1.Expert recommendations on vision friendly built environments for myopia prevention and control in children and adolescents

Abstract
The prevention and control of myopia in Chinese children and adolescents has become a major public health issue. While maintaining increased outdoor activity as a cornerstone intervention, there is an urgent need to explore new complementary approaches that can be effectively implemented in both indoor and outdoor settings. In recent years, environmental spatial frequency has gained increasing attention as one of the key environmental factors influencing the development and progression of myopia. Both animal studies and human research have confirmed that indoor environments lacking mid to high spatial frequency components, often characterized as "visually impoverished", can promote axial elongation and myopia through mechanisms such as disruption of retinal neural signaling, impaired accommodative function, and altered expression of related molecules. Based on the scientific consensus, it is recommended that "enriching of environmental spatial frequency" should be integrated into the myopia prevention and control framework. Following the principles of schoolled organization, family cooperation, community involvement, and student participation, specific measures are put forward in three areas:optimizing school visual settings, improving home spatial environments, and promoting healthy visual behavior. The aim is to create "visually friendly" indoor environments as an important supplement to outdoor activity, thereby providing a novel perspective and strategy for comprehensively advancing myopia prevention and control among children and adolescents.
2.Reflections on Status Quo and Development Pathways of Traditional Chinese Medicine Technology Transfer in Context of Digital-intelligent Transformation

As a distinctive resource of Chinese civilization, traditional Chinese medicine (TCM) technology transfer faces significant opportunities under the background of digital and intelligent transformation, while also being constrained by unique challenges such as the complexity of its theoretical system, lengthy industrial chains, and multidimensional policy restrictions, resulting in a "high-value-high-threshold" paradox. At present, TCM technology transfer is deeply trapped in a "threefold reluctance" dilemma, i.e., unwillingness to transfer, inability to transfer, and lack of capacity to transfer. Specifically, the disconnection between scientific research evaluation systems and market demand leads to low conversion rates of research achievements, unclear ownership and compliance risks suppress innovation incentives, and the absence of professional services intensifies supply-demand mismatches. This article systematically analyzes the specific characteristics of TCM technology transfer and proposes a breakthrough pathway centered on full-chain digital and intelligent transformation. By integrating technologies such as intelligent sorting systems, blockchain-based traceability, and AI diagnostic models, the TCM ecosystem spanning "cultivation-production-service" can be reconstructed. In terms of standardization, promoting the progression from "experience-based data conversion" to "data standardization" and further to "intelligent standardization" is advocated to resolve quality control challenges. For example, a "three-no-one-full" certification system can strengthen quality trust. Policy coordination should focus on optimizing mechanisms for the transformation of scientific and technological achievements, while exploring intellectual property securitization and risk-sharing models to stimulate research momentum. In terms of internationalization, reliance on the Belt and Road Initiative platform to promote the export of geo-authentic medicinal material brands and standards is recommended to build a dual-driven model of "technology plus culture". Looking ahead, through the construction of national-level databases, the cultivation of interdisciplinary talent, and the mutual recognition of international standards, a new paradigm of "scientific intelligent manufacturing" can be formed, providing systematic solutions for the modernization of TCM and global health governance.
3.Identification and Analysis of bHLH Genes Related to Color Formation of Gastrodia elata Stem

ObjectiveGastrodia elata has evolved ecological types with shortened rhizome internodes and diversified flower and fruit coloration in response to different altitudes. Studying the genetic mechanisms of different ecotype germplasm is significant for guiding variety breeding in different cultivation areas. MethodsThe bHLH gene family was identified based on the whole-genome datasets of G. elata f. elata and G. elata f. glauca. Subsequently, the gene family members were subject to analysis, including gene structure, chromosomal localization, cis-acting elements, gene synteny, and phylogeny. Combined with transcriptome data and quantitative Real-time PCR, the expression patterns of bHLH genes in the stems of the different G. elata ecotype germplasm were analyzed. Finally, correlation analysis was conducted between gene expression patterns and color to obtain the key bHLH genes regulating the color formation of stem. ResultsA total of 63 bHLH genes were identified in both G elata f. elata and G. elata f. glauca, unevenly distributed across 17 chromosomes and clustered into 16 subfamilies, with significant expansion in some family members. Obvious inversions of bHLH genes on the same chromosome and interchromosomal translocations were detected in the two ecotype germplasm. Among these genes, 12 bHLH genes (such as bHLH62-3 and bHLH74) were associated with the bright yellow color of G elata f. elata stem, while 9 bHLH genes (such as PIL13, UNE12, and bHLH130) were correlated with the red color of G. elata f. glauca stem. Compared to G. elata f. glauca, the bHLH48 expression level was significantly higher in flowers and scale leaves of G elata f. elata, and the bHLH62-3 expression level was significantly higher in all organs of G elata f. elata. ConclusionsFunctional pathway divergence of the bHLH family members has occurred across different chromosomes in G elata f. elata and G. elata f. glauca. Through synergism or antagonism with other genes, 21 bHLH genes participate in the coloration metabolic pathway regulation of stems, flowers, and fruits. Specifically, bHLH62-3 is involved in regulating stem color differentiation in the anthocyanin biosynthesis pathway of G. elata, thus relevant to the color formation of stem. Additionally, GebHLH48 positively regulates flowering-related pathways to promote the early-flowering phenotype of G. elata f. elata. These findings have laid the foundation for analyzing the genetic regulatory mechanisms underlying the color formation of the G. elata stem.

