1.The inhibitory effect of lidocaine by different administration routes on cardiovascular stress response during tracheal intubation of anesthetic intubation

Objective To investigate the preventive effects of lidocaine administered through different routes on cardiovascular stress responses during anesthesia tracheal intubation. Methods Total 120 patients scheduled for elective surgery under general anesthesia were randomly divided into three groups: intravenous injection group (group IV), throat spray group (group LJ), and control group (group CT), with 40 patients in each. Group IV received 50 mg of lidocaine via intravenous injection 1 minute before tracheal intubation. Group LJ received 50 mg of lidocaine sprayed into the pharyngeal cavity, glottis, and subglottic area. Group CT did not receive any treatment, and the remaining procedures were performed following the routine general anesthesia induction protocol. Heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), and mean arterial pressure (MAP) were recorded at four time points: T0 (before tracheal intubation), T1 (immediately after tracheal intubation), T2 (3 minutes after intubation), and T3 (5 minutes after intubation). Statistical analysis of the data was performed using SPSS 22.0. Results There were no significant differences in HR at various time points within the group LJ. The changes in HR in the group IV and group CT were different statistically from those in the throat spray group. The blood pressure of patients in all three groups increased to varying degrees immediately after tracheal intubation, with the group CT showing particularly significant changes that differed significantly from both the group IV and the group LJ. The group LJ rapidly returned to levels close to those before intubation. Conclusion The preventive effects of lidocaine on stress responses during tracheal intubation were different depending on the route of administration. The inhibitory preventive effect of the throat spray method was superior to that of intravenous lidocaine, especially in preventing changes in heart rate.

The prevalence of periodontitis in China is as high as 74.2%, making it the leading cause of tooth loss in adults and severely impacting both oral and overall health. The treatment of periodontitis and periodontal tissue regeneration are global challenges of significant concern. GE Shaohua' s group at School and Hospital of Stomatology, Shandong University has focused on the key scientific issue of "remodeling the periodontal inflammatory microenvironment and optimizing tissue repair and regeneration". They have elucidated the mechanisms underlying the persistence of periodontitis, developed bioactive materials to enhance stem cell regenerative properties, and constructed a series of guided tissue regeneration barrier membranes to promote periodontal tissue repair, leading to the establishment of a comprehensive technology system for the treatment of periodontitis. Specific achievements and progress include: (1) Elucidating the mechanism by which key periodontal pathogens evade antimicrobial autophagy, leading to inflammatory damage; developing intelligent antimicrobial hydrogels and nanosystems, and creating metal-polyphenol network microsphere capsules to reshape the periodontal inflammatory microenvironment; (2) Explaining the mechanisms by which nanomaterial structures and electroactive interfaces regulate stem cell behavior, developing optimized nanostructures and electroactive biomaterials, thereby effectively enhancing the regenerative repair capabilities of stem cells; (3) Creating a series of biphasic heterogeneous barrier membranes, refining guided tissue regeneration and in situ tissue engineering techniques, stimulating the body' s intrinsic repair potential, and synergistically promoting the structural regeneration and functional reconstruction of periodontal tissues. The research outcomes of the group have innovated the fundamental theories of periodontal tissue regeneration, broken through foreign technological barriers and patent blockades, established a cascade repair strategy for periodontal regeneration, and enhanced China' s core competitiveness in the field of periodontal tissue regeneration.

Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

4.The Effect of Blood Lipid Profiles on Chronic Kidney Disease in a Prospective Cohort:Based on a Regression Discontinuity Design

Objective Previous studies on the association between lipid profiles and chronic kidney disease(CKD)have yielded inconsistent results and no defined thresholds for blood lipids. Methods A prospective cohort study including 32,351 subjects who completed baseline and follow-up surveys over 5 years was conducted.Restricted cubic splines and Cox models were used to examine the association between the lipid profiles and CKD.A regression discontinuity design was used to determine the cutoff value of lipid profiles that was significantly associated with increased the risk of CKD. Results Over a median follow-up time of 2.2(0.5,4.2)years,648(2.00%)subjects developed CKD.The lipid profiles that were significantly and linearly related to CKD included total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),TC/HDL-C,and TG/HDL-C,whereas low-density lipoprotein cholesterol(LDL-C)and LDL-C/HDL-C were nonlinearly correlated with CKD.TC,TG,TC/HDL-C,and TG/HDL-C showed an upward jump at the cutoff value,increasing the risk of CKD by 0.90%,1.50%,2.30%,and 1.60%,respectively,whereas HDL-C showed a downward jump at the cutoff value,reducing this risk by 1.0%.Female and participants with dyslipidemia had a higher risk of CKD,while the cutoff values for the different characteristics of the population were different. Conclusion There was a significant association between lipid profiles and CKD in a prospective cohort from Northwest China,while TG,TC/HDL-C,and TG/HDL-C showed a stronger risk association.The specific cutoff values of lipid profiles may provide a clinical reference for screening or diagnosing CKD risk.

