ObjectiveTo explore the relationship between negative life events and depression severity in adolescent patients with depressive disorder, as well as the chain mediating role of insomnia symptoms and quality of life. Methods374 outpatient patients and hospitalized patients with adolescent depressive disorders were enrolled. The Adolescent Life Event Scale (ASLEC), the Insomnia Severity Index (ISI), the World Health Organization Quality of Life Questionnaire Short Form (WHOQOL-BREF), and the Center for Epidemiology Depression Scale (CES-D) were used to evaluate the negative life event situation, insomnia symptoms, quality of life level and depression severity of the subjects, respectively. In addition, the PROCESS 4.0 macroprogram was used to analyze the chain mediating effect of insomnia symptoms and quality of life between negative life events and depression severity in patients with adolescent depressive disorder. ResultsThe results of correlation analysis showed that there was a significant correlation between negative life events and insomnia symptoms, quality of life, and depression severity (all P<0.05). In addition, the results of chain mediation showed that negative life events had a significant direct effect on depression severity, with an effect size of 0.12 (P<0.001). Insomnia symptoms and quality of life played a mediating role in the relationship between negative life events and depression severity in patients with adolescent depressive disorders, with indirect effect sizes of 0.062 (95%CI: 0.040-0.087) and 0.091 (95%CI: 0.059-0.123), respectively. It could also play a chain mediation role, and the effect size was 0.039 (95%CI: 0.024-0.057). ConclusionNegative life events experienced by patients with adolescent depressive disorder not only directly affect the severity of depressive symptoms, but may also indirectly exacerbate depression through insomnia symptoms and quality of life.

Diabetic kidney disease (DKD), a common complication of diabetes mellitus, is a leading global cause of end-stage renal disease (ESRD). Current therapeutic strategies primarily focus on symptomatic management but exhibit limited efficacy in halting disease progression to ESRD, and some drugs carry non-negligible toxic side effects. Traditional Chinese medicine (TCM) has a long history in treating DKD, with single TCM and TCM compounds demonstrating unique advantages in multi-target, multi-pathway, and multi-effect therapeutic interventions. Tripterygium wilfordii (TW), known for its effects in promoting blood circulation, dredging collaterals, dispelling wind, removing dampness, reducing swelling, and alleviating pain, contains bioactive components such as Tripterygium glycosides (TWG), triptolide (TPL), tripdiolide (TPD), and celastrol (CEL). The active ingredients possess various functions, including regulating immune-inflammatory balance, ameliorating renal fibrosis and glomerulosclerosis, combating oxidative stress, protecting podocytes, and improving glucose and lipid metabolism, all of which play a significant role in the treatment of DKD. This review summarized the mechanisms underlying the therapeutic effects of T. wilfordii and its active ingredients on DKD, aiming to provide insights for clinical management and novel drug development of DKD.

Intravitreal anti-vascular endothelial growth factor(anti-VEGF)therapy has been widely used, but the variability in its therapeutic efficacy limits individualized treatment. In recent years, the application of artificial intelligence(AI)has opened up new avenues for personalized treatment response prediction, and its core branches include machine learning(ML)and deep learning(DL). This review systematically retrieved and analyzed 41 relevant studies published up to April 2025. Comprehensive analysis reveals that AI predictive models are evolving from forecasting single endpoints(such as visual acuity or central retinal thickness)to integrating multi-dimensional endpoints(encompassing anatomical, functional, and treatment demand parameters)and generating predictive imaging outputs. In terms of technical approaches, DL models(28 studies, accounting for 68.3%)dominate this field due to their robust image interpretation capabilities, while ML models(10 studies, 24.4%)retain significant value in the analysis of structured clinical data. Cross-disease comparisons indicate that research efforts are most concentrated on age-related macular degeneration(ARMD)and diabetic macular edema(DME), with shared conceptual frameworks for model construction, yet distinct anatomical and functional indicators are prioritized for each disease. Currently, the field confronts several key challenges, including insufficient prospective clinical validation, limited model interpretability(the “black box problem”), and a scarcity of high-quality multi-center datasets. Moving forward, it is imperative to advance real-world validation and develop explainable AI techniques to expedite the clinical translation of these predictive models.

