1.Develop a rehabilitation nursing model of external treatment of traditional Chinese medicine for knee osteoarthritis

Objective To develop a rehabilitation nursing model for knee osteoarthritis(KOA)of external treatment in traditional Chinese medicine(TCM).Methods Between February and June 2023,a preliminary KOA rehabilitation nursing model was developed through literature search and semi-structured interview method.Two rounds of Delphi consultations were conducted with the selected experts,to establish a rehabilitation nursing model for KOA of TCM external treatment.Results A total of 24 experts from different regions participated in the consultation.The final rehabilitation nursing model of TCM external treatment for KOA included 3 primary indicators,16 secondary indicators and 91 tertiary indicators.The response rates from the two rounds of expert consultation were 96.00%and 100.00%,respectively,and the rates of expert opinion proposal were 58.33%and 8.33%,respectively.The expert authority coefficient were 0.906 and 0.923.The two rounds of expert consultation were 0.137 and 0.236 in Kendall's coefficient of concordance(W),with statistically significant differences(both P<0.001).The importance scores of each item in the second inquiry ranged from 3.75 to 4.88,and the coefficient of variation ranged from 0.07 to 0.30,and the full score ratio ranged from 20.83%to 87.50%.Conclusion The rehabilitation nursing model for KOA of TCM external treatment developed in this study is significant,scientific and feasible.It provides a guidance for medical professionals.
2.Application of expanded carrier screening for autosomal recessive monogenic diseases in 1384 patients undergoing assisted reproductive technology

Objective:To explore the guiding significance of Expanded Carrier Screening (ECS) for the fertility process of individuals undergoing Assisted Reproductive Technology (ART) in Dalian.Methods:A retrospective analysis was conducted on the ECS results of patients who visited the Dalian Maternal and Child Health Care Center (Group) from January 2023 to June 2024. The study included the screening of 155 monogenic diseases and the ART status of carrier couples.Results:Among the 1 384 patient samples, 490 carriers were identified, yielding a detection rate of 35.41% (490/1 384). A total of 100 diseases and 91 pathogenic genes were detected. The most frequently identified disease was autosomal recessive hearing loss type 4 (SLC26A4), with a carrier rate of 3.54% (49/1384). For the 490 carriers, their partners were recalled for testing and, 322 partners participated. Eight high-risk couples were identified, accounting for 2.48% (8/322). Among the 322 couples, 4 used artificial insemination by husband (AIH, 1.24%, 4/322), 166 used in vitro fertilization (IVF, 51.55%, 166/322), 139 used intracytoplasmic sperm injection (ICSI, 43.17%, 139/322), and 1 underwent preimplantation genetic testing for monogenic diseases (PGT-M, 0.31%, 1/322). Twelve couples (3.73%, 12/322) did not undergo ART, were not pregnant, or chose gamete donation. Among the eight high-risk couples, three underwent IVF, four underwent ICSI, and one underwent PGT-M. One couple conceived through ICSI, and the fetus was followed up. Amniotic fluid Sanger sequencing revealed that the fetus had compound heterozygous mutations in the PAH gene, with two suspected pathogenic variants at c.532G>A and c.1174T>A. One couple undergoing PGT-M currently has one retrieved oocyte and zero usable blastocysts. The embryo carries a paternal mutation and is aneuploid.Conclusions:This study not only identified the common pathogenic diseases in Dalian, providing a reference for clinical treatment, but also validated the critical significance of ECS for individuals undergoing ART.
3.Mendelian randomization study of causality between opioids and chronic renal failure