5.Analysis of clinical characteristics and risk factors for infection in patients with multiple myeloma treated with bortezomib

OBJECTIVE To study the clinical characteristics and potential risk factors for infection in patients with multiple myeloma (MM) following treatment with bortezomib. METHODS Clinical data were retrospectively collected from MM patients who received bortezomib-based treatment regimens at the Department of Hematology, the Second Affiliated Hospital of Soochow University, from October 2021 to February 2025. The collected data primarily included demographic characteristics, disease characteristics of MM, treatment regimens, occurrence of infections and corresponding management measures, and prophylactic medication use. Univariate and multivariate Logistic regression analyses were conducted to identify potential risk factors for MM complicated with infection. RESULTS Among the 284 MM patients treated with bortezomib, 132 patients (46.5%) experienced at least one infection. The predominant types of infections were respiratory tract infections and gastrointestinal infections. Univariate analysis showed that age at initial diagnosis, pathological classification, and grade of myelosuppression were influencing factors for infection in MM patients ( P <0.05). Further analysis of influencing factors for the two main types of infections revealed that sex, age at initial diagnosis, pathological classification, treatment regimen, and smoking history were influencing factor s for respiratory tract infections in MM patients ( P <0.05); BMI, pathological classification, treatment regimen, and grade of myelosuppression were influencing factors for gastrointestinal infections in MM patients ( P <0.05). Multivariate Logistic regression analysis indicated that age≥70 years and the presence of grade Ⅳ myelosuppression before treatment were risk factors for infection in MM patients, while the IgG-λ type was a protective factor against infection ( P <0.05). CONCLUSIONS The incidence of infection is relatively high in MM patients receiving bortezomib-based treatment regimens, with respiratory and gastrointestinal infections being the most common. Age at initial diagnosis, grade of myelosuppression, and pathological classification are influencing factors for infection in MM patients.
6.Experimental study on the ultrasound visibility of a novel interventional catheter and delivery system: In vitro and animal experiments

Objective To assess the echogenicity of the ultrasound-guided catheter and its associated delivery system. Methods The study consisted of in vitro characterization experiments and animal studies. In the in vitro phase, the acoustic and mechanical properties of the ultrasound-guided catheter were compared with those of the traditional MPA2 catheter, including parameters such as echo intensity, recognizability, and angle dependence. In the animal experiments, a ventricular septal defect (VSD) model was established in miniature pigs to compare the procedural performance of the ultrasound-guided delivery system versus the conventional system. Evaluation indicators included the time required for the system to cross the VSD, the detection rate of the system within the right ventricle, and the occurrence of intraoperative complications. Results The ultrasound-guided catheter demonstrated a significantly higher mean echo intensity than the MPA2 catheter [(237.3±1.8) dB vs. (190.9±13.1) dB, P<0.001] and a markedly improved recognizability rate (82.3%±5.6% vs. 26.7%±3.2%, P<0.001), along with better angle independence and image quality. In animal experiments, the ultrasound-guided delivery system significantly reduced the time required to cross the VSD [(18.5±5.7) min vs. (30.3±4.5) min, P<0.001] and substantially increased the detection rate within the right ventricle (100.0% vs. 30.0%). No severe complications occurred in any experimental animal. Conclusion The ultrasound-guided catheter and its corresponding delivery system exhibite superior ultrasound visibility and operational performance in both in vitro and animal experiments, indicating strong potential for clinical application.
7.Efficacy and learning curve of three-lobe holmium laser enucleation of the prostate for benign prostatic hyperplasia in county-level hospitals

