1.Efficacy and safety of omadacycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneu-monia in children

OBJECTIVE To investigate the efficacy and safety of omadacycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneumonia (MUMPP) in children. METHODS A retrospective study was conducted on children aged 1-18 years old with MUMPP who were hospitalized in the Department of Pediatrics, the First Affiliated Hospital of Xinjiang Medical University from January 2022 to June 2025. According to the selection of secondary antibiotics after 72 h of initial treatment with macrolides, they were divided into the omadacycline group and the doxycycline group. Based on conventional treatment, children in the omadacycline group were given intravenous infusion of 2.4 mg/kg (once daily) of omadacycline tosylate, while children in the doxycycline group were given oral doxycycline hydrochloride tablets at 2 mg/kg (twice daily). The efficacy and safety were compared between the two groups of pediatric patients. Univariate analysis and multivariate Logistic regression analysis were performed on clinical efficacy, and subgroup analysis along with multiple sensitivity analyses were conducted to verify the robustness of the conclusions. RESULTS A total of 284 children with MUMPP were included in this study, with 142 in the omadacycline group and 142 in the doxycycline group. In terms of efficacy, although the hospitalization time of children in the omadacycline group was longer than that in the doxycycline group ( P <0.05), the lung lesion absorption rate and clinical efficacy were significantly higher or better than those in the doxycycline group ( P <0.05). The results of multivariate Logistic regression analysis showed that medication (OR=5.300, 95%CI: 2.526-11.123), length of hospital stay (OR=1.348, 95%CI: 1.167-1.556), and medication duration (OR=1.422, 95%CI: 1.169-1.729) were influencing factors of clinical efficacy ( P <0.05). The subgroup analysis results showed that the clinical efficacy of omadacycline was significantly better than that of doxycycline in all subgroups ( P <0.05). The results of multiple sensitivity analysis showed that the regression coefficients B of the four models (gradually adjust variables) before and after inverse probability of treatment weighting were significantly greater than 1 ( P <0.05). In terms of safety, there was no statistically significant difference in the inci dence of adverse drug reactions between the two groups of patients ( χ 2 =0.447, P =0.504). CONCLUSIONS In the case of hospitalization and prolonged medication, the efficacy of omadacycline in treating childhood MUMPP is superior to that of doxycycline, and its safety is good.
2.Comorbidity of myopia, and overweight and obesity among primary and middle school students in Yanqing District

Objective:
To investigate the prevalence of comorbidity of myopia, and overweight and obesity among primary and middle school students in Yanqing District, Beijing Municipality, so as to provide the evidence for \prevention and control of comorbidity.
Methods:
In September from 2020 to 2024, a stratified cluster sampling method was adopted annually to select primary, junior high school, senior high school, and vocational high school students in Yanqing District as survey subjects. Myopia was screened based on Screening Protocol for Myopia in Children and Adolescents. Height an weight were measured, and overweight and obesity were determined using the Screening for overweight and Obesity in School-Age Children and Adolescents.
Results:
A total of 9 883 individuals were surveyed, including 5 219 primary school students (52.81%), 2 486 junior high school students (25.15%), and 2 178 senior high school / vocational high school students (22.04%). There were 5 154 boys (52.15%) and 4 729 girls (47.85%). From 2020 to 2024, the numbers of primary and middle school students in Yanqing District with myopia, overweight, and obesity were 4 426, 1 897, and 3 164, respectively, with detection rates of 44.78%, 19.19%, and 32.01%. The detection rates of myopia, overweight, and obesity showed an increasing trend with the elevation of school stage (all P<0.05). The detection rate of myopia was higher in girls than in boys, while the detection rate of obesity was lower in girls than in boys (all P<0.05). There were 2 588 individuals with both myopia and overweight/obesity, representing a detection rate of 26.19%, which showed an increasing trend (P<0.05), rising from 25.04% in 2020 to 34.41% in 2024. The detection rates of comorbidity of myopia, and overweight and obesity among primary school, junior high school, and senior high school / vocational high school students were 13.22%, 36.93%, and 45.00%, respectively, showing an increasing trend with the elevation of school stage (P<0.05). The difference between genders was not statistically significant (P>0.05).
Conclusions
The detection rates of comorbidity of myopia, and overweight and obesity among primary and middle school students in Yanqing District is relatively high and shows an upward trend, with students in higher school stage being the key target group for prevention and control. It is recommended to strengthen health education on the co-prevention of multiple health issues and implement differentiated interventions based on school stages.

