1.Mechanisms of Dihuang Yinzi in Treating Advanced Parkinson's Disease Based on Gut Microbiota-SCFAs-inflammation Axis

ObjectiveTo observe the effects of Dihuang Yinzi (DY) on motor dysfunction in rats with advanced Parkinson's disease (PD) and to investigate the mechanisms by which DY improves advanced PD symptoms through the "gut microbiota-short-chain fatty acids (SCFAs)-inflammation-neuroprotection pathway". MethodsAn advanced PD rat model was induced by rotenone. Rats were divided into a normal group, model group, positive drug group (levodopa, 50 mg·kg-1), and DY low-, medium-, and high-dose groups (5.2, 10.4, 20.8 g·kg-1). After 7 days of administration, motor function was evaluated using the open-field, pole-climbing, and inclined plate tests. Hematoxylin-eosin (HE) staining was used to observe pathological changes in the substantia nigra and colon, and immunohistochemistry was performed to detect α-Synuclein (α-Syn) and tyrosine hydroxylase (TH) expression in the substantia nigra. Enzyme-linked immunosorbent assay (ELISA) was used to measure levels of dopamine (DA), 5-hydroxytryptamine (5-HT), 3,4-dihydroxyphenylacetic acid (DOPAC), Levodopa, homovanillic acid (HVA), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-1β (IL-1β). Western blot analysis was used to detect the expression of zonula occludens-1 (ZO-1) and occludin. Gut microbiota diversity was analyzed by 16S rRNA sequencing, and gas chromatography (GC) was used to determine the content of SCFAs in colonic contents. ResultsCompared with the normal group, the model group showed significantly decreased movement speed and distance in the open-field test, prolonged pole-climbing time, and reduced retention angle on the inclined plate (P<0.01), accompanied by increased α-Syn expression (P<0.01) and decreased TH expression (P<0.01) in the brain. Compared with the model group, all DY dose groups improved motor dysfunction in advanced PD rats to varying degrees (P<0.05, P<0.01) and alleviated pathological damage in the brain and colon. High-dose DY significantly reduced α-Syn aggregation in the substantia nigra (P<0.01) and increased TH expression (P<0.01). ELISA and Western blot results showed that, compared with the normal group, the model group exhibited decreased levels of DA, 5-HT, DOPAC, Levodopa, and HVA in the striatum (P<0.01), increased levels of TNF-α, IL-6, and IL-1β in the colon and striatum (P<0.01), and significantly reduced expression of ZO-1 (P<0.05) and occludin in the colon (P<0.01). Compared with the model group, all DY dose groups increased the levels of DA, 5-HT, DOPAC, Levodopa, and HVA in the striatum to varying degrees (P<0.05, P<0.01). In the high-dose DY group, the levels of TNF-α, IL-6, and IL-1β in the colon and striatum were reduced (P<0.01), while the expression of ZO-1 (P<0.05) and occludin in the intestine was increased. The 16S rRNA sequencing results indicated that the relative abundances of Actinobacteriota, Enterobacteriaceae, and Erysipelotrichaceae were increased in the model group, whereas the relative abundances of Bacteroidota, class Clostridia, Lachnospiraceae, and Akkermansia muciniphila were decreased. These changes were effectively reversed after high-dose DY intervention. GC analysis showed that the content of SCFAs in the colonic contents of rats in the model group was decreased (P<0.05, P<0.01), while after high-dose DY intervention, the levels of acetate, propionate, isobutyrate, and butyrate were significantly increased (P<0.05, P<0.01). ConclusionDY may exert therapeutic effects in advanced PD by regulating the gut microbiota-SCFAs-inflammation pathway.
2.Identification and clinical implication of a novel variant of SPAG17 gene resulting in Familial severe asthenozoospermia

