1.Exploration on the effects of Zhiganqing Prescription on insulin resistance and gluconeogenesis in NAFLD mice based on the PI3K/Akt/FoxO1 signaling pathway

Objective:To observe the protective effects of Zhiganqing Prescription on the liver of C57BL/6J non-alcoholic fatty liver disease (NAFLD) mice induced by high fat diet and its effects on PI3K/Akt/FoxO1 signaling pathway, insulin resistance (IR) and gluconogenesis.Methods:A total of 48 8-week-old male C57BL/6J mice were divided into control group ( n=8) and modeling group ( n=40) according to random number table method. The control group was fed with ordinary diet, and the model group was fed with high-fat diet. The NAFLD model was established after 8 weeks of feeding. The modeling group was divided into model group, Pioglitazone group, Zhiganqing Prescription low-, medium-, and high-dosage group ( n=8 in each group) according to random number table method, and drug intervention lasted for 8 weeks. The body mass of mice was measured regularly during administration. Fasting blood glucose (FBG) levels were measured at 0 and 8 weeks of administration, and oral glucose tolerance tests (OGTT) were conducted. After the experiment, serum levels of GPT, GOT, TC, TG, LDL-C, HDL-C, FINS and C-P were detected and HOMA-IR was calculated. The pathological morphology of liver was observed by HE and PAS staining. The expression levels of PI3K and p-Akt were detected by IHC staining. The protein expression levels of PI3K, Akt, p-Akt, FoxO1, p-FoxO1, G6PC and PCK1 were detected by Western blot. Results:Compared with model group, the body weight of mice in each administration group decreased at 4, 6 and 8 weeks ( P<0.05 or P<0.01). At the 8th week of administration, the levels of FBG and OGTT AUC in each administration group decreased ( P<0.05 or P<0.01), the levels of GPT, TC, TG and LDL-C decreased ( P<0.01), and the GOT levels in Zhiganqing Prescription medium- and high-dosage groups decreased ( P<0.01). The HDL-C level in Zhiganqing Prescription medium-dosage group decreased ( P<0.05 or P<0.01), and the HOMA-IR level in Zhiganqing Prescription low- and medium-dosage groups decreased ( P<0.05 or P<0.01). The levels of FINS and C-P in each administration group increased ( P<0.05 or P<0.01), and the expressions of PI3K protein and p-Akt/Akt, p-FoxO1 /FoxO1 protein in liver tissues increased ( P<0.05 or P<0.01). The protein expressions of G6PC and PCK1 decreased ( P<0.05 or P<0.01). Conclusion:Zhiganqing Prescription can effectively control the body mass, blood glucose, liver function and blood lipids of NAFLD mice, improve IR and gluconeogenesis, the mechanism of which may be related to the activation of PI3K/Akt/FoxO1 signaling pathway.
2.Echocardiographic manifestations of infective endocarditis complicated with valve damage in children

Objective To observe the echocardiographic manifestations of infective endocarditis(IE)complicated with valve damage in children.Methods Totally 104 children with IE were retrospectively enrolled and divided into non-damage group(n=34),mild damage group(n=39)and dysfunction group(n=31)according to whether complicated with valve damage and damage's degree.The general and echocardiographic data were compared among groups,and the echocardiographic characteristics of IE complicated with valve damage in children were analyzed.Results Significant difference of the proportion of combining with other congenital heart diseases(excluding bicuspid aortic malformations),the incidence of embolization events during hospital stay,also of endocarditis of left cardiac system were found among groups(all P<0.05).Pairwise comparison showed that in non-damage group,the proportion of combining with other congenital heart diseases was higher,while the incidence of endocarditis of left cardiac system was lower than those in both mild damage group and dysfunction group(all P<0.05).The incidence of embolization events during hospital stay in non-damage group was lower than that in dysfunction group(P<0.05).Among 70 cases of IE complicated with valve damage,mitral valve(30/70,42.86%)was the most common involved valve,mostly presented as valve stenosis(63/70,90.00%).No significant difference of valve involvement site,valve structural lesions nor the incidence of valve stenosis was found between mild damage group and dysfunction group(all P>0.05).Conclusion IE complicated with valve damage in children mostly involved left cardiac system,and the risk of embolization events was higher than that of IE children without valve damage.Echocardiography could be used as an important method for evaluating the site of valve involvement and the degree of damage.
3.Investigation results of iodine in drinking water in Henan Province from 2017 to 2020

