Objective:To investigate the effect of dandelion polysaccharide(DP)on inflammatory response and the protein expression of S100 calcium binding protein A8/A9(S100A8/A9)in lung tissue and small intestinal tissue of rats with multiple organ dysfunction syndrome(MODS).Methods:The two-hit method of hemorrhagic shock and intraperitoneally injected lipopolysaccharide was used to establish a rat model of MODS,and the rats were divided into sham-operation group, model group,low-dose DP group, and high-dose DP group. The organ coefficient and wet/dry weight ratio of the lung and the small intestine were observed for each group of rats;HE staining was used to observe the pathomorphological changes of lung tissue and small intestinal tissue;immunohistochemical staining was used to measure the expression of interleukin-1β(IL-1β),interleukin-6(IL-6),and interleukin-10(IL-10)in lung tissue and small intestinal tissue;Western blot was used to measure the protein expression level of S100A8/A9 in lung tissue and small intestinal tissue. Results:Compared with the sham-operation group, the model group had significant increases in the organ coefficient of the lung(5.849±0.824),the wet/dry weight ratio of the lung(6.556±0.631),the wet/dry weight ratio of the small intestine(6.356±0.535),and the wet weight/length ratio of the small intestine(73.950±5.569).HE staining showed that that the model group had massive in-flammatory cell infiltration in alveolar space and pulmonary interstitium, thickened alveolar wall, and disintegration and fragmentation of the villi of the small intestine, with inflammatory cell infiltration and proliferation of segmental aggregated lymphoid follicles. In the model group,S100A8/A9 was mainly expressed in neutrophils and macrophages, and there were increases in the expression of S100A8/A9,IL-1β,and IL-6 and a reduction in the expression of IL-10 in the lung tissue and small intestinal tissue of rats. After treatment with high-dose DP,there were reductions in the organ coefficient of the lung(4.297±0.462),the wet/dry weight ratio of the lung(5.313±0.495),the wet/dry weight ratio of the small intestine(5.398±0.388),and the wet weight/length ratio of the small intestine(59.417±2.891).The high-dose group also had alleviation of pathological injury in the small intestine, with reductions in the expres-sion of S100A8/A9,IL-1β,and IL-6 and an increase in the expression of IL-10 in lung tissue and small intestinal tissue. Conclusion:DP may alleviate inflammatory response in lung and small intestinal injuries of rats with MODS by inhibiting the expression of S100A8/A9.

Objective To construct quality evaluation indicators for perioperative nursing in heart transplantation, and to provide standard and professional quantitative bases for monitoring and management of perioperative nursing quality. Methods This study was conducted based on the frame work of the three-dimensional"structure-process-outcome"quality model, using literature review,Delphi method and analytic hierarchy to determine the content of the indicators, and the weight of each index. Results A total of 22 experts from 14 qualified heart transplantation hospitals were included, and a total of 2 rounds of consultations were conducted. The effective recovery rates of 2 rounds of expert consultation questionnaires were 100%.The authority coefficients were 0.817.The variation coefficients of each item ranged from 0.025~0.169 and 0.039~0.157.The Kendall harmony coefficients were 0.126 and 0.225(P<0.001).The final evaluation indicators for perioperative nursing quality in heart transplantation included 3 first-level indicators,12 second-lever indicators and 59 third-level indicators. Conclusion The evaluation indicators of perioperative nursing quality in heart transplantation was scientific, comprehensive and specialized, which can provide references for the evaluation of perioperative nursing quality in heart transplantation.

To summarize the nursing care of a patient with acute myocardial infarction complicated with cardiogenic shock before operation. The main nursing points are as follows:in the acute stage, the integrated rescue was implemented with rapid on-machine coordination of external cardiopulmonary resuscitation, multidisciplinary collaboration to ensure safe patient transport;during the bridging period, the combined operation care of extracorporeal membrane oxygenation,intra-aortic balloon counter pulsation and continuous kidney replacement therapy was carried out with goal-oriented anticoagulation care, prevention of catheter-related infection with the assistance of mechanical circulation, and neurological function monitoring. The patient successfully passed the waiting period for heart transplantation and underwent heart transplantation 21 days after admission. With the follow-up for 1 year, the patient recovered well.

Objective To develop SYBR Green I real-time PCR assay for detection and identification of Hepatitis B virus. Methods Based on the sequences of Hepatitis B virus gp1 gene, primers were designed. The reaction assay and thermal cyc-ling profile were optimized. The positive standard was from recombinant clone. Both the developed assay and Zhejiang kuake biotechnology company’s assay were applied in 100 patients serum. Results The detection limit was between 5×102 copies/ml to 5×108 copies/ml with a good liner correlation and no cross reaction. The whole process just needed 2.5 h.Comparing with the company products, the sensitivity and specificity of the developed assay were 100% and 92.5% respectively.Con-clusion The established assay is rapid, simple, high sensitivity and specificity. It is not only valuable for the identification of Hepatitis B virus patients, but also provide accurate quantitative analysis for HBV patients.
