1.Clinical characterization of 602 older patients with cancer related anemia received multi-line anti-tumor treatment

Objective To analyze the clinical characteristics of cancer related anemia(CRA)in older cancer patients undergoing multi-line anti-tumor treatment.Methods A cross-sectional study was conducted,including patients with CRA who were≥65 years old and had received or were currently undergoing multi-line treatment at 9 hospitals in Beijing from June 1,2018 to September 30,2023.Data on gender,age,past history,family history,tumor type,metastasis site,clinical symptoms,blood routine,specialized examination for anemia,anti-tumor and anemia correction treatment were collected.The basic characteristics of the included patients,clinical symptoms,the status of blood cell reduction,treatment conditions,and the distribution of traditional Chinese medicine(TCM)syndrome characteristics were analyzed.Results A total of 602 patients were included.The tumor types included lung cancer,colorectal cancer,gastric cancer,esophageal cancer,breast cancer,gynecological tumors,lymphoma.The overall median hemoglobin(HGB)level was 99.00(88.00,107.00)g/L.The HGB levels of patients with different tumor types showed significant differences(P<0.001).The HGB levels of patients with gynecological tumors were significantly lower than those of patients with colorectal cancer,lung cancer and esophageal cancer(P<0.05).The most common clinical symptoms were fatigue,loss of appetite,irregular bowel movements and pain.In terms of anti-tumor treatment,405 patients were receiving multi-line anti-tumor treatment(including combined chemotherapy,combined radiotherapy,combined targeted therapy,and combined immunotherapy),197 patients were receiving symptomatic supportive treatment after multi-line anti-tumor treatment.In terms of anemia correction treatment,183 patients received erythropoietin,folic acid,iron,vitamin B12,or blood transfusion,while 293 patients received TCM for correcting anemia(including Chinese patent medicines,Chinese herbal decoctions,combinations of Chinese patent medicines and Chinese herbal decoctions).Specialized tests for anemia,such as serum iron,total iron binding capacity,transferrin saturation,transferrin,erythropoietin et al,have a detection rate of only 3.65%~27.91%.In terms of the characteristics of TCM syndromes,the main TCM syndrome types of the patients were as follows:syndrome of spleen-stomach weakness,syndrome of qi and blood deficiency,syndrome of heart-spleen deficiency,syndrome of spleen-kidney yang deficiency,syndrome of liver and kidney yin deficiency.Some patients also have the accompanying symptoms of internal binding of static blood and poison,phlegm dampness retention.Further comparison revealed that the HGB level of older patients with CRA who had syndrome of spleen-stomach weakness was significantly lower than patients with other symptom types(P<0.001).The HGB level of patients with syndrome of qi and blood deficiency was significantly lower than patients with syndrome of spleen-kidney yang deficiency(P<0.01)and syndrome of liver and kidney yin deficiency(P<0.001).Patients with symptoms of internal binding of static blood and poison,or symptoms of phlegm dampness retention had significantly lower HGB levels compared to those without accompanying symptoms(P<0.01).Conclusion The rate of specialized blood test for anemia in older patients with CRA was relatively low.The most common TCM syndrome types were syndrome of spleen-stomach weakness and syndrome of qi and blood deficiency.The overall treatment rate was also low.The screening,specialized diagnosis and monitoring of older patients with CRA need to be given more attention,and treatment methods such as TCM,Western medicine,and integrated TCM-Western medicine should be selected based on the patient's condition.

3.Improvement Effects of Sanggenone C on Lipid Accumulation in FFA-induced Human Liver Cancer HepG 2 Cells

