1.Study on the value of T-piece resuscitator as a respiratory support strategy for the transpot of critically ill premature infants

Objective:To evaluate the effectiveness of T-piece resuscitator as a respiratory support strategy during the transport of critically ill premature infants,and to provide a scientific basis for clinical decision-making.Methods:A total of 280 critically ill premature newborns hospitalized in the NICU of Fifth Medical Center of Chinese People's Liberation Army General Hospital from January 2017 to December 2023 were included.Infants were categorized into three groups based on the respiratory support method given during transport: the ventilator group(108 cases),the T-piece group(102 cases),and the resuscitation sac group(70 cases).The transport distance,general condition at birth,prenatal conditions,dyspnea symptoms at admission,blood gas analysis results,clinical diagnosis,clinical intervations,and related treatment among the three groups were retrospectively analyzed.Results:There were no significant differences in the transport distance,the number of endotrached intubations during transport,the main complications during pregnancy,the general condition at birth,and the history of asphyxia among the three groups(all P>0.05).The incidence of triple-concave sign at admission in T-piece group was significantly lower than that in resuscitation sac group (41.7% vs.62.9%, P=0.005),and the arterial carbon dioxide tension(PaCO 2) at admission was also significantly lower in T-piece group than that in resuscitation sac group[(41.194±8.720) mmHg vs.(45.360±13.998) mmHg, P=0.034].Furthermore,the T-piece group had significantly lower rates of type II respiratory failure(0.9% vs.22.9%),respiratory acidosis(9.3% vs.27.1%),hypoxemia(7.4% vs.28.6%),hyperoxygen partial pressure(1.9% vs.28.6%),neonatal respiratory distress syndrome(66.7% vs.87.1%),and intracranial hemorrhage(18.5% vs.38.6%) during hospitalization compared to the resuscitation sac group (all P<0.05).The proportion of tracheal intubations(63.9% vs.87.1%) and the time of using non-invasive ventilator[1.0(1.0,2.0)d vs.1.0(1.0,6.0)d] were also significantly lower in T-piece group compared to the resuscitation sac group(both P<0.05).Compared with the respiratory group,there were no statistically significant differences in the aforementioned indicators for the T-piece group. Conclusion:The T-piece resuscitator can provide stable and adjustable positive end-inspiratory pressure and positive expiratory pressure,as well as a stable inspired oxygen flow rate,without increasing the risk of invasive procedures and severe complications.Its application during the transport and treatment of critically ill premature infants has definite clinical value.
2.Characteristic differences between award-winning and first-time blood donors in Guangzhou: a role theory perspective

Objective: To preliminarily develop a multidimensional blood donor role scale based on role theory and systematically compare the psychosocial characteristic differences between award-winning donors and first-time donors in Guangzhou, and to provide an empirical reference for formulating differentiated donor retention strategies. Methods: A cross-sectional survey design was adopted. A random sample of award-winning donors and concurrently sampled first-time donors yielding 1 361 valid responses collected (721 from the award group, 640 from the first-time group). Exploratory factor analysis was used to assess the scale structure. Data were post-stratified and weighted according to the gender and age distributions of the general donor population. Independent samples t-tests, multivariate analysis of covariance (MANCOVA), and generalized linear models were employed to compare dimensional scores between the two groups. A paired t-test was conducted to analyze the annual donation frequency of award-winning donors before and after receiving the award. Results: Exploratory factor analysis yielded a 5-factor structure, including Role Identity and Expectations, Role Adaptation and Maintenance, Role Environment and Experience, Role Relationships and Conflict, and Role Incentives and Rewards, with a cumulative variance contribution rate of 56.43%. The scale demonstrated good internal consistency reliability (Cronbach's α=0.906). Known-group validity test showed that award-winning donors scored significantly higher than first-time donors on Role Identity and Expectations (t=4.366, P<0.001, d=0.240), Role Adaptation and Maintenance (t=5.436, P<0.001, d=0.500), and Role Relationships and Conflict (t=4.844, P<0.001, d=0.220). These differences remained significant after controlling for selected demographic variables (MANCOVA, Wilks' λ=0.943, P<0.001). Generalized linear models suggested that donation frequency was an independent predictor for these dimensions. Additionally, the annual donation frequency of award-winning donors was slightly higher after receiving the award than before (t=2.007, P=0.045). Conclusion: The preliminary blood donor role scale demonstrates acceptable reliability and validity and can effectively distinguish groups with different donation behavior characteristics. The study reveals that award-winning donors exhibit more positive psychological characteristics across multiple role identity dimensions and maintain their donation behavior after receiving an award. External incentives and internal role identity may jointly contribute to behavioral persistence. The findings provide a preliminary reference for further exploring the formation pathways of donor role identity and developing differentiated donor retention strategies.
3.Spondyloarthritis with monoclonal garmmopathy of undetermined significance: a report of five cases with literature review