Objective:To investigate the association between SPAG17 gene variant and Familial severe asthenozoospermia, and to assess its impact on the outcome of intracytoplasmic sperm injection (ICSI). Methods:Two siblings (Probands 1 and 2) with severe asthenozoospermia from a Chinese family who presented at the Reproductive Medicine Center Ⅱ of Gansu Maternity and Child Health Care Hospital (Gansu Provincial Central Hospital) in May 2023 were selected as study subjects. Clinical data were collected, and sperm morphology and ultrastructure (assessed by transmission electron microscopy) were analyzed. Pathogenic variants were screened using whole exome sequencing (WES) and verified by Sanger sequencing. This study was approved by the Medical Ethics Committee of Gansu Maternity and Child Health Care Hospital (Ethics No.: 2023GSFYLS78).Results:Probands 1 and 2 had primary infertility for 10 and 3 years, respectively, and both exhibited normal semen concentration, but the percentage of progressive motile sperm (PR) was significantly lower than the normal reference value (>32.00%), measuring 2.33%±0.58% and 0.80%±0.45%, respectively. Additionally, the percentage of sperms with normal morphology was slightly below the reference range (>4.00%), with the values of 3.36%±0.35% and 2.93%±1.36%. Both probands were found to harbor homozygous c. 2188C>T (p.Q730*) nonsense variant of the SPAG17 gene (NM_206996.4), for which their mother was a heterozygous carrier (their father had already deceased). Both sibs underwent ICSI treatment using a long gonadotropin-releasing hormone agonist protocol during the follicular phase combined with assisted oocyte activation (AOA). The wife of Proband 1 ultimately gave birth to a healthy girl, whilst the wife of Proband 2 delivered two healthy girls.Conclusion:The homozygous c. 2188C>T (p.Q730*) nonsense variant of the SPAG17 gene is closely related with the severe asthenozoospermia phenotype. Live births can be achieved through ICSI combined with AOA technology, though the overall utilizable embryo rate may be relatively low.
3.Survey on the current situation of management of sterilization and supply centers in response to emerging and outbreaks of infectious diseases in 442 hospitals and analysis of influencing factors

Objective To investigate the current situation of emergency response management during new outbreaks of infectious diseases in central sterile supply department(CSSD)of hospitals in 18 provinces(autonomous regions and municipalities),and to provide a reference basis for the establishment of a sound emergency response management system.Methods A convenience sampling method was used to investigate and study the CSSDs in 447 hospitals from June to December 2023 using a self-developed questionnaire.The questionnaire included 3 aspects,including general information,emergency disposal management situation(including environment management,personnel management,material management,and process management),and satisfaction with emergency disposal management,and the relevant factors affecting the satisfaction with the emergency disposal management of CSSDs responding to new outbreaks of infectious disease devices were analyzed.Results A total of 447 questionnaires were recovered,with 442 valid questionnaires and a valid questionnaire recovery rate of 98.88%.In terms of environmental management,CSSDs in 88(19.91%)hospitals set up independent instrument pre-sterilization rooms for infected device disposal.In terms of material management,CSSDs in 42(9.50%)hospitals conducted daily emergency material management.In terms of personnel management,CSSDs in 236(53.39%)hospitals conducted emergency response knowledge assessments.In terms of process management,CSSDs in 225(50.90%)hospitals conducted information traceability system full-process closed-loop management.The results of multiple linear regression analysis showed that whether or not to carry out pre-treatment,emergency supplies stockpiling and management,emergency disposal knowledge assessment,establishment of standard operating procedures,and full-process closed-loop management of the traceability system were the influencing factors affecting the satisfaction of CSSD's emergency disposal management(P<0.05).Conclusion The current situation of CSSD's emergency disposal management of new outbreaks of infectious diseases needs to be improved,and the environmental management of the decontamination area should be strengthened to improve the staff's ability of emergency disposal,guarantee the reserve of emergency supplies,and strengthen the informatization of the whole process management,to reduce the risk of hospital infections and safeguard the quality of medical care.

5.Fufang Kangjiaolv Capsules Treat Anxiety in Rat Model of Chronic Restraint Stress via Microbiota-gut-brain Axis

