1.Analysis of drug resistance and infection-related risk factors of carbapenem-resistant Klebsiella pneumoniae and extended-spectrum β-lactamase-producing Escherichia coli in patients with severe pneumonia in ICU

Objective To analyze the rates of drug resistance of carbapenem-resistant Klebsiella pneumoniae (CRKP) and extended-spectrum β-lactamase-producing Escherichia coli (ESBL-E.coli) in bronchoalveolar lavage fluid (BALF) of critically ill patients with severe pneumonia and artificial airway and risk factors of CRKP. Methods Patients with severe pneumonia and artificial airway treated in the Center of Emergency and Intensive Care Unit, Jinshan Hospital, Fudan University from May 2022 to May 2025 were selected. Klebsiella pneumoniae (K. pneumoniae) and Escherichia coli (E.coli) isolated and cultured from BALF of these patients were analyzed. The drug resistance rates of the two bacteria were compared. The multivariate logistic regression was used to analyze related risk factors for CRKP infection. Results A total of 384 patients were included, and 384 strains were cultured, including 330 strains (85.9%) of K. pneumoniae and 54 strains (14.1%) of E.coli. The infection rate of K. pneumoniae was higher. Compared with E.coli, the drug-resistant gene expression rate of K. pneumoniae was lower (37.3% vs 57.4%, P<0.01). The resistance rates of CRKP to cefotaxime, cefepime, aztreonam, and imipenem were higher than those of ESBL-E.coli (P<0.003). Both CRKP and ESBL-E.coli had high sensitivity to polymyxin and tigecycline. Multivariate logistic regression showed duration of mechanical ventilation ≥ 7 days, and combination of ≥ 2 antibiotics for over 7 days were independent risk factors for CRKP infection (P<0.01). Conclusions For patients with severe pneumonia and artificial airway in ICU, K. pneumoniae is the main bacterial infection. CRKP and ESBL-E.coli in BALF exhibit different degrees of drug resistance to various antibacterial agents, both have the characteristic of multidrug resistance, and are sensitive to polymyxin and tigecycline. Prevention and control of risk factors related to CRKP should be formulated particularly.
2.Correlation between choreiform symptoms of hepatolenticular degeneration and caudate nucleus atrophy on brain magnetic resonance imaging

Objective To quantitatively analyze the volumetric characteristics of each subregion of the basal ganglia in patients with hepatolenticular degeneration (also known as Wilson disease ,WD) using brain magnetic resonance imaging (MRI) and brain segmentation technology, to explore the specific imaging findings of choreiform symptoms, and to assess the clinical value of caudate nucleus atrophy as an imaging indicator for this symptom. Methods A retrospective analysis was performed for 40 WD patients with choreiform symptoms and 40 patients without choreiform symptoms from June 2023 to June 2025, and clinical indicators were compared between the two groups. In addition, the two groups were compared in terms of the volume of the basal ganglia after estimated total intracranial volume (eTIV) correction, and the correlation between the volume of differential brain regions and the chorea subscale score of Unified Wilson's Disease Rating Scale (UWDRS) was explored. Results There were no significant differences in baseline data between the two groups. UWDRS scores showed that the choreiform group had a higher neurological function score (P=0.005), a significantly higher chorea subscale score (P<0.01), and a lower hepatic function score (P<0.01). The choreiform group had a significantly smaller caudate nucleus volume than the non-choreiform group (P<0.001), suggesting severe subregional atrophy, and in contrast, the choreiform group had a significant increase in thalamus volume (P=0.002). Caudate nucleus volume ratio was significantly negatively correlated with chorea subscale score in the choreiform group (P<0.001). Conclusion Caudate nucleus atrophy is a specific imaging finding of choreiform symptoms in WD patients, and a quantitative analysis of caudate nucleus volume is expected to become an objective imaging indicator for assessing the severity of choreiform symptoms and monitoring disease progression in WD.
3.Empirical Exploration of the Determination Rules for Medical Insurance Payment Standards for Non-Winning Drugs in Centralized Volume-Based Procurement