Tumor metastasis is closely related to high mortality rate of cancer.It is well known that glutamine plays an important role in the malignant progression of cancer.Notably,as an important carbon and nitrogen donor,glutamine has been found to be closely related to tumor metastasis in recent years.Glutamine is not only involved in regulating the proliferation of tumor cells,but is also closely related to the migration and invasion of tumor cells.Furthermore,various enzymes along with transporters in the metabolism of glutamine are involved in the process of tumor metastasis through different signaling pathways.This review provided a summary of the role of glutamine in tumor metastasis in recent years and proposed therapeutic targets to provide new strategies for the clinical treatment of tumor metastases.
6.Accuracy of sacroiliac screw placement in robot-assisted navigation

BACKGROUND:Orthopedic robots have been widely used in clinical practice,and relevant reports have shown that they have many advantages such as minimal trauma and short surgical time.However,there is currently no clear report on how accurate they are. OBJECTIVE:To evaluate the accuracy of robot-assisted sacroiliac screw insertion. METHODS:A total of 131 patients with sacroiliac joint fracture and dislocation and sacral fracture admitted to the Department of Trauma Surgery,Gansu Provincial Hospital from January 2020 to April 2023 were retrospectively collected,including 131 S1 screws and 46 S2 screws,totaling 177 screws.They were divided into two groups based on whether robot-assisted navigation was performed.There were 63 cases of sacroiliac screws inserted under robot-assisted navigation(observation group),with 36 males and 27 females,aged 19-72 years,with a mean age of(45.3±17.6)years.Among them,39 cases were fixed with only S1 screws,while 24 cases were fixed with S1S2 screws,resulting in a total of 87 sacroiliac screws.Under C-arm fluoroscopy,68 cases of sacroiliac screws were inserted with bare hands(control group),including 41 males and 27 females,aged 23-67 years,with a mean age of(42.6±21.3)years.Among them,46 cases were fixed with simple S1 screws,while 22 cases were fixed with S1S2 screws,resulting in a total of 90 sacroiliac screws.A postoperative CT scan was performed to evaluate the number of S1 screws,S2 screws,total screw level,and calculate accuracy based on the method introduced by SMITH et al. RESULTS AND CONCLUSION:(1)In the observation group,62 S1 screws were accurately placed(62/63),with an accuracy rate of 98%.24 S2 screws were accurately placed(24/24),with an accuracy rate of 100%.The total number of screws accurately placed was 86(86/87),with an accuracy rate of 99%.(2)In the control group,58 S1 screws were accurately inserted(58/68),with an accuracy rate of 85%.19 S2 screws were accurately inserted(19/22),with an accuracy rate of 86%.The total number of screws accurately inserted was 77(77/90),with an accuracy rate of 86%.(3)There was a statistically significant difference in the accuracy of the S1 screw,S2 screw,and total screw between the two groups(P<0.05).It is suggested that the placement of sacroiliac screws under robot navigation has higher accuracy compared to manual placement under C-arm fluoroscopy,but still has a lower error rate in placement.
7.Current status of indoor microbial pollution and evaluation of air purifier effectiveness in Xi'an

