1.Practice of clinical pharmacist participating in the treatment of a case of mixed shock caused by severe scrub typhus

OBJECTIVE To provide valuable insights for the adjustment of anti-infectious regimens, identification of adverse reactions, and individualized pharmaceutical care in patients with critically severe scrub typhus. METHODS Clinical pharmacists actively participated in the pharmaceutical care process for a patient with severe scrub typhus leading to mixed shock undergoing continuous renal replacement therapy and extracorporeal membrane oxygenation. Initially, the patient received meropenem (1 g, q12 h, ivdrip), in combination with doxycycline (0.1 g, q12 h, po), which was later switched to meropenem (1 g, q8 h, ivdrip) along with omacycline (100 mg, qd, ivdrip) due to impaired gastrointestinal function. However, as the patient’s condition progressively deteriorated and the infection became uncontrolled, the clinical pharmacists recommended that the clinicians adjust the anti-infective regimen to meropenem (2 g, q8 h, ivdrip) combined with tigecycline (100 mg for first dose; 50 mg, q12 h for maintenance; ivdrip). The clinicians followed the advice of the clinical pharmacists. After treatment, the patient’s symptoms exhibited significant improvement, accompanied by a notable decrease in inflammatory markers, indicating that the infection had been successfully controlled. However, due to continuously increasing bilirubin levels, in order to reduce the risk of drug-induced liver injury, the clinicians changed tigecycline to azithromycin (0.5 g, qd, ivdrip) following the recommendation of the clinical pharmacists. RESULTS Ultimately, metagenomic next-generation sequencing of the bronchoalveolar lavage fluid and blood specimens indicated that Orientia tsutsugamushi had been completely eradicated in the patient. CONCLUSIONS Tigecycline may be a viable therapeutic choice for patients with severe scrub typhus. In the context of critically ill patients with scrub typhus, combining tigecycline with azithromycin might potentially enhance the efficacy in eliminating Orientia tsutsugamushi.
2.Development of the Social Isolation Scale for people with type 2 diabetes mellitus and its reliability and validity test

Objective:The Social Isolation Scale for people with type 2 diabetes mellitus (T2DM) was developed and tested for reliability and validity, which provided an effective tool for measuring the social isolation level of patients with T2DM.Methods:The initial scale was developed through literature review, qualitative interviews, and expert consultation. Convenience sampling method was used to select T2DM patients who met the inclusion and exclusion criteria for questionnaire survey. The item analysis method was used to select the items of the scale. Exploratory and confirmatory factor analyses were used to evaluate construct validity. The content validity of the scale was evaluated by the scale-level content validity index and the item-level content validity index. The criterion validity was verified by using the Lubben Social Network Scale-6 and the De Jong Gierveld Loneliness Scale. Reliability was tested through internal consistency and test-retest reliability.Results:Through literature review and qualitative interviews, 30 items of the scale were selected to form the first version scale. After two rounds of inquiries from 16 experts (expert authority coefficient = 0.897), the second version of the scale was formed. A total of 407 questionnaires were distributed in two stages using the second version scale. Among the 407 patients, there were 214 males and 193 females. Exploratory factor analysis extracted 4 common factors, with a cumulative variance contribution rate of 61.338%. The final scale was determined to include 4 dimensions and 16 items, with the dimensions being "social support network", "frequency of social participation", "satisfaction with interpersonal relationships", and "diabetes-related sense of isolation".The average content validity index at the scale level was 0.929, the content validity index at the item level was 0.830 - 1.000, and the criterion validity was - 0.647 and 0.681. The Cronbach′s α coefficient of the total scale was 0.822, and the test-retest reliability was 0.858.Conclusions:The Social Isolation Scale for people with T2DM has good reliability and validity. It is a reliable and valid tool for assessing social isolation in T2DM patients.
3.Occurrence risk establishment of predictive mode of surgically induced astigmatism in elderly patients with diabetic cataract

