1.Role of Different Central Venous Pressure Levels in Septic-associated Acute Kidney Injury

Objective To explore the role of different CVP levels in the occurence and development of sepsis-associated acute kid-ney injury(SA-AKI).Methods Sepsis model was induced using the cecal ligation and puncture(CLP)method for 24h in male SD rats,with a sham group as the control.The rats in two groups were intravenously infused with Ringer's solution to induce an increase in CVP to 10,15 and 20mmHg,respectively.The hemodynamic parameters including mean arterial pressure(MAP),heart rate(HR),and arterial blood lactate(Lac)were dynamically monitored in rats.And side stream darkfield imaging(SDF)technique was used to de-tect the renal cortex microcirculation perfusion parameters including perfused small vessel density(PVD)and microvascular flow index(MFI).Renal injury markers including serum creatinine(CRE),urine neutrophil gelatinase associated lipocalin(NGAL),renal inflam-matory factor interleukin-6(IL-6)levels and renal histopathological changes were also detected.Results Compared with the sham group,the MAP of CLP rats significantly decreased,while HR and Lac significantly increased(P<0.05).The renal PVD and MFI were significantly decreased(P<0.05),while serum CRE,urine NGAL,and renal IL-6 were significantly increased(P<0.05).Com-pared with CVP 10mmHg,the PVD and MFI of CLP rats were significantly decreased at CVP 20mmHg(P<0.05);The PVD and MFI of sham group at CVP 15mmHg and 20mmHg were significantly lower than those of the CVP at CVP 10mmHg(P<0.05).The IL-6 of CLP rats at CVP 15mmHg was significantly higher than that at CVP 10mmHg(P<0.05),and the IL-6 levels at CVP 20mmHg were significantly higher than at CVP 15mmHg(P<0.05).In CLP rats,increasing CVP led to cortical and medullary edema,vacuolar degen-eration of renal tubular epithelial cells,loss of brush borders,tubular obstruction,and disappearance of Bowman's space;medullary ede-ma was more pronounced than in the cortex.In the sham group,glomerular and tubular swelling occurred at CVP 20mmHg,and Bowman's space became narrower.Conclusion Excessive CVP levels can decrease renal cortical microcirculation perfusion and promote the acti-vation of local inflammation and renal tissue edema in CLP rats,which exerting a detrimental effect during the process of S-AKI.

Objective:To characterize the histopathological subtypes and their clinicopathological parameters of gender and onset age by common, rare and sparse primary esophageal malignant tumors (PEMT).Methods:A total of 272 437 patients with PEMT were enrolled in this study, and all of the patients were received radical surgery. The clinicopathological information of the patients was obtained from the database established by the State Key Laboratory of Esophageal Cancer Prevention & Treatment from September 1973 to December 2020, which included the clinical treatment, pathological diagnosis and follow-up information of esophagus and gastric cardia cancers. All patients were diagnosed and classified by the criteria of esophageal tumor histopathological diagnosis and classification (2019) of the World Health Organization (WHO). The esophageal tumors, which were not included in the WHO classification, were analyzed separately according to the postoperative pathological diagnosis. The χ 2 test was performed by the SPSS 25.0 software on count data, and the test standard α=0.05. Results:A total of 32 histopathological types were identified in the enrolled PEMT patients, of which 10 subtypes were not included in the WHO classification. According to the frequency, PEMT were divided into common (esophageal squamous cell carcinoma, ESCC, accounting for 97.1%), rare (esophageal adenocarcinoma, EAC, accounting for 2.3%) and sparse (mainly esophageal small cell carcinoma, malignant melanoma, etc., accounting for 0.6%). All the common, rare, and sparse types occurred predominantly in male patients, and the gender difference of rare type was most significant (EAC, male∶ female, 2.67∶1), followed with common type (ESCC, male∶ female, 1.78∶1) and sparse type (male∶ female, 1.71∶1). The common type (ESCC) mainly occurred in the middle thoracic segment (65.2%), while the rare type (EAC) mainly occurred in the lower thoracic segment (56.8%). Among the sparse type, malignant melanoma and malignant fibrous histiocytoma were both predominantly located in the lower thoracic segment (51.7%, 66.7%), and the others were mainly in the middle thoracic segment.Conclusion:ESCC is the most common type among the 32 histopathological types of PEMT, followed by EAC as the rare type, and esophageal small cell carcinoma and malignant melanoma as the major sparse type, and all of which are mainly occur in male patients. The common type of ESCC mainly occur in the middle thoracic segment, while the rare type of EAC mainly in the lower thoracic segment. The mainly sparse type of malignant melanoma and malignant fibrous histiocytoma predominately occur in the lower thoracic segment, and the remaining sparse types mainly occur in the middle thoracic segment.
3.Robot-assisted gait training improves the walking ability of stroke survivors

