1.Association between sleep quality and executive functions among middle school students

Objective:
To explore the relationship between sleep quality and executive function among middle school students, so as to provide theoretical support for the promotion of adolescents physical and mental health development.
Methods:
From September to December 2023, 5 713 junior and senior high school students aged 13 to 18 were selected by stratified cluster random sampling method from Shanghai, Suzhou, Taiyuan, Wuyuan, Xingyi, and Urumqi. Pittsburgh Sleep Quality Index was used to conduct sleep quality survey. And conduct executive function was tested on middle school students, including inhibitory function, refresh function and conversion function tests. Spearman correlation and linear regression were used to analyze the relationship between sleep quality and executive function of middle school students.
Results:
The total Pittsburgh Sleep Quality Index (PSQI) score of boys was [4.0(2.0,6.0)] and that of girls was [5.0(3.0,6.0)], and the difference was statistically significant ( Z=-10.90, P < 0.01 ). The total PSQI score of boys was positively correlated with both 2-back reaction time and conversion function of executive function ( r =0.04, 0.04); the total PSQI score of girls was negatively correlated with 2-back reaction time ( r =-0.04) ( P <0.05). After controlling for variables such as mental health, physical activity and nutritional status,linear regression analyses showed that PSQI total score of middle school students was positively correlated with the inhibitory function and the conversion function response time [ B (95% CI )=1.28(0.21-2.34), 7.62(2.34-12.90), P <0.05]; the associations of total PSQI scores among middle school students with both 2-back and 1-back response time were not statistically significant [ B (95% CI )=-5.88(-16.14-4.37), 8.05( -3.39 -19.50), P >0.05].
Conclusion
Positive correlations are observed on sleep quality with inhibitory and conversion functions of executive function among middle school students.
2.Analysis of clinical diagnosis treatment and aeromedical evaluation of thyroid carcinoma in military pilots

Objective To summarize the clinicopathological characteristics,diagnostic and therapeutic process and aeromedical evaluation of 17 cases of thyroid cancer in Chinese military pilots in order to provide a reference for the diagnosis and treatment of disease and aeromedical evaluation of patients with thyroid cancer in military pilots in the future.Methods A retrospective analysis was conducted on the clinicopathological characteristics,treatment methods and aerospace medical evaluation conclusions of 17 military pilots with thyroid cancer who were hospitalized and underwent diagnosis,treatment and aeromedical evaluation at the Department of Aviation Medicine of Xijing Hospital from March 2021 to March 2025.Results All 17 pilots with thyroid cancer were male,and the age of onset ranged from 22 to 51 years old(with a median age of 40 years old).All patients underwent fine-needle aspiration biopsy of thyroid nodules or enlarged cervical lymph nodes before the operation.The postoperative pathological types were all papillary thyroid carcinoma.Among them,16 cases were papillary thyroid microcarcinoma,9 cases were combined with regional lymph node metastasis,and 1 case with postoperative complications of Horner's syndrome recovered after treatmentwas.14 patients underwent unilateral glandular lobe and isthectomy of the thyroid,3 patients underwent total/near-total thyroidectomy,2 patients received radioactive 131I treatment after the operation,and 17 patients received thyroid stimulating hormone(TSH)suppression therapy after the operation.After aerospace medical evaluation,15 cases were qualified for flight duties,the observation time was 3 to 6 months(with an average observation time of 4.2 months),1 case reached the age limit,and 1 case was temporarily unqualified for failure to achieve TSH suppression.After 0.5 to 4.0 years of follow-up,no patient showed signs of recurrence or metastasis.Conclusion For thyroid cancer in military pilots,universal screeningprotocols and earlydetection,as well as standardized therapeutic regimensshould be carried out.Evidence-based aeromedical evaluations and long-term monitoring should be conducted based on the aircraft type,jobs,and flight experience to preserve military combat effectiveness to the greatest extent.
3.Association between household and leisure-time physical activity and the risk of cardiometabolic multimorbidity in the elderly

Objective:To analyze the association between household and leisure-time physical activity (HLPA) and the risk of cardiometabolic multimorbidity (CMM) in the elderly.Methods:This retrospective cohort study utilized three longitudinal datasets from the Chinese Longitudinal Healthy Longevity and Happy Family Study (CLHLS-HF) covering 2008-2018, 2011-2018, and 2014-2018. A total of 10 876 participants aged ≥65 years who completed at least one follow-up were enrolled for cohort analysis. The general data, lifestyle, histories of chronic diseases, and physical examination results were collected. The HLPA score was utilized as an indicator to assess the degree of HLPA participation of the study subjects, the subjects were divided into 4 groups by quartile of HLPA score (Q1, Q2, Q3, and Q4), and were stratified jointly by sex, type of residence and household income; stratified Cox regression was applied to analyze the association between HLPA score and the risk of CMM morbidity, and Cox regression combined with restricted cubic spline (RCS) model was utilized to analyze the dose-response relationship between HLPA score and CMM morbidity.Results:The cohort was observed for a total of 51 630.24 person-years, with a median follow-up time of 3.95 (2.57, 6.04) years, with 576 new cases of CMM and an incidence density of 11.16 per 1 000 person-years. Compared with the HLPA score Q1 group, the risk of CMM morbidity was significantly reduced by 31%, 41%, and 44% in the elderly in the Q2 ( HR=0.69, 95% CI: 0.53-0.90), Q3 ( HR=0.59, 95% CI: 0.45-0.77) and Q4 ( HR=0.56, 95% CI: 0.42-0.74) groups, respectively (all P<0.05). The HLPA score was non-linearly associated with CMM morbidity ( P for nonlinear=0.010), with the risk of CMM morbidity decreasing with the increase of HLPA score, with a significant downward trend up to the median, after which the decline leveled off. There was an interaction between the HLPA score and exercise ( P for interaction=0.016); there was a negative association between the HLPA score and the risk of CMM morbidity in the no-exercise population ( HR=0.65, 95% CI: 0.52-0.80, P<0.001). Conclusion:There is a nonlinear negative association between HLPA and the risk CMM morbidity in elderly in China, and interventions focused on those who do not exercise may yield better results.
4.Evaluating the effectiveness of a training program for faculty supervisory specialists in standardized residency training based on adult learning theory