OBJECTIVE:To study the improvement ef fects of sanggenone C on lipid accumulation in human liver cancer HepG2 cells induced by free fatty acid (FFA). METHODS :HepG2 cells were divided into control group ,model group , fenofibrate group (10 μmol/L),sangerone C low ,medium and high concentration groups (2,4,8 μmol/L). Except for control group,other groups were treated with 1 mmol/L FFA to induce lipid accumulation model ,and administration groups were cultured with relevant medium containing drugs. The lipid accumulation was observed by oil red O staining ,and lipid level and triglyceride (TG) content were also determined. Real-time PCR and Western blot assay were adopted to detect the mRNA and protein expression of PPARα,CPT-1,SREBP-1c,FAS,SIRT1 and PGC- 1α in HepG2 cells. RESULTS :Compared with control group , the nucleus was atrophied significantly and the volume became smaller ,and the number of lipid droplets was significantly increased;the level of lipid ,TG content ,mRNA and protein expression of SREBP- 1c and FAS were significantly increased (P< 0.05 or P<0.01),mRNA and protein expression of PPARα,CPT-1,SIRT1 and PGC- 1α were decreased significantly(P<0.01). Compared with model group ,no obvious nucleus atrophy and normal volume were observed in sangerone C groups ,and the number of lipid droplets was significantly reduced ;the levels of lipid ,TG content ,mRNA and protein expression of PPARα pathway related genes (except for SREBP- 1c protein in saggenone C low concentration group )were significantly reversed (P< 0.05 or P<0.01). CONCLUSIONS :Sangenone C can significantly improve the lipid accumulation of HepG 2 cells,and its mechanism may associated with regulating PPAR α signaling pathway,improving cell lipid oxidation ability and inhibiting lipid synthesis.
4.Serotype distribution and antibiotic resistance pattern of 225 Streptococcus pneumoniae isolates from Urumqi Children′s Hospital in 2018

Objective:To investigate the serotype distribution and drug resistance of Streptococcus pneumoniae ( S. pneumoniae) isolated in Urumqi Children′s Hospital and to evaluate the significances of 13-valent pneumococcal conjugate vaccine (PCV13) in preventing infection and controlling drug resistance. Methods:The S. pneumoniae isolates stored in clinical laboratory of Urumqi Children′s Hospital from January to December in 2018 were re-cultured.The serotypes were detected by capsule swelling experiment to assess the coverage rate of PCV13.The minimum inhibitory concentration (MIC) of Penicillin, Amoxicillin, Cefotaxime and Ceftriaxone were detected by E-test method, and the susceptibility of the isolates to Meropenem and other 9 antibiotics was detected by VITEC 2 Compact system. Results:A total of 225 S. pneumoniae strains were identified.The common serotypes were 19F (32.9%), 23F (12.0%), 19A (10.7%), 6B (10.2%) and 6A (8.0%). PCV13 coverage rate was 80.4%.There was no significant difference in serotype distribution and PCV13 coverage between children < 2 years old and ≥ 2 years old, as well as between Han and minority people.The 57.8% and 31.7% strains showed intermediate susceptibility and resistance against oral Penicillin, respectively.Based on the breakpoints for meningitis, 89.4% strains were resistant against pare-nteral Penicillin, and 47.5% and 64.6% strains were non-susceptible (mainly intermediately susceptible) to Ceftria-xone and Cefotaxime, respectively.The resistance rates of strains against Erythromycin, Sulfamethoxazole-trimethoprim and Tetracycline were as high as 98.1%, 67.6% and 89.6%, respectively.More than 90% tested isolates were susceptible to Amoxicillin, Meropenem, Levofloxacin or Moxifloxacin.PCV13 strains were more resistant to Penicillin than non-PCV13 strains. Conclusions:The serotypes 19F, 23F, 19A, 6B and 6A are common among the S. pneumoniae isolated in Urumqi.The coverage rate of PCV13 is about 80%.There was no significant difference in serotype distribution between Han and minority nationality children. S. pneumoniae were frequently resistant against Erythromycin.The high resistance to Penicillin and other beta-lactams should be taken into account when treatment is decided for suspected pneumococcal meningitis.Universal administration of PCV13 would be effective strategy to prevent pneumococcal infection in children and to control the drug resistance of S. pneumoniae.
5.Influence of aerobic exercise and relaxation therapy on quality of life and symptoms for breast cancer patients with chemotherapy

Objective To study the influence of aerobic exercise and relaxation therapy on quality of life and symptoms of breast cancer patients undergoing chemotherapy.Methods A total of 116 female breast cancer patients in our hospital were divided into study group and control group by random number table.The control group took routine nursing intervention,and the study group applied aerobic exercise and relaxation therapy on the basis of the control group.And the symptoms,quality of life and emotions before and after intervention were compared.Results After intervention,the psychological symptoms,hormone related symptoms,the digestive system symptoms and self-image disorder symptoms scores in the study group were lower than that in the control group (P < 0.05),the physiological status,functional status,emotion,social/family conditions and additional attention scores were higher than that in the control group (P < 0.05).Conclusion Aerobic exercise and relaxation therapy for patients with breast cancer chemotherapy has a significant effect,and it can effectively improve the quality of life and symptoms,so it is suitable for clinical promotion.

