1.Analyzing Differences in Volatile Components of Citri Reticulatae Pericarpium Before and After Being Stir-fried with Halloysitum Rubrum Based on HS-GC-MS and Intelligent Sensory Technology

ObjectiveTo analyze the differences in color, odor and volatile components of Citri Reticulatae Pericarpium(CRP) before and after being stir-fried with Halloysitum Rubrum, and to explore the material basis of enhancing the effect of strengthening spleen after processing and the scientific connotation of decoction pieces processed with Halloysitum Rubrum as the auxiliary material. MethodsThe volatile components of the samples before and after processing were identified and relatively quantified by headspace gas chromatography-mass spectrometry(HS-GC-MS), and the volatile components were analyzed by principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA). According to the principle of variable importance in the projection(VIP) value>1.5, volatile differential components before and after processing were screened. And combined with intelligent sensory technologies such as colorimeter and electronic nose, the chroma and odor information of CRP before and after being stir-fried with Halloysitum Rubrum were identified. Pearson correlation analysis was used to explore the correlation between volatile differential components and chroma values. ResultsA total of 112 volatile components were identified from CRP and CRP stir-fried with Halloysitum Rubrum, of which 84 were from CRP and 97 were from CRP stir-fried with Halloysitum Rubrum. And 7 differential components were selected, including α-pinene, β-myrcene, linalool, sabinene, ocimene isomer mixture, A-ocimene, and δ-elemene. After being processed with Halloysitum Rubrum, the brightness value(L*), yellow-blue value(b*) and total chromatic value(E*ab) of CRP were decreased(P<0.01), and red-green value(a*) was increased(P<0.01), the response values of S4, S5, S10 and S13 sensors were significantly increased(P<0.05), and the response values of S3 and S8 sensors were significantly decreased(P<0.05). Correlation analysis showed that α-pinene and β-myrcene were negatively correlated with L* and E*ab, but positively correlated with a*. Sabinene was positively correlated with L* and E*ab. Linalool was positively correlated with L* and E*ab, and negatively correlated with a*. The ocimene isomer mixture was positively correlated with the L*. ConclusionAfter being processed with Halloysitum Rubrum, the appearance color, odor and volatile components of CRP change significantly, and α-pinene, β-myrcene, sabinene, linalool and A-ocimene are the characteristic volatile components before and after processing, which can provide references for the quality evaluation and clinical application of CRP and its processed products.

3.Current status of pruritus care in dermatology departments in China: a survey of 607 nurses from 85 public hospitals

Objective:To investigate the current status of pruritus care in dermatology departments of public hospitals in China.Methods:A multicenter, cross-sectional survey was conducted. By convenience sampling, dermatology nurses were selected from 85 public hospitals across 17 provinces in China from September 2 to September 14, 2024, and a self-designed pruritus care questionnaire survey was conducted. The survey content included two parts: general information (e.g., education levels, professional titles, regions, hospital grades) and current status of pruritus care (e.g., pruritus care systems or standards, establishment of pruritus clinics, pruritus assessment, pruritus intervention measures, pruritus nursing training, and challenges in pruritus care) .Results:A total of 607 nurses were investigated, including 322 (53.0%) from general hospitals and 285 (47.0%) from specialized hospitals; 359 (59.1%) were from tertiary grade-A hospitals. In total, 264 (43.5%) nurses reported that pruritus clinics had been established in their hospitals, 218 (35.9%) reported that pruritus nursing teams had been set up, and 283 (46.6%) noted a lack of pruritus care systems or standards in their hospitals. The most commonly used anti-pruritus measures were physical therapy (51.2%) , drug treatment (37.9%) , and environmental management (10.9%) . There were 418 (68.9%) nurses who had received pruritus nursing training. The most frequently used pruritus assessment tools were the numeric rating scale ( n = 341, 56.2%) and the visual analog scale ( n = 268, 44.2%) , while 165 (27.2%) nurses had not used any assessment tools. The most challenging issues in pruritus care were the selection of pruritus assessment tools ( n = 303, 49.9%) and poor efficacy of pruritus control ( n = 113, 18.6%) . Tertiary grade-A hospitals and hospitals with pruritus clinics exhibited higher rates of establishing pruritus care systems/standards and providing pruritus nursing training compared with non-tertiary grade-A hospitals and hospitals without pruritus clinics respectively (all P < 0.05) . Conclusions:Public hospitals in China currently face issues such as inadequate pruritus care management systems, non-standardized pruritus assessment, and insufficient pruritus nursing training. Pruritus nursing levels were variable among different hospitals, and the standardization and homogeneity of pruritus nursing urgently need to be improved.
4.Design and implementation of online continuing education platform for transfusion medicine

