Ulcerative colitis (UC) is a chronic nonspecific inflammatory intestinal disease with a complex pathogenesis. Its recurrent attacks easily lead to persistent intestinal mucosal damage, resulting in a year-by-year increase in incidence and imposing a heavy burden on patients. Currently, the treatment of UC mainly relies on corticosteroids, immunosuppressants, and aminosalicylate preparations. However, these therapies are limited by significant side effects, strong drug resistance, and poor long-term prognosis. As one of the important inflammatory pathways, the Janus kinase 2 (JAK2)/signal transducer and activator of transcription 3 (STAT3) signaling pathway can regulate the occurrence and development of UC through multiple pathways by modulating multiple downstream targets. Therefore, the JAK2/STAT3 signaling pathway offers a new research direction for the treatment of UC. Numerous studies have shown that the JAK2/STAT3 signaling pathway plays an important role in intestinal inflammatory responses, intestinal mucosal barrier damage, T helper cell 17 (Th17)/regulatory T cell (Treg) immune imbalance, intestinal epithelial cell apoptosis, and oxidative stress. Chinese medicine has attracted increasing attention because of its unique advantages, including fewer side effects, individualized diagnosis and treatment, and enhancement of efficacy with reduction of toxicity. In addition to traditional Chinese herbal formulas, more Chinese medicine monomers and extracts have played key roles because of their more precise therapeutic targets. In recent years, many studies have confirmed that Chinese medicine can exert therapeutic effects on UC by interfering with the JAK2/STAT3 signaling pathway, thereby alleviating intestinal inflammatory responses, repairing the intestinal mucosal barrier, regulating the balance of Th17/Treg immune cells, inhibiting intestinal epithelial cell apoptosis, and suppressing oxidative stress. This article summarizes and reviews recent research progress in this field from three aspects, including an overview of the JAK2/STAT3 signaling pathway, the relationship between the JAK2/STAT3 signaling pathway and UC, and the regulatory mechanisms of traditional Chinese medicine, with the aim of providing a theoretical basis for the future clinical treatment of UC and drug development.

With the full implementation of the Diagnosis Related Groups(DRG)payment model, its institutional advantages in optimizing resource allocation and controlling medical costs through fixed disease payment standards have gradually emerged. However,it has also triggered structural value conflicts in the doctor-patient relationship.Based on the four principles of medical ethics, this paper constructed an analytical framework for the value conflicts in doctor-patient relationships under the DRG payment model.Starting from the manifestations of value conflicts, the inducements creating them were analyzed in depth. On these foundations,multi-dimensional optimization paths were proposed, including repairing respect-related conflicts through information transparency and decision-making co-governance;constructing a refined cost management system and embedding an ethical review mechanism to resolve non-harm conflicts;implementing a phased payment mechanism for innovative technologies and an ethical review exemption mechanism to alleviate benefit conflicts;as well as designing dynamic payment rules, unifying payment standards for insurance participation types, and strengthening dynamic monitoring to address justice conflicts.Under this framework, this paper aimed to promote the gradual transformation of DRG from a cost-control tool to a governance tool. While ensuring the security of the fund,it was necessary to maintain the bottom line of quality, stimulate technological innovation, and return to the patient-centered concept, thereby promoting the doctor-patient relationship to shift from a zero-sum game to a symbiotic and win-win situation.

Objective:To dynamically monitor the prevalence and trend of dental fluorosis in children in Shandong Province, and to evaluate the prevention and control measures for drinking water-borne endemic fluorosis (referred to as drinking water-borne fluorosis), and to provide scientific basis for the next step. Methods:Totally 40 counties (cities, districts) were selected as project counties (cities, districts) from drinking water-borne fluorosis areas in Shandong Province in 2018, and all counties (cities, districts) were selected in 2019 and 2020, to investigate the situation of water improvement, detect water fluoride content, and investigate the prevalence of dental fluorosis in children aged 8 - 12 years. Results:From 2018 to 2020, the detection rates of dental fluorosis in children aged 8 - 12 years were 10.30% (503/4 884), 8.94% (25 895/289 539) and 8.66% (24 061/277 689), respectively, with statistically significant differences (χ 2 = 27.10, P < 0.001), and the dental fluorosis indexes were 0.21, 0.18 and 0.17, respectively. The total detection rates of dental fluorosis in children of different age groups in the 3 years were 7.26% (6 590/90 775), 7.97% (9 303/116 680), 9.29% (12 167/130 915), 9.29% (12 238/131 670) and 9.95% (10 161/102 072), the differences were statistically significant (χ 2 = 615.71, P < 0.001). In the 3 years, the total detection rate of dental fluorosis was 8.93% (28 101/314 737) in boys and 8.69% (22 358/257 375) in girls, the difference was statistically significant (χ 2 = 10.27, P = 0.001). In 2018 and 2019, the detection rates of dental fluorosis of children aged 8 - 12 years in water fluoride qualified villages [5.74% (235/4 095) and 7.98% (20 200/253 082)] were significantly lower than those in villages with excessive water fluoride [33.97% (268/789) and 15.62% (5 695/36 457), χ 2 = 570.61, 2 283.76, P < 0.001]. Conclusions:The prevalence of dental fluorosis among children aged 8 - 12 years has been effectively controlled, and remarkable results have been achieved in prevention and treatment of drinking water-borne fluorosis in Shandong Province. However, the detection rate of dental fluorosis among children in a few endemic villages is high, so it is necessary to continue to strengthen the monitoring of fluoride content in drinking water and the condition of dental fluorosis among children.

OBJECTIVE: To investigate the features of adverse drug reaction(ADR) in our hospital and evaluate the quality of ADR report in order to promote rational clinical drug use and find out the existing problems in the report. METHODS: Microsoft Excel was designed to analyze statistically the sex and age of the patients, category of the used drugs and the clinical manifestations of ADR in 311 cases of ADR report collected between 2002 and 2005.RESULTS: The occurrence of ADR induced by antibacterials ranked the first.ADR affecting skin and its appendages counted for 50% of the total. The numbers of ADR reports were found to be less than expected.CONCLUSIONS: Professional staff training should be strengthened, the use of antibacterials should be rationalized to lessen and avoid the occurrence of ADR.