The construction of a training system for visiting physicians in the department of rare diseases in China is an important measure to improve the overall diagnosis and treatment capacity for rare diseases and address the critical challenge of insufficient knowledge and skills among clinicians in practice. This article systematically describes the visiting physician training system established by the Department of Rare Diseases at Peking Union Medical College Hospital. It summarizes the training objectives and positioning, design logic, and learning modules of the system, aiming to provide a reference for the construction of the specialized talent team for rare diseases in China.
4.A Case of Multidisciplinary Treatment for a Patient with Gorham-Stout Disease

Gorham-Stout disease(GSD) is a rare osteolytic disorder characterized by spontaneous and progressive osteolysis, along with abnormal angiogenesis and lymphangiogenesis, with no new bone formation. We present a case of a 15-year-old female admitted due to " recurrent right leg pain for 5 years, 11 months after undergoing right femoral fracture surgery". Through comprehensive integration of the patient's clinical phenotype, laboratory tests, imaging findings, pathological examinations, and molecular biological test results, GSD was considered highly likely. A multidisciplinary treatment approach was conducted, including a combination of zoledronic acid and sirolimus to inhibit osteolysis, along with rehabilitation training and orthopedic intervention, providing a personalized and comprehensive treatment strategy.
5.Perioperative Management of Duchenne Muscular Dystrophy and Accompanying Spinal Deformity: a Case Report

Duchenne muscular dystrophy (DMD) is an X-linked recessive genetic disorder caused by mutations in the dystrophin gene, classified as a rare congenital muscular disease. Its clinical features include progressive skeletal muscle weakness, often involving respiratory and cardiac muscles, and frequently associated with spinal deformities. This paper reports the diagnosis, perioperative management, and follow-up of a case of DMD with multisystem involvement and severe scoliosis, aiming to provide a reference for clinicians in the diagnosis and treatment of such diseases.
6.Correlation of emotional and behavioral problems among preschool children with maternal parenting competence and family rearing environment

Objective:
To investigate the emotional and behavioral problems among preschool children and the correlations with maternal parenting competence and family rearing environment, so as to provide a scientific basis and practical guidance for the physical and mental health development of preschool children.
Methods:
In June 2025, 660 preschool children aged 3-6 years old were selected from 10 kindergartens in Hefei, Anqing, and Tongling in Anhui Province by using a stratified cluster random sampling method. A questionnaire survey was conducted among all parents of preschool children using the Parent version of the Strengths and Difficulties Questionnaire, the Child Family Rearing Environment Scale, and the Chinese version of the Parenting Sense of Competence Scale. Pearson correlation analysis and multiple linear regression analysis were used to analyze the related factors of emotional and behavioral problems among preschool children. SPSS macro program Process 4.1 and Bootstrap method were used to test the mediating effect of family parenting environment between emotional and behavioral problems among preschool children and maternal parenting competence.
Results:
The detection rate of emotional and behavioral problems among preschool children was 20.15%. The total scores of family rearing environment and maternal parenting competence were negatively correlated with emotional and behavioral problems among preschool children ( r =-0.45,-0.79), and the total score of family rearing environment was positively correlated with the total score of maternal parenting competence ( r =0.43) (all P <0.01). Multiple linear regression showed that, after controlling whether being only child, parents educational level, registered residence location and other variables, family rearing environment, self efficacy and satisfaction were all negative predictors of emotional and behavioral problems among preschool children ( B =-0.07, -0.42, -0.42, all P <0.01). The mediation effect results showed that maternal parenting competence could positively predict the family rearing environment ( B =0.75), while maternal parenting competence and family rearing environment could both negatively predict emotional and behavioral problems among preschool children ( B =-0.49, -0.06 ) (all P <0.05). The Bootstrap sampling method test results showed that the effect value of the indirect effect of maternal parenting competence on emotional and behavioral problems through the family rearing environment was -0.04, and the effect proportion was 8.21 %.
Conclusion
Preschool children with a better family rearing environment and stronger maternal parenting competence are less likely to have emotional and behavioral problems, among which the family rearing environment has a mediating effect.
7.Shared genetic architecture between arrhythmia and obsessive-compulsive disorder: a heart-brain axis study based on multi-omics integration

