BACKGROUND:The level of peripheral erythrocytes in rats is significantly increased under hypoxia exposure, and the proliferation of nucleated erythrocytes in the bone marrow may be one of the direct causes of the increase in peripheral erythrocytes.Previous studies have focused on the effects of factors such as erythropoietin and hypoxia-inducible factor, but little research has been done on related factors such as inflammation and immunity.OBJECTIVE:To study the expression of interleukin-18 in bone marrow nucleated erythrocytes, bone marrow supernatant and peripheral blood of rats after hypoxia exposure, and to explore the possible role of interleukin-18 in the pathogenesis of chronic mountain sickness.METHODS:Sixteen healthy male SD rats were randomly divided into two groups:the experimental group was kept in a hypobaric oxygen chamber at a simulated altitude of 5 000 m for 28 days, and the control group was kept in a laboratory at an altitude of 2 260 m for 28 days. The blood routine tests of the two groups of rats were performed. The proportion of CD71+nucleated erythrocytes in the bone marrow of the two groups of rats was determined by flow cytometry. The expressions of interleukin 18 mRNA and protein in CD71+nucleated erythrocytes in the bone marrow of the two groups of rats were determined by RT-qPCR and western blot assay. The expressions of interleukin 18 protein in the sternum of the two groups of rats were determined by immunofluorescence. The levels of interleukin 18 in the peripheral blood and bone marrow supernatant of the two groups of rats were determined by ELISA.RESULTS AND CONCLUSION:(1)The indexes of erythrocyte count, hemoglobin, hematocrit, and mean hemoglobin content in peripheral blood of the experimental group were higher than those of the control group(P<0.05).(2)The proportion in bone marrow CD71+erythroblasts was significantly higher in the experimental group than that in the control group(P<0.05).(3)RT-qPCR results showed that the expression of interleukin 18 mRNA in CD71+nucleated erythrocytes in the bone marrow of rats in the experimental group was significantly higher than that in the control group(P<0.05).(4)Western blot assay results showed that the expression of interleukin 18 protein in CD71+nucleated erythrocytes in the bone marrow of rats in the experimental group was significantly higher than that in the control group(P<0.05).(5)The immunofluorescence results showed that the expression of interleukin 18 protein in the sternum of rats in the experimental group was significantly higher than that in the control group(P<0.05).(6)ELISA results exhibited that the level of interleukin 18 in the serum of rats of the experimental group was higher than that in the control group(P<0.05),but the level of interleukin 18 in the bone marrow supernatant of rats in the experimental group was lower than that in the control group(P<0.05).The results indicate that the increased expression of interleukin 18 in bone marrow CD71+erythroblasts and peripheral blood of rats under hypobaric hypoxia may be involved in the proliferation of erythroblasts in bone marrow.

Journal of Traditional Chinese Medical Sciences 2024;11(3):357-362
Objective:
To observe and compare the clinical effects of different electroacupuncture waveforms on primary dysmenorrhea.
Methods:
This was a prospective, randomized, three-group, parallel-controlled trial. Participants with primary dysmenorrhea were randomly divided into dense-sparse wave, continuous wave, and discontinuous wave groups in a 1:1:1 ratio. Two lateral Ciliao (BL 32) points were used. All three groups started treatment 3–5 days before menstruation, once a day for six sessions per course of treatment, one course of treatment per menstrual cycle, and three menstrual cycles. The primary outcome measure was the proportion with an average visual analog scale (VAS) score reduction of ≥50% from baseline for dysmenorrhea in the third menstrual cycle during treatment. The secondary outcome measures included changes in dysmenorrhea VAS scores, Cox Menstrual Symptom Scale scores and the proportion of patients taking analgesic drugs.
Results:
The proportion of cases where the average VAS score for dysmenorrhea decreased by ≥50% from baseline in the third menstrual cycle was not statistically significant (P > .05). Precisely 30 min after acupuncture and regarding immediate analgesia on the most severe day of dysmenorrhea, there was a statistically significant difference in the dense-sparse wave group compared with the other two groups during the third menstrual cycle (P < .05). Additionally, there was a statistically significant difference between the dense-sparse wave and discontinuous wave groups 24 h after acupuncture (P < .05).
Conclusions
Waveform electroacupuncture can alleviate primary dysmenorrhea and its related symptoms in patients. The three groups showed similar results in terms of short- and long-term analgesic efficacy and a reduction in the number of patients taking analgesic drugs. Regarding achieving immediate analgesia, the dense-sparse wave group was slightly better than the other two groups.

