OBJECTIVE To conduct a comprehensive clinical evaluation of four nucleoside (acid) analogues that have been approved and marketed in China, such as entecavir, tenofovir disoproxil fumarate, tenofovir alafenamide fumarate, and tenofovir amibufenamide. METHODS According to the Guideline for the Administration of Clinical Comprehensive Evaluation of Drugs (2021 edition, trial implementation), a comprehensive search was conducted across databases including CNKI, Wanfang Data, VIP, PubMed, the Cochrane Library, Embase, as well as relevant official websites. Drug package inserts, guidelines, consensus statements, and relevant literature for the four drugs were collected and subjected to a comprehensive evaluation across six dimensions: safety, efficacy, cost-effectiveness, innovativeness, suitability, and accessibility. RESULTS The scores for entecavir in terms of safety, efficacy, cost-effectiveness, innovativeness, suitability, and accessibility-along with its comprehensive score-were 13, 14, 13, 10, 18, and 6, totaling 74 points. For tenofovir disoproxil fumarate, the respective scores were 13, 17, 18, 8, 18, and 7, totaling 81 points. For tenofovir alafenamide fumarate, the scores were 14, 20, 12, 8, 18, and 5, totaling 77 points. Finally, for tenofovir amibufenamide, the scores were 10.5, 17, 10, 6, 15, and 4, totaling 62.5 points. CONCLUSIONS Tenofovir disoproxil fumarate, with the highest score, is recommended as the first-line option, suitable for adults, children, and pregnant women. However, caution is warranted for potential renal impairment. Tenofovir alafenamide fumarate is recommended as a second-line alternative, particularly for individuals at high risk for bone and renal damage. Entecavir has a score similar to tenofovir alafenamide fumarate but requires dosing on an empty stomach and dose adjustment based on renal function of patients. Tenofovir amibufenamide received the lowest score and is considered a weak recommendation. The clinical application of these nucleoside (acid) analogues should be individualized based on the patient’s age, physiological status, and risk factors.

BackgroundIt has been shown that academic procrastination, cognitive emotion regulation strategies, anxiety and mobile phone dependence are closely related, but the detailed mechanism by which academic procrastination contributes to mobile phone dependence remains largely unclear and mediation analysis is currently lacking. ObjectiveTo explore the mediation role of cognitive emotion regulation strategies and anxiety in the relationship between academic procrastination and mobile phone dependence among college students, so as to provide references for the prevention and intervention of college students' mobile phone dependence. MethodsIn March 2023, 474 students from Xinjiang Normal University were selected by random sampling technique, and Mobile Phone Addiction Index (MPAI), Procrastination Assessment Scale-Students (PASS), Beck Anxiety Inventory (BAI) and Cognitive Emotion Regulation Questionnaire-Chinese version (CERQ-C) were used to conduct the survey. Pearson correlation analysis was adopted to examine the correlation among all variables, and Process 3.5 macro program was utilized to determine the mediation effect of cognitive emotion regulation strategies and anxiety on the relationship between academic procrastination and mobile phone dependence among college students. Results①PASS scores were positively correlated with the scores on CERQ-C negative cognitive emotion regulation strategy, BAI and MPAI (r=0.374, 0.229, 0.661, P<0.01), CERQ-C negative cognitive emotion regulation strategy scores were positively correlated with BAI and MPAI scores (r=0.372, 0.498, P<0.01), and BAI scores were positively correlated with MPAI scores (r=0.340, P<0.01). ② Both negative cognitive emotion regulation strategy and anxiety exerted an mediation effect on the relationship between academic procrastination and mobile phone dependence among college students, with an effect value of 0.094 (95% CI: 0.056~0.137) and 0.013 (95% CI: 0.001~0.029), and a chained mediation effect of negative cognitive emotion regulation strategy and anxiety on the relationship between academic procrastination and mobile phone dependence among college students was also documented, with an effect value of 0.015 (95% CI: 0.006~0.026). ConclusionAcademic procrastination is proved to be effective in predicting college students' mobile phone dependence both directly and indirectly through either separate or chained mediation of negative cognitive emotion regulation strategy and anxiety. Objective:To summarize the clinical, pathological, and molecular genetic features of dermatofibrosarcoma protuberans (DFSP) in children. Methods:A retrospective analysis was conducted on clinical data from 8 children with DFSP in the Department of Dermatology, Beijing Children′s Hospital, Capital Medical University from January 2017 to December 2024. General information, clinical manifestations, pathological examinations, molecular genetic examinations, and treatments were analyzed, and clinical features and prognosis were summarized. Results:All the 8 cases were females, with ages at onset ( M[ Q1, Q3]) of 1.3 (0.3, 2.6) years, including 2 congenital cases; their ages at diagnosis were 5.1 (3.7, 7.4) years. Skin lesions manifested as solitary dark red patches, plaques, or nodules, and were located on the trunk in 5 cases (3 on the back, 1 on the abdomen, and 1 on the chest) and on the lower limbs in 3 cases. Histopathological examinations of all the 8 cases showed tumor cells diffusely infiltrating the dermis and subcutaneous tissue without epidermal involvement. Immunohistochemical staining showed that all the cases were strongly positive for CD34 and negative for soluble 100 protein; the Ki-67 labeling index ( M[ Q1, Q3]) was 9.0% (8.0%, 17.5%). Fluorescence in situ hybridization revealed the COL1A1-PDGFB fusion gene or PDGFB gene rearrangement in all 8 cases. All the patients underwent surgical excision of the primary skin lesions, including 3 treated with wide local excision, 2 with traditional Mohs surgery, and 3 with modified slow Mohs surgery, and all achieved negative margins. During the follow up of 6 months to 7 years, no tumor recurrence was observed. Conclusions:DFSP often occurred at a relatively young age in children, and mostly presented as atrophic patches or plaques. PDGFB gene alterations were commonly observed, and prognosis after surgical treatment was generally favorable.

