1.Analysis of urban cancer screening results in Qinghai Province from 2019 to 2024

Objective:To analyze the screening results of the Urban Cancer Early Diagnosis and Treatment Project in Qinghai Province from 2019 to 2024.Methods:A summary and statistical analysis were conducted on six years of screening data from the Urban Cancer Early Dia-gnosis and Treatment Program in Qinghai Province,with the high-risk rate,screening rate,and detection rate calculated separately for each type of cancer.Results:From 2019 to 2024,56,882 high-risk individuals were identified.The high-risk rates for lung,colorectal,breast,up-per gastrointestinal,and liver cancer were 22.02%,21.57%,14.23%,13.52%,and 6.10%,respectively.Overall,13,592 individuals com-pleted clinical screening,with detection rates of 0.32%for lung cancer,0.41%for liver cancer,0.08%for precancerous gastric lesions,3.63%for precancerous colorectal lesions,0.08%for esophageal cancer,0.16%for gastric cancer,and 0.14%for colorectal cancer.Conclusions:The implementation of the Urban Cancer Early Diagnosis and Treatment Program in Qinghai Province aids in the early detection of cancer,improves early diagnosis and survival rates,and reduces mortality.Nevertheless,due to low public awareness and limited participation,en-hancements in program management and public outreach are required.

Suspected variant loci were screened, and Sanger sequencing was used to validate the variant in pedigree members. Bioinformatics analysis was performed on the variant loci. This study was approved by the Medical Ethics Committee of Tongji Hospital of Tongji University(Ethics No.K-W-2024-004).Results:① The proband is a 67-year-old female with clinical features of DD, such as keratotic papules in sebaceous areas. ② Whole-genome exom sequencing revealed a missense variant, c. 68G>A (p.Gly23Glu), in the exon 1 of ATP2A2 gene of the proband. Sanger sequencing showed that the proband′s eldest daughter also carried this variant. This variant was not detected in other pedigree members, indicating a co-segregation of the variant with the disease phenotype in the pedigree. ③ According to the interpretation principles of gene variants by the American College of Medical Genetics and Genomics (ACMG), this variant was classified as pathogenic (PS1+ PM1+ PM2_Supporting+ PP1+ PP3+ PP4). Conclusions:The c. 68G>A (p.Gly23Glu) variant in the ATP2A2 gene may be the genetic cause of the disease in this pedigree. This finding further enriches the genetic variant spectrum in DD patients and provides a basis for clinical diagnosis and genetic counseling for patients.

Objective·To compare the effectiveness of hydrodynamic debridement versus conventional debridement in the treatment of burn wounds through a systematic review,focusing on differences in time to complete healing after graft,time to debride a 1%total body surface area(TBSA)wound,hospitalization duration,skin graft survival rate at 7 d post-surgery,secondary debridement rate,and positive rate of bacterial culture of wound exudate at 3 d post-surgery,aiming to select a more effective debridement method for burn wounds requiring debridement.Methods·A systematic literature search was conducted in PubMed,Embase,Web of Science,Cochrane Library,CNKI,SinoMed,China Science and Technology Journal Database,and Wanfang Database,for studies comparing hydrodynamic debridement and conventional debridement in the treatment of burns.The search included articles published in Chinese and English,and the search period was from the inception of the databases to October 1,2024.The study type was randomized controlled trials(RCTs).After literature search and screening,the included studies was evaluated for quality,and relevant data were extracted.Qualitative variables were presented as relative risk(RR),and quantitative variables as mean difference(MD).Forest plots were created by using RevMan 5.4 software with fixed-or random-effects models.Funnel plots were generated and Egger's test was performed by using Stata 14.0 software.Results·Fifteen high-quality RCTs were included in this study,involving 1 261 patients with burn injuries requiring debridement.The analysis results showed that compared to the conventional debridement group,the hydrodynamic debridement group had significantly shorter time to complete healing after graft(MD=-3.29,95%CI-3.88?-2.70,P<0.001),shorter time required to debride a 1%TBSA wound(MD=-0.63,95%CI-0.76?-0.50,P<0.001),and reduced hospitalization duration(MD=-4.22,95%CI-6.17?-2.28,P<0.001).The skin graft survival rate at 7 d post-surgery in the hydrodynamic debridement group(MD=8.62,95%CI 7.21?10.04,P<0.001)was significantly higher,while the secondary debridement rate(RR=0.21,95%CI 0.12?0.37,P<0.001)and the positive rate of bacterial culture of wound exudate at 3 d post-surgery(RR=0.30,95%CI 0.17?0.53,P<0.001)were significantly lower compared with the conventional debridement group.There was no statistically significant difference in the postoperative infections rates between the two groups(RR=1.06,95%CI 0.66?1.69,P=0.820).Conclusion·In the treatment of burn wounds,hydrodynamic debridement outperforms traditional debridement.In the management of burn wounds,hydrodynamic debridement outperforms conventional debridement by shortening debridement and hospitalization durations,reducing the need for secondary debridement,decreasing early bacterial colonization,and improving skin graft survival.In terms of postoperative infection risk,no significant difference was observed between the two methods,indicating comparable safety profiles.

