BACKGROUND:At present, there is a lack of an internal fixation system with good reduction and simple operation for the treatment of atlas burst fracture by posterior single-segment fixation. OBJECTIVE:Based on the atlas CT measurement data,a new posterior atlas fracture reduction and internal fixation system was designed and optimized, which was in line with the characteristics of human local anatomical structure, easy to operate and with good reduction. METHODS:CT images of 347 adults were analyzed retrospectively. In the images, the length of pedicle screw track with a medial inclination of 0°,the angle of the maximum medial inclination angle and the length of pedicle screw track at this time, the height of vertebral artery groove, the distance between the entry points of bilateral pedicle screws and the midline, the radius of the posterior arch of atlas, the distance between the posterior tubercle of the atlas and the posterior edge of the foramen magnum, and the distance between the posterior tubercle of the atlas and the upper surface of the axial arch were measured. The imaging data were analyzed and a reduction and internal fixation system was designed and optimized for atlas fracture in line with human anatomical characteristics. RESULTS AND CONCLUSION:(1)There was no statistically significant difference in the the length of pedicle screw track with a medial inclination of 0°,the maximum medial inclination angle and the length of pedicle screw track at this time, the height of vertebral artery groove, the distance between the entry points of bilateral pedicle screws and the midline, the radius of the posterior arch of atlas, the distance between the posterior tubercle of the atlas and the posterior edge of the foramen magnum, and the distance between the posterior tubercle of the atlas and the upper surface of the axial arch measured on the left and right sides of all subjects(P>0.05).There were statistically significant differences in each index measured between the male and female groups(P<0.05).(2)The new posterior atlas fracture reduction and internal fixation system has been successfully designed and obtained the national patent. The internal fixation system is suitable for the anatomical characteristics of the posterior arch of the atlas. It can not only effectively treat the atlas burst fracture, but also retain the movement function of the occipital atlantoaxial joint.

Objective To analyze the expression level of ATP5A1 in gastric carcinoma and its influence on the prognosis of the patients and glucose metabolism in the tumor cells. Methods We retrospectively analyzed the data of 115 patients undergoing radical resection of gastric carcinoma in our hospital from February,2013 to November,2016.ATP5A1 expression in the surgical specimens were detected using immunohistochemistry, and the long-term prognosis of the patients with high(n=58)and low ATP5A1 expression(n=57)were analyzed. In gastric carcinoma MGC803 cells, the effects of lentivirus-mediated ATP5A1 knockdown or overexpression on glucose metabolism were investigated. We also observed the growth and glucose metabolism of xenografts derived from MGC803 cells with ATP5A1 knockdown or overexpression in nude mice. Results ATP5A1 was significantly overexpressed in gastric carcinoma tissues in close correlation with blood CEA and CA19-9 levels, pathological grade,T stage and N stage(P<0.05).ATP5A1 overexpression was an independent risk factor for a significantly lowered 5-year survival rate of patients with gastric carcinoma(P<0.05).ROC curve analysis demonstrated the predictive value of high ATP5A1 expression for the patients'prognosis(P<0.001).In MGC803 cells,ATP5A1 overexpression significantly up-regulated cellular glucose uptake and lactate production and increased the protein levels of HK2,PFK1,and LDHA(P<0.05),while ATP5A1 knockdown produced the opposite changes(P<0.05).In the tumor-bearing mice, overexpression of ATP5A1 increased glucose metabolism of the tumor cells and promoted tumor growth(P<0.05).Overexpression of ATP5A1 promoted the expressions of p-JNK and p-JUN in MGC803 cells(P<0.05),and the JNK inhibitor SP600125 significantly inhibited the enhancement of cellular glucose metabolism induced by ATP5A1 overexpression(P<0.05).Conclusion High ATP5A1 expression in gastric cancer is associated a poor long-term prognosis of the patients, and its effect is mediated at least partly by promoting glucose metabolism of the cells through the JNK/JUN pathway.

Chinese Journal of Thoracic and Cardiovascular Surgery 2024;40(8):473-479
Objective:To evaluate the relationship between preoperative free thyroxine (FT4) levels and the risk of new-onset atrial fibrillation (AF) after cardiac valve surgery in patients with normal thyroid function, and to identify high-risk individuals for postoperative AF among patients undergoing cardiac valve surgery. Methods:This retrospective study enrolled patients(≥18 years old)with valvular heart disease who underwent heart valve replacement or valvoplasty surgery from December 2019 to January 2023 in Beijing Anzhen Hospital. Patients with thyroid function tests exceeding the normal reference range(7.64-16.03 pmol/L) were excluded, resulting in a final cohort of 2 645 patients, including 1 597 males and 1 048 females; aged 18-83 years, with a median age of 56 (46, 64) years. Among them, 1 891 patients (71.5%) had hypertension, and 176 (6.7%) had type 2 diabetes. The patients were divided into four groups based on quartiles of FT4 levels: Group 1 (n=661) with FT4 levels of 7.64-10.33 pmol/L, Group 2 (n=661) with 10.33-11.36pmol/L, Group 3 (n=661) with 11.36-12.54 pmol/L, and Group 4(n=662) with 12.54-13.66pmol/L. The risk of postoperative AF was compared among the four groups, and logistic regression was used to adjust for relevant risk factors. The odds ratio ( OR) and 95% confidence interval( CI) for postoperative AF across different groups were analyzed. Subgroup analyses were performed based on age, gender, hypertension status, and B-type natriuretic peptide (BNP) levels to compare the association between FT4 levels and new-onset AF within different subgroups. Results:Statistically significant differences were observed among the four groups in terms of age, body mass index, hypertension, hyperlipidemia, BNP, glomerular filtration rate, triglyceride, and total cholesterol levels ( P<0.05), while no significant differences were found in other indicators( P<0.05). Using the lowest FT4 group as the baseline, a higher risk of postoperative new-onset AF was observed in groups with higher FT4 levels, with a significant trend of incremental increase in postoperative AF with rising FT4 levels. Similar trends were observed across subgroups stratified by age, gender, hypertension status, and BNP levels. The Youden index indicated a cut-off value of 11.485pmol/L for FT4. Conclusion:Among patients with normal thyroid function and valvular heart disease, preoperative higher FT4 levels are significantly associated with an increased risk of new-onset AF after cardiac valve surgery. Close monitoring for postoperative AF risk is recommended for patients with preoperative FT4 levels exceeding 11.485 pmol/L, even if the thyroid function is still within the normal.