Objective To construct and verify the risk profile model of surgically induced astigmatism(SIA)in elderly patients with diabetic catarac.Methods A total of 309 patients admitted to our hospital for phacoemulsification were selected as the study objects.Patients were divided into the SIA group(n=158)and the non-SIA group(n=151)according to whether SIA occurred after surgery.Multivariate Logistic regression analysis was used to analyze the factors influencing the occurrence of SIA in diabetic cataract patients,and the nomogram model of SIA in diabetic cataract patients was constructed.ROC curve,calibration curve and decision curve were used for evaluation and verification of the model.Results The patient age and the lens nucleus hardness level Ⅲ-Ⅳ,incision size≥3.00 mm,incision shape as trapezoidal transparent corneal incision,corneal incision length≥1.75 mm and horizontal diameter of the cornea were significantly higher in the SIA group than of the non-SIA group(P<0.05).There were no significant differences in gender,body mass index(BMI),duration of diabetes,history of hypertension,history of cardiovascular disease,grading of diabetic retinopathy,axial length,anterior chamber depth,central corneal thickness,total corneal refractive power,astigmatism axis position and ultrasound phacoemulsification time between the two groups of patients(P>0.05).Multivariate Logistic regression results showed that incision size,incision shape,corneal incision length and corneal horizontal diameter were the factors affecting the occurrence of SIA in diabetic cataract patients(P<0.05).The R software was used to incorporate four influencing factors to generate a nomogram model that could be used to predict the occurrence of SIA in diabetic cataract patients.ROC curve analysis results showed that the area under the curve(AUC)value of this histogram to predict SIA in diabetic cataract patients was 0.810(95%CI=0.761-0.860).Hosmer-Lemeshow test and calibration curve analysis showed that the model fit well(χ2=11.142,P=0.194).The results of decision curve analysis showed that when the probability of high risk threshold 0.35-0.8,the clinical net benefit rate of the column graph model was greater than 0.Conclusion Incision size,incision shape,corneal incision length and corneal horizontal diameter are influential factors for the occurrence of SIA in patients with diabetic cataract.Based on the above influential factors,the columniogram model for predicting the occurrence risk of SIA in patients with diabetic cataract has good differentiation,accuracy and clinical applicability.

5.Role of Traditional Chinese Medicine in Pathogen-Induced Metabolic Reprogramming and Immune Suppression

Traditional Chinese medicine(TCM),an essential component of the traditional medicine practiced in China,has demonstrated unique therapeutic efficacy in combating infectious diseases caused by pathogens and various types of tumors.In particular,TCM plays a vital role in enhancing immune function,maintaining homeostasis,and improving metabolic balance.However,the complex ingredients used in TCM and its broad range of therapeutic targets present challenges for comprehensive analysis of the mechanisms involved.Moreover,the molecular and cellular mechanisms underlying TCM's effects remain underexplored,limiting its broader application in modern medicine.Recent studies have increasingly revealed that TCM can not only directly inhibit the activity of pathogens,such as bacteria,fungi,viruses,and tumor cells,but also exert profound effects on immune remodeling by regulating the metabolism of both pathogens and hosts.Therefore,a comprehensive understanding of the role of TCM in metabolic regulation is crucial for elucidating its anti-pathogenic effects.This review is focused on the metabolic pathways of pathogens and host metabolic reprogramming induced by pathogens.We systematically reviewed the mechanisms by which TCM regulates pathogen metabolism,influences pathogen-induced metabolic reprogramming in hosts,and mitigates immune suppression caused by pathogens.This review may provide new ideas for investigating the molecular mechanisms of TCM in disease development and progression.
6.Mechanisms of Neiyiting Decoction in Preventing Postoperative Recurrence of Endometriosis by Inhibiting Macrophage M1 Polarization Through the TREM1/TLR4/NF-κB Signaling Pathway