Objective:It aims to conduct empirical exploration on the rules for determining the medical insurance payment standards for non-winning drugs with the same generic name in the national medical centralized volume-based procurement,and provided decision-making references for further determination of medical insurance payment standards.Methods:Taking the first three batches of non-winning drugs for centralized procurement as research objects,based on real-world data from medical institutions nationwide,using commonly used pricing methods such as the German reference pricing method and the volume-price weighting method,different calculation schemes are designed to calculate the medical insurance payment standards of the above research objects.Comparation is conducted on the measurement for the decrease in payment standards of highest price of non-winning drugs and the individual burden under the payment standard between different calculation methods.Results:The German reference pricing method has slightly lower medical insurance payment standards compared to the volume-price weighting method,resulting in slightly higher price reductions and personal burdens.The results of different volume-price weighting method calculation schemes are similar.Conclusion:It is recommended to adopt the volume-price weighting method to formulate the medical insurance payment standard for the national centralized procurement of non-winning drugs,the reduction is more reasonable and the patient's personal burden is lower,and it can be better connected with the existing policy to gradually form a unified payment standard with the common name and return to the bid-winning price.
4.Knockdown of GPER1 aggravates neuronal injury and cognitive dysfunction after epilepsy

Aim To investigate the impact of G pro-tein-coupled estrogen receptor 1(GPER1),also known as GPR30 playing a significant role in the nerv-ous system,on neuronal damage and cognitive dysfunc-tion following epileptic seizures.Methods The pro-tein expression levels of GPER1 and the DNA damage marker γ-H2AX in epileptic rats were assessed using Western blot.The hippocampal neuronal damage and apoptosis in pilocarpine-induced epilepsy models were evaluated using Nissl and TUNEL staining techniques,compared with GPER1 knockdown(GPER1-KD)rats with wild-type(WT)controls.The behavioral activi-ties,including memory and spatial learning,were mo-nitored during the chronic phase of epilepsy using the IntelliCage system.Results Compared to the control group,GPER1 protein expression in the cerebral cortex and hippocampus significantly increased 24 hours post-epilepsy onset.In the GPER1-KD+EP group,hipp-ocampal neuronal damage was more severe,with a sig-nificant increase in apoptotic neurons compared to the WT+EP group.The IntelliCage data revealed that during free exploration,nose contact,position learn-ing,and reverse position learning stages in the GPER1-KD+EP group exhibited fewer visits and a higher error rate than in the WT+EP group.Conclu-sions Deficiency in GPER1 impairs memory and spa-tial learning abilities following epilepsy,potentially due to exacerbated neuronal injury,apoptosis,and inflam-mation.GPER1 represents a promising therapeutic tar-get for mitigating post-epileptic nerve damage and cog-nitive impairment.
5.Analysis of the effect and influencing factors of auditory and speech rehabilitation after cochlear im-plantation in children with bilateral cochlear deficiency

Objective To study the effect of auditory and speech development after cochlear implant(CI)in children with bilateral cochlear nerve deficiency(CND)and its influencing factors.Methods A total of 20 children with bilateral CND were included in the study,of which 5 were implanted bilaterally and 15 unilaterally.CT of the temporal bone showed stenosis of the cochlear aperture in 14 cases and atresia of the cochlear aperture in 6 cases.There were 8 cases accompanied by other inner ear malformations,and 12 cases with no accompanying inner ear mal-formations.MRI of the internal auditory canal showed 1 nerve in 5 cases,2 nerves in 6 cases,3 nerves in 8 cases,and 4 nerves in 1 case.There were 6 cases in which the EABR was not elicited and 14 cases in which it was elicited.The postoperative auditory and speech abilities of the subjects were evaluated using categories of auditory perform-ance(CAP)and speech intelligibility rating(SIR).Results ① The CAP(P<0.001)and SIR(P<0.001)scores of the children with stenosis of the cochlea nerve canal were higher than those of the patients with atresia of the cochlea nerve canal.② The more nerve roots in the internal auditory canal,the higher the score of CAP(P=0.003)and SIR(P=0.008).③ CAP score of the children with EABR elicited was higher than that of the children without EABR elicited(P=0.030).The difference in SIR scores was not statistically significant(P=0.14).④The differences in CAP and SIR between those with bilateral CI and unilateral CI,as well as between those with and without other inner ear malformations,were not statistically significant(P>0.05).Conclusion Children with bi-lateral CND had significant postoperative improvement in auditory function but poor speech development after CI.Postoperative auditory speech ability was related to the condition of the cochlear foramen,the number of nerve roots in the internal auditory canal,and whether or not the EABR was elicited intraoperatively.
6.Self-determination experience in home-based exercise rehabilitation of stroke patients:a qualitative meta-synthesis