The course, progression, and prognosis of fetal congenital cystic adenomatoid malformation (CCAM) depend on the size of the mass, whether the mediastinum is displaced, changes in fetal hemodynamics, and the occurrence of fetal hydrops, with large CCAM being relatively rare. In this case, a fetus was suspected of having a large CCAM with fetal hydrops and cardiac insufficiency at 27 weeks of gestation. After multiple imaging evaluations and multidisciplinary consultations, a delivery plan and neonatal resuscitation plan were formulated. At 31 +4 weeks of gestation, in-utero thoracentesis and drainage were performed. Postoperatively, the fetal CCAM was reduced compared to before, and the maternal symptoms of chest tightness and shortness of breath were also improved. Three days after the procedure, the mother experienced premature rupture of membranes and delivered a male infant vaginally at 32 weeks of gestation. On the fifth day after birth, the newborn underwent resection of the pulmonary cystic adenomatoid malformation, confirming the diagnosis. Follow-up for two years post-birth showed good prognosis for both the mother and the child. Therefore, when CCAM is detected during pregnancy, thorough in-utero evaluation should be conducted, and a monitoring plan should be developed based on potential perinatal conditions to avoid unnecessary termination of pregnancy. If the pregnancy continues, multidisciplinary evaluation and preparation for perinatal surgery are necessary.

Objective:To screen and analyze the differentially expressed genes between lacrimal gland benign lymphoepithelial lesions (LGBLEL) and mucosa-associated lymphoid tissue (MALT) lymphoma. Methods:A cross-sectional study was performed.Ten consecutive patients were included in Beijing Tongren Hospital Affiliated to Capital Medical University from January 2015 to November 2017, including five patients with LGBLEL and five patients with MALT lymphoma.Clinical data and peripheral blood sample were collected from each patient.DNA was extracted from peripheral blood. The whole-exome sequencing (WES) was employed for gene sequencing. The BWA software was used for the screen of differentially expressed gene; GATK software was used to detect genomic variation; ANNOVAR software was used to annotate and predict the effects of the variation; Varscan software was used to analyze single nucleotide polymorphisms (SNPs) and insertion-deletions (InDels), and ExomeCNV software was used to identify copy number variations (CNVs). The mutated hub gene with the maximal clique centrality was screened out by the analysis of protein interaction network and construction of functional module network. This study was approved by an Ethics Committee of Beijing Tongren Hospital Affiliated to Capital Medical University.Written informed consent was obtained from each patient prior to any medical examination. Results:There was 16.63 Gb sequencing data per sample on average.Synonymous mutation and missense mutation were the most common SNPs mutation types in the LGBLEL group and MALT lymphoma group, and no significant difference was found in gene mumber of synonymous mutation and missense mutation between the two groups. The number of terminating codon missing mutation genes in the LGBLEL group was more than that in the MALT lymphoma group ( P<0.05). The most common InDels types were frameshift mutation, non-frameshift insertion and non-frameshift deletion, and there was no significant difference in gene number of InDels between the LGBLEL group and MALT lymphoma group. The number of exon CNVs was few in both two groups and showed no significant influence in final result.Six differentially expressed hub genes were found, including IGFN1, TCP10, SLC45A4, BTBD7, PHGR1 and PIEZ02. Conclusions:IGFN1, TCP10, SLC45A4, BTBD7, PHGR1 and PIEZ02 genes may participate in the development of LGBLEL into MALT lymphoma.

Background The residual astigmatism following Toric intraocular lens (IOL) rotation have received much attention. However, the variation of the optical performance and the wavefront abrrveation with Toric IOL rotation are unclear. Objective The aim of this study was to evaluate the optical performance, wavefront abrrveation and residual diopter spherical and cylinder lens with Toric IOL rotation. Methods T3,T4 and T5 Toric IOLs of +22.0 D were placed in Hwey-Lan Liou model eye respectively, with the posterior surface flat on the X axis and steep on the Y axis.Corneal astigmatism model was established by mimicing the model eye with Toric IOL using Zemax optical software. Then the Toric IOLs were rotated 5° to 10° individually under the 4 mm pupil diameter and 550 nm monochromatic light, and the image performance and wavefront abrrveation were recorded with all conditions, including modulation transfer function (MTF),out-of-focus aberration, astigmatism aberration, coma, trefoil aberration and spherical aberration. The refractive error of spherical power and cylinder power were calculated. Results Corneal astigmatism was fully corrected when Toric IOLs in the middle, and the MTF curves were near in T3,T4,T5 Toric IOLs.The image performance was worse under the high spatial frequency with the increase of rotation degrees of Toric IOLs,showing the gradually low of MTF curves, especially T5 Toric IOL.No obvious changes was seen in coma, trefoil aberration and spherical aberration after rotation of Toric IOLs,while out-of-focus aberration, astigmatism aberration were obviously increased. In addition, residual astigmatism and spherical error increase with the rotation of Toric IOLs.Conclusions Toricl IOL rotation leads to increase of astigmatism and spherical refractive error but not high order aberration.