Objective To investigate the current situation and health effects of indoor microbial pollution in Xi'an, to analyze the purification effect of air purifiers on indoor microbial pollution, and to provide reference for improving the indoor environment. Methods Through stratified random sampling, 20 families from rural areas and 20 families from upwind and downwind urban areas respectively were selected from Xi'an. Data was collected by questionnaire surveys and on-site environmental sampling. Non-parametric analysis and correlation analysis were used for statistical analysis. Results Overall, the standard-exceeding rate of total count of bacteria was 5.00%. The medians of the total count of bacteria and fungi were 312.50 cfu/m3 and 260.00 cfu/m3, respectively. In terms of health effects, the correlation between rhinitis and cold with total bacterial count was statistically significant (P<0.05), with the correlation coefficients of 0.182 and 0.223, respectively. Purification effect of air purifiers on microbial pollution was statistically significant (P<0.05). After opening for 2 hours, the total numbers of bacteria and fungi decreased significantly. Conclusion The occurrence risk of colds and rhinitis is increased by indoor microbial pollution. Air purifiers have a certain effect on decreasing the total number of bacteria and fungi. It is recommended to use air purifiers with high CADR of particulate matter, double-layer filter and sterilization and dehumidification function, and replace the filters regularly to reduce indoor microbiological contamination.
8.An experimental study of hydroxysafflor yellow A combined with hyaluronidase in the treatment of hyaluronic acid arterial embolism

Objective:To explore the therapeutic effect of hydroxysafflor yellow A (HSYA) combined with hyaluronidase (HAase) for arterial embolism caused by hyaluronic acid (HA).Methods:Thirty-two white male rabbits were randomly divided into four groups, with 8 rabbits in each group. Groups A, B and C were experimental groups, while group D served as the control group. An axial rectangular composite tissue flap sized 2.0 cm × 5.0 cm, with a pedicle width of 1.0 cm, and located 4.0 cm from the root, was designed with the central auricular artery as the long axis on the dorsal side of the ear. The incision depth reached the ventral perichondrium of the ear, and the flap was sutured continuously in place and divided into three equal parts (areas Ⅰ, Ⅱ, Ⅲ) from the proximal to the distal area. The proximal end, located 1 cm from the flap, and the central artery was the intersection point, where 50 μl of HA was injected to establish the model of HA arterial embolism. Each group was treated after 60 minutes. Group A: 20 ml of HSYA solution was slowly injected into the saphenous vein of the thigh (the dosage of HSYA was calculated at 10 mg/kg). Group B: 0.5 ml of HAase solution was injected into the central auricular artery (400 U/ml). Group C: 0.5 ml of HAase solution with the same dosage as in group B was injected into the central auricular artery, while 20 ml of HSYA solution with the same dosage as in group A was slowly injected into the saphenous vein. Group D and other parts of groups A and B were injected with the same dosage of normal saline (NS). The thigh saphenous veins of all groups were injected with the same dosage of solution once daily for 14 days. Flaps were observed immediately, 1, 7 and 14 days after establishing hyaluronic acid arterial embolism models of tissue flaps. Dorsal and backlight auricular photographs were taken. On the 14th day postoperatively, the survival areas of the flaps were calculated. Samples were taken from areas Ⅱof tissue flaps, stained with hematoxylin-eosin (HE) and Masson, to detected the activity of superoxide dismutase (SOD) and the content of malondialdehyde (MDA). The measurement data that conformed to a normal distribution was represented as Mean ± SD. Single-factor analysis of variance (ANOVA) was used to compare the differences among groups, followed by head-to-head comparison using the LSD test. P<0.05 was considered statistically significant. Results:Tissue flaps from all groups appeared pale immediately after the operation. On the first day after the operation, a dark ischemic area appeared at the distal end of each group. On the 7th day postoperatively, the ischemic area of each group showed varying degrees of necrosis and blackening, while the non-necrotic area exhibited significant swelling. On the 14th day post-operation, the ischemic area in each group showed further necrosis, blackening, and curling, with clear boundaries. Group C was the best, group D was the worst, and both group A and B were in between the two. The swelling of non-necrotic areas in groups A and C was reduced. HE staining revealed numerous thrombi and infiltration of inflammatory cells in group D, with group B following closely behind. Thrombi were rare in groups A and C. Masson staining showed that collagen fibers were organized regularly in group C, while abundant collagen fibers were disintegrated and disordered in group D. Groups A and B exhibited characteristics that fell between the other two groups. The percentages of survival areas of the flaps in groups A, B, C and D were as follows: (69.87±5.04)%, (85.03±6.58)%, (93.93±4.25)% and (49.22±9.64)%. There were statistical differences in pairwise comparisons between groups (all P<0.05). SOD activity of groups A, B, C, and D were as follows: (49.83±8.08), (36.65±5.49), (55.61±7.93) and (22.45±5.47) U/mg prot. Except for the group A vs. C, there were statistical differences between the groups (all P<0.01). The MDA content of groups A, B, C and D were as follows: (0.77±0.17), (1.03±0.16), (0.68±0.12), and (0.41±0.09) nmol/mg prot. Except that group A vs. C, there were statistical differences between groups (all P<0.01). Conclusion:In animal experiments, it was found that compared to HAase alone, the combination of HSYA with HAase significantly improves the therapeutic outcomes of HA arterial embolism and increases the proportion of tissue flap survival area.
9.Aqueous extract of Chuan Xiong Rhizoma enhances inhibitory effect of temozolomide against brain metastasis of melanoma in mice

