Polygonatum sibiricum, as a traditional Chinese medicine with both medicinal and edible properties, has attracted considerable attention due to its functions of nourishing Yin and moistening the lungs, tonifying the spleen and benefiting Qi, and nourishing the kidneys and filling essence. Recent studies have demonstrated that Polygonatum sibiricum plays a significant role in regulating the immune system, effectively enhancing and improving the morphology and function of immune organs, stimulating the proliferation and activation of immune cells, and regulating the secretion and release of immune factors, thereby enhancing the immune function of the body and improving various immune-related diseases. Although a large number of studies have explored the pharmacological effects and mechanisms of P. sibiricum, there has been no systematic review and summary of its immune regulatory activity and mechanisms. Therefore, this article comprehensively reviews the research achievements of P. sibiricum polysaccharides and saponins in the field of immune regulation in recent years, and further sorts out the immune regulatory mechanisms of P. sibiricum in multiple aspects: including increasing the organ index of the spleen and thymus, increasing the number and activity of tumor-suppressive bone marrow hematopoietic stem cells, improving intestinal flora imbalance, regulating the quantity and proportion of T lymphocyte subsets, increasing the level of immunoglobulin, promoting the proliferation of macrophages, enhancing the activity of natural killer cells, increasing the number of white blood cells, and promoting the maturation of dendritic cells, providing a solid theoretical basis and scientific evidence for the research and application of P. sibiricum, and promoting its development and application in traditional Chinese medicine immune enhancers and various functional products.

Objective:To understand the baseline viral load (VL) of newly reported HIV- infected patients before antiretroviral therapy and related factors in Tianjin. Methods:Data were obtained from the China Disease Control and Prevention Information System, and the study subjects were HIV-infected patients before the first antiretroviral therapy in Tianjin from 2019 to 2022, and the information about their socio-demographic characteristics, baseline CD4 +T lymphocyte (CD4) counts before antiretroviral therapy and baseline VL test results were collected, the baseline high VL was defined as ≥100 000 copies/ml. The effect of different factors on viral load were analyzed. Software SPSS 24.0 was used for statistical analysis. Results:A total of 1 296 newly reported HIV-infected patients were included in the study, in whom 15.89% (206/1 296) had high baseline VL, and multifactorial logistic regression analysis showed that those with history of STD (a OR=1.45, 95% CI:1.00-2.08) were more likely to have high baseline VL. Compared with those with baseline CD4 counts <200 cells/μl, those with baseline CD4 counts 200-350 cells/μl (a OR=0.40, 95% CI: 0.27-0.57), 351-500 cells/μl (a OR=0.32, 95% CI: 0.20-0.49), and >500 cells/μl (a OR=0.30, 95% CI: 0.18-0.49) were less likely to have high baseline VL. Conclusions:The proportion of HIV-infected patients with high baseline VL before antiretroviral therapy was low in Tianjin during 2019-2022. History of STD and baseline CD4 counts <200 cells/μl were associated with high baseline VL in HIV-infected patients, to which close attention needs to be paid in AIDS prevention and control.

Objective :
To investigate the efficacy of standardized allergen subcutaneous immunotherapy (SCIT) in
children with asthma sensitized to single dust mite allergens versus multiple allergens and to assess the safety of SCIT.
Methods :
62 children with confirmed allergic asthma who received standardized allergen SCIT were retro⁃
spectively analyzed and divided into the monosensitized group (dust mite results≥ + + + ) and the polysensitized group (dust mite results ≥ + + + combined with other positive allergens) according to the results of skin pricktest , we observed the changes of pulmonary function , medication score and visual analog scale (VAS) scores , children asthma control test (C - ACT) scores , asthma control questionnaire (ACQ) scores before and after treatment in both groupsand compared the efficacy of the two groups. The incidence of local and systemic adverse effects was recorded during treatment in all children to assess the safety of SCIT.
Results :
Standardized allergen SCIT treatmentimproved lung function parameters , medication scores and VAS scores , C ⁃ACT scores , ACQ scores in both the monosensitized and polysensitized groups , with statistically significant differences before and after treatment (P < 0. 05) . In comparison between the two groups , lung function parameters [forced expiratory flow at 50% vital capacity(FEF50% ) , maximum midexpiratory flow(MMEF)] , medication scores , C ⁃ACT scores and ACQ scores
improved significantly in the monosensitized group compared with the polysensitized group after treatment ( P <0. 001) . 62 patients received a total of 2 606 injections during the treatment of SCIT , 6 children had a total of 10 local adverse reactions and 3 children had 3 mild to moderate systemic adverse reactions , with an incidence of 0. 38% for local adverse reactions and 0. 12% for systemic adverse reactions.
Conclusion
The children with asthma in both the monosensitized group and polysensitized group achieved significant and safe clinical outcomes under standardized allergen SCIT. The children in the monosensitized group had more obvious clinical effects than the polysensitized group under standardized allergen SCIT.

Journal of Chinese Physician 2023;25(11):1707-1712
Objective:To evaluate the safety and effectiveness of distal femoral osteotomy combined with arthroscopy in the treatment of valgus knee. Methods:A retrospective analysis was conducted on 24 patients (25 knees) with valgus knee osteoarthritis admitted to the First Hospital of Jiaxing from January 2017 to December 2020 who underwent distal femoral osteotomy combined with arthroscopic surgery and were fixed with domestically produced distal femoral locking plates. The changes in hip knee ankle angle (HKA), distal lateral femoral angle (LDFA), knee joint range of motion (ROM), and knee joint function score (HSS) of the American Special Surgery Hospital before and after surgery were statistically analyzed.Resultsl:A total of 24 patients (25 knees) were followed up for (15.6±2.3)months. HKA increased from (171.22±2.51)° before surgery to (179.24±1.86)° at the last follow-up; LDFA increased from (82.17±2.03)° before surgery to (88.57±1.53)° at the last follow-up; The differences were statistically significant ( P<0.05). The HSS score improved from preoperative (60.27±6.04) to the last follow-up (87.80±5.50), with a statistically significant difference (all P<0.05). One out of 25 knees experienced loss of osteotomy angle, without insufficient or excessive correction, infection, and non union of fractures. Conclusions:The use of distal femoral osteotomy combined with arthroscopic surgery for the treatment of valgus knee osteoarthritis is a safe and reliable method. Retaining the lateral bone hinge during osteotomy is an important factor in maintaining the osteotomy angle and fracture healing.