Objective:To explore the clinical characteristics, treatment, and prognosis of patients with spondyloarthritis (SpA) and clinically insignificant monoclonal garmmopathy of undetermined significance (MGUS).Methods:A retrospective study was performed to analyze the clinical characteristics of patients simultaneously diagnosed with both SpA and MGUS at the First Affiliated Hospital to Zhengzhou University between January 2020 and December 2024, supplemented by a literature review.Results:Among the 5 patients (2 males, 3 females; age range 33~65 years), 4 presented with peripheral arthritis and 2 with extra-articular manifestations. All patients demonstrated elevated globulin and IgG levels, with 4 being HLA-B27 positive. M-protein typing revealed IgG/κ in 3 cases and IgA/λ in 2. All five patients underwent both X-ray and MRI examinations, with sacroiliitis being detected in 4 cases. Three patients with long disease duration showed poor response to conventional therapy (NSAIDs/DMARDs). Targeted therapies yielded variable outcomes: one patient achieved normalized globulin/IgG levels with etanercept (3-year follow-up without MGUS progression); another showed marked clinical improvement and significant globulin/IgG reduction with JAK inhibitor (tofacitinib, treated for 7 years); while a third demonstrated no symptom relief then switching from TNF-α inhibitor to IL-17 inhibitor.Conclusion:Persistent hyperglobulinemia in SpA patients warrants MGUS screening. Those with SpA-MGUS may require targeted therapies, where JAK inhibitors and monoclonal TNF-α inhibitors appear to be the preferred options, though long-term monitoring for MGUS progression remains essential.

6.Textual Research on Classical Formula Juanbitang

This paper discussed the historic evolution of Juanbitang and similar decoctions, clarified the historic development of Yangshi Juanbitang and Chengshi Juanbitang, and probed into the key information of the meaning, original plants, processing methods, and modern dosage and usage of Chengshi Juanbitang. A total of 267 pieces of relevant information were collected from ancient traditional Chinese medicine (TCM) books, among which 53 pieces of effective data were included in this study. The results showed that both Chenshi Juanbitang and Yangshi Juanbitang were originated from Duhuo Jishengtang recoded in the Important Prescriptions Worth a Thousand Gold for Emergency (Bei Ji Qian Jin Yao Fang). According to the standard of "1 qian roughly equals 3.73 g" in the measurement system of the Qing Dynasty, we suggest Chenshi Juanbitang is composed of 3.73 g Notopterygii Rhizoma et Radix, 3.73 g Angelicae Pubescentis Radix, 3.73 g Gentianae Macrophyllae Radix, 11.19 g Angelicae Sinensis Radix, 11.19 g Mori Ramulus, 2.61 g Chuanxiong Rhizoma, 1.87 g Cinnamomi Cortex, 1.87 g stir-fried Glycyrrhizae Radix et Rhizoma, 7.46 g Piperis Kadsurae Caulis, 2.98 g Olibanum, and 2.98 g Aucklandiae Radix, which should be decocted with 600 mL water to reach a volume of 300 mL. The decoction should be taken 3 times a day before meals. Juanbitang, a classical formula specialized for treating impediment diseases, has the effects of dispelling wind, removing dryness, and alleviating impediment to relieve pain. It can be used for treating vexing pain in body, spasm of nape and back, and heaviness in waists and legs. Modern studies have shown that Juanbitang can be used for treating rheumatoid arthritis, knee osteoarthritis, and periarthritis of shoulder. The above results served as a reference for the future development of Juanbitang.
7.Textual Research of Pediatric Famous Classical Formula Daochisan Based on Ancient and Modern Literature