To provide in-service medical technicians and nurses with convenient access to continuing education resources in transfusion medicine, reduce transfusion-related adverse events, and ensure the safety, rationalization, and effectiveness of clinical transfusion, we designed and developed an online transfusion continuing education platform. The platform was based on the new managed code programming model.NET Core and the powerful functions of hypertext preprocessor PHP 7.4, addressing current issues in transfusion online continuing education. Through in-depth analysis of student attributes, learning behaviors, and teaching behaviors, a comprehensive online continuous teaching quality evaluation index system was established. This system not only facilitates the quantitative assessment of teaching quality but also successfully integrates the two core functions of teaching and management, thereby achieving unified online teaching.

7.Analysis of Color and Odor Changes of Different Processed Products of Paeoniae Radix Alba Based on HS-GC-MS and Electronic Sensory Techniques

ObjectiveTo investigate the correlation between the color, odor and volatile components of Paeoniae Radix Alba(PRA) and its processed products, and to examine the effects of different processing methods on the odor and color formation of PRA. MethodThe odor and chromaticity information of PRA, honey chaff-fried PRA and honey bran-fried PRA were identified by electronic nose and colorimeter, and the volatile components in the different processed products of PRA were identified and relatively quantified by headspace gas chromatography-mass spectrometry(HS-GC-MS), and analyzed using principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA), then the differential key flavor components among the three were screened according to the principles of variable importance in the projection(VIP) value>1 and relative odor activity value(ROAV)≥1. Pearson correlation analysis was used to investigate the association between the differential flavor components and the colorimetric values and electronic nose sensors, respectively. ResultAfter being fried with honey chaff and honey bran, the lightness value(L*) of PRA decreased, and red-green value(a*) and yellow-blue value(b*) increased significantly(P<0.05, P<0.01). The odor differences were mainly reflected in the S1, S2, S4, S5, S6, S8 and S11 sensors, and the results of PCA of the electronic nose indicated that the odor differences among PRA, honey chaff-fried PRA and honey bran-fried PRA were obvious, among which the overall odor intensity of honey bran-fried PRA was higher than that of honey chaff-fried PRA. A total of 47 volatile components were identified from PRA and its processed products, including 21 for PRA, 36 for honey chaff-fried PRA, and 37 for honey bran-fried PRA. Odor analysis revealed that 12, 24 and 22 volatile components may be the key flavor components in PRA, honey chaff-fried PRA and honey bran-fried PRA, respectively. Correlation analysis showed that the L* of the decoction pieces was negatively correlated with the content of the Maillard reaction products, the a* and b* were positively correlated with the content of the Maillard reaction products, the S1 and S8 sensors were negatively correlated with the content of the Maillard reaction products, and the S2, S4, S5, S6 and S11 sensors were positively correlated with the Maillard reaction products. ConclusionThe color of PRA is deepened after being stir-fried with honey chaff and honey bran, and 5-hydroxymethylfurfural, furfural and other components are generated at the same time, which is in line with the theory of burning aroma strengthens the spleen of stir-fried with honey chaff and honey bran. Honey bran-fried PRA has a stronger Maillard reaction than honey chaff-fried PRA, which makes honey bran-fried PRA with a burnt flavor and a dark yellow color, while honey chaff-fried PRA has a sweet flavor and a bright yellow color.
8.Orphan nuclear receptor LRH-1 promotes oxaliplatin resistance in hepatocellular carcinoma cells by regulating MDR1 gene