Objective To systematically evaluate and integrate real experience and feelings of self-determination in home-based exercise rehabilitation of stroke patients.Methods We searched databases including PubMed,Embase,Web of Science,Cochrane Library,CINAHL,CNKI,Wanfang,VIP,Sinomed.The literature of qualitative research on self-determination experience in home-based exercise rehabilitation of stroke patients from the establishment of the database to February 2024 was retrieved.The literature was evaluated using the Australian JBI Quality Evaluation Criteria for Qualitative Research in Evidence-based Health Care Centers(2016),and the results were consolidated using an aggregative integration approach.Results A total of 23 theme were extracted,and 10 new categories were formed after induction and integration,which were finally merged into 3 integrated results,including multiple dilemmas in rehabilitation self-determination,motivation transformation during self-determination,strategies to achieve self-determination.Conclusion During the process of home-based exercise rehabilitation for stroke patients,medical personnel should pay attention to patients'determination dilemma,motivation changes and related strategies they adopt.Targeted decision-making programs should be developed for stroke patients,aiming for the scientific and precise advancement of exercise rehabilitation.
7.Clinical Study on the Efficacy of Yiqi Huayu Jiedu Decoction in Reducing the Risk of Recurrence and Metastasis of Postoperative Gastric Cancer

Objective To evaluate the effects of Yiqi Huayu Jiedu Decoction combined with chemotherapy on disease-free survival rate and disease-free survival(DFS)of patients with stage Ⅱ and Ⅲ postoperative gastric cancer.Methods Totally 102 patients with stage Ⅱ and Ⅲ postoperative gastric cancer treated by Affiliated Hospital of Nanjing University of Chinese Medicine and Nanjing Drum Tower Hospital were selected.The patients were divided into control group(52 cases)and test group(50 cases)with block random method.The control group received conventional postoperative adjuvant chemotherapy for 4-6 cycles.The test group was treated with Chinese materia medica for 6 months on the basis of chemotherapy.Continue follow-up until the patients had recurrence,metastasis or death.DFS was analyzed by Kaplan-Meier method,and the average DFS of the two groups was estimated.Log Rank test was used to test the significance of differences in survival distribution between groups.Univariate and multivariate analyses of variables affecting prognosis were conducted using the Cox regression model.Adverse reactions of both groups were monitored.Results The recurrence and metastasis rate of the test group was 12.0%(6/50),while that of the control group was 34.6%(18/52),which was lower in the test group than in the control group(P<0.05).The 1-year and 2-year DFS rates and DFS of the control group were 86.5%,72.2%and 25.8 months respectively,while those of the test group were 95.6%,84.1%and 33.9 months respectively,with statistical significance(P=0.012).Cox-regression analysis suggested:The independent factors influencing the disease-free survival time of patients with gastric cancer were the test group(P=0.022),surgical methods of subtotal gastrectomy(P=0.038)and clinical stages Ⅱ(P=0.040).No adverse reactions related to Chinese materia medica were reported in the two groups.Conclusion Yiqi Huayu Jiedu Decoction combined with chemotherapy can improve the 1-year and 2-year DFS rate,prolong DFS,reduce the recurrence and metastasis rate,and improve the prognosis.
8.Association between modelling cycle and antioxidant pathways in rat models of non-alcoholic fatty liver disease

Objective:To construct a rat model of non-alcoholic fatty liver disease(NAFLD)by choline-deficient high fat diet(CDHFD),and to observe the association between feeding cycle and antioxidant pathway.Methods:The study lasted 16 weeks and was divided into 4 cycles.Detection of pathological changes and expression of antioxidant enzymes in rats liver in different cycles.Results:The early stage of liver steatosis and inflammation in model rats was 2~4 weeks,non-alcoholic steatohepatitis(NASH)stage was 4~8 weeks,and liver fibrosis progression was 8~16 weeks.Mechanistic studies had shown that the expressions of antioxidant enzymes Nrf2,SOD and GSH-Px in the liver of NAFLD rats gradually decreased with the extension of the feeding cycle.Conclusion:Different modeling cycles can successfully induce the pathological changes of steatosis,inflammation and liver fibrosis in rat liver,and the pathological changes are time-dependent with the expressions of antioxidant enzymes.
