1.Application of ultrasound multimodal diagnostic mode combined with ultrasonic precision interventional technology in differentiating the benign from malignant solid breast masses

Objective To discuss the application of ultrasound multimodal diagnostic mode combined with ultrasonic precision interventional technology in differentiating the benign from malignant solid breast masses.Methods A total of 396 patients who underwent breast surgery at the Wuwei Cancer Hospital of China from May 2021 to July 2023 were enrolled in this study.Within 2 weeks before surgery,multimodal ultrasound examination(including two-dimensional ultrasound,color Doppler ultrasound,elastic ultrasound,breast three-dimensional ultrasound,and contrast-enhanced ultrasound)and ultrasound-guided needle biopsy were performed in all patients.The consistency between the results of multimodal ultrasound examination,ultrasound-guided needle biopsy,combination diagnosis and the postoperative pathological diagnoses was analyzed.Results Of the 396 patients with solid breast mass,malignant lesion was seen in 237(59.85%)and benign lesion was seen in 159(40.15%).The sensitivity of multimodal ultrasound in diagnosing benign and malignant solid breast masses was 77.64%(184/237),the specificity was 90.57%(144/159),the positive predictive value was 92.46%(184/199),the negative predictive value was 73.10%(144/197),the accuracy was 82.83%(328/396),and the consistency with the postoperative pathological diagnosis was 0.656.The sensitivity of ultrasound-guided needle puncture in diagnosing benign and malignant solid breast masses was 94.51%(224/237),the specificity was 100.00%(159/159),the positive predictive value was 100.00%(224/224),the negative predictive value was 92.44%(159/172),the accuracy was 96.72%(383/396),and the consistency with the postoperative pathological diagnosis was 0.933.The sensitivity of multimodal ultrasound combined with ultrasound-guided needle puncture in diagnosing benign and malignant solid breast masses was 100.00%(228/228),the specificity was 94.64%(159/168),the positive predictive value was 96.20%(228/237),the negative predictive value was 100.00%(159/159),the accuracy was 97.73%(387/396),and the consistency with the postoperative pathological diagnosis was 0.937.Conclusion The ultrasound multimodal diagnostic mode and ultrasonic precision interventional technology can be used in differentiating the benign from malignant solid breast masses with high diagnostic accuracy.

Objective:We aimed to develop a novel visualized model based on nomogram to predict postoperative overall survival.Methods:This was a multicenter, retrospective, observational cohort study, including participants with histologically confirmed gastric and colorectal cancer who underwent radical surgery from 11 medical centers in China from August 1, 2015 to June 30, 2018. Baseline characteristics, histopathological data and nutritional status, as assessed using Nutrition Risk Screening 2002 (NRS 2002) score and the scored Patient-Generated Subjective Global Assessment, were collected. The least absolute shrinkage and selection operator regression and Cox regression were used to identify variables to be included in the predictive model. Internal and external validations were performed.Results:There were 681 and 127 patients in the training and validation cohorts, respectively. A total of 188 deaths were observed over a median follow-up period of 59 (range: 58 to 60) months. Two independent predictors of NRS 2002 and Tumor-Node-Metastasis (TNM) stage were identified and incorporated into the prediction nomogram model together with the factor of age. The model's concordance index for 1-, 3- and 5-year overall survival was 0.696, 0.724, and 0.738 in the training cohort and 0.801, 0.812, and 0.793 in the validation cohort, respectively.Conclusions:In this study, a new nomogram prediction model based on NRS 2002 score was developed and validated for predicting the overall postoperative survival of patients with gastric colorectal cancer. This model has good differentiation, calibration and clinical practicability in predicting the long-term survival rate of patients with gastrointestinal cancer after radical surgery.
3.Correlation Between L3 Level Trunk Muscle Mass and Severity of Lumbar Facet Joint Osteoarthritis