Objective To establish the fingerprint map and method of multi-component content determination of ginger and its processed products, and to evaluate their quality. Methods High performance liquid chromatography (HPLC) was used to establish the fingerprint of ginger and its processed products (dried ginger, baked ginger, ginger charcoal) and the method for the simultaneous determination of content of five components. The data were analyzed by similarity evaluation and chemical pattern recognition. Results The fingerprint of ginger and its processed products was established. The similarity was ranged from 0.931 to 0.996. A total of 15 common peaks were confirmed. Five components (6-ginger phenol,8-ginger phenol,10-ginger phenol,6-ginger enol, zingiberone) were identified when compared with the standard. The content of 6-ginger phenol,8-ginger phenol and 10-ginger phenol in ginger and its processed products(dried ginger, baked ginger, ginger charcoal),which were determined by HPLC,met the standard of Chinese Pharmacopoeia(2020 edition). The content of 6-ginger enol and zingiberone in fresh ginger was very low,but their content increased significantly after processing into dried ginger and baked ginger. Ginger and its processed products can be divided into different category in cluster analysis. Five components with VIP greater than one were selected by orthogonal partial least squares discriminant analysis. Conclusion The HPLC fingerprint and assay method established in this study are specific, simple and feasible, stable and reliable. With the help of chemical pattern recognition method,it can provide reference for the quality evaluation of ginger and its processed products.

Objective:To investigate the effects of continuous renal replacement therapy (CRRT) on plasma concentration, clinical efficacy and safety of colistin sulfate. Methods:Clinical data of patients received with colistin sulfate were retrospectively analyzed from our group's previous clinical registration study, which was a prospective, multicenter observation study on the efficacy and pharmacokinetic characteristics of colistin sulfate in patients with severe infection in intensive care unit (ICU). According to whether patients received blood purification treatment, they were divided into CRRT group and non-CRRT group. Baseline data (gender, age, whether complicated with diabetes, chronic nervous system disease, etc), general data (infection of pathogens and sites, steady-state trough concentration, steady-state peak concentration, clinical efficacy, 28-day all-cause mortality, etc) and adverse event (renal injury, nervous system, skin pigmentation, etc) were collected from the two groups. Results:A total of 90 patients were enrolled, including 22 patients in the CRRT group and 68 patients in the non-CRRT group. ① There was no significant difference in gender, age, basic diseases, liver function, infection of pathogens and sites, colistin sulfate dose between the two groups. Compared with the non-CRRT group, the acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) and sequential organ failure assessment (SOFA) were higher in the CRRT group [APACHE Ⅱ: 21.77±8.26 vs. 18.01±6.34, P < 0.05; SOFA: 8.5 (7.8, 11.0) vs. 6.0 (4.0, 9.0), P < 0.01], serum creatinine level was higher [μmol/L: 162.0 (119.5, 210.5) vs. 72.0 (52.0, 117.0), P < 0.01]. ② Plasma concentration: there was no significant difference in steady-state trough concentration between CRRT group and non-CRRT group (mg/L: 0.58±0.30 vs. 0.64±0.25, P = 0.328), nor was there significant difference in steady-state peak concentration (mg/L: 1.02±0.37 vs. 1.18±0.45, P = 0.133). ③ Clinical efficacy: there was no significant difference in clinical response rate between CRRT group and non-CRRT group [68.2% (15/22) vs. 80.9% (55/68), P = 0.213]. ④ Safety: acute kidney injury occurred in 2 patients (2.9%) in the non-CRRT group. No obvious neurological symptoms and skin pigmentation were found in the two groups. Conclusions:CRRT had little effect on the elimination of colistin sulfate. Routine blood concentration monitoring (TDM) is warranted in patients received with CRRT.

Chinese Journal of Hospital Administration 2022;38(2):97-100
In recent years, the medical insurance coverage of China has been increased significantly, and the medical insurance policies have been launched continuously, so the traditional manual audit method is unable to support medical institutions to effectively supervise the medical insurance fund. In view of this situation, a tertiary hospital in Beijing had successfully built an intelligent medical insurance audit system for drugs, diagnosis and treatment projects under key supervision, realized the prior audit and in-process control of the illegal use of medical insurance funds, through the establishment of intelligent audit rules, the design of audit trigger points and the interception level of illegal medical orders, and the establishment of a pre-operational preparation system and continuous improvement mechanism. In March 2021, the hospital officially launched the system. After the application of the system, the amount and quantity of outpatient medical insurance refusal from April to September 2021 were 10 587 yuan and 72 respectively, which decreased by 79.21% and 77.50% compared with the same period in 2020. This system effectively improved the quality and efficiency of medical insurance fund supervision.