8.Biological significance of glucose metabolism in keloids

Keloids are often accompanied by clinical manifestations such as itching, pain, and discomfort. Their biological characteristics bear similar to those of tumors, making keloids difficult to cure and prone to relapse. Recent research has identified a crucial role for alterations in cellular glucose metabolism in the pathogenesis of keloids. This paper summarized the research progress on cell glucose metabolism in keloids, explaining the tumor-like proliferation of keloid fibroblasts through aerobic glycolytic metabolism. Furthermore, it explored the influence of the relationship between endothelial cell and immune cell metabolism and inflammatory response on keloids. The roles of glucose transporter, lactate dehydrogenase, and pyruvate kinase in keloid were summarized here, providing more insights for the prevention and treatment of keloids.
9.Associations between air pollution exposure and mortality risk of critically ill patients and mediation effects of blood cell count ratio in associations

Objective:To investigate the association between exposure to major air pollutants (including PM 2.5, PM 10, NO 2, SO 2, O 3 and CO) and 28-day all-cause mortality in patients admitted to the intensive care unit (ICU). Methods:The electronic medical records of critically ill patients admitted to the ICU in Hubei Province General Hospital and Jingzhou Central Hospital were collected from August 2018 to August 2019 and from May 2021 to May 2022. Patients' exposure to air pollutants was assessed based on the average concentrations at their place of residence during the previous two months. A generalized linear regression model was used to estimate the association between air pollutant exposure and 28-day all-cause mortality in critically ill patients. Subgroup analysis was then conducted to examine the impact of individual air pollutant exposure on 28-day mortality, which served as the primary outcome in this study. The effect size and confidence interval were adjusted for patient characteristics including age, gender, smoking or drinking habits, length of hospital stay, and SOFA score. Additionally, mediation analysis was conducted using the mediation package (Bruce R) in R software. The direct effect represented the association between exposure to air pollutants and risk of mortality, while the indirect effect aimed to assess whether neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and monocyte- lymphocyte ratio (MLR) served as mediating variables in the correlation between exposure to air pollutants and mortality risk.Results:The study included a total of 3 772 critically ill patients admitted to the ICU, with a 28-day all-cause mortality rate of 40.0% (1 509/3 772). A significant positive correlation was observed between an incremental increase of 1 μg/m 3 in air pollutants (specifically PM 2.5, NO 2 and CO) and an elevated risk of mortality within 28 days among critically ill patients. Conversely, there is an inverse relationship between O 3 levels and mortality risk. Additionally, male gender and SOFA rating >3 scores were identified as independent risk factors for all-cause mortality in ICU patients exposed to NO 2 or CO. The Neutrophil/lymphocyte ratio (NLR) played a mediating role in the correlation between PM 2.5 or CO exposure and mortality risk, accounting for 9.09% or 4.71% of the correlations, respectively. Conclusions:The exposure to air pollutants (PM 2.5, NO 2, CO) significantly associate with a high risk of 28-day all-cause mortality in ICU patients. Even at low levels of air pollution, NO 2, CO remains positively correlate with the mortality risk in critically ill patients, who belong to a vulnerable population.
