1.The effects of maxillary protraction on soft and hard tissue in patients with cleft lip and palate in the mixed dentition period

Objective To study the three-dimensional changes of soft and hard tissue in male patients with unilateral non-syndromic cleft lip and palate in the mixed dentition period before and after maxillary protraction.Methods Twenty male patients with unilateral non-syndromic cleft lip and palate in the mixed dentition period treated by maxillary anterior traction in the Department of Orthodontics of Affiliated Stomatological Hospital of Nanjing Medical University were selected(average age(10.6±1.23)years old).Cone beam CT was taken before and after treatment.Dolphin 3D 11.95 software was used for three-dimensional measurement and analysis.SPSS 25.0 software package was used for statistical analysis.The self-controlled paired t test was used to compare the changes in soft and hard tis-sues of male patients with unilateral cleft lip and palate before and after treatment.The changes in the anterior displacement of the ANS point,the anterior displacement of point A,and the posterior displacement of point B were tested using the one-sample t test.Results The sagittal skeletal changes were significantly increased in ∠ SNA(P<0.01),∠ANB(P<0.01),Y axis(P<0.05),the forward displacement of ANS point(P<0.01)and A point(P<0.01)and the backward displacement of B point(P<0.01),but ∠SNB(P<0.05)was decreased significantly.The vertical skeletal changes showed that ∠MP-FH(P<0.01),∠MP-SN(P<0.05)and the dis-tance of ANS-Me(P<0.05)were increased significantly,but ∠SN-PP(P<0.01)was decreased significantly.The dental changes inclu-ding ∠U1-NA(P<0.01),the distance of U1-NA(P<0.01),∠U1-SN(P<0.01),overjet and the Wits were increased significantly,but ∠ L1-NB(P<0.01),the distance of L1-NB(P<0.01)and ∠L1-MP(P<0.01)were decreased significantly.The changes of soft tis-sue including ∠ S-Ns-Sn(P<0.01),∠ Sn-Ns-Bs(P<0.01),the distance of UL-EP(P<0.01)and LL-UL(P<0.01)were increased sig-nificantly.Conclusion After the treatment of maxillary protraction,the forward growth of maxilla will be possibly promoted on patients with cleft lip and palate in the peak of growth timing,as well as the intermaxillary relationship and soft tissue profile,but the side effects should be paid attention to.
2.One case of anaphylaxis due to recurrent cervical cancer treated with candonilimab

A 49-year-old female patient with recurrent cervical cancer was treated with injectable paclitaxel(albumin-bound)(400 mg,ivd,d1)+injectable cisplatin 100 mg,intraperitoneal instillation,d1+candonilimab injection(375 mg,ivd,d7).During the 2nd cycle of treatment,when the intravenous infusion of candonilimab was about 8 minutes,the patient suddenly had symptoms such as flushing,shortness of breath,cardiac discomfort,blurred consciousness with profuse sweating,etc.The candonilimab was immediately suspended,and after resuscitation,the patient's vital signs stabilised,and the treatment of the primary disease with candonilimab was terminated.According to the principle of correlation evaluation in Adverse Drug Reaction Reporting and Monitoring Workbook,the correlation between anaphylaxis and candonilimab was evaluated to be"very likely",and the common types of adverse reactions to candonilimab,the mechanism of anaphylaxis and pretreatment measures were also reviewed,with a view to providing referable opinions on the whole course of management of candonilimab and promoting the rational use of the drug in clinical safety.
3.Study on quality changes and shelf life of space pre-packaged scallion pancakes during storage

