1.A Study on Optimization Strategies for Bidirectional Referral of Diseases Based on DRG in Graded Diagnosis and Treatment

Objective To analyze the current status of medical services in secondary and tertiary hospitals in Shandong Province based on DRG,and to provide effective support for the implementation strategies of bidirectional referral.Methods The DRG group was used to analyze 42.348 3 million cases from 75 secondary hospitals and 69 tertiary hospitals in Shandong Province from 2019 to 2023.Experts were organized to establish standards for bidirectional referral of diseases.Results(1)35 high-frequency DRG disease groups with diagnosis,treatment ability and medical resources in secondary hospitals were selected,(2)The medical expenses and medical quality in the high-frequency DRG disease groups within secondary hospitals were lower than those in tertiary hospitals,(3)To develop standardized referral standards and programs with esophageal cancer as an example.Conclusion It is urgent to triage patients gradually and accurately through disease classification management,and formulate disease diagnosis and treatment plans and bidirectional referral standards to improve the medical quality of secondary hospitals.
2.Correlation Between Cortical Thickness and Putamenial Dopamine Transporter in Parkinson's Disease

Purpose To investigate the cortical thickness features in Parkinson's disease(PD)patients at various stages and their association with dopamine transporter(DAT)levels in the putamen.Materials and Methods We retrospectively enrolled 30 PD patients and 15 healthy subject who underwent 11C-CFT PET and T1 MRI scans at the Department of Nuclear Medicine/PET Center of Huashan Hospital from August 2016 to October 2020.DAT average radioactivity in the anterior and posterior putamen was analysis using SPM12 software,with the occipital lobe as the reference region.Cortical segmentation and reconstruction were performed on T1 images using Freesurfer v7.2.The differences in cortical thinning between the groups were compared using a general linear model.Additionally,the relationship between cortical thickness in various brain regions and DAT uptake in the putamen were assessed.Results Compared to healthy subjects,significant cortical thinning was observed in the left inferior parietal lobule and the right and left inferior middle frontal gyrus of PD patients(all P<0.05).There was a significant positive correlation between the cortical thickness of the left inferior parietal lobule and right inferior middle frontal gyrus and DAT uptake in the corresponding anterior/posterior parts of the putamen(r=0.30-0.47,all P<0.05).Furthermore,the DAT uptake in the right precentral gyrus was positively correlated with the ipsilateral posterior putamen,exhibiting a stronger correlation than on the contralateral side(r=0.32,P=0.029).Conclusion The results show that the thickness of the thinning cortex area in the PD patients correlates significantly positively with DAT levels in the putamen,highlighting the importance of the basal ganglia cortical circuit and providing a basis for further research into the neural mechanisms of PD.

4.Effects of transcutaneous electrical nerve stimulation on knee joint pain,function,muscle strength and gait in patients with knee fracture surgery

Objective:To explore the effects of transcutaneous electrical nerve stimulation(TENS)on knee joint pain,func-tion,muscle strength and gait in patients with knee fracture surgery.Method:Forty patients with knee fractures were divided into the control group and the experimental group,with 20 cases in each group.Strength training,joint mobilization techniques,neuromuscular electrical stimula-tion,and routine rehabilitation training were provided to the control group.The experimental group was given the same routine rehabilitation program,except the TENS intervention instead of neuromuscular electrical stimu-lation.For six weeks,each treatment was given once daily over five days.All the patients were evaluated by Visual analogue score(VAS),American hospital for special surgery knee score(HSS-KS),isometric muscle strength test and 3-D gait analysis before and after treatment.Result:There was no significant difference between the two groups in each evaluation scores before treatment(P>0.05).HSS-KS score,peak torque per bodyweight of knee extensors and flexors under 60°/s and 180°/s,and gait parameters in both groups were significantly improved after 6 weeks of treatment,compared to those before treatment(P<0.05).Meanwhile,the experimental group were significantly better than the control group in terms of the VAS score and HSS-KS score,peak torque/body weight(PT/BW)at 60°/s and 180°/s of knee extensors,and subparts of gait parameters(P<0.05).Conclusion:In patients with knee fractures surgery,TENS can significantly minimize the pain,improve knee function and gait pattern,and strengthen the absolute force of knee extensors.