Objective:To explore the effectiveness of the Exowalk gait training robot in improving the walking ability of stroke survivors.Methods:Forty stroke survivors were randomly divided into a control group and an experimental group, each of 20. In their rehabilitation, the control group was given routine walking training, while the experimental group′s training was assisted with the Exowalk robot. Both groups trained for 60 minutes a day, five days a week for four weeks. Before as well as after 2 and 4 weeks of training functional ambulatory categories (FACs), the Berg balance scale (BBS), the 6-minute walking test (6MWT), the 10-minute walking test (10MWT), the Rivermead mobility index and an exercise index were used to evaluate those in both groups.Results:After 2 weeks significant improvement was observed in the average FAC, BBS, 6MWT and 10MWT results of both groups, without significant differences between them. After 4 weeks there was still no significant difference in the groups′ average BBS scores. However, the average FAC rating in the experimental group had improved significantly while there was no significant increase in the control group′s average score.Conclusions:The Exowalk robot can help to improve the balance and walking ability of hemiplegic stroke survivors.
4.Test time affects the detection of cognitive dysfunction by Montreal Cognitive Assessment in elderly patients after stroke

Objective To compare if the Montreal cognitive assessment (MoCA) performed in the morning or afternoon would affect abnormal rate of cognitive function in the elderly with stroke.Methods A total of 378 senile patients (≥ 65 years) with acute ischemic stroke and low NIHSS score (≤ 3) were enrolled in the prospective study,which was held in the Department of Neurology at Cangzhou Hospital of Integrated Traditional Chinese Medicine.MoCA was assessed after one month of hospitalization.Based on the time of MoCA assessment,all patients were randomly divided into the group A (assessed in the morning,9 am-12 am) and the group B (assessed in the afternoon,12 am to 5 pm).Clinical data were collected,and RANKIN scale (mRS) examination was performed.Moreover,patients were further divided into severe cognitive impairment (SCI) subgroup (score < 20),mild cognitive impairment (MCI) subgroup (score 20-25) and no cognitive impairment (NCI) subgroup (score > 26) according to the MoCA score.Results There were 189 patients in the group A (50%),and 189 cases in the group B (50%).There was no significant difference in age,gender,education level,disability (mRS score < 1),history of hypertension,diabetes,hyperlipidemia,smoking and atrial fibrillation between the two groups.Based on the MoCA score,211 cases had NCI,142 had MCI,and 25 had SCI.Compared with patients in group B,patients in group A was associated with significantly higher positive rate of SCI[12.2% (23/189)vs.1.1% (2/189),P=0.000],MCI[40.2% (76/189)vs.34.9% (66/189),P=0.013]and slightly higher positive rate of NCI[56.6% (107/189)vs.55.0% (104/189),P=0.214].Conclusions The test time of MoCA may have an effect on the cognitive function detection rate in elderly patients with stroke,and the time of MoCA examination should be considered in clinical examination.
5.Measurement of Phase-controlled Characteristics of HIFU Based on Infrared Thermal Imaging

Phase-Controled high intensity focused ultrasound 3D temperature distribution is the key indicator for measuring the efficiency of HIFU transducer. Considerable progress has been achieved in the use of infrared (IR) imaging techniques for qualitative mapping of acoustic and thermal field of high intensity focused ultrasound (HIFU) transducers. This article proposes a method to measure phased-controled characteristics of HIFU based on infrared thermal imaging and establishes a whole measurement system to make the method more quantitative and reliable. The result shows that the proposed measurement system is able to measure pHIFU characteristics rapidly and precisely, which wil be of great significance in standardizing the measurement of pHIFU acoustic field.
6.Research on Shielding of Emboli with the Phase-Controlled Ultrasound.

The postoperative neurological complications is associated with intraoperative cerebral emboli, which results from extracorporeal circulation and operation. It can effectively reduce the incidence of neurological complications with ultrasonic radiation. In fluids, a particle will change it's motion trail when it is acted by the radiation force generated by the ultrasound. This article mainly discuss how to shielding emboli with ultrasound. The equipment can transmit phased ultrasonic signals, which is designed on a FPGA development board. The board can generate a square wave, which is converted into a sine wave through a power amplifier. In addition, the control software has been developed on Qt development environment. The result indicates it's feasible to shielding emboli with ultrasonic radiation force. This article builds a strong foundation for the future research.

7.A focused sound field measurement system by LabVIEW.