Objective To explore the potential causal relationship between opioids and chronic renal failure(CRF)using Mendelian randomization analysis.Methods Screening for single nucleotide polymorphisms(SNPs)strongly associated with opioids and CRF firstly,causality was analyzed by using two-sample Mendelian randomization methods,heterogeneity,horizontal pleiotropy and sensitivity analyses were then performed.Results A total of 41 opioid-related SNPs were screened in this study,inverse variance weighting analysis suggested that opioid use is a risk factor for CRF(OR=1.15,95%CI:1.04-1.27,P<0.01).The heterogeneity test showed that there was no heterogeneity in the SNPs,and the horizontal multivariate analysis and sensitivity analysis confirmed the robustness of the results.Conclusion Genetically,the study suggests a positive causal association between opioids and CRF,opioid use is a risk factor for CRF.
4.Genomic characterization of a respiratory syncytial virus BA9 genotype clinical strain associated with a severe pneumonia outbreak

Objective:To investigate the genomic characteristics of a clinical strain of respiratory syncytial virus (RSV) causing a severe pneumonia outbreak in a maternity center in a city in northern China in 2021.Methods:The whole genome sequence of RSV from a clinical sample obtained from a child with respiratory failure and heart failure was determined and analyzed using Sanger sequencing method. Sequence splicing and alignment were performed using bioinformatics software such as Sequencher 5.0, MEGA 7.0, and BioEdit 7.0. Besides, its phylogenetic relationship with the representative strains of RSV-B genotype, amino acid variations, and glycosylation sites were analyzed.Results:The genome of the clinical strain (named RSV/SY/2021) was 15 242 bp in length, consisting of 10 genes encoding 11 proteins, and possessed all the structural features of RSV. Phylogenetic analysis showed that the RSV/SY/2021 strain belonged to BA9 genotype and had the closest genetic relationship with the 2018 Netherlands epidemic strain MZ515558/Netherlands/2018. The termination codon mutation at the end of its attachment glycoprotein (G) gene resulted in the elongation of seven amino acids "Q-R-L-Q-S-Y-A" and the introduction of two additional O-glycosylation sites.Conclusions:This study suggests that RSV BA9 genotype may cause severe clinical symptoms, and clarifies the genome-wide characteristics and nucleotide/amino acid variation patterns of the RSV/SY/2021 strain. These findings enrich both national and global genome databases of RSV, and provide crucial etiological data for tracking RSV transmission, nucleic acid testing, and the development and evaluation of vaccines, antibodies and drugs against RSV.
5.Clinicopathologic analysis of 17 cases of malacoplakia

Purpose To explore the clinicopathologic features,pathogenesis and differential diagnosis of Malaco-plakia.Methods The clinical features,imaging manifestations,cytopathologic features,histopathologic features,im-munohistochemistry,special staining and molecular pathological manifestations of 17 patients with Malacoplakia were analysed and the relevant literature was reviewed.Results Seventeen patients with Malacoplakia were mostly female,of which 13 lesions were located in the bladder,1 in the prostate,1 in the colon,1 in the right external auditory canal,and 1 in the retroperitoneum.Cytologic morphology varied depending on the site of the lesion,with phagocytes and MG-like microsomal analogues seen in renal puncture cytology,and small numbers of squamous epithelial cells and uroepi-thelial cells(NHGUC)seen in urinary cytology specimens.Histologic morphology showed a large number of foamy his-tiocytes and small numbers of neutrophils and eosinophils against a background of chronic inflammation dominated by lymphocytes and plasma cells;the cytoplasm of the histiocytes was eosinophilic and granular,with blue calcified vesi-cles scattered throughout,some of which were in the form of target-ring or concentric-circle-like structures,which are known as Michaelis-Gutmann bodies(MG bodies).Special stains showed PAS and iron staining(+);immunohisto-chemistry showed diffuse histiocyte CD68(+),CD163(+),CK(AE1/AE3)(-);molecular pathology showed TB-DNA(-).Conclusion Malacoplakia is a chronic granulomatous disease that can be cured.Imaging often shows occupancy,which is easily misdiagnosed as a tumour clinically,and confirming the diagnosis mainly relies on patholog-ical diagnosis,differential diagnosis includes xanthogranulomatous cystitis,xanthogranulomatous pyelonephritis,colon cancer,granulosa cell tumour and Langerhans histiocytosis.

