Abstract
On September 26, 2024, a municipal hospital in Jiaxing City reported a maternal case of Listeria monocytogenes infection. In order to clarify the source of infection, the Jiaxing Center for Disease Control and Prevention immediately conducted the epidemiological investigation, laboratory testing and related disposal work. The case presented with fever (37.9 ℃), gradually intensifying paroxysmal abdominal pain without obvious cause, and went to hospital on the day of onset. Due to fetal intrauterine distress, a male infant was delivered by cesarean section on the same day. The epidemiological investigation identified that the case usually consumed fruits, often store fruits such as watermelon and grapes in the refrigerator alongside raw meat, and the refrigerator had never been cleaned or disinfected, posing a risk of cross contamination. Laboratory tests on amniotic fluid sample from the pregnant woman, infant blood sample showed positive results for Listeria monocytogenes infection. One strain of Listeria monocytogenes was detected in a smear sample from the inner wall of the refrigerator, and all the strains were ST2 type. Consuming fruits contaminated with Listeria monocytogenes may be the main source of infection. Food safety education for pregnant women and their family members should be strengthened to reduce the risk of infection.

Journal of Preventive Medicine 2025;37(7):673-676,681
Objective:
To explore the impact of heat wave on the incidence of foodborne diseases, so as to provide the basis for preventing foodborne diseases in summer.
Methods:
Data on foodborne disease cases from June to September each year in Jiaxing City from 2018 to 2023 were collected through the Zhejiang Provincial Foodborne Disease Surveillance Platform. Meteorological data were obtained from the Jiaxing Meteorological Bureau. Air quality data were collected from the National Urban Air Quality Real-time Release Platform. Heat wave were defined based on temperature thresholds and durations. The impact of heat wave on the incidence of foodborne diseases were analyzed by a time stratified case-crossover design with a distributed lag nonlinear model (DLNM). The cumulative relative risk (CRR) and attributable fraction (AF) were calculated to compare the cumulative lagged risks and disease burdens across different genders, age groups, and disease types.
Results:
From June to September each year between 2018 and 2023, a total of 13 026 foodborne disease cases were reported in Jiaxing City, including 6 646 male cases and 6 380 female cases. Among them, 746 cases were aged <15 years, 10 910 cases were aged 15-65 years, and 1 370 cases were aged >65 years. There were 1 425 cases of infectious diarrhea, 9 956 cases of acute gastroenteritis, and 1 645 cases of other foodborne diseases. The DLNM analysis revealed that heat waves defined in different ways increased the risk of foodborne diseases (all CRR>1, P<0.05), and the risk of disease incidence rose with increasing temperature thresholds and durations. When the daily mean temperature was not lower than the 95th percentile of the daily mean temperature from 2018 to 2023 and lasted for at least 4 days, the Akaike Information Criterion was minimized, and the cumulative lagged risk and disease burden of foodborne diseases were relatively high, with a CRR value of 1.32 (95%CI: 1.09-1.60) and an AF value of 2.81% (95%CI: 1.05%-4.31%). Among them, females, individuals aged 15-65 years, and cases of acute gastroenteritis were more significantly affected by heat wave days, with CRR values of 1.37 (95%CI: 1.12-1.67), 1.35 (95%CI: 1.06-1.73), and 1.35 (95%CI: 1.09-1.66), respectively, and AF values of 3.00% (95%CI: 0.82%-4.73%), 3.08% (95%CI: 1.27%-4.61%), and 3.00% (95%CI: 1.04%-4.64%), respectively.
Conclusions
Heat wave have lagged and cumulative effects on the risk of foodborne disease incidence, increasing both the risk and disease burden. Females, individuals aged 15-65 years, and cases of acute gastroenteritis are more significantly affected by heat wave days.