Chronic obstructive pulmonary disease (COPD) is the most common chronic respiratory disease around the world, and pharmacotherapy is the foremost treatment method currently. In recent decades, with the rapid development of bronchoscopic interventional therapy, endoscopic physical ablation technology presents a therapeutic effect in treating COPD, with few treatment-related side effects, showing excellent application prospects in treating COPD. Since ablation techniques in this field are emerging technologies with low patient acceptance, they are not widely used in the clinical treatment of COPD. This article reviews the development process of physical ablation techniques. Moreover, their current application status and the prospects in the field of COPD treatment are also summarized and analyzed. We hope to promote the application of physical ablation in the clinical treatment of COPD and provide practical references and a theoretical basis for the clinical treatment of COPD.

Pegylated interferon α-2b (PEG-IFN-α-2b) is currently a first-line drug for the treatment of chronic hepatitis B virus infection and is widely used in clinical practice. This drug has multiple effects of inhibiting viral replication, regulating immunity, and improving liver function, and some patients can achieve clinical cure. However, it often causes various adverse reactions during treatment, which are important factors for compromising treatment compliance and efficacy. This article systematically reviews the adverse reactions and their mechanisms during PEG-IFN-α-2b therapy for chronic hepatitis B, including influenza-like symptoms, peripheral blood cytopenia, thyroid dysfunction, and neuropsychiatric symptoms, and it also summarizes the monitoring and management strategies for these adverse reactions, in order to provide evidence-based guidance for clinical decision-making and emphasize the importance of individualized treatment.

OBJECTIVE To explore and establish a full-cycle management model for drug traceability codes that aligns with national policy requirements and the practical needs of healthcare institutions, thereby enhancing the refinement of drug management and the level of medication safety. METHODS A tripartite strategy integrating “hardware deployment, system transformation, and process re-engineering” was adopted. This involved the introduction of intelligent identification devices (personal digital assistant, high-definition industrial reader), the modification of the hospital information system interface, and the re-engineering of workflows (drug warehousing, dispensing and distribution, drug withdrawal, uploading to the insurance platform) to achieve comprehensive, informatized collection and association of drug traceability codes throughout all stages. RESULTS A full-cycle management model for drug traceability codes was successfully established, realizing the goals of making drugs “traceable to their source, trackable in their distribution, and accountable in their responsibility”. The patient waiting time for medication dispensing before and after the implementation was [3.08(1.67,5.58)] min and [3.28(1.77,5.98)] min, respectively. Among them, the patient waiting time under the pre-preparation mode was [3.60(2.13,6.35)] min and [3.50(2.03,6.30)] min, respectively; the patient waiting time under the real-time mode was [2.05(0.83,4.03)] min and [2.78(1.18,5.38)] min, respectively; the number of dispensing errors was 3, 0, respectively; the staffing of relevant positions had not been increased. CONCLUSIONS The drug traceability code management model constructed from a full-cycle perspective effectively meets national policy requirements. It provides data support for refined hospital management and offers solid technical and procedural safeguards for ensuring patient medication safety and strengthening medical insurance fund supervision, demonstrating practical value.