Objective Pre-packaged space scallion pancakes are prone to quality deterioration during storage due to starch retrogradation and lipid oxidation,which affects their food safety and sensory characteristics.Therefore,investigating their quality changes during storage and predicting shelf life are of great significance for ensuring the quality of space food.Methods study conducted accelerated shelf-life tests at(37±1)℃ and(47±1)℃ under simulated spacecraft food storage conditions.Comprehensive analyses were performed,including physicochemical properties(acid value,peroxide value,carbonyl value),microbiological indicators(total bacterial count,coliforms),color parameters(L*,a*,b*,c*,h°),and texture characteristics(hardness,adhesiveness,etc.).XRD and FTIR were employed to verify starch retrogradation and lipid oxidation.Results Physicochemical properties:The acid value gradually decreased and stabilized with prolonged storage.The peroxide value increased rapidly at 37℃and 47℃,with samples stored at 47℃approaching the safety limit(0.25 g/100 g)within 40 days,while the carbonyl value exhibited a decreasing trend.Microbiological indicators:All samples met the requirements of GJB 4992A-2012.No microbial contamination was detected initially,with only minor bacterial growth observed in later stages,and no coliforms were detected throughout the storage period.Color changes:During storage at 20℃,the a*value increased by 91.67%,while b* and c* values showed significant increases.Meanwhile,L* and h° values decreased by 7.42%and 8.75%,respectively.Texture deterioration:Overall texture characteristics declined,and XRD and FTIR analyses confirmed aggravated starch retrogradation.Shelf-life prediction:Correlation analysis identified peroxide value as the key factor.The established prediction model had an error margin of less than 10%,with predicted shelf lives of 236,121,and 57 days at 20℃,37℃,and 47℃,respectively.Conclusion This study reveals the quality change patterns of space scallion pancakes during storage.The established shelf-life prediction model provides theoretical and methodological support for quality monitoring of pre-packaged space foods.
4.Visualization of flagella and its applications in research on flagellar functions.

Flagella are important protein structures on the cell surface of bacteria and the main appendage for bacterial swimming. Flagella play a crucial role in bacterial motility, chemotaxis, pathogenicity, and environmental sensing. With the development of microscopic tracking technology and flagellum visualization tools, new forms of flagellar motility and increasing roles of flagella in the physiological activities of bacteria have been discovered. This review introduces the visualization methods of flagella and the applications of these methods in studying flagellar functions, giving insights into exploring the functions of flagella and laying a theoretical foundation for its future applications in inhibiting bacterial transmission and treating bacterial infections.

5.Clinical application of metagenomic next-generation sequencing for identifying non-tuberculous mycobacteria from bronchoalveolar lavage fluid

Objective:To evaluate the application of metagenomic next-generation sequencing(mNGS)in the identification of non-tuberculous mycobacteria(NTM).Methods:A retrospective analysis was conducted on mNGS results of 358 bronchoalveolar lavage fluid(BALF)samples positive for NTM collected at the First Affiliated Hospital of Zhejiang University School of Medicine from February 2021 to January 2024. The analysis included the distribution of NTM species,the detection of mixed pathogens,and the performance of conventional mycobacterial detection methods.Results:The results showed that 362 strains of 15 NTM species were identified from 350 specimens,8 specimens were not precise to the species level. The most frequently detected species were Mycobacterium intracellulare(37.3%,135/362), Mycobacterium abscessus(26.8%,97/362),followed by Mycobacterium avium(11.0%,40/362), Mycobacterium kansasii(8.0%,29/362)and Mycobacterium chelonae(7.7%,28/362). Single NTM species were detected in 339 specimens,while two or three NTM species were simultaneously detected in 11 specimens(3.1%,11/358). Non-NTM microorganisms co-infected were detected in 53.4%(191/358)of NTM-positive BALF samples,including common pathogens such as Pseudomonas aeruginosa, Staphylococcus aureus,and Aspergillus fumigatus;and difficult-to-identify pathogens such as Legionella pneumophila and Talaromyces marneffei. In NTM-positive patients detected by mNGS,the results supported the diagnosis of NTM infection in 298 cases(298/358,83.2%)and 105 cases(105/358,29.3%)initiated anti-NTM treatment accordingly;while in 60 cases(60/358,16.8%)the positive results were considered as colonization or unrelated to clinical infection. For samples tested with acid-fast staining,mycobacterial liquid culture,and DNA microarray,the positivity rates for NTM were 31.5%(73/232),48.7%(57/117),and 43.0%(46/107),respectively. Conclusions:mNGS demonstrates advantages in identification of NTM. However,the test may detect multiple microorganisms,in that case,the interpretation with clinical and radiological results is requried to determine the main pathogens.
6.Left ventricular myocardial work in detecting early cardiac dysfunction in patients with Fabry disease