7.Occupational Exposure of Medical Personnel and Their Protection

OBJECTIVE To evaluate the reason and protection measures of occupational exposure.METHODS From Jan 2005-Mar 2008,all medical personnel in our hospital were investigated by face to face investigation and checking recording data.The causes,modalities,management and prophylaxis of occupational exposure were analyzed.RESULTS Seventy-six person suffered occupational exposure,including 51 nurses(67.11%),18 doctors(23.68%) and the other 7 workers.Occupational exposure mainly occurred in 20-30 years old medical workers.61 persons(80.26%) suffered stab injury and sharp instrument injury.All 76 front medical personnel could make appropriate management after occupational exposure and without further infection.CONCLUSIONS Some first front medical personnel do not recognize the hazard of the occupational exposure.It′s necessary to enhance the management and training and to establish a corresponding standard to protect medical personnel from being harmed by occupational exposure.
8.Prenatal sonographic diagnosis on agenesis of fetal corpus callosum

Objective To assess the value of prenatal sonographic diagnosis on fetal agenesis of corpus callosum(ACC). Methods A retrospective study was on prenatal sonographic findings of 10 cases with ACC malformation and their abnormalities in central nervous system (CNS) or extra-CNS.Results The special sonographic findings established the diagnosis of ACC malformation in all 10 cases,with 7cases diagnosed absence agenesis. Among all patients with ACC,6 cases were accompanied with abnormalities in extra-CNS,8 in CNS and 5 in both. Conclusions Prenatal ultrasonography plays a vital role in accurate diagnosis on fetal ACC. Attentions should be paid to the indirect encephalic features and complicated abnormalities so as to make accurate and prompt diagnosis.
9.Comparative diagnosis of malformations of forearm and crus by prenatal and fetus in vitro ultrasonography

Objective To assess the value of prenatal ultrasonography on fetal forearm and crus malformations by studying the ultrasonographic characteristic in relation to methods of examination. Methods All fetus were evaluated by using a systematic continuous sequence approach (SCSA) with ultrasonography. A close attention was paid on shapes, structures and movement of fetal forearm and crus, and fetus specimens after induced labor were rescanned by ultrasonography under the condition of mimic intra-uterus. Results Thirty of 33 cases (90.9%) with fetal forearm and crus malformations (totally 48 of 54 limb anormalies, 88.9%) were correctly diagnosed by prenatal ultrasonography. Six limb malformations of 3 cases were missed. Conclusion Our study demonstrates that the malformations of forearm and crus detected with prenatal ultrasonography are highly accordant with the malformations revealed in fetus in vitro. It is important to abide by the SCSA in ultrasonographic diagnosis of fetal forearm and crus malformation.
10.Prenatal ultrasound diagnosis of fetal radial ray abnormalities

Objective To investigate prenatal ultrasound diagnosis of radial ray abnormalities and its clinical value. Methods All the fetus that entered usual prenatal ultrasound diagnosis were scanned each limb using a systematic continuous sequence approach (SCSA) with Acuson Sequoia 512 and GE Logiq 400 equipments. All the artificial aborted fetal radius malformation also had ultrasound examination in a simulating intrauterine estate and /or X-ray examination. Results Correctly diagnosis was made in 13 cases fetal radial ray abnormalities by prenatal ultrasonography following SCSA through the scaning course. All the fetal radial ray abnormalities had radius absence(only one skeleton in the forearm) or radius dysplasia and characteristic radial club hand. Ray abnormalities can be classifyied into three types on ultrasonography: type Ⅰ, radius absence; type Ⅱ,radius part absence; type Ⅲ,radius dysplasia.Conclusions Mastering characteristic radial club hand and SCSA are the keys to diagnose fetal radial ray abnormalities by prenatal ultrasonography.