Objective:To explore the effectiveness of designing a training program for faculty supervisory specialists guided by adult learning theory.Methods:Based on the theory of adult learning, we designed a training program for faculty supervisory specialists in standardized residency training. A questionnaire survey was conducted to collect satisfaction and learning outcomes from 31 trainees who participated in this training program from 2019 to 2022. SPSS 25.0 was used to conduct nonparametric tests of supervisory course ratings for four core teaching activities in 2019 and 2022.Results:The study showed that 100.00% (31/31) of training participants were very satisfied or satisfied with the training content and training methods, 96.77% (30/31) were very satisfied or satisfied with the training sessions, and 90.32%(28/31) of training participants felt that they had mastered the supervisory requirements. The course grades for mini-lectures ( Z=-3.01, P<0.05) and overall course supervision ( Z=-2.47, P<0.05) were significantly different between 2019 and 2022. Compared with 2019, the number of externally assigned instructors and externally assigned assessment experts increased significantly in 2022. The training program for faculty supervisory specialists has a high value for promotion. Conclusions:The supervisory specialist training program designed under the guidance of adult learning theory helps increase the interest of training participants in the training program and ultimately improves the effectiveness of the training program.
5.Exploration of combined teaching evaluation based on characteristic analysis of raw scores and standard scores

Objective:To focus on clinical mini-lectures characterized by flexible teaching formats and significant challenges in scoring during supervision, systematically explore the roles and characteristics of raw scores (directly assigned by supervisory experts) and standard scores (revealing the relative position of instructors withing the group through Z-score conversion) in evaluating the teaching level and development trends of residency training teachers, investigate the feasibility of flexibly combining these two scoring methods, and provide new ideas for comprehensive, scientific, and objective teaching evaluations. Methods:In May 2024, 24 teachers who were supervised by three or more experts at a standardized training base for residents from 2019 to 2023 were selected as the research subjects. The study analyzed differences in mean rankings and development trends between raw and standard scores. Excel and SPSS were used for score transformation and statistical analysis of differences.Results:The mean rankings of standard scores differed from those of raw scores for 17 (70.83%) teaching staff members, among which 8 (33.33%) had a lower mean ranking of standard scores than raw scores, and 9 (37.50%) had an improved mean ranking of standard scores. For 10 (41.67%) teaching staff members, the differences in raw scores given among different supervisors were reversed after standardization conversion. The changes in the trends of scores for 17 (70.83%) teaching staff members throughout the years were inconsistent before and after score conversion.Conclusions:There were significant differences between the statistical results of standard scores and raw scores, and the two types of results evaluated the teaching level of teachers in terms of absolute situation and their relative positions in the entire teaching staff group, respectively. Therefore, combining the two types of scores with distinct characteristics for teaching evaluation can help teaching management departments comprehensively and objectively reveal the teaching level of teachers, while enhancing the confidence of faculty in teaching.
6.Anesthesia Management and Perioperative Outcome in Patients Receiving Left Ventricular Assisted Device Implantation

Objectives:Recently,domestical developed left ventricular assist devices(LVAD)have been frequently introduced into clinical practice.This study aims to report the anesthesia protocol and perioperative outcomes in Chinese patients receiving LVAD implantation surgery.Methods:This retrospective analysis included patients who underwent LVAD implantation at our center from June 2017 to November 2024.During and after separation from cardiopulmonary bypass,we optimized right heart function through careful adjustments in heart rate,rhythm,preload,myocardial contractility,and afterload.Vasoactive agents were administered as needed,and mechanical ventilation parameters were optimized.We implemented blood conservation strategies and established strict transfusion criteria to minimize allogenic blood transfusions.Results:A total of 100 patients were included in the analysis,with 54.0%classified as Interagency Registry for Mechanically Assisted Circulatory Support(INTERMACS)I or II.Before leaving the operating room,the mean arterial pressure(MAP),mean pulmonary arterial pressure(mPAP),central venous pressure(CVP),lactic acid levels,and urinary output after cardiopulmonary bypass were recorded as(74±7)mmHg,(25±7)mmHg,(7±3)mmHg,(2.3±1.9)mmol/L,and(8.2±5.4)ml/(kg·h),respectively.The transfusion rates for red blood cells and fresh frozen plasma were 20.0%and 28.0%.The in-hospital mortality rate was 3.0%,with a low incidence of severe complications including right heart failure(12%).Left ventricular ejection fraction increased from(23.7±4.8)%preoperatively to(25.3±10.5)%prior to discharge.Conclusions:Patients who received LVAD at our center exhibited low rates of postoperative mortality and complications and significant improvement in left heart function before discharge.