Objective The high post-surgery recurrence rate of endometriosis(EMs)has emerged as a challenge in the long-term manaagement of the condition.This study is aimed at investigating the mechanisms of Neiyiting(NYT)decoction in preventing postoperative recurrence of EMs.Methods An animal model of EMs postoperative recurrence and a model of endometrial stromal cells(hEM15A)cocultured with macrophages(RAW 264.7 cell line)were established for both in vivo and in vitro experiments.An autotransplantation method was used to establish a rat model of EMs.The rats were divided into 4 groups(6 rats per group)and received the corresponding treatments:a Model group receiving distilled water,a Gestrinone group receiving gestrinone at 0.325 mg/kg,a low-dose NYT(NYT-L)group receiving NYT decoction at 5.04 g/(kg-d),and a high-dose NYT(NYT-H)group receiving NYT decoction at 10.08 g/(kg-d).The treatment was administered for 3 weeks via intragastric gavage.In addition,6 SD rats were randomly selected for the control group(Control group),and were given distilled water for 3 weeks via intragastric gavage.The sizes and pathological changes of recurrent lesions in EMs rats were observed.Immunohistochemistry and qRT-PCR were performed to assess the expression of M1 macrophage marker CD86 protein and mRNA in vivo.Additionally,immunohistochemistry and qRT-PCR were used to assess the expression of indicator proteins related to the triggering receptor expressed on myeloid cells 1(TREM1)/Toll-like receptor 4(TLR4)/nuclear factor kappa B(NF-κB)signaling pathway and mRNA.The proliferation of hEM15A cells in the coculture experiment was observed.Flow cytometry was performed to determine the polarization of RAW264.7 macrophages,and qRT-PCR was used to determine the expression levels of inducible nitric oxide synthase(iNOS)and interleukin 1β(IL-1β)mRNA.Western blot was performed to determine the expression of signaling pathway-related indicator proteins in vitro.ELISA was performed to determine the levels of inflammatory factors in vitro.Results Compared with the Model group,the volume of recurrent lesions in the NYT-H group was reduced(P<0.01).Findings from the macrophage M1 polarization assessment showed that the expression levels of CD86 protein and mRNA in the recurrent lesions of the Model group were higher than those in the control group(P<0.01).The expression levels of CD86 protein and mRNA in the recurrent lesions of the NYT-H group were lower than those of the Model group(P<0.01).In addition,the RAW 264.7 cell experiment further verified that NYT decoction could reduce the number of CD86-positive macrophages induced by plasmids overexpressing TREM1 and reduce the expression of IL-1β and iNOS mRNA(P<0.01).The results of the hEM15A cell proliferation assay showed that NYT decoction down-regulated KI-67 protein expression in hEM15A cells induced by macrophage M1 polarization(P<0.01).The results of TREM1/TLR4/NF-κB signaling pathway showed that the protein and mRNA expression levels of TREM1,TLR4,and NF-κB in the recurrent lesions of the Model group were higher than those of the control group(P<0.01).Compared with those in the Model group,the protein and mRNA expression levels of TREM1,TLR4,and NF-κB in the recurrent lesions of the NYT-H group were lower(P<0.01).In addition,the coculture experiment of RAW264.7 and hEM15A cells further confirmed that NYT decoction reduced the expression of TREM1,TLR4,and P-P65 proteins(P<0.01).Conclusion NYT decoction can inhibit macrophage M1 polarization through the TREM1/TLR4/NF-κB signaling pathway,improve the inflammation level,and inhibit the formation of ectopic endometrial lesions,thereby preventing postoperative recurrence of EMs.

8.Research on the association between tetracyclines use during pregnancy and congenital malformations in offspring

Objective:To explore the association between the use of tetracyclines during pregnancy and congenital malformations, with the aim of providing evidence-based guidance for the rational use of antibiotics during pregnancy.Methods:Data from the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) and the Canada Vigilance Adverse Reaction (CVAR) database from January 2015 to September 2024 were collected. Five methods including Tree-based scan statistic (TreeScan), proportional reporting ratio (PRR), reporting odds ratio (ROR), the UK Medicines and Healthcare Products Regulatory Agency (MHRA) comprehensive standard, and the Bayesian confidence propagation neural network (BCPNN) were used to detect signals of risk for congenital malformations in offspring following maternal use of tetracyclines during pregnancy. A signal that met the threshold criteria of all above 5 methods was considered as a risk signal. Based on population-based cohort of the drug exposures and adverse pregnancy outcomes (DEEP) data from January 2013 to December 2021 in Xiamen City, propensity score matching (PSM)-based Poisson regression was applied to evaluate the association between the first-trimester tetracyclines exposure and congenital malformations in offspring. Adjusted relative risk (a RR) and its 95% confidence interval ( CI) were calculated. Sensitivity analysis was conducted to validate the reliability of the results. Results:A total of 304 098 reports of adverse events during pregnancy were obtained from the FAERS and CVAR databases. Among them, 5 028 reports were related to tetracyclines, including 1 026 reports of congenital malformations in offspring, involving congenital malformations of musculoskeletal system, other digestive system, and other congenital malformations. Signal detection results suggested that tetracyclines may be a risk signal for above congenital malformations in offspring. The DEEP data included 411 936 pregnant women. After PSM, 240 pregnant women exposed to tetracyclines were included. The results showed no significant association between the first-trimester tetracyclines exposure and congenital malformations in offspring (a RR=0.75, 95% CI: 0.26-2.17), sensitivity analysis also showed no correlation. Conclusions:Data mining from the FAERS and CVAR databases suggests a potential association between tetracyclines use during pregnancy and congenital malformations in offspring. However, the DEEP data study shows no significant correlation.
9.The relationship between triglyceride glucose body mass index,high density lipoprotein cholesterol to apolipoprotein A ratio,glycemic risk index and diabetic retinopathy