Daochisan is a pediatric famous classical formula in traditional Chinese medicine(TCM) specializing in clearing the heart, disinhibiting water and enriching Yin, which has been inherited and developed by medical experts of successive generations. The formula can be traced back to Xiaoer Yaozheng Zhijue written by QIAN Yi from Song dynasty, and most of the later-developed Daochisan(Daochitang) is based on this formula with additions and subtractions. So as to promote the literature excavation of this formula, this paper conducts a thorough textual research and analysis of the composition, processing, preparation methods, clinical applications of Daochisan based on a systematic collation of relevant ancient and modern literature in the method of bibliometrics. A total of 127 pieces of data involving 86 antient TCM books were collected. Statistical analysis showed that the drug origin of Daochisan is basically clear, we suggest that Rehmanniae Radix is the root tuber of Rehmannia glutinosa of Scrophulariaceae, Akebiae Caulis is the dry stem of Akebia quinata of Lardizabalaceae, Glycyrrhizae Radix et Rhizoma is the dry root and stem of Glycyrrhiza uralensis of legume, bamboo leaf is the dry leaves of Phyllostachys nigra var. henonis of gramineous plants. Four herbs are recommended to use its raw products. The key information of the dosage and decocting method is supposed to be "4.13 g each of raw Rehmanniae Radix, raw Glycyrrhizae Radix et Rhizoma, Akebiae Caulis, adding 300 mL of water, and then adding 4.13 g of bamboo leaf, decocting together to 150 mL, taking warmly after meal". Daochisan has the effect of clearing the heart and enriching Yin, disinhibiting water and relieving stranguria, and is widely used in treating symptoms such as heart vexed hotness, hydrodipsia and reddish complexion. Modern studies have showed that Daochisan are frequently used in treating oral ulcer, herpetic stomatitis, urinary tract infection, herpes zoster and other diseases. The above research results can provide scientific reference for the future development and research of Daochisan.
8.Rapid construction of rPRV-ΔTK/EGFP variant strain using CRISPR/Cas9 sys-tem

Pseudorabies virus(PRV)is the etiological agent of pseudorabies in pigs,which is char-acterized by dyspnea,reproductive disorders,and neurological diseases,and it spreads widely a-round the world.Since 2011,the newly emerged PRV variants have resulted in poor immunity pro-tection of traditional vaccine strains,and the original method of vaccine strain preparation is time-consuming and labor-intensive.Therefore,it is urgently needed to develop an efficient screening method of the vaccine strain at present.Using CRISPR/Cas9 gene editing technology in this study,two single guide RNAs(sgRNA)were designed targeting the virulence gene TK of PRV variant strain CH/JX/2016,and then the enhanced green fluorescent protein the reporter(EGFP)gene was inserted at the TK locus by a homologous repair plasmid.After multiple rounds of plaque puri-fication,the rPRV-ΔTK/EGFP strain was obtained.The results showed the cleavage efficiency of the two sgRNAs was extremely high.The preparation of rPRV-ΔTK/EGFP strain was succeed af-ter only three rounds of purification,and the EGFP expressed normally.The CRISPR/Cas9 system can edit the PRV gene simply,rapidly,and efficiently,and exhibits great potential in the construction of vaccine candidate strains.Meanwhile,the rescued rPRV-ΔTK/EGFP strain not only could be used as a tracer strain in PRV variant infection progresses,but also for subsequent antivi-ral drug screening.
9.Diagnostic value and characteristic analysis of multimodal imaging in subretinal drusenoid deposit in age-related macular degeneration