8.Goal-oriented nursing with integrated traditional Chinese medicine and western medicine for a patient with a complication of early postoperative inflammatory bowel obstruction after the surgery for abdominal cocoon syndrome:a case report

This paper highlights the specific nursing experiences in an early inflammatory bowel obstruction of a patient after the surgery for abdominal cocoon syndrome.Immediately after the surgery,the nursing care focused on prevention of postoperative metabolic disorders,including dynamic monitoring and correction of refeeding syndrome and goal-oriented sequential nutritional support management.Over the period of postoperative recovery,the nursing care shifted to prevention and treatment of early postoperative inflammatory bowel obstruction,including dynamic monitoring and identification of an early postoperative inflammatory bowel obstruction,goal-oriented early postoperative inflammatory small bowel obstruction(EPISBO)fluid therapy together with the traditional Chinese medicine to promote bowel movement and control inflammation,the auricle copper-blade Gua Sha combined with auricular point sticker compression to regulate qi and reduce bowel flatulence,as well as a goal-oriented early mobilisation to prevent a re-obstruction of bowel.The patient discharged at 28 days after surgery without an event.At the 6 months of postoperative follow-up,the patient was found in continuous improvement of relevant indicators and increase of body weight.
9.Analysis of risk factors for parenteral nutrition related water-soluble vitamin accumulation

Objective To explore the influencing factors of abnormal increase of plasma water-soluble vitamins in patients with parenteral nutrition(PN).Methods A total of 862 hospitalized patients receiving nutritional diagnosis and treatment were enrolled.Daily water-soluble vitamin supply and total urine output were monitored.If patients underwent renal replacement therapy(RRT),the ultrafiltration volume was recorded.Plasma concentrations of nine water-soluble vitamins(Vit-C,B1,B2,B3,B5,B6,B7,B9 and B12)were measured.According to the test results,patients were divided into the accumulation group and the non-accumulation group.Renal function(BUN,Cr,UA),inflammatory indicators(CRP,WBC,NEU,SII)and nutritional indicators(ALB,PA,Hb,LYM)were compared between the two groups.Logistic regression was used to analyze the independent risk factors for the accumulation of water-soluble vitamins.Results There was a high incidence of abnormal plasma water-soluble vitamin levels,including Vit-B2(36.08%),Vit-B6(16.01%)and Vit-B5(13.81%).Compared with the non-accumulation group,the Vit-B2 accumulation group had higher levels of age,BUN,Cr,UA and NEU,and lower levels of ALB,PA and urine volume.The Vit-B5 accumulation group had higher levels of BUN,Cr,UA and NEU,and lower levels of LYM,Hb,ALB,PA and urine volume.Patients in the Vit-B6 accumulation group was older,with increased levels of BUN,Cr and UA,and decreased levels of LYM,Hb,ALB,urine volume and vitamin intake(P<0.05).Multivariate Logistic regression analysis showed that the elevated level of BUN was independent risk factors for Vit-B2 accumulation,the elevated BUN and reduced urine volume were independent risk factors for Vit-B5 accumulation,and elevated BUN and Cr,as well as reduced urine volume,were independent risk factors for Vit-B6 accumulation.Receiver operating characteristic curve analysis showed that the area under the curve for BUN in predicting Vit-B2,B5 and B6 accumulation were 0.659(95%CI:0.620-0.697),0.728(95%CI:0.675-0.781)and 0.785(95%CI:0.738-0.831),respectively,with high specificity but low sensitivity.Conclusion The serum levels of water-soluble vitamins are affected by renal function.When implementing parenteral nutrition water-soluble vitamins supplementation,the patient's renal function status should be evaluated,and the risk of abnormal elevation of Vit-B2,B5 and B6 should be vigilantly monitored.