ObjectiveTo observe the intervention effect of Fufang Kangjiaolv capsules on anxiety-like behaviors in the rat model of chronic restraint stress (CRS) and explore the mechanism underlying the anti-anxiety effect via the microbiota-gut-brain axis. MethodsRats were assigned into blank, model, positive drug (diazepam, 1 mg·kg-1), and low-, medium-, and high-dose (0.75, 1.5, 3 g·kg-1, respectively) Fufang Kangjiaolv capsules groups. After 14 days of administration, the elevated plus maze test, open field test, light and dark box test, and marble burying test were performed. Hematoxylin-eosin staining was employed to observe the pathological changes in the hippocampus and colon of rats, and Nissl staining was conducted to observe the damage of hippocampal neurons. The gut microbiota was analyzed by 16S rRNA gene sequencing. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was employed to determine the mRNA levels of zonula occludens-1 (ZO-1) and occludin in the colon of rats. The levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-1β (IL-1β) in the colon, serum, and hippocampus were determined by enzyme-linked immunosorbent assay. Western blot was employed to determine the protein levels of ZO-1, occludin, nuclear factor-κB p65 (NF-κB p65) in the colon tissue and NF-κB p65 and brain-derived neurotrophic factor (BDNF) in the hippocampal tissue. ResultsCompared with the blank group, the model group showed reductions in the time and frequency ratio of rats entering the elevated plus maze, the time and frequency of rats entering the central area of the open field, the time of entering the open box, the times of passing through the light and dark box, and the number of unburied beads (P<0.05, P<0.01). Compared with the model group, Fufang Kangjiaolv capsules ameliorated the anxiety of the model rats to varying degrees, and the high-dose group had the best effect, with increases in the proportions of time and frequency of rats entering the open arm in the elevated plus maze (P<0.05), the number of rats entering the central area in the open field (P<0.05), the time of entering the open box, the times of passing through the light and dark boxes, and the number of unburied beads (P<0.01). Moreover, the high-dose group showed alleviated pathological damage of hippocampal neurons and colon. The results of 16S rRNA gene sequencing showed that the model group had increased relative abundance of Firmicutes, Deferribacterota, Romboutsia, and Phascolarctobacterium, while it had decreased relative abundance of Bavcteroidota and Lactobacillus. The drug administration groups showed increased relative abundance of Bavcteroidota, Bacteroides, norank f norank o Clostridia UCG-014, and Blautia and decreased relative abundance of Firmicutes and Deferribacterota. Compared with the blank group, the model group showed down-regulated protein and mRNA levels of ZO-1 and occludin in the colon (P<0.01), elevated levels of TNF-α, IL-6, and IL-β in the colon, serum, and hippocampus (P<0.01), up-regulated protein level of NF-κB p65 in the colon and hippocampus (P<0.01), and down-regulated protein level of BDNF in the hippocampus (P<0.05). Compared with the model group, high-dose Fufang Kangjiaolv capsules up-regulated the mRNA levels of ZO-1 and occludin in the colon (P<0.01), lowered the levels of TNF-α, IL-6, and IL-β in the colon, serum, and hippocampus (P<0.01), up-regulated the protein levels of ZO-1 (P<0.01) and occludin (P<0.05) in the colon, down-regulated the protein level of NF-κB p65 in the colon and hippocampus (P<0.05), and up-regulated the protein level of BDNF in the hippocampus. ConclusionFufang Kangjiaolv capsules can reduce the anxiety-like behaviors in the rat model of CRS by regulating the gut microbiota disturbance, up-regulating the expression of tight junction proteins in the colon, repairing intestinal mucosal mechanical barrier, and down-regulating NF-κB/BDNF signaling pathway, thereby reducing peripheral and central inflammation. This study proves the hypothesis that Fufang Kangjiaolv capsules play an anti-anxiety role via the microbiota-gut-brain axis, providing a new idea for further research.

8.Prediction model of insomnia disorder in perimenopausal women based on machine learning method

Objective:To explore the influencing factors of insomnia disorder in perimenopausal women and construct a prediction model of insomnia disorder in perimenopausal women based on machine learning method.Methods:A case-control study was used in this study. A total of 140 perimenopausal women who were examined in Lishui Maternal and Child Health Hospital from January 2019 to June 2021 were selected as the study objects for retrospective analysis by convenient sampling method. They were divided into occurrence and non-occurrence groups based on the presence of insomnia disorders. Relevant data of the patients were collected and risk factors analysis was conducted. Multivariate Logistic regression, decision classification regression tree (CRT) and back propagation neural network (BPNN) algorithm based on machine learning, the prediction model of insomnia disorder in perimenopausal women was constructed.Results:A total of 140 perimenopausal women were included, including 88 patients (62.86%) in the occurrence group, aged (50.16 ± 4.73) years old, and 52 patients (37.14%) in the non-occurrence group, aged (47.33 ± 4.54) years old. Multivariate Logistic regression analysis showed that percapita family monthly income ( OR = 0.019, 95% CI 0.001-0.422, P<0.05), Hamilton Depression Scale (HAMD) score ( OR = 1.665, 95% CI 1.108-2.502, P<0.05) and Self-rating Anxiety Scale (SAS) score ( OR = 1.407, 95% CI 1.085-1.826, P<0.05) of the two groups were independent risk factors for the occurrence of insomnia disorder in perimenopausal women. The prediction model constructed by CRT showed that SAS score, HAMD score and percapita family monthly income were the influencing factors for the occurrence of insomnia disorder in perimenopausal women. The results of BPNN model showed that the importance ranking of influencing factors was SAS score>percapita family monthly income>HAMD score>body mass index>age>work status>daily exercise cumulative time. Among the models constructed by the three machine learning algorithms, the area under the curve of multivariate Logistic regression analysis was 0.998, the sensitivity was 96.6%, the specificity was 100.0%, which had the best predictive performance. Conclusions:In this study, the prediction model of insomnia disorder in perimenopausal women based on machine learning method has good prediction efficiency, among which the multivariate Logistic regression model has the best diagnostic efficiency, and the established prediction model has good prediction accuracy.
9.Baicalin alleviates dermatitis injury of acne rats by suppressing SDF-1/CXCR4 signaling pathway

