1.Epidemiological characteristics of surgical site infection outbreaks at home and abroad

OBJECTIVE To analyze the characteristics of global surgical site infection(SSI)outbreaks and provide references for targeted prevention and control measures.METHODS SSI outbreak events from Jan.1,1990 to Dec.31,2023 were searched from Wanfang Med,CNKI,VIP and PubMed.Data on department distribution dur-ing the outbreak,duration,investigation methods,main causes,transmission modes,pathogen composition and outbreak outcomes were summarized for analysis.RESULTS A total of 111 SSI outbreaks in 20 countries were identi-fied,involving 1 382 patients and 24 deaths.The source of the outbreak was identified in 78 cases,mainly involving med-ical personnel in 27 cases(34.62%),hospital equipment in 19 cases(24.36%),environmental factors in 11 cases(14.10%),workflow factors in 10 cases(12.82%),hospital water factors in 7 cases(8.97%)and disinfectant fac-tors in 4 cases(5.13%).The duration of SSI outbreaks abroad was 5.00(1.50,12.00)months,longer than that in China[1.00(1.00,2.00)month](P<0.05).In China,the outbreaks mainly occurred in neurosurgery,cardiac surger-y,orthopedics,obstetrics and general surgery,while at abroad,they mainly occurred in cardiac surgery,orthopedics,and involved multiple surgical departments.Environmental hygiene methods were adopted in up to 90.63%of cases in China.Compared with China,analytical epidemiological methods and molecular epidemiological methods were more com-monly used abroad.Targeted measures and strengthened basic measures were implemented for different outbreak sources to terminate the SSI outbreaks.Pulsed-field gel electrophoresis(34 times)was a widely used molecular typing method in outbreak investigations.CONCLUSIONS Based on the analysis of SSI outbreak characteristics and risk factors,it is neces-sary to strengthen medical personnel training and monitoring of special pathogens.At the same time,the epidemiologi-cal investigation capabilities of infection control professionals should be strengthened.

Lung function serves as a clinical indicator reflecting respiratory system function,ai-ding in the diagnosis and monitoring of pulmonary diseases.Both genetic and environmental factors ex-ert significant influences on lung function.A comprehensive understanding of the role of genetic factors in lung function is crucial for gaining deeper insights into the regulatory mechanisms of lung function.In recent years,with the application of advanced technologies such as genome-wide association stud-ies,whole-exome sequencing,whole-genome sequencing,and epigenome-wide association studies,numerous genetic loci related to lung function have been identified.This review aimed to summarize the current research progress in genetics related to lung function,providing a reference for subsequent in-depth exploration of the pathological and physiological mechanisms affecting lung function.
3.Cannabidiol inhibits neuronal endoplasmic reticulum stress and apoptosis in rats with multiple concussions by regulating the PERK-eIF2α-ATF4-CHOP pathway.

OBJECTIVES:
To explore the effects of cannabidiol on endoplasmic reticulum stress and neuronal apoptosis in rats with multiple concussions (MCC).
METHODS:
SD rats were randomized into sham group, MCC group, 1% tween20 (TW) treatment group, and low-dose (10 mg/kg) and high-dose (40 mg/kg) cannabidiol treatment groups. In all but the sham group, MCC models were established using a metal pendulum percussion device, after which the rats received daily intraperitoneal injections of the corresponding agents for 2 weeks. The expressions of PERK, eIF2α, ATF4, CHOP, TRIB3, p-Akt and pro-caspase-3 in the brain tissue of the rats were detected with qRT-PCR, Western blotting and immunofluorescence staining. The core targets of cannabidiol in treatment of traumatic brain injury (TBI) were identified by network pharmacology analysis, and molecular docking was carried out to simulate the interaction of cannabidiol with the factors related to endoplasmic reticulum stress and apoptosis.
RESULTS:
Compared with the sham-operated rats, the rat models of MCC showed significantly increased mRNA expressions of PERK, eIF2α and CHOP and protein expressions of PERK, eIF2α, ATF4, CHOP, TRIB3, p-AKT and pro-caspase-3 in the cerebral cortex. CBD treatment, especially at the high dose, obviously increased the expression of p-Akt and lowered the expression levels of the other factors tested in the rat models. Network pharmacology analysis indicated interactions of the core targets of CBD with the factors related to endoplasmic reticulum stress and TBI, and molecular docking study showed a high binding energy of CBD with multiple factors pertaining to endoplasmic reticulum stress and apoptosis.
CONCLUSIONS
MCC induce endoplasmic reticulum stress and apoptosis in rat brain tissues, for which CBD, especially at a high dose, provides neuroprotective effects by inhibiting endoplasmic reticulum stress and cell apoptosis.