Background Meteorological factors are among the key extrinsic triggers for the onset and exacerbation of cardiovascular and cerebrovascular diseases (CVD). Against the backdrop of sustained global warming, elucidating the impact of ambient temperature and atmospheric pressure on CVD, especially on pre-hospital CVD emergent events, has become imperative for evidence-based prevention and emergency preparedness. Objective To quantify the temporal trends of daily mean temperature and atmospheric pressure and their associations with pre-hospital CVD emergent events in Yantai, and to explore effect modification by demographic subgroups and geographic areas, thereby providing an empirical basis for the rational allocation of emergency medical resources. Methods Pre-hospital CVD emergency data from January 1, 2016 to December 31, 2022 were selected from the Yantai 120 Emergency Medical Command System. Synchronous meteorological factors and environmental pollutant data were obtained from the websites of the National Oceanic and Atmospheric Administration and the National Centers for Environmental Information of the United States. Time-series analysis combined with distributed lag non-linear model was used to analyze the association between daily temperature, atmospheric pressure, and pre-hospital CVD emergencies. Average annual percentage changes (AAPC) were calculated using Joinpoint (version 5.2.0.0) to reflect temporal trends. Spearman correlation analysis was employed to screen variables with low collinearity for inclusion in the multi-pollutant adjusted models. Results From 2016 to 2022, a total of

Objective To establish a rat model of venous thrombosis in a plateau hypobaric hypoxic environment and to investigate the effect of this environment on venous thrombosis. Methods A total of 144 healthy male SD rats were assigned randomly to four groups(n=36 rats per group):a plains sham operation(A)group, plains operation(B)group, plateau altitude 6000 m+sham operation(C)group, and plateau altitude 6000 m+surgery(D)group.Rats in A and B groups were maintained in a plains normoxic environment, while rats in C and D groups C and D were subjected to a plateau environment.Rats in the surgical groups underwent quantitative constriction to incompletely obstruct the inferior vena cava blood flow. Each group was further divided into subgroups based on time:1,3,5,7,14,and 21 d(n=6 rats per group).Regular vascular ultrasound monitoring was conducted, and blood samples were taken for whole blood viscosity testing and the assessment of inflammatory indicators, including endothelin-1(ET-1),interleukin-6(IL-6)and tissue factor(TF).Coagulation function was evaluated through the activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(FIB)and D-dimer.After the observation period, the experimental animals were sacrificed and the limbs were removed.Thrombus samples were stained with hematoxylin/eosin(HE),and the thrombus wet mass was measured. Results The thrombosis incidence was significantly higher in the plateau D group than in B group, accompanied by a marked increase in blood viscosity and hematocrit(P<0.01).Additionally,levels of ET-1,IL-6,and TF were significantly elevated(P<0.05),indicating a coagulation disorder. Conclusions A plateau hypoxic environment model can be successfully simulated by quantitative coarctation of the inferior vena cava, combined with a specialized environmental chamber. The findings of this study suggest that a plateau hypoxic environment promotes venous thrombosis.

To analyze the epidemiological characteristics of COVID-19 in Fujian Province from the 49th week of 2022 to the 5th week of 2023,after further optimization of China's COVID-19 prevention and control measures on December 7,2022(the 49th week of 2022),this study used multi-dimensional surveillance data to dynamically assess population infection levels and their changing trends. The aim of the study was to provide a scientific basis for early warning of epidemic risk, medical resource allocation, and evalu-ation of socio-economic impact.A multi-source data surveillance system was constructed, encompassing surveillance of fever clinics at medical institutions(weekly collection of visits, positive nucleic acid and antigen test results, inpatients, and severe cases in sec-ondary or above hospitals),population nucleic acid test monitoring(weekly person-times and positivity rates of single-tube tests from the provincial system),sentinel hospital monitoring(weekly proportion of influenza-like illness visits at 18 sentinel hospitals and re-lated viral testing data),and monitoring of novel coronavirus variants(weekly systematic collection of genomic sequences of local and imported cases).Line charts were plotted weekly, and time series analysis, molecular epidemiological methods, and an improved SEIAR model were used to simulate epidemic spread.During the study period, the COVID-19 epidemic in Fujian Province exhibited three distinct stages. In the infection peak stage(52nd week of 2022),the provincial fever clinic visits reached 606 893 person-times,and a 49.2%positivity rate in population single-tube nucleic acid tests and 63.8%positivity rate in sentinel hospital monitoring were observed. In the medical load peak stage(2nd week of 2023),274 460 inpatients and 28 487 severe cases were recorded. In the epidemic decline stage(4th to 5th weeks of 2023),fever clinic visits decreased by 96.3%with respect to the peak, the single-tube nucleic acid test positivity rate decreased to 6.3%,and the sentinel hospital COVID-19 nucleic acid test positivity rate was 6.4%.All 508 sequenced local cases were Omicron variants, predominantly BA.5.2 and its sub-lineages(67.4%).Among 56 imported se-quenced cases,BA.5.2 and its sub-lineages accounted for 50.0%,and 16.1%comprised nine variants of interest including XBB and BQ.The model predicted the infection peak in the 52nd week of 2022,whereas the hospitalization peak lagged by approximately 10.6 days.Multi-source data monitoring revealed a three-stage development of the COVID-19 epidemic in Fujian. The BA.5.2 strain was dominant during the epidemic. The combination of multi-source monitoring data and modeling provides important references for epi-demic prevention and control, and highlights the need to improve the monitoring system in follow-up.