In this paper, according to the requirement of the focused sound field measurement, a focused sound field measurement system was established based on the LabVIEW virtual instrument platform. The system can automatically search the focus position of the sound field, and adjust the scanning path according to the size of the focal region. Three-dimensional sound field scanning time reduced from 888 hours in uniform step to 9.25 hours in variable step. The efficiency of the focused sound field measurement was improved. There is a certain deviation between measurement results and theoretical calculation results. Focal plane--6 dB width difference rate was 3.691%, the beam axis--6 dB length differences rate was 12.937%.

8.Changhai fulcrum bending radiographic technique to assess curve flexibility in patients with adolescent idiopathic scoliosis.

OBJECTIVETo evaluate the role and value of Changhai fulcrum bending radiograph(CH-FBR) in curve flexibility assessment of adolescent idiopathic scoliosis(AIS) patients.
METHODSThirty-seven AIS patients treated between June 2012 and August 2013 were enrolled, including 31 female and 6 male patients whose age ranged from 10 to 19 years, averaged of 15.0 years. The assessment of radiographs included preoperative standing posterior-anterior radiograph, supine side-bending radiograph, traditional fulcrum bending radiograph, Changhai fulcrum bending radiograph and postoperative standing posterior-anterior radiograph. Postoperatively, radiographs were assessed at one week. The CH-FBR was performed at the lowest height and the optimized height which means the weight on the fulcrum touch the maximum. All measurements of angle were made with use of the Cobb method. The flexibility of the curve as well as the correction rate and fulcrum bending correction index (FBCI) were calculated for all patients. The maximum height of CH-FBR, basic weight and maximum weight were measured for all AIS. Paired t-tests were used to assess differences between preoperative and postoperative curves within group samples. The Pearson correlation coefficients were calculated using bivariate analysis between CH-FBR flexibility rate and correction rate, the maximum height of CH-FBR and maximum weight, the height changes of CH-FBR and weight changes.
RESULTSA total of 46 curves were involved in this study, including 28 thoracic and 18 thoracolumbar/lumbar curves. Preoperatively, the mean Cobb angle of the 46 structural curves was 47° ± 11°. Postoperatively, the mean Cobb angle was 11° ± 5°. Cobb's angle in supine side-bending(t = 7.2, P = 0.001), traditional fulcrum bending (t = 7.1, P = 0.001) and lowest height of Changhai fulcrum bending (t = 6.5, P = 0.001) were significantly different from the postoperative Cobb angle; Cobb's angle in traditional FBR (t = 11.0, P = 0.001) and lowest height of Changhai fulcrum bending (t = 13.6, P = 0.001) were significantly different from the optimized height CH-FBR Cobb angle. There was no significant difference found between traditional FBR Cobb angle and lowest height CH-FBR Cobb angle (t = 2.0, P = 0.051), optimized height CH-FBR Cobb angle and postoperative Cobb angle (t = 0.9, P = 0.36), lowest height CH-FBR Cobb angle and traditional FBR Cobb angle(t = 2.0, P = 0.051). The maximum height of CH-FBR, basic weight and maximum weight were (29.6 ± 1.4)cm,(20 ± 6)kg, and (40 ± 6) kg. Preoperatively, the mean Cobb angle of the 28 structural curves(main thoracic curves) was 46° ± 11°. Postoperatively, the mean Cobb angle was 12° ± 6°. Preoperatively, the mean Cobb angle of the 18 structural curves(thoracolumbar/lumbar curves) was 49° ± 12°. Postoperatively, the mean Cobb angle was 10° ± 5°. The results were same in 28 structural curves, 18 structural curves as well as 46 curves. Correlation analysis of 46 curves indicated that the maximum height of CH-FBR positively correlated with maximum weight (r = 0.69, r(2) = 0.47, P = 0.001), the height changes of CH-FBR positively correlated with weight changes on CH-FBR (r = 0.62, r(2) = 0.38, P = 0.001).
CONCLUSIONSCH-FBR is a more reliable and effective method than traditional FBR and supine side-bending for curve flexibility evaluation in AIS patients. Moreover, compared to the traditional FBR and side-bending radiograph, the flexibility suggested by the optimized height CH-FBR more closely approximates the postoperative result made by pedicle screws fixation and fusion.

10.Temperature distribution based on Monte Carlo method of optical transmission in tissues of laser ablation.

Monte Carlo method was used for calculation of finite-diameter laser distribution in tissues through convolution operation. Photo-thermal ablation model was set up on the basis of Pennes bioheat equation, and tissue temperature distribution was simulated by using finite element method by ANSYS through the model. The simulation result is helpful for clinical application of laser.