Purpose To investigate the correlation between lumbar trunk muscle mass at the L3 level and the severity of lumbar facet joint osteoarthritis (FJOA) in the middle-aged and elderly population. Materials and Methods The analysis involved 312 individuals aged between 44 and 82 years in Beijing Jishuitan Hospital from March to June 2016. The total area and CT values of the lumbar trunk muscles at the L3 vertebral body's lower endplate level were measured to calculate the skeletal muscle mass index via OsiriX image processing software. Furthermore,the severity of bilateral lumbar facet osteoarthritis at the L4-5 level was assessed using the Weishaupt grading criteria. CT value of trunk muscle at L3 lower endplate level,trunk muscle area at L3 lower endplate level,skeletal muscle index,body mass index,age and FJOA grade at L4-5 right and left sides were compared between genders. The correlation between the mean value of trunk muscle CT and FJOA grade at the lower endplate level of L3 was analyzed. Results In 109 male patients,the mean trunk muscle was negatively correlated with FJOA grade on the right and left sides of L4-5 (r=-0.25,P=0.002;r=-0.28,P=0.003),while age was positively correlated with FJOA grade on the right and left sides of L4-5 (r=0.23,P=0.034;r=0.21,P=0.033). In 203 female patients,there was a negative correlation between trunk muscle mean and both right and left FJOA grade at L4-5 (r=-0.29,P<0.001;r=-0.33,P<0.001),there was a positive correlation between age and both right and left FJOA grade at L4-5 (r=0.27,P<0.001;r=0.34,P<0.001),and there was a positive correlation between body mass index and both right and left FJOA grade at L4-5 (r=0.20,P=0.018;r=0.23,P=0.004). Partial correlation analysis,controlling for age,showed that the severity of bilateral FJOA at L4-5 was somewhat negatively correlated with the mean value of the L3 trunk muscle in the male and female groups (r=-0.239,-0.249,-0.177,-0.169,all P<0.05). Conclusion Trunk muscle mass at the L3 level is negatively correlated with the severity of FJOA,and FJOA is one of the independent factors of chronic low back pain,strengthening nutritional intake and physical exercise is beneficial to prevent the occurrence and development of sarcopenia,thereby reducing the occurrence and aggravation of FJOA and reducing the burden on patients with chronic low back pain.

5.Value of prophylactic anticoagulation therapy in the 28-day prognosis of severe COVID-19 patients

Objective To explore the value of prophylactic anticoagulation in the 28-day prognosis of patients with severe COVID-19.Methods The clinical data of COVID-19 patients admitted to Guangdong Provincial People's Hospital from December 2022 to January 2023 were collected,including age,gender,past medical history,routine blood test,liver and renal function,procalcitonin,coagulation function,Padua prediction score,antiviral therapy,hormonotherapy,immunotherapy and anticoagulation therapy.The patients were divided into an anticoagula-tion group and a non-anticoagulation group.The differences in various indicators were compared between the two groups.Cox regression was performed to assess the independent risk factors for 28-day mortality and the anticoagula-tion efficacy between the subgroups.Results Among 158 patients,128 received anticoagulation;There were significant differences between the two groups in case number of hypertension and critical condition,lymphocyte count,prothrombin time,D-dimer,and case number of immunotherapy.COX logistic regression showed that antico-agulation(HR = 2.25,95%CI:1.01~5.01,P = 0.048)was an independent risk factor for all-cause mortality of COVID-19 patients within 28 days.Subgroup analysis showed that anticoagulation therapy led to an increase in the 28-day mortality as level of procalcitonin of≥0.5 pg/mL(HR = 2.72,95%CI:1.05~7.04)or D-dimer of<2 000 ng/mL(HR = 9.16,95%CI:1.63~51.48).Conclusions Prophylactic anticoagulation did not reduce all-cause mortality of COVID-19 within 28 days.
6.A recommendation on minimization of wastes from natural uranium purification and conversion

In the process of natural uranium purification and conversion, low-level uranium-containing radioactive wastes will be produced. On the basis of introducing the characteristics of wastes from natural uranium purification and conversion and their treatment, we review the main strategies and good practices of minimization of wastes from natural uranium purification and conversion, analyze the main problems in minimization of wastes from natural uranium purification and conversion in China, and propose corresponding improvement measures, which provides a reference for the implementation and management of minimization of wastes from natural uranium purification and conversion.
7.Dynamic Characteristics of an Adolescent Idiopathic Scoliotic Spine