BackgroundArrhythmia and obsessive-compulsive disorder (OCD) frequently co-occur in clinical and epidemiological settings, yet their shared genetic basis and potential heart-brain axis mechanisms remain unclear. ObjectiveTo systematically evaluate the genetic correlation between arrhythmia and OCD, and to elucidate their underlying molecular genetic mechanisms, so as to provide molecular evidence for the "heart-brain axis" to support risk assessment and integrated clinical strategies for these comorbidities. MethodsThe aggregated data from the genome-wide association study (GWAS) of arrhythmia in the UK Biobank (7 207 cases and 477 391 controls) and the GWAS data of OCD released by the Psychiatric Genomics Consortium (2 688 cases and 7 037 controls) were integrated, all of which were limited to individuals of European ancestry. The genome-wide genetic correlations were estimated using the linkage disequilibrium score regression (LDSC) and the high-definition likelihood (HDL). Local genetic correlation analysis was conducted using the local analysis of variance annotation (LAVA). Multi-trait analysis of GWAS (MTAG) was employed to identify pleiotropic loci. Shared risk genes were identified by combining summary-data based Mendelian randomization (SMR) and transcriptome-wide association study (TWAS). Functional enrichment analysis was performed based on the functional mapping and annotation (FUMA) platform. ResultsBoth LDSC (rg=0.248, 95% CI: 0.159–0.336, P=4.82×10-3) and HDL (rg=0.294, 95% CI: 0.237–0.351, P=5.87×10-4) revealed significant positive genetic correlation between arrhythmia and OCD. LAVA identified 23 significantly local correlated regions in the genome (P<2.0×10-5). MTAG discovered 11 genome-wide significant pleiotropic SNPs, among which rs12754189 (intron of KCNN3) had potential functional harmfulness (CADD>12.37). SMR and TWAS jointly identified 20 shared genes, enriched in neural-cardiovascular tissues such as the cerebral cortex, amygdala, and left ventricle, and involved in DNA damage response, RNA metabolism, transcriptional regulation, and FAS signaling pathway (FDR<0.05). ConclusionArrhythmia and OCD share a common genetic basis. The co-morbidity mechanism may involve the common vulnerability of neurons and cardiac muscle cells in terms of gene expression regulation and stress response, supporting the role of the brain-heart axis in the pathophysiology of both conditions.
8.Surveillance of Oncomelania hupensis snails following interruption of schistosomiasis transmission in Yunnan Province

