Objective:To share our experience of single-port transoral Da Vinci robotic surgery for laryngeal and pharyngeal benign tumors. Methods:Three patients aged over 18 years and diagnosed respectively with epiglottic cyst, aryepiglottic fold cyst and thyroglossal duct cyst at tongue base were included who received operations in Hainan Hospital of Ruijin Hospital affiliated to Shanghai Jiao Tong University School of Medicine (Boao Research Hospital of Hainan) between August 1, 2023 and April 10, 2024. There were 1 male and 2 females, with an average age of 59 years old. The clinical diagnosis of these 3 patients was respectively epiglottic cyst, aryepiglottic fold cyst and thyroglossal duct cyst at tongue base. Intraoperative conversion rate, blood loss, operation time, hospital stay, pain score and swallowing function score were analyzed. Results:All three patients successfully underwent the operations with the single-port Da Vinci robotic system. The clean-contaminated resection rate of the tumors was 100%. The conversion rate was nil. The blood loss was 0-7 ml. The operation time was 5-30 minutes. There were no adverse events such as loosening of teeth and soft tissue abrasions of pharyngeal side wall. The rate of oral feeding within 24 hours after surgery was 100%. No postoperative coughing, bleeding, or dyspnea occurred. The average length of hospital stay was 3.7 days. The VAS score decreased 3 days after surgery compared to 1 day after surgery. The swallowing function recovered 1 month after surgery. Conclusion:The single-port transoral robotic surgery for laryngeal and pharyngeal benign lesions is safe and efficient, with fewer complications.

Objective To explore the mechanism of IgA nephropathy(IgAN)caused by multi-glycosides of Tripterygium wilfordii(GTW)through the regulation of Silent information regulatory factor 1(SIRT 1)/nuclear transcription factor E2-related factor 2(Nrf 2)/antioxidant enzyme heme oxygenase 1(HO-1)signaling pathway. Methods Forty-five male SD rats were selected and randomly divided into two groups:the blank group(n=9)and the model group(n=36).In addition to the blank group, the BSA+CCl4+LPS group was used.At the end of 12 weeks, two rats were randomly selected for verification, and the model was successfully established. The 34 model rats were randomly divided into 3 groups:the model group(n=10),prednisone group(n=12),and GTW group(n=12).Urine,blood and kidney tissues were harvested 4 weeks after drug administration.Urinary erythrocyte number,24-h urinary protein quantification(24 h-UTP),alanine transaminase(ALT),serum albumin(ALB),urea nitrogen(BUN),and blood creatinine(SCr)were performed for each group;the protein expression of SIRT1,Nrf2,HO-1 and PINK1 was detected by Western blotting analysis;real-time polymerase chain reaction(RT-PCR)detection of SIRT1,Nrf2,HO-1 and PINK1 mRNA expression in rat kidney tissue;and detection of IgA deposition in the renal mesangial area by immunofluorescence.Kidney histopathological changes were observed in all the rats by hematoxylin-eosin(HE)staining.Results The results compared with those in the blank group, the urinary red blood cell count and 24 h-UTP,ALT,BUN,and SCr levels were significantly greater(P<0.01);The ALB level was significantly lower(P<0.01);renal tissue SIRT1,Nrf2,HO-1,PINK1 protein and mRNA expression were significantly lower(P<0.01);IgA deposition in the mesentery was obvious;renal pathological damage was severe;and the difference was statistically significant. Compared with those in the model group, urinary red blood cell counts and 24 h-UTP,ALT,BUN,and SCr levels in the prednisone and GTW groups were significantly lower (P<0.01);ALB levels were significantly greater (P<0.01);SIRT1,Nrf2,HO-1,PINK1 protein and mRNA expression were significantly greater (P<0.01);IgA deposition in the mesangial area was reduced, and renal pathology was improved, with statistically significant difference. Conclusions GTW may alleviate oxidative stress injury, protect renal function, and improve renal injury by activating the SIRT 1/Nrf 2/HO-1 signaling pathway.

Objective:
To investigate the expression, prognosis and biological role of hepatocyte nuclear factor 4A (HNF4A) in gastric cancer, and to study its effect on the proliferation of gastric cancer cells.
Methods:
Tumor Immune Estimation Resource 2. 0 (TIMER2. 0) and Gene Expression Profiling Interactive Analysis ( GEPIA2) databases were used to analyze the relative expression levels of HNF4A in gastric cancer and normal tissue,KM Plotter was used to analyze the correlation between the expression level of HNF4A and the survival rate of gastric cancer patients,TISIDB database and R language (4. 1. 2) were used to analyze whether HNF4A was involved in the immune regulation process of gastric cancer.cBioPortal database was used to analyze the mutations of HNF4A in gastric cancer,GSEA 4. 2 was used to analyze the functional enrichment of HNF4A,and LinkedOmics database was used to predict the genes that might be regulated by HNF4A.The relative expression of HNF4A in gastric cancer and adjacent tissues was detected by qRT-PCR , Western blot and immunohistochemistry (IHC) .The proliferation and cell cycle of gastric cancer cells were analyzed by CCK-8,EdU,colony forming assay and flow cytometry.
Results :
The expression of HNF4A increased in gastric cancer tissues (P <0. 05) ,and the overall survival rate of gastric cancer patients with high HNF4A expression was worse (P<0. 001) .HNF4A was mainly missense mutated in gastric cancer.Immune cell infiltration showed that HNF4A was associated with B lymphocytes,CD8 + T cells, neutrophils, macrophages and dendritic cells ( all P <0. 001) . HNF4A was also associated with tumor mutation burden (r = 0. 28,P<0. 0001) and microsatellite instability (r = 0. 13,P<0. 01) .After knockdown of HNF4A, cell proliferation ability was significantly inhibited ,and cell cycle was arrested at G0 / G1 phase.
Conclusion
HNF4A expression significantly increased in gastric cancer tissues, which is associated with poor prognosis ,and may also be involved in immune regulation. Knockdown of HNF4A can inhibit the proliferation of gastric cancer cells.