Chinese Medical Ethics 2025;38(10):1240-1245
There are numerous ethical dilemmas in families of children with kidney disease during the treatment stage. From the perspective of Giddens’ structured theory, this paper analyzed the ethical dilemmas, such as individual and family wealth disparity at the micro-level, doctor-patient information asymmetry and the attribution of medical decision-making rights at the meso-level, as well as unequal medical resources and an incomplete medical security system for children at the macro-level. The ethical dilemmas faced by families of children with kidney disease are the result of the structural constraint effect. The coping strategies they adopt in response to these dilemmas are the basis of structural reproduction and the products of the structural effect. As a group with subjective initiative, they are good at self-reflection. Through repeated cognitive evaluation, they can make a series of effective coping strategies to achieve their own goals, such as relying on family support and linking resources to seek social support, establishing online support groups and building an information sharing platform, assessing children’s best-interests judges and safeguarding their reasonable and legitimate rights and interests, planning and allocating high-quality medical resources and promoting the construction of the medical service system, as well as promoting the reform of the basic medical insurance system for children and improve the protection mechanism for major illnesses.

Objective:To analyze any effect of combining hyperbaric oxygen with qingkailing on the metabolic biomarkers of consciousness disorder (CD) caused by brain trauma using metabolomics technology. Also, to screen out diagnostic markers for clinical treatment and prognosis.Methods:Thirty-six patients with a CD caused by brain trauma formed the observation group, while 40 healthy gender-, age- and ethnicity-matched individuals were the control group. Both groups were given routine supportive treatment, while the observation group additionally received hyperbaric oxygen and oral qingkailing medication. Before and after 14 days consciousness disturbance was evaluated using the Glasgow Coma Scale (GCS). Venous blood was collected for metabolomic analysis using liquid chromatography linked to a mass spectrometer to screen out specific metabolic biomarkers. Metabolic markers associated with the disease and of diagnostic significance were thus identified. Results:After the treatment the average GCS score of the observation group had improved significantly and the degree of coma was significantly relieved. Twenty-one metabolic markers were found to be significant, with creatine, proline, uric acid, acetyl-L-carnitine, histidine, proline leucine, tryptophan and 9E-octagenoic acid potentially of high diagnostic value. After the treatment, all of those markers came close to the levels observed in the healthy control group. Conclusions:Proline, leucine, 9E-octagenoleic acid, uric acid and acetyl-L-carnitine could be used for diagnosis and evaluating efficacy with such patients. Hyperbaric oxygen supplemented with qingkailing can relieve coma, correct metabolic disorders and accelerate patients′ awakening. Metabolomics provides a new method for identifying endogenous metabolic abnormalities in patients with post-traumatic consciousness disorders. It can be useful in prognosis and clinical treatment.

Journal of Traditional Chinese Medicine 2024;65(11):1104-1108
This paper summarized Professor ZHOU Zhongying's experience in differentiating and treating hepatitis and liver cirrhosis from deficiency and excess. It is considered that the pathogenesis of hepatitis and liver cirrhosis belongs to deficiency in root and excess in branch, with depletion of liver, spleen and kidney as the root, and constraint and bind of damp-heat and stasis toxin as the branch. Moreover, mutual cause and promotion between deficiency and excess leads to the disease. For general principle of treatment, it is recommended to clear and transform pathogenic excess, supplement deficiency and rectify the healthy qi. In the early stage of hepatitis and cirrhosis, excess pathogen hyperactivity is the main manifestation, which can be treated by clearing and transforming damp-heat and stasis toxin, supplemented by regulating spleen and stomach, with modified Yinchenhao Decoction (茵陈蒿汤) and Biejiajian Pill (鳖甲煎丸). In the middle and late stages, cases with deficiency-excess complex were more common, which should be treated by clearing damp-heat and stasis toxin, regulating and supplementing liver-spleen-kidney, using medicinals with the function of clearing heat and dispelling damp, dissolving stasis and resolving toxins to treat the branch. Moreover, Liujunzi Decoction (六君子汤), Yiguan Decoction (一贯煎)plus Erzhi Pill (二至丸) and Buzhong Yiqi Decoction (补中益气汤) modifications are suggested respectively in correspondence to the different kinds of root deficiency including irregular liver and spleen, liver and kidney yin deficiency, and liver-spleen-kidney deficiency.