Objective To investigate the distribution characteristics of Oncomelania hupensis snails in Yunnan Province fol-lowing interruption of schistosomiasis transmission, so as to provide the evidence for assessing the risk of schistosomiasis transmission and scientifically formulating the schistosomiasis surveillance program. Methods According to the requirements of the National Schistosomiasis Surveillance Scheme (2020 Edition), O. hupensis snail surveillance data were collected from 18 schistosomiasis-endemic counties (cities, districts) in Yunnan Province from 2020 to 2024, including area of snail survey, area of snail habitats, area of re-emerging snail habitats, number of frames surveyed, number of frames with O. hupensis snails, number of O. hupensis snails captured, and number of living snails, and the occurrence of frames with snails and mean density of living snails were calculated. Changes in snail status over the 5-year period from 2020 to 2024 and the differences in snail distributions specified by epidemic intensity, environmental type, and vegetation type were analyzed. Results The areas of snail survey increased from 1 727.96 hm2 in 2020 to 3 894.45 hm2 in 2024 (peak) across 18 schistosomiasis-endemic counties (cities, districts) in Yunnan Province during the period from 2020 through 2024. The areas of snail habitats increased from 70.36 hm2 in 2020 to a peak in 2023 (172.04 hm2), followed by a reduction to 132.36 hm2 in 2024, and the areas of re-emerging snail habitats increased from 42.71 hm2 in 2020 to a peak in 2022 (78.43 hm2), followed by a reduction to 40.21 hm2 in 2024. The occurrence of frames with snails and mean density of living snails increased from 1.24% (3 025/244 404) and (0.033 2 ± 0.038 7) snails/0.1 m2 in 2020 to peaks at 2.03% (6 231/307 563) and (0.066 9 ± 0.068 4) snails/0.1 m2 in 2023, followed by reductions to 1.04% (5 829/559 941) and (0.032 6 ± 0.057 7) snails/0.1 m2 in 2024, respectively. There was a significant difference in the occurrence of frames with snails over the 5-year study period (χ2 = 1 962.95, P < 0.05), and the occurrence of frames with snails reduced by 48.71% in 2024 relative to in 2023 (χ2 = 1 411.05, P < 0.005); however, there was no significant difference in the mean density of living snails over the 5 years (H = 5.310, P > 0.05). There were significant differences in the occurrence of frames with snails (χ2 = 481.27, P < 0.05) and mean density of living snails (H = 6.872, P < 0.05) in schistosomiasis-endemic areas with different epidemic intensities. The occurrence of frames with snails (χ2 = 25.32 and 38.70, both P values < 0.017) and mean density of living snails (Z = 28.55 and 49.96, both P values < 0.017) were higher in schistosomiasis transmission-interrupted and eliminated areas with snails than in schistosomiasis-eliminated areas without snails, and the occurrence of frames with snails (χ2 = 453.54, P < 0.017) and mean density of living snails (Z = −56.97, P < 0.017) were higher in schistosomiasis-eliminated areas with snails than in schistosomiasis transmission-interrupted areas with snails. O. hupensis snails were mainly distributed in paddy fields, dry farmlands and ditches; however, the occurrence of frames with snails (13.40%, 424/3 164) and mean density of living snails [(0.252 8 ± 0.158 7) snails/0.1 m2] were higher in ponds/weirs than in other types of environments (both P values < 0.05). Rice, dry farmland crops and weeds were main vegetations in which O. hupensis snails were distributed, and the occurrence of frames with snails (2.29%, 7 111/310 140) and mean density of living snails [(0.072 3 ± 0.018 9) snails/0.1 m2] were higher in weeds than in other types of environments (both P values < 0.05). Conclusions O. hupensis snails have been effectively controlled in Yunnan Province following implementation of integrated schistosomiasis control measures; however, there are still risk factors for schistosomiasis transmission, including reduced attention to schistosomiasis control and snail re-emergence. Improved control efforts and surveillance system construction and timely identification of risk factors of snail status and timely management are recommended to ensure the achievement of the target of schistosomiasis elimination as scheduled.
9.Clinical characteristics of neonatal necrotizing enterocolitis and analysis of risk factors for early-onset children