Objective:To investigate the iodine content in drinking water of residents in Henan Province, and clarify the distribution characteristics of water iodine in Henan Province.Methods:In 2017, in all counties (cities and districts, hereinafter referred to as counties) of Henan Province, taking township (town, subdistrict office, hereinafter referred to as township) as the unit to carry out an investigation of iodine content in drinking water; and in the township with water iodine content of 10 μg/L or more, taking administrative village (neighborhood committee, hereinafter referred to as the administrative village) as the unit to carry out the drinking water iodine content investigation. Supplementary investigation was conducted from 2018 to 2020 in administrative villages where water iodine levels had never been tested or had not been tested after replacing water sources. At least 25 ml water samples were collected at each sampling site, and the water iodine content was determined by cerous sulfate catalytic spectrophotometry.Results:From 2017 to 2020, the median water iodine in Henan Province was 8.20 μg/L. A total of 50 124 administrative villages in 2 465 townships, 160 counties and 18 provincial-level cities were investigated for iodine content in drinking water, of which 65.5% (32 807/50 124) of the administrative villages had a median water iodine < 40 μg/L, belonging to iodine deficiency area; 16.9% (8 473/50 124) of the administrative villages had a median water iodine of 40-100 μg/L, suitable for iodine; and 17.6% (8 844/50 124) of the administrative villages had a median water iodine > 100 μg/L, belonging to water source high iodine area.Conclusions:Henan Province as a whole is at the state of iodine deficiency in the external environment. Most administrative villages are iodine deficiency areas. There are certain proportion of water source areas with high iodine and areas with suitable iodine.
4.Research advances in the protective effect of sulforaphane against liver injury and related mechanisms

Sulforaphane is a phytochemical with a variety of biological activities that exists widely in Cruciferae plants. This article summarizes the recent experimental studies of sulforaphane in the treatment of various types of liver injury in China and globally and reviews the role and mechanism of sulforaphane in protecting against liver injury. Based on the experimental animal models of liver injury, this article summarizes the therapeutic effect of sulforaphane on the models of chemical liver injury, drug-induced liver injury, alcoholic liver injury, immunological liver injury, and ischemia/reperfusion liver injury and analyzes the mechanism of action of sulforaphane in improving experimental liver injury, so as to provide a reference for in-depth research on sulforaphane in protecting against liver injury.
5.An intervention study on the effects of vitamin D 3 on cognitive function and blood inflammatory cytokines in patients with Alzheimer's disease

Objective:To assess the effects of a 12-month vitamin D 3 intervention on cognitive function and blood inflammatory cytokines in subjects with Alzheimer's disease(AD). Methods:Two hundred and ten AD patients were randomly divided into the intervention and control groups, with 105 patients in each group.The intervention group received vitamin D 3 800 U/day and the control group received a placebo for 12 months.Repeated-measures analysis of variance was used to evaluate the effects of vitamin D and placebo on cognitive performance and blood inflammatory cytokines at baseline, month 6 and month 12. Results:During the follow-up, serum 25(OH)D 3( F=712.086, P<0.001)and 1, 25(OH) 2D 3( F=372.038, P<0.001)concentrations in the intervention group were significantly higher than those in the control group.Changes in FSIQ, VIQ and PIQ between the groups, between different time points and with interaction between the variables taken into account were statistically significant(all P<0.001). Compared with the control group, interleukin-6(IL-6)( F=341.344, P<0.001)and tumor necrosis factor-α(TNF-α)( F=9.580, P<0.001)levels in the intervention group were significantly decreased, and the ratio of Aβ40 to Aβ42 was increased( F=4.617, P=0.010). There were interaction effects between group and time.Except for intervention effects, the baseline concentration of 25(OH)D 3 was positively correlated with scores of FSIQ( β=0.016, P=0.031)and VIQ( β=0.017, P=0.031). Conclusions:Daily oral vitamin D 3 supplementation(800 U/day)for 12 months may improve cognitive function and change inflammatory cytokines in elderly patients with AD.
6.The value of echocardiography in the diagnosis of persistent fifth aortic arch and its classification

Objective:To study the classification of persistent fifth aortic arch (PFAA) and the value of echocardiography in the diagnosis of PFAA.Methods:A total of 16 cases (male 6, female 10, at ages from 7 days to 4 years and 2 months old, the median age was 3 months) diagnosed with PFAA in Beijing Children′s Hospital Affiliated to Capital Medical University from January 2013 to June 2019 were studied retrospectively. The diagnosis standard, differential methods and misdiagnosed analysis of different subtypes of PFAA by echocardiography were summarized and analyzed.Results:The 16 cases included 1 case of type A1 double lumen aortic arch, 8 cases of type A2 single-lumen aortic arch, 3 cases of type B1 with pulmonary atresia and 4 cases of type B3 pulmonary artery branch arising from the distal end of ascending aorta. Only one patient of double lumen aortic arch missed diagnosis by echocardiography, and the rest were accurately diagnosed by echocardiography. CTA was performed in 13 cases, including 9 cases of type A, 1 case of type B1 and 3 cases of type B3, which confirmed the echocardiography diagnosis. Seven cases of Type A2 were operated.Conclusions:PFAA is a rare and complicated aortic arch malformation, which is divided into four major classification and multiple subtypes. Echocardiography can diagnose the PFAA and its classification, it is of great clinical significance for the early diagnosis, treatment and prognosis of children.
7.Research hotspots and trends analysis of traditional Chinese medicine nursing for stroke based on co-word analysis