Objective:To utilize the left ventricular myocardial work(LVMW)technique for early identification of myocardial damage in patients with fabry disease(FD).Methods:In an observational cross-sectional study,35 patients with FD who visited the Second Hospital of Hebei Medical University from February 2023 to April 2024 were included. They were categorized into two groups based on left ventricular mass index(LVMI):the non-left ventricular hypertrophy group(LVH-FD group,14 cases)and the left ventricular hypertrophy group(LVH+FD group,21 cases). Further gender-stratified analysis was performed on parameters related to left ventricular global longitudinal strain(LVGLS)and LVMW. Additionally,for the FD group with normal LVGLS,a comparison of their LVMW-related parameters was made with a control group. A control group consisting of 28 healthy subjects from the same period was selected for comparison. The LVGLS,global work index(LVGWI),global constructive work(LVGCW),global wasted work(LVGWW),and global work efficiency(LVGWE)of the three groups were analyzed using two-dimensional speckle tracking and non-invasive myocardial work techniques,and intergroup comparisons of these parameters were performed. Pearson's linear correlation was applied to analyse the correlation between LVGWI,LVGWE and LVMI.Results:① According to the LVMI grouping results,compared with the control group,the LVH-FD group showed decreased LVGWI( P<0.05),while the LVH+FD group exhibited reductions in LVGLS,LVGWI,and LVGWE(all P<0.05). Compared to the LVH-FD group,the LVH+FD group demonstrated lower LVGLS,LVGWI,and LVGWE(all P<0.05). ② Gender subgroup analysis revealed that in the female subgroup,no statistically significant differences were observed in LVGLS,LVGWI,and LVGWE between the female control group and the female LVH-FD group(all P>0.05). However,the female LVH+ FD group showed decreased LVGLS,LVGWI,and LVGWE compared to both the female control group(all P<0.05)and the female LVH-FD group(all P<0.05). In the male subgroup,compared to the male control group,the male LVH-FD group had reduced LVGWI( P<0.05)but no significant differences in LVGLS or LVGWE(both P>0.05). The male LVH+ FD group displayed lower LVGLS,LVGWI,and LVGWE compared to both the male control group(all P<0.05)and the male LVH- FD group(all P<0.05). ③Compared with the control group,the LVGLS-normal FD group had a reduced LVGWI( P<0.05),while there were no statistically significant differences in LVGLS,LVGWE,LVGCW,and LVGWW between these two groups(all P>0.05). ④LVGWI and LVGWE were negatively correlated with LVMI( r=-0.617,-0.707;both P<0.001). Conclusions:LVMW can detect early cardiac dysfunction in patients with FD and is of guiding value for their clinical management.
7.Analysis of the therapeutic efficacy of one-stage endoscopic retrograde cholangiopancreatography combined with laparoscopic cholecystectomy surgery in patients with common bile duct stones and gallbladder stones

Objective To evaluate the safety and feasibility of primary endoscopic retrograde cholangiopancreatolithiasis(ERCP)plus laparoscopic cholecystectomy(LC)in patients with choledocholithiasis combined with cholecystolithiasis.Methods From January 2023 to August 2024,185 patients with common bile duct stones combined with gallbladder stones were treated at our hospital.Patients who underwent ERCP+LC during the same surgical period were assigned to Group A(94 patients),and those who underwent LC+ERCP in separate sessions during the same hospital stay were assigned to Group B(91 patients).The gender,age,total hospitalization costs,number and size of stones,whether a drainage tube was left in place,intraoperative blood loss,duration of anesthesia,time to postoperative bowel gas passage,length of hospital stay,incidence of postoperative cholangitis,incidence of postoperative pancreatitis(PEP),rate of residual stones in the bile ducts,and changes in preoperative and postoperative neutrophils,neutrophil percentage,total serum bilirubin,indirect serum bilirubin,alanine aminotransferase,aspartate aminotransferase,and γ-glutamyl transferase were recorded for both groups of patients.Results Compared to Group B[(9.7±2.0)d],the hospital stay duration for patients in Group A[(8.6±2.6)d]was significantly reduced(P<0.05).The duration of anesthesia for patients in Group A[(2.5±0.8)h]was also significantly shorter than that for Group B[(3.7±0.5)h],with a statistically significant difference(P<0.05).Similarly,the total hospitalization costs for patients in Group A[(23 969±7 421)yuan]were lower than those for Group B[(25 427±5 236)yuan],and this difference was statistically significant(P<0.05).Conclusion The one-stage ERCP combined with LC treatment protocol for common bile duct stones with gallbladder stones is safe and feasible,with the potential advantages of reducing hospital stay,costs,and duration of anesthesia.