Objective:To explore the clinical characteristics of neonatal necrotizing enterocolitis(NEC)and analyze the risk factors for early-onset NEC.Methods:A total of 220 children with NEC admitted to the Department of Pediatrics,Tianjin Medical University General Hospital from January 1st,2018 to February 29th,2024 were retrospectively selected as the research objects. According to the time of onset,the early-onset group( n=120)and the late-onset group( n=100)were established,and the clinical characteristics of the two groups were compared. Another 150 cases of normal healthy newborns born in this hospital in the same period were selected as the control group,and the clinical data of the control group were collected. The clinical characteristics of the early-onset group and the control group were compared,and the risk factors of early-onset NEC were analyzed by multivariate Logistic regression. Results:Compared with the late-onset group,the early-onset group had fever[50.0%(60/120)vs. 40%(40/100), χ2=7.333, P=0.007],apnea[39.17%(47/120)vs. 28%(28/100), χ2=7.568, P=0.006],no rise in body temperature[56.67%(68/120)vs. 39%(39/100), χ2=6.815, P=0.009],abdominal distension[25%(30/120)vs. 40%(40/100), χ2=13.200, P<0.001],vomiting[30.83%(37/120)vs. 45%(45/100), χ2=12.797, P<0.001]was significantly different(all P<0.05);Multivariate Logistic regression analysis:weight<1 500 g( OR=5.871,95% CI:3.153~9.673, P<0.001),gestational age<30 weeks( OR=4.256,95% CI:2.641~7.896, P=0.007),hemodynamically significant patent ductus arteriosus(hs-PDA)( OR=3.113,95% CI:1.865~5.133, P=0.033),severe anemia( OR=3.057,95% CI:2.165~4.802, P=0.001),feeding intolerance( OR=4.215,95% CI:1.579~10.802, P=0.005),amniotic fluid pollution( OR=2.452,95% CI:1.579~3.111, P<0.001)were the independent risk factors for early-onset NEC(all P<0.05). Conclusion:Weight<1 500 g,gestational age<30 weeks,hs-PDA,severe anemia,feeding intolerance,and amniotic fluid contamination are independent risk factors for early-onset NEC. In clinical practice,more attention should be paid to these factors for disease prevention,early identification,and timely intervention in newborns to reduce the occurrence of NEC.
10.Predictive value of stone burden on the therapeutic effect of extracorporeal shock wave lithotripsy for upper ureteral stones

Objective:To explore the value of applying stone burden in evaluating the treatment efficacy of patients with upper ureteral stones undergoing extracorporeal shock wave lithotripsy.Methods:A retrospective selection was conducted on 328 patients with upper ureteral stones admitted to Beijing Friendship Hospital, Capital Medical University from October 2018 to August 2024. The maximum diameter of the stones was 1.0-2.0 cm, and the patients were divided into successful lithotripsy group ( n=306) and failed lithotripsy group ( n=22) based on the success or failure of lithotripsy. Using multi-slice spiral CT scanning to collect the longest diameter, maximum CT value, volume, and average CT value of the stones in the enrolled patients, evaluate the predictive value of stone burden on the effectiveness of extracorporeal shock wave lithotripsy in patients with upper ureteral stones. Measurement data were expressed as mean ± standard deviation ( ± s), and t-test was used for comparison between groups; count data were expressed as the cases and percentage, and Chi-square test was used for comparison between groups. Univariate and multivariate Logistic regression analysis were used to evaluate the factors affecting the efficacy of extracorporeal shock wave lithotripsy in patients with upper ureteral stones. The area under the curve (AUC) of receiver operating characteristic (ROC) curve was used to evaluate the predictive value of each influencing factor for the therapeutic effect. Results:The results of univariate analysis showed that there were no statistically significant differences in age, gender, disease duration, and stone side between the successful and failed lithotripsy groups ( P>0.05). However, there were statistically significant differences in the longest diameter, maximum CT value, volume, average CT value of the stones and stone burden ( P<0.05). The results of multivariate Logistic regression analysis showed that the longest diameter, maximum CT value, volume, average CT value of the stones and the stone burden were all risk factors affecting the therapeutic effect of extracorporeal shock wave lithotripsy for upper ureteral stones ( P<0.05). The results of ROC curve analysis showed that the AUC of the longest diameter, maximum CT value, volume, average CT value of the stones and the stone burden, burden 1, burden 2, and burden 3 were 0.844, 0.827, 0.948, 0.873, 0.889, 0.835, 0.911 and 0.865, respectively. Conclusion:The stone burden has good predictive value for the therapeutic effect of extracorporeal shock wave lithotripsy in the treatment of upper ureteral stones, and is worthy of promotion and application.