Objective:Using ecological niche width and situation model,the operation and management of public Traditional Chinese Medicine(TCM)hospitals in 16 cities in Shandong Province were evaluated to put forward countermeasures.Methods:The relevant index data of public TCM hospitals in 16 cities in Shandong Province from 2019 to 2022 were collected to analyze the development status.The ecological niche width model and niche situation model were applied to evaluate the operation and management of public TCM hospitals in 16 cities in Shandong.Results:The top three public TCM hospitals in terms of average ecological niche width were City G,City P and City F.The ecological niche situation value of public TCM hospitals in City K,City A and City J ranked the top three.Conclusion:The dimensions of operation management structure of public TCM hospitals in Shandong need to be enhanced and the development of operation management space needs to be balanced.It is feasible to evaluate operation management by using ecological niche width and situation model.It is suggested to improve the allocation efficiency of health personnel,facilities,funds and other resources;promote the spatial balanced development of operation management;promote the application of ecological niche width and situation model in the field of medicine and health management.
8.Mild cognitive impairment and emotional problems among the elderly in a community of Shanghai, China: A follow-up study

Objective To investigate the public health issues associated with the follow-up outcomes of mental health in elderly community residents with a focus on mild cognitive impairment (MCI) and emotional problems (anxiety and depression), as the burden of various diseases increases with global population growth and aging. Methods In this study, Patient Health Questionnaire-9, Early Dementia Screening Scale, Montreal Cognitive Assessment, Generalized Anxiety Disorder Scale, Geriatric Depression Scale, and Activities of Daily Living scale were used to analyze the data on MCI and emotional problems among the elderly in a district of Shanghai, China. The t-test, Spearman correlation analysis, and linear regression analysis were used to identify related protective factors and evaluate the effectiveness of comprehensive intervention measures. Results The first session of intervention led to an improvement in cognitive impairment (P<0.001). During the first session of intervention, exercise and skill training were very effective in improving cognitive impairment (r=0.71 and 0.68, P<0.001), while group instruction and the distribution of educational materials failed to achieve an ideal effectiveness. The second session of intervention also had a significant effect on cognitive impairment (P<0.05); however, the second session of intervention lacked a significant overall effect, and skill training remained the most effective intervention method (Pr=0.0344, P<0.05). The research findings showed that the two sessions of community intervention were protective factors against MCI and depression, while they could not alleviate anxiety. Conclusion This study validates the effectiveness of follow-up intervention measures in improving cognitive function in elderly patients with MCI, which provide scientific prediction and reference for early screening, prediction, and follow-up intervention of elderly dementia and MCI at the community level.
9.Epidemiology and influencing factors of skin complications at the puncture site following femoral artery compression in patients with primary hepatocellular carcinoma after TACE

Objective To investigate the epidemiological characteristics and influencing factors of Femoral Artery Compression-Related Skin Complications Around the Puncture Site(FACR-SCAPS)in patients with primary hepatocellular carcinoma(HCC)undergoing transarterial chemoembolization(TACE).Methods A multicenter cross-sectional study was conducted using convenience sampling.A total of 1 573 HCC patients who underwent TACE between April 2023 and October 2024 were recruited from interventional radiology departments,oncology units,and specialized centers across 10 hospitals in Beijing,Tianjin,Shandong,Hebei,Qinghai,and Inner Mongolia.Descriptive statistics,univariate analysis,and multivariate logistic regression were used to explore the epidemiological characteristics and influencing factors of FACR-SCAPS in this population.Results Among the 1 573 primary HCC patients undergoing TACE interventional therapy,FACR-SCAPS occurred in 28.99%(456/1 573),with a total of 476 complication instances recorded(30.26 per 100 patients).Patients with a single complication accounted for 96.93%,whereas those with multiple complications constituted 3.07%.The most prevalent types of complications were skin erythema,skin ecchymosis,and hard lumps formation,collectively accounting for 96.49%of all complications.Hematoma,blisters,and rupture complications collectively accounted for only 4.61%.Logistic regression analysis revealed that peak diastolic blood pressure during compression(OR=1.024,95%CI:1.013-1.035,P<0.001),use of rotary compression hemostasis devices(OR=3.220,95%CI:2.120-4.891,P<0.001),elevated PT-INR(OR=19.630,95%CI:6.039-63.810,P<0.001),and anticoagulant use within the last three months(OR=1.909,95%CI:1.064-3.427,P=0.030)were significant influencing factors associated with FACR-SCAPS post-TACE.Conclusion FACR-SCAPS is commonly seen among primary HCC patients after TACE,its risk factors include peak diastolic blood pressure during compression,use of rotary compression devices,elevated PT-INR,and recent anticoagulant use.