Objective To analyze the incidence of cardiovascular events among patients with comorbid chronic obstructive pulmonary disease(COPD)and hypertension(HTN),as well as to identify the associated influ-encing factors. Methods A retrospective analysis was conducted on patients diagnosed with primary hypertension(HTN group,n=64),chronic obstructive pulmonary disease(COPD group,n=64),and concomitant primary hypertension and COPD(combined group,n=64)at our hospital between December 2021 and January 2025.Cardiovascular event incidence, pulmonary function parameters-including forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),peak expiratory flow(PEF),and percent predicted FEV1(FEV1%pred)-and blood pressure levels(systolic blood pressure[SBP],diastolic blood pressure[DBP])were compared across the three groups.Kaplan-Meier curves were constructed to illustrate and compare the cumulative incidence of cardiovas-cular events.Covariance analysis was performed to assess the impact of hypertension on pulmonary function, and Cox proportional hazards regression was employed to identify factors associated with cardiovascular outcomes. Results The total incidence rate of cardiovascular events in the HTN group was 14.06%,which was significantly lower than that in the COPD group(29.69%)and the combined HTN-COPD group(48.44%)(P<0.05).A highly significant difference was observed in the cumulative incidence rates across the three groups(P<0.05).Specifically,the COPD group exhibited a higher cumulative incidence than the HTN group(P<0.05),while the combined group showed the highest incidence, exceeding both the COPD and HTN groups(P<0.05).Pulmonary function parameters-including FVC,FEV1,PEF,and FEV1%pred-were significantly higher in the HTN group compared to both the COPD and combined groups(P<0.05).Moreover,these indices were also higher in the COPD group than in the combined group(P<0.05).Systolic and diastolic blood pressure(SBP and DBP)levels in the combined group were significantly elevated compared to both the HTN and COPD groups(P<0.05),and SBP and DBP in the HTN group were higher than those in the COPD group(P<0.05).After adjusting for potential confounders, the intergroup dif-ference in FVC remained statistically significant(P<0.05),and the effect of group on FEV1%pred was particularly robust(partial η2=0.754,P<0.05).Compared with patients without cardiovascular events, those who experienced events differed significantly by disease type(P<0.05).The proportions of patients with HTN alone and HTN com-bined with COPD were higher in the event group than in the non-event group(P<0.05).Additionally,FVC and FEV1%pred were lower in the event group, whereas SBP and DBP were higher(all P<0.05).Multivariate analysis identified disease type,FEV1%pred,and SBP as independent predictors of cardiovascular events(P<0.05).Deci-sion tree analysis further highlighted that the coexistence of HTN and COPD constitutes a critical determinant in cardiovascular risk stratification. Conclusions The incidence of cardiovascular events in patients with COPD com-bined with HTN was significantly higher than in those with either condition alone.Furthermore, the coexistence of these diseases, along with impaired lung function(as indicated by reduced FEV1%predicted)and increased SBP,were independent risk factors for cardiovascular events.