Objective To investigate the effect of the aqueous extract of Chuan Xiong Rhizoma(CR)on brain metastasis of melanoma B16F10 cells in mice.Methods C57BL/6J mouse models of brain metastasis of melanoma were established by ultrasound-guided intraventricular injection of Luc-labeled B16F10 cells,and brain tumor growth was monitored by in vivo imaging.The mouse models were then randomized for daily gavage of saline or aqueous extract of CR(equivalent crude drug concentration of 1 mg/g).Behavioral tests were used to evaluate the neuroprotective effects of CR in the tumor-bearing mice,and the changes in proteins associated with blood-brain barrier integrity,neuronal cell proliferation and apoptosis,and microglial cell apoptosis and activation were observed using immunofluorescence assay.The efficacy of CR combined with temozolomide(25 mg/kg)against brain metastases of B16F10 cells was observed by in vivo imaging.Results CR-treated mouse models did not show obvious progression of brain metastases and had a reduced rate of body weight loss and lowered protein expressions of ZO-1,claudin-5,occludin,P-gp,TNF-α,AQP4 and PDGFRβ.In the behavioral tests,the CR-treated mice showed prolonged stay on the wooden stick with a shortened time of sticky stick removal.Immunofluorescence assay showed increased proliferation and decreased apoptosis of neuronal cells and microglia in CR-treated mice.CR treatment significantly increased the levels of CD86,CD206,IL-4 and IL-10 and decreased the levels of CD163 and IL-1β in the microenvironment of brain metastases.The mice receiving combined treatments with CR and temozolomide showed significantly lower intensity of fluorescent signals in the brain than those treated with temozolomide alone.Conclusion CR does not promote brain metastasis of melanoma while inducing opening of the blood-brain barrier,and its combined use with TMZ results in enhanced inhibition against brain metastasis of melanoma B16F10 cells in mice.
10.Application value of generative artificial intelligence in the field of stomatology

Objective This study aims to compare and analyze three types of generative artificial intelligence(GAI)and explore their application value and existing problems in the field of stomatology in the Chinese context.Methods A total of 36 questions were designed,covering all the professional areas of stomatology.The questions encompassed various aspects including medical records,professional knowledge,and translation and editing.These questions were submitted to ChatGPT4-turbo,Gemini(2024.2)and ERNIE Bot 4.0.After obtaining the answers,a blinded evaluation was conducted by three experienced oral medicine physicians using a four-point Likert scale.The value of GAI in vari-ous application scenarios was evaluated.Results Gemi-ni scored 45,ERNIE Bot scored 38,and ChatGPT scored 33 for clinical documentation and image production.For research assistance,Gemini achieved 45,ERNIE Bot had 39,and ChatGPT scored 35.Teaching assistance capabilities were rated at 54 for ERNIE Bot,50 for Gemini,and 48 for ChatGPT.In patient consultation and guidance,Gemini scored 78,ERNIE Bot scored 59,and ChatGPT scored 48.Over-all,the total scores were 218,190,and 164 for Gemini,ERNIE Bot,and ChatGPT,respectively.Among GAI applica-tions,the top scoring categories were article translation and polishing(26),patient-doctor communication documentation(23),and popular science content creation(23).The lowest scoring categories were literature search and reporting(13)and image generation(12).Conclusion In the Chinese context,the application value of GAI is the highest for Gemini,followed by ERNIE Bot and ChatGPT.GAI shows significant value in translation,patient-doctor communication,and popular science writing.However,its value in literature search,reporting,and image generation remains limited.