Objective To analyze the efficacy, safety, and learning curve of the three-lobe holmium laser enucleation of the prostate(HoLEP)in the treatment of benign prostatic hyperplasia(BPH)in a county-level hospital.Methods A retrospective analysis was conducted on the clinical data of 65 BPH patients who underwent the three-lobe HoLEP performed by a single surgeon at the Department of Urology, Yining County People's Hospital, during Dec.2023 and Jun.2024.The enucleation efficiency was calculated by dividing the weight of the enucleated prostatic tissue by the enucleation time.A case scatter diagram of enucleation efficiency was plotted according to the chronological order of the operations, and the learning curve was analyzed after fitting.Based on the inflection points of the learning curve, the learning process was divided into the initial learning, mastery, and proficiency phases.The basic clinical data, perioperative indicators, postoperative complications, and follow-up indicators were compared among the different learning phases.Results All 65 procedures were successfully completed.Marked by the enucleation efficiency reaching a plateau, cases 1-20 were defined as the initial learning phase, cases 21-40 as the mastery phase, and cases 41 onwards as the proficiency phase.The prostate volume in the proficiency phase was significantly larger than that in the initial learning and mastery phases(both P<0.05).There were no statistically significant differences in other baseline characteristics among the three groups(all P>0.05).The operation time [(105.50±19.12)min vs.(85.25±26.92)min vs.(69.00±23.58)min] and enucleation time [(76.90±14.19)min vs.(63.70±22.24)min vs.(48.80±20.48)min] showed a decreasing trend across the three groups(all P<0.05).The enucleation efficiency in both the mastery and proficiency phases was significantly higher than that in the initial learning phase [(1.16±0.44)g/min vs.(0.85 ±0.25)g/min, P<0.05;(1.36±0.49)g/min vs.(0.85±0.25)g/min, P<0.05].The enucleation efficiency in the proficiency phase was slightly higher than that in the mastery phase, but the difference was not statistically significant(P= 0.389).There was no significant difference in the incidence of perioperative complications among the three groups(all P>0.05).At the 6-month follow-up, the international prostate symptom score(IPSS), post-void residual(PVR), and maximum urinary flow(Qmax)were significantly improved compared to preoperative values in all three groups(all P<0.05);however, no significant differences were observed among the three groups(all P>0.05).Conclusion The three-lobe HoLEP for the treatment of BPH is safe and effective in a county-level hospital setting.Surgeons with some experience in endoscopic surgery can preliminarily master this technique after a learning period of approximately 20 procedures.
8.Risk analysis of drug-related medical insurance claim denials in healthcare institutions and exploration of phar-macist-led intervention model

OBJECTIVE To develop a pharmacist-led intervention model for reducing drug-related medical insurance claim denial risks, so as to provide practical references for healthcare institutions in fulfilling their primary responsibility for medical insurance fund supervision. METHODS A risk matrix method was employed by the pharmacists to assess the risks associated with drug‑related medical insurance claim rejections in our hospital during 2024. Intervention measures were developed targeting high‑risk factors (including inappropriate indications for medical insurance coverage, exceeding the covered treatment course, and inappropriateness of gender and age) from three dimensions, namely organizational structure, technical prevention and control, and closed‑loop management. Changes in drug‑related claim rejection data were compared between 2024 (pre‑intervention) and 2025 (post‑intervention), and the interception effect of the pre‑prescription review system was evaluated after the intervention. RESULTS After the intervention, the total amount of drug‑related medical insurance claim rejections in our hospital was reduced by 57.24% compared with that before the intervention, and the number of rejected items decreased from 10 108 to 2 892. Specifically, the number of rejected items due to inappropriate indications, exceeded treatment course, inappropriate gender, and inappropriate age was reduced by 70.94%, 74.26%, 83.86%, and 96.70%, respectively. Throughout the year following the intervention, a total of 5.605 yuan million in outpatient prescription drug amounts and 4.683 million yuan in inpatient medication order amounts that did not meet the insurance‑limited payment requirements were intercepted by the pre‑prescription review system. The proportion of drug‑related claim rejection amount to the total medical service claim rejection amount of our hospital decreased to 19.7%. CONCLUSIONS The pharmacist-led multi-department collaborative, technical prevention and control, and dynamic optimization integrated full-process intervention model is highly operable and has significant practical effects. It has important promotion value in reducing drug-related medical insurance claim denial risks.
9.Exploration and effect evaluation of pharmacy practice research path for PIVAS

