens according to the histo-logical types of lung cancer: CAP regimen for lung squamous carcinoma, EP regimen for lung adenocarci-noma and CE regimen for small cell carcinoma of lung, In Group C, 30 patients were treated with chemo therapy combined with XD. Symptomatic relief, life quality, tumor size, distant metastasis, sub-types of T lymphocytes, survival period and side effects in the three groups were evaluated after one course of treatment. [Results] The effects of XD on tumor size were evaluated by the effective rate and stabilized rate.