Objective To establish the three-dimensional (3D) finite element (FE) model of thoracolumbosacral T1-S spine based on the computed tomography (CT) images of patients with scoliosis and study its dynamic characteristics. Methods The established scoliotic model was validated by axial compression and shear loading, and the predicted responses were in good agreement with the experimental data. The modal and harmonic analyses were performed using the ABAQUS software, and during the harmonic analysis, the dynamic response of the model was collected at frequencies 5 Hz and 10 Hz. Results From the modal analysis, the first fourth-order modal was extracted. The first- and second-order resonant frequencies of the model were 1.097 Hz and 1.384 Hz, respectively, and the vibration mode was longitudinal bending and lateral bending, respectively. The distribution of the second- and third-order modal resonant frequencies were 5.688 Hz and 28.090 Hz, and the vibration mode was vertical vibration and twisting around the long axis, respectively. The peak amplitude in the harmonic analysis appeared near the modal frequencies, and the average amplitude of vertebral body of the lateral convex segment was larger than that of other segments of the scoliotic spine. Under the vibration frequencies of 5 Hz and 10 Hz, the stress inhomogeneously concentrated on the concave and convex sides of the segments of the vertebral deformity as well as on the intervertebral disc. Conclusions The segments of the spinal deformity in patients with scoliosis were the weak links of their spines and more vulnerable to damage in a vibrating environment. Patients with scoliosis should avoid a vibrating environment, particularly in a sensitive frequency range. The research outcomes provide methodological assistance and mechanical analysis references for the protection, rehabilitation treatment, and clinical pathological studies of patients with scoliosis.
8.Dynamic characteristics of the lumbosacral vertebrae based on three-dimensional finite element models

BACKGROUND: Inherent modal analysis and harmonic response analysis on the human normal lumbosacral vertebraehave been reported, but there is a lack of comparative research on their modal analysis results before and after pediclescrew fixation.OBJECTIVE: To explore the dynamic characteristics of human lumbosacral vertebrae using three-dimensional finiteelement method.METHODS: Finite element model of lumbosacral vertebrae (L1-S1) before and after pedicle screw fixation was developedand validated based on CT images, and the modal analysis and harmonic response analysis were then conducted.RESULTS AND CONCLUSION: (1) Representative nodes were selected at the spinous process segments of L1, L3 andL5, and numbered as A, B, and C, respectively. (2) The maximum displacement of each node in Y and Z directions oflumbosacral vertebral model after internal fixation was significantly decreased compared with those of the normallumbosacral vertebral model, suggesting that screw fixation system plays a protective role in lumbosacral vertebrae, andreduces its amplitude under external load, thus diminishing its sensitivity to external load. (3) The lumbosacral vertebralmodal analysis can provide basis for further study on dynamic analysis, and the parameters such as natural frequency,modal shape and vibration amplitude of the lumbar spine have been determined.
9.The suggestions on the entry of Chinese medicines into European Community monographs for traditional herbal medicinal products

Directive 2004/24/EC of the European Parliament and the Council entered into force on April 30th, 2004. After 7 years, there is no Chinese medicine to be registered successfully in European market as traditional herbal medicinal products. The thesis gives some ideas to tackle this problem. Procedure for the Preparation of Community Monographs for Traditional Herbal Medicinal Products (EMEA/HMPC/182320/2005) published by European Medicines Agency is an important guidance for traditional herbal products to enter European Community monographs. The thesis introduces and details the procedure as well as gives feasible suggestions about the procedure. It suggests that Chinese medicines enter European Community monographs first, and then apply the registration according to the directive 2004/24/EC. This is an easier access to European market.

The present paper introduced the management status of wild medicinal resources (WMR) including the law system, the government system and the rule system, and analyzed the main problems and their reasons for WMR management. It pointed out that the old management system for WMR was not fit the need of conversation and management of WMR. It suggested to revise the "the Law of Conversation and Manage System of Wild Medicinal Resources", and discussed the law type, the objective and the principle he government system, management field and rule system for the revised law in details.