To investigate the effect of baicalin(BA)on acne-induced dermatitis injury of acne rats through regulating stromal cell-derived factor-1/CXC chemokine receptor 4(SDF-1/CXCR)signaling pathway and its mechanism.The rat model of acne was established,and the rats were divided into control group,model group,BA groups(BA-L,BA-M,BA-H groups,intragastric administration of 25,50,100 mg·kg-1·d-1 BA)and combined groups(intragastric administration of 100 mg·kg-1·d-1 BA+intraperitoneal injection of 7 μg·kg-1·d-1 SDF-1α recombinant protein).The apparent changes of auricle skin tissue were observed and the thickness of auricle was measured.HE and Masson staining were used to observe the pathological changes and fibrosis of auricle skin;ELISA was used to detect levels of IL-1β,IL-6,IL-8 and TNF-α in serum;immunofluorescence was used to detect the expression of NLRP3 in auricle skin;Western blot was used to detect the level of SDF-1 and CXCR4 proteins in auricle skin.Compared with the control group,the auricle of the model group was red and swollen,papules and telangiectasia were obvious,the stratum corneum of the ear was thickened,inflammatory cells infiltrated and the size of sebaceous gland was increased,and the hair follicle sebaceous gland was excessively keratinized.Furthermore,rats in model group demonstrated increases in thickness of auricle skin tissue,fibrotic area,hydroxyproline content,levels of IL-1β,IL-6,IL-8,and TNF-α,positive expression rate of NLRP3,and expression of SDF-1 and CXCR4 proteins(P<0.05).BA could reverse these changes mentioned above in model rat(P<0.05),while SDF-1α recombinant protein could suppress the effect of BA-H on the above indexes(P<0.05).In conclusion,BA may alleviate dermatitis injury in acne rats by inhibiting SDF-1/CXCR4 signaling pathway.

To investigate the effect of baicalin(BA)on acne-induced dermatitis injury of acne rats through regulating stromal cell-derived factor-1/CXC chemokine receptor 4(SDF-1/CXCR)signaling pathway and its mechanism.The rat model of acne was established,and the rats were divided into control group,model group,BA groups(BA-L,BA-M,BA-H groups,intragastric administration of 25,50,100 mg·kg-1·d-1 BA)and combined groups(intragastric administration of 100 mg·kg-1·d-1 BA+intraperitoneal injection of 7 μg·kg-1·d-1 SDF-1α recombinant protein).The apparent changes of auricle skin tissue were observed and the thickness of auricle was measured.HE and Masson staining were used to observe the pathological changes and fibrosis of auricle skin;ELISA was used to detect levels of IL-1β,IL-6,IL-8 and TNF-α in serum;immunofluorescence was used to detect the expression of NLRP3 in auricle skin;Western blot was used to detect the level of SDF-1 and CXCR4 proteins in auricle skin.Compared with the control group,the auricle of the model group was red and swollen,papules and telangiectasia were obvious,the stratum corneum of the ear was thickened,inflammatory cells infiltrated and the size of sebaceous gland was increased,and the hair follicle sebaceous gland was excessively keratinized.Furthermore,rats in model group demonstrated increases in thickness of auricle skin tissue,fibrotic area,hydroxyproline content,levels of IL-1β,IL-6,IL-8,and TNF-α,positive expression rate of NLRP3,and expression of SDF-1 and CXCR4 proteins(P<0.05).BA could reverse these changes mentioned above in model rat(P<0.05),while SDF-1α recombinant protein could suppress the effect of BA-H on the above indexes(P<0.05).In conclusion,BA may alleviate dermatitis injury in acne rats by inhibiting SDF-1/CXCR4 signaling pathway.