Objective:To observe the multi-modal fundus imaging features of subretinal drusenoid deposit (SDD) in age-related macular degeneration (AMD), and observe image features.Methods:A prospective clinical study. From December 2019 to December 2023, 65 patients (104 eyes) with a diagnosis of AMD-SDD by spectral domain optical coherence tomography (SD-OCT) examination in Shandong Eye Hospital were included. All eyes were examined by best corrected visual acuity (BCVA), traditional color fundus photography (CFP), ultra-wide-angle scanning laser fundus imaging (UWF), multicolor scanning laser fundus imaging (MC) and SD-OCT. The standard MC images were obtained by using Spectralis HRA+OCT for MC examination. The multi-mode image characteristics of SDD were analyzed retrospectively. Area under curve (AUC) was used to evaluate the sensitivity and specificity of CFP, MC and UWF in detecting SDD.Results:Among 65 patients with SDD, 29 cases of males (52 eyes) and 36 cases of females (52 eyes) was included. There were 26 patients with unilateral SDD and 39 patients with bilateral SDD. The average age was (71.74±10.97) years. The early, middle and late stages of AMD were 31 (29.8%, 31/104), 24 (23.1%, 24/104), 49 (47.1%, 49/104) eyes, respectively. The SDD detected by CFP, MC and UWF was 76 (73.1%, 76/104), 94 (90.4%, 94/104), 96 (92.3%, 96/104) eyes. CFP showed that the edge of SDD in the macular area was blurred. UWF showed that the dot and the ribbon SDD were light yellow pale discrete deposits and light yellow interlaced network deposits respectively. MC showed the dot SDD had a strong yellow-green circular reflection, while the edge of the ribbon SDD was surrounded by a weak reflection, and the boundary was clear. SD-OCT showed that SDD had strong reflection signal, which was located between the retinal pigment epithelium layer and the photoreceptor cell layer. The dot SDD could break through the ellipsoid zone and caused slight uplift or interruption of the external membrane, showing a cone-like strong reflection signal. While the ribbon SDD showed a continuous "hill-like" protrusion, which hardly broke through ellipsoid zone. The sensitivity and specificity of CFP, MC and UWF for SDD were 73.1%, 90.4%, 92.3% and 61.1%, 94.4% and 83.3%, respectively.Conclusions:MC and UWF show high sensitivity and specificity in diagnosing AMD-SDD, which is superior to CFP. SD-OCT can effectively reveal the location and morphoLogical characteristics of SDD under retina.
10.Clinical value of 1, 3-β-D glucan and mannan IgM/IgG antibodies in the early diagnosis of invasive candidiasis

Objective:To evaluate the diagnostic value of 1, 3-β-D glucan (BDG), mannan IgM antibody (Mn-IgM) and mannan IgG antibody (Mn-IgG) in invasive candidiasis and to compare the differences in the diagnostic capability of serological markers used alone or in combination.Methods:Serum samples of 126 patients with invasive candidiasis and 104 healthy people who took physical examination during the same period were collected. BDG was detected by dynamic chromogenic method, and Mn-IgM and Mn-IgG were detected by ELISA. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index, coincidence rate and Kappa value of the three serological markers used alone or in combination in the diagnosis of invasive candidiasis were analyzed and compared. The receiver operating characteristic (ROC) curves were drawn and the areas under the curves (AUCs) were calculated.Results:The levels of BDG, Mn-IgM and Mn-IgG in patients with invasive candidiasis were significantly higher than those in healthy people ( P<0.01). The sensitivity, specificity, Kappa value and AUC of BDG were 48.41%, 92.31%, 0.389 and 0.842. The sensitivity, specificity, Kappa value and AUC of Mn-IgM were 64.29%, 91.35%, 0.540 and 0.829. The sensitivity, specificity, Kappa value and AUC of Mn-IgG were 27.78%, 95.19%, 0.214 and 0.737. The sensitivity, specificity, Kappa value and AUC of BDG+ Mn-IgM were 76.19%, 88.46%, 0.637 and 0.921. The sensitivity, specificity, Kappa value and AUC of BDG+ Mn-IgG were 59.52%, 91.35%, 0.491 and 0.856. The sensitivity, specificity, Kappa value and AUC of Mn-IgM+ Mn-IgG were 69.84%, 90.38%, 0.588 and 0.891. The sensitivity, specificity, Kappa value and AUC of BDG+ Mn-IgM+ Mn-IgG were 80.16%, 88.46%, 0.679 and 0.922. Conclusions:The sensitivity of Mn-IgM was higher than that of BDG and Mn-IgG in the diagnosis of invasive candidiasis. When the serological biomarkers were used in combination, BDG+ Mn-IgM and BDG+ Mn-IgM+ Mn-IgG had relatively high Kappa value and AUC, showing high accuracy. The clinical diagnostic value of multiple serological biomarkers used in combination was significantly higher than that of any serological biomarkers used alone. Early combined detection and continuous monitoring of multiple serological biomarkers in patients with high risk of invasive candidiasis could be used clinically to adjust antifungal treatment strategies timely.