Objective:To understand the basic situation of occupational internal exposure to 131I among nuclear medicine staff in Jiangsu province, and to analyze the relevant factors that contribute to the formation of internal exposure. Methods:A four-year external direct measurement of thyroid organ counting was conducted on nuclear medicine staff who underwent 131I treatment in Jiangsu province. The result were statistically analyzed, and the effective dose of accumulated internal exposure was estimated. The relationship between the detection rate of internal exposure and 10 possible factors was analyzed. Results:A total of 847 person-times were monitored during the monitoring period, of which 345 person-times were detected for 131I, with a detection rate of 40.7%. The highest individual monitoring result for internal exposure was 24.83 kBq, with an average value of 77.3 Bq and a median of 28.3 Bq among the detected individuals. The average committed effective dose of internal exposure for staff was 0.201 mSv, with a median of 0.074 mSv among the detected individuals. Conclusions:There is statistically significant difference in the detection rate of individual doses of internal exposure among nuclear medicine staff in each year. It has been observed that the main factors related to the detection rate are age, hospital grade, job position, location, whether there is an automatic dispensing operation, operating method, exposure level and length of exposure. Sufficient attention should be given to the situation of internal exposure, and further effective monitoring should be conducted on all such staff members.

Chinese Journal of Clinical Oncology 2025;52(16):854-859
The epidermal growth factor receptor(EGFR)is a key oncogenic driver in non-small cell lung cancer(NSCLC),and its mutations have significant clinical implications. While classical mutations, such as exon 19 deletions and exon 21 L858R substitutions, are well estab-lished,increasing attention has shifted toward less common,non-classical EGFR mutation subtypes. The widespread adoption of high-throughput sequencing technologies such as next-generation sequencing(NGS)has substantially improved the detection rate of non-classic-al EGFR mutations. Thus, their molecular characteristics and therapeutic responses have been increasingly elucidated. However, due to their significant heterogeneity, substantial variability exists in the sensitivity of different non-classical mutations to EGFR tyrosine kinase inhibitors(EGFR-TKIs).Furthermore,the scarcity of clinical samples limits the availability of robust evidence.Additionally, prolonged clinical use of EGFR-TKIs can also lead to acquired resistance, further complicating treatment strategies.Despite these challenges, ongoing research continues to explore targeted therapies for patients with non-classical EGFR mutations. This review summarizes recent studies on non-classical EGFR mutations in NSCLC,examines current therapeutic approaches, and outlines clinical recommendations for managing patients after EGFR-TKI treatment failure.By integrating existing evidence and clinical experience, this review aims to optimize individualized treatment strategies for these patients, with the ultimate goal of improving their prognosis and quality of life.

Journal of Audiology and Speech Pathology 2025;33(1):66-72
Objective To study the dynamic changes in the secretion of neurotransmitters glutamic acid(Glu)and γ-aminobutyric acid(GABA)in the central auditory brain area,in order to explore the effects of sodium salicy-late on different locations of the auditory pathway. Methods A total of 126 SD rats were injected intraperitoneally with salicylate, and were divided into 10 groups including injection groups for 1,2,4,8,and 24 hours, chronic in-jection groups for 3,7,and 14 days, and chronic recovery groups for 21 and 28 days with 6 rats in each group,as well as their corresponding blank control groups.Rats in each group were anesthetized and materials were collected for further use.High-performance liquid chromatography(HPLC)was performed to detect and compare the dynam-ic changes in the levels of Glu and GABA in the auditory cortex, inferior colliculus, cochlear nucleus, and hippocam-pus of the auditory center of rats in each group at different time points. Results Compared with the control group, within 24 hours of acute injection of salicylate, the Glu content in the auditory cortex reached the peak in 1 hour, and the hippocampus reached the peak at the 4th hour after injection, and then decreased slowly. The GABA con-tent in the four brain regions showed a slow upward trend in the chronic injection period, reached the peak on the 7th day, decreased and approached normal level on the 14th day,and basically returned to the normal level in the re-covery period. Conclusion These findings indicate that salicylate has a certain short-term excitatory and stimulating effect on the central auditory system.Under the mechanism of central plasticity, after long-term injection of salicy-late,the release of neurotransmitters reaches a new excitation/inhibition balance in the central area.Glu and GABA may each play a different role that may ultimately lead to the development of tinnitus.