Objective:To explore the research hotspots and trends of traditional Chinese medicine (TCM) nursing for stroke.Methods:Literatures on TCM nursing for stroke were retrieved in Chinese National Knowledge Infrastructure (CNKI) , WanFang Data, VIP, China Biological Medicine (CBM) from building database to September 2018. The data management tools included BICOMB 2.0, SPSS 22.0 and UCINET 6.0. The research hotspots and trends of TCM nursing for stroke were analyzed with the methods of co-word analysis, clustering analysis and social network analysis.Results:A total of 7 253 literatures were included involving 17 236 keywords and 30 high-frequency keywords. Clustering analysis showed that research hotspots of TCM nursing for stroke focused on the application of auricular buried beans in symptomatic nursing for stroke, negative emotion management of stroke, body function treatment after stroke and its research on model and TCM rehabilitation appropriate technology of stroke sequela. Social network analysis showed that scope and quantity of research hotspots of TCM nursing for stroke expanded year by year, exploration field was excavated and deepened constantly and inheritance emerged gradually.Conclusions:Co-word, clustering and social network analysis objectively reflects the research hotspots and trends of TCM nursing for stroke which could provide the information support for researchers and clinical workers and provide a reference for research selected topic as well as clinical practice.
8.Current status of evidence-based health education for postdialysis fatigue

Objective:To investigate the gap between the health education and the best evidence among nurses and patients with postdialysis fatigue (PDF) , find out the problems and analyze the reasons, and provide a reference and basis for future clinical nursing decisions among the dialysis room nurses.Methods:By searching the evidence-based guideline websites and integrating the best evidence comprehensively, a health knowledge questionnaire for dialysis room nurses and patients with PDF was made. In December 2018, a total of 7 nurses and 56 patients in the dialysis room of Dongzhimen Hospital, Beijing University of Chinese Medicine were selected by purpose sampling method, and they were investigated by the health knowledge questionnaire for dialysis room nurses and patients.Results:The score of the PDF knowledge questionnaire for the 7 nurses was (56.43±4.37) , with a highest score of 64 and a lowest score of 51, and the standard-reaching rate was 28.57% (2/7) . The score of the PDF knowledge questionnaire for the 56 patients was (52.41±9.54) , with a highest score of 70 and a lowest score of 20, and the standard-reaching rate was 28.57% (16/56) . Patients and nurses received verbal health education and small lectures in the department, and there was no unified education content.Conclusions:There is no unified and scientific education content nor standardized education model. Evidence-based health education should be carried out for nurses to improve the awareness of nurses and patients in this regard, thereby reducing patients' fatigue.
9.Analysis of extreme obesity in two pedigrees due to leptin receptor mutation

This study reported two women with extreme obesity who underwent metabolic surgery due to their mutations in leptin receptor (LEPR).Genomic DNA was extracted from the anticoagulant blood samples of the two patients and their parents.A panel of genes related to metabolic diseases or whole exon sequencing was screened and the results were confirmed by Sanger sequencing.This is the first time that these three mutations in LEPR were reported.Two patients complained insatiety and early-onset obesity since childhood at clinics.Patient 1 was a 39-year-old woman with height 150 cm,weight 130 kg,and BMI 57.8 kg/m2.Serum leptin level was 156.4 μg/L.A homozygous mutation of c.2317G>T was found in exon 15 of LEPR gene in patient 1,which was descended from her father and mother respectively.Patient 2 was a 37-year-old woman with height 158 cm,weight 167 kg,and BMI 67 kg/m2.Serum leptin level was 193.4 μg/L.Genetic analysis showed compound heterozygous mutations of c.1482delT and c.1892C > A.Her father showed heterozygous c.1482delT mutation,and her mother carried heterozygous c.1892C > A mutation.Two patients all underwent metabolic surgery with body weight reduction of about 22 kg and 40 kg respectively after first six months.However,the follow-up studies showed that the body weight of patient 1 rebounded to pre-surgery level in two years and patient 2 did not further lose weight in the following six months.
10.Analysis of the research hotspots of traditional Chinese medicine nursing of diabetes mellitus based on data mining and co-word analysis

Objective? To understand and analyze the research status of traditional Chinese medicine nursing for diabetes mellitus, and to provide reference for its clinical application and research direction. Methods? China Journal Full Text Database (CNKI), VIP database and WanFang Database were searched to get the relevant articles on diabetes Traditional Chinese medicine nursing published from the establishment date to December 2018. Data mining and keyword clustering analysis were used to analyze the literature features and research hotspots. Results? Finally, 1 963 articles related to traditional Chinese medicine nursing of diabetes mellitus were included, and 2 389 keywords and 27 high frequency keywords were obtained. Bibliometric analysis showed that the amount of relevant research fluctuated and increased year by year. Cluster analysis result revealed that the research of traditional Chinese medicine nursing of diabetes mainly focused on the nursing of elderly patients in community, the nursing of diabetic complications, the application of traditional Chinese medicine nursing technology and the evaluation of traditional Chinese medicine nursing program. Conclusions? Diabetes traditional Chinese medicine nursing has made some progress in the traditional field, but the expansion and innovation of the research still need to be strengthened. In the later stage, we should attach importance to the exploration of talent training mode, encourage the development of basic theory and interdisciplinary exchanges and cooperation, adhere to the combination of inheritance and innovation, and improve the scientific nature of diabetes traditional Chinese medicine nursing.