Objective To investigate the relationship between triglyceride glucose body mass index(TyG-BMI),high density lipoprotein cholesterol to Apolipoprotein A ratio(HAR),glycemic risk index(GRI)and diabetic retinopathy(DR).Methods A total of 159 patients with type 2 diabetes mellitus(T2DM)complicated with DR(the DR group)were divided into the non-proliferative retinopathy group(NPDR group,66 cases)and the proliferative retinopathy group(PDR group,93 cases)according to DR international clinical grading criteria,and 159 T2DM patients without DR were selected as the control group.Clinical information and baseline laboratory test results were recorded,and TyG-BMI,HAR and GRI were calculated.Multivariate Logistic regression analysis was conducted to analyze the related factors of the incidence of DR,and receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of TyG-BMI,HAR and GRI for DR.Results The duration of T2DM,the proportion of hypertension,diabetic nephropathy and diabetic foot,levels of HbA1c,homeostasis model insulin resistance index(HOMA-IR),total cholesterol(TC),C-reactive protein(CRP),TyG-BMI,HAR and GRI were higher in the DR group than those in the control group(P<0.05).TyG-BMI,HAR and GRI were higher in the PDR group than those in the NPDR group(P<0.05).Multivariate Logistic regression analysis showed that the longer course of T2DM disease[OR(95%CI):2.781(1.398-5.534)],high TyG-BMI[2.036(1.169-3.546)],high HAR[1.890(1.090-3.280)]and high GRI[1.836(1.065-3.167)]were risk factors for DR(P<0.05).ROC curve analysis results showed that,the area under the curve(AUC)of combined TyG-BMI,HAR and GRI diagnosis of DR was higher than that of single diagnosis[0.940(0.908-0.964),0.864(0.821-0.900),0.796(0.747-0.839)and 0.836(0.790-0.875),all P<0.05].Conclusion The increased TyG-BMI,HAR and GRI in T2DM patients is associated with the onset and severity of DR,and which can be used to assess the risk of DR.
10.Analysis of factors influencing the trough concentration of voriconazole and adverse drug reactions in renal transplant patients

OBJECTIVE To analyze the influencing factors of voriconazole trough concentration and adverse drug reactions (ADR) in renal transplant recipients. METHODS Data from inpatients who received voriconazole and therapeutic drug monitoring in our hospital between January 2022 and August 2023 were retrospectively analyzed. Patients were divided into renal transplant group and non-renal transplant group based on transplantation status. A 1∶1 propensity score matching (PSM) method was used to balance differences in baselines between the two groups. Voriconazole trough concentrations, target attainment rate, clinical efficacy, and ADR were compared between the two groups. Multiple linear regression (backward) was used to analyze the factors influencing voriconazole trough concentrations in the renal transplant group. Univariate analysis and binary Logistic regression were used to identify independent risk factors for ADR in the renal transplant group. RESULTS After PSM, 48 patients were included in each group. There were no statistically significant differences in the mean voriconazole trough concentration, target attainment rate or efficacy rate between the two groups (P>0.05). The total incidence of ADR was significantly higher in the renal transplant group than in the non-renal transplant group (P<0.05). Multiple linear regression analysis showed that age, average daily dose, pulmonary infection, total bilirubin during medication, day-1 loading dose, use of the original drug, concomitant immunosuppressant use, and the occurrence of ADR were factors influencing voriconazole trough concentration in renal transplant patients (P<0.05). Binary Logistic regression analysis showed that abnormal direct bilirubin during medication [OR=7.747, 95%CI (1.334, 45.005), P=0.023] was an independent risk factor for ADR in renal transplant patients receiving voriconazole. CONCLUSIONS Age, average daily dose, pulmonary infection, use of the original drug, day-1 loading dose, total bilirubin during medication, concomitant immunosuppressant use, and the occurrence of ADR are the factors influencing voriconazole trough concentration in renal transplant patients. Furthermore, patients with abnormal direct bilirubin during medication are more susceptible to ADR.