OBJECTIVE To improve the research capacity and research enthusiasm of dispensing pharmacists in pharmacy intravenous admixture services (PIVAS), facilitate their career development, and further promote the sustainable development of hospital pharmacy disciplines. METHODS A full-chain pharmacy practice research path covering topic orientation, topic evaluation, and implementation was explored and constructed for PIVAS. Multiple approaches including framework condensation for topic selection, department empowerment and improved incentive mechanisms were adopted to systematically boost the research competence and enthusiasm of PIVAS dispensing pharmacists. Four dimensions of indicators, including PIVAS research output, operational level, career development of staff and external influence, were compared between the pre-implementation period (2020- 2022) and post-implementation period (2023-2025) to evaluate the implementation effect of the path. RESULTS After the application of the PIVAS pharmacy practice research path, the number of published papers and comprehensive quality score of papers increased by 150.00% and 161.90% respectively compared with those before implementation. The number of approved research projects and comprehensive quality score of projects rose by 166.67% and 225.00%, and the number of authorized patents increased by 100.00%. The monthly number of external errors decreased from (0.42±0.18) pieces/month pre-implementation to (0.28±0.23) pieces/month post-implementation (P>0.05). Dispensing efficiency was elevated from (121.83±2.48) bags/(hour· person) to (139.00±3.46) bags/(hour·person) (P<0.05). The research participation rate of PIVAS dispensing pharmacists grew from 15.00% to 50.00%, and the overall satisfaction rate of staff increased from 55.00% to 85.00% (P<0.05). Within the 3 years after path implementation, our hospital PIVAS received a total of 22 trainees from external medical institutions, and the number of external institutional visits increased by 374.07% versus the pre-implementation stage. CONCLUSIONS The constructed pharmacy practice research path for PIVAS can significantly improve PIVAS research output, operational efficiency and external influence, and effectively promote the career growth of dispensing pharmacists working in PIVAS.
10.Investigation of the current status and related factors of vancomycin-associated acute kidney injury in critically ill patients

OBJECTIVE To investigate the incidence and outcomes of vancomycin-associated acute kidney injury (VA-AKI) in critically ill patients, to analyze the independent risk factors for the development of VA-AKI and renal function outcomes in VA-AKI patients, and to evaluate the predictive performance of vancomycin treatment duration for VA-AKI occurrence, so as to provide reference for the rational and safe use of this drug.METHODS Clinical data of critically ill patients who received vancomycin during hospitalization in the intensive care unit of Zhongshan Hospital, Fudan University,from January 2022 to December 2024 were retrospectively collected. Patients were divided into the VA‑AKI group and the non‑AKI group according to the occurrence of VA‑AKI. Multivariate Logistic regression analysis was performed to screen for independent risk factors for VA‑AKI development and renal function outcomes in patients with VA‑AKI. The receiver operating characteristic (ROC) curve was used to analyze the predictive performance of vancomycin treatment duration for VA‑AKI.RESULTS A total of 1 484 patients were enrolled, among whom 287 developed VA‑AKI, with an incidence of 19.3%. The overall median time to onset was 2.5 days after vancomycin administration. Complicated hypertension, cardiac or renal insufficiency, hospital‑acquired sepsis/bacteremia, multiple organ failure, invasive mechanical ventilation during hospitalization, vancomycin treatment duration ≥10 days, and concomitant use of carbapenems or tacrolimus were independent risk factors for VA‑AKI ( P <0.05). Concomitant use of vasopressors was an independent risk factor for renal function outcomes in patients with VA‑AKI ( P <0.05). The area under the ROC curve (AUC) of the model using vancomycin treatment duration of predict the occurrence of VA-AKI was 0.624. At the optimal cut‑off value of 9.7 days, the sensitivity and specificity were 47.4% and 74.9%, respectively.CONCLUSIONS Critically ill patients have a relatively high incidence of VA-AKI, which mostly occurs in the early stage of vancomycin therapy. Patients with underlying cardiac-renal diseases, severe infection, requirement for mechanical ventilation, prolonged vancomycin therapy, and concomitant administration of carbapenems or tacrolimus are at high risk for VA-AKI. Concomitant vasopressor use affects renal function recovery in patients with VA-AKI. The predicted risk of acute kidney injury is higher when vancomycin treatment duration is ≥10 days.