Abstract: Objective To investigate the function and molecular mechanism of LRH-1 in regulating the sensitivity of
hepatocellular carcinoma (HCC) cells to oxaliplatin, providing new ideas for the treatment of liver cancer. Methods Knockdown
and overexpression of LRH-1 in HCC cell lines were constructed, and the effect of LRH-1 on oxaliplatin resistance of HCC
cells was explored by detecting IC50, cell proliferation, and plate colony formation assay. The transcriptional regulation of the
MDR-1 gene by LRH-1 was detected through quantitative PCR. The transcriptional activation ability of LRH-1 on the MDR1
gene was evaluated by luciferase reporter assay. Results In HuH7 cells overexpressing LRH-1, the IC50 significantly
increased to 18.012 μmol/L under oxaliplatin treatment, significantly higher than the 2.042 μmol/L in the HuH-7 control
group, showing statistically significant differences (P<0.05). After overexpression of LRH-1 in HuH-7 cells, the cell
proliferation ability was significantly increased, with a noticeable increase in MDR1 mRNA level. In HepG2 cells with
knockdown LRH-1 expression, the IC50 significantly dropped to 1.012 μmol/L, significantly lower than the 6.294 μmol/L in the
HepG2 control group, with statistically significant differences (P<0.05). After knockdown of LRH-1 in HepG2 cells, the cell
proliferation and plate colony formation ability were significantly inhibited, with a notable decrease in MDR1 mRNA expression
level. Luciferase reporter assay confirmed that LRH-1 can activate the transcriptional activity of the MDR1 promoter in a dosedependent manner, and its specific inhibitor ML-180 can significantly reduce LRH-1′s transcription activation ability on the
MDR1 promoter. Conclusions LRH-1 may promote oxaliplatin resistance in hepatocellular carcinoma cells by regulating
the transcriptional activity of MDR1 gene. Since its specific small molecule inhibitor has been successfully synthesized, LRH-1
can potentially become a target for the treatment of drug resistance in hepatocellular carcinoma.
9.Value of transvaginal ultrasonography combined neutrophil-to-lymphocyte ratio in diagnosis of endometrial cancer in postmenopausal women

Objective:To explore the value of transvaginal ultrasound (TVS) combined with neutrophil to lymphocyte ratio (NLR) in postmenopausal women with endometrial cancer(EC).Methods:A retrospective analysis was conducted on 348 postmenopausal patients who underwent hysteroscopy or surgery in the Obstetrics and Gynecology Hospital of Fudan University from January 2019 to December 2021 and had pathological results of EC, and they were included in the malignant group. One hundred and six benign patients with pathological results in the same period were randomly selected for inclusion in the benign group. The differences in age, menopausal time, gravidity, body mass index (BMI), post-menopausal bleeding, TVS and NLR were compared between the two groups. Binary Logistic regression analysis was performed to determine the independent predictive factors for endometrial cancer in postmenopausal women. ROC curves were plotted to evaluate the diagnostic efficacy of predicting EC in postmenopausal women.Results:Parity (≥2 times), post-menopausal bleeding, intrauterine fluid, the diameter of the mass, blood flow and NLR showed statistically significant differences between the malignant group and the benign group (all P<0.05). Logistic regression analysis showed that post-menopausal bleeding, intrauterine fluid and long diameter of the mass were independent predictors for EC in postmenopausal women( OR=0.433, P=0.006; OR=14.933, P<0.001; OR=1.062, P=0.001). The area under the curve (AUC) of EC in postmenopausal women was 0.901(95% CI=0.867-0.934) by the combination of the above independent factors and the parameters of P<0.10 (blood flow and NLR), the sensitivity was 78.4% and the specificity was 90.6%. Conclusions:The combination of post-menopausal bleeding, intrauterine fluid, the diameter of the mass, blood flow and NLR have certain clinical application value in predicting EC in postmenopausal women.
10.Trilogy of drug repurposing for developing cancer and chemotherapy-induced heart failure co-therapy agent.

Chemotherapy-induced complications, particularly lethal cardiovascular diseases, pose significant challenges for cancer survivors. The intertwined adverse effects, brought by cancer and its complication, further complicate anticancer therapy and lead to diminished clinical outcomes. Simple supplementation of cardioprotective agents falls short in addressing these challenges. Developing bi-functional co-therapy agents provided another potential solution to consolidate the chemotherapy and reduce cardiac events simultaneously. Drug repurposing was naturally endowed with co-therapeutic potential of two indications, implying a unique chance in the development of bi-functional agents. Herein, we further proposed a novel "trilogy of drug repurposing" strategy that comprises function-based, target-focused, and scaffold-driven repurposing approaches, aiming to systematically elucidate the advantages of repurposed drugs in rationally developing bi-functional agent. Through function-based repurposing, a cardioprotective agent, carvedilol (CAR), was identified as a potential neddylation inhibitor to suppress lung cancer growth. Employing target-focused SAR studies and scaffold-driven drug design, we synthesized 44 CAR derivatives to achieve a balance between anticancer and cardioprotection. Remarkably, optimal derivative 43 displayed promising bi-functional effects, especially in various self-established heart failure mice models with and without tumor-bearing. Collectively, the present study validated the practicability of the "trilogy of drug repurposing" strategy in the development of bi-functional co-therapy agents.