4.Application of peripheral blood inflammatory markers in prognosis evaluation of patients with acute-on-chronic liver failure

Acute-on-chronic liver failure (ACLF) refers to severe liver dysfunction that occurs on the basis of chronic liver diseases, and it is characterized by rapid disease progression, poor prognosis, and high mortality rate. In recent years, inflammatory markers have become a research hotspot due to their significant role in assessing the prognosis of ACLF. This article reviews the advances in the application of inflammatory markers in assessing the prognosis of ACLF, such as systemic immune inflammatory index, lymphocyte-white blood cell ratio, neutrophil-lymphocyte ratio, and C-reactive protein, and discusses their clinical value and future research directions, in order to provide a theoretical basis for the early intervention and prognosis management of ACLF patients.
5.Epidemiological characteristics and trends of other infectious diarrhea among children during 2014-2020

Objective:
To analyze the epidemiological characteristics and trends of other infectious diarrhea among children under 18 years old in Guangzhou City from 2014 to 2020, and to explore the correlation between climatic factors and the incidence of the disease, so as to provide reference for the early prevention of infectious diseases.
Methods:
The data of cases of other infectious diarrhea and meteorological data of children under 18 years old in Guangzhou City from 2014 to 2020 were collected through the Chinese Infectious Disease Reporting System and the Guangzhou Meteorological Bureau. The correlation between meteorological factors and the incidence of other infectious diarrhea was analyzed using negative binomial regression.
Results:
A total of 104 566 cases of other infectious diarrhea among children under 18 years old were reported in Guangzhou City from 2014 to 2020, with a male to female ratio of 1.48∶1. The incidence rate was the highest in 2017 (980.83 per 100 000) and the lowest in 2020 (388.22 per 100 000). The peak of incidence occurred from October to March of the following year. Children under 5 years old accounted for 87.95% of all cases. The number of cases of other infectious diarrhea was negatively correlated with the temperature of the previous 6 days ( IRR = -0.07 ), and positively correlated with the temperature difference on the day of onset ( IRR =0.02) (both P <0.05). It was also positively correlated with the wind speed of the previous 7 days ( IRR=0.07, P <0.05), but there was no statistically significant correlation with the relative humidity on the day of onset ( IRR=-0.00, P >0.05).
Conclusions
Low temperature, large temperature difference, and high wind speed can increase the risk of other infectious diarrhea. It is necessary to strengthen the prediction and early warning in conjunction with meteorological changes, and warn kindergartens and schools to enhance preventive measures against the clustering of other infectious diarrhea cases.

Obesity, as a chronic recurrent disease, has become a major public health challenge in China. To implement the requirements of the Healthy China Initiative (2019—2030), under domestic guidelines or consensus statements on overweight and obesity, and in alignment with the latest scientific advances globally, the Quality control protocol for adult overweight and obesity screening in health management (examination) institutions (2025 edition) was developed. This protocol was drafted by the Health Management Center of Shanghai Changzheng Hospital and formulated through multiple rounds of deliberation by experts in China’s health examination quality control field. The protocol establishes unified standards for screening facilities, personnel qualifications, and measurement or testing procedures. It defines specific screening items, outlines a standardized screening pathway, and sets requirements for the final medical review, ensuring the scientific validity, effectiveness, and safety of the screening process. The implementation of this protocol will enhance the consistency of weight management practices for adults across health examination institutions and strengthen the quality control of overweight and obesity screening programs.

Background:To compare the efficacy and safety of monthly administrations of gonadotropin releasing hormone (GnRH) agonists LY01005 and Zoladex ? in Chinese patients with premenopausal breast cancer. Methods:From October 2020 to November 2021, 188 premenopausal breast cancer patients were enrolled in 34 hospitals and randomized 1:1 to receive either LY01005 or Zoladex ? every 28 days for a total of three injections. All patients concomitantly received oral tamoxifen (TAM). The primary efficacy endpoint was cumulative probability of maintaining menopausal level [oestradiol (E2) ≤30 pg/ml] from day 29 to day 85. The second efficacy endpoint included changes in E2, luteinizing hormone (LH), and follicle-stimulating hormone (FSH) compared with the baseline. Pharmacokinetics (PK), pharmacodynamics (PD), and safety were analyzed. The study also evaluated the pharmacokinetic and pharmacodynamic characteristics of LY01005. Results:A total of 188 patients were randomised and 187 patients received either LY01005 or Zoladex ?. Cumulative probabilities of maintaining menopausal level (E2≤30 pg/ml) from day 29 to day 85 were 93.1% for LY01005 and 86.3% for Zoladex ?. The between-group difference was 6.8% (95% CI: -2.3%, 15.9%) and primary efficacy in the LY01005 group was not inferior to that in the Zoladex ? group. Changes in E2, LH, and FSH levels compared with the baseline were equivalent between the two groups (E2: 89.34% to 90.23% vs. 82.11% to 85.02%; LH: 88.89% to 95.52% vs. 89.70% to 97.02%; FSH: 75.36% to 80.85% vs.73.07% to 80.24%, respectively). After three consecutive doses of LY01005, the LH and FSH levels of the subjects showed a transient increase after the first dose, reached a peak on the second day and then started to decrease. The LH and FSH reached a lower level and remained at or below that level until the 85th day. Both treatments were well-tolerated. Conclusion:LY01005 is as effective as Zoladex ? in suppressing E2 to menopausal levels in Chinese patients with premenopausal breast cancer, with a similar safety profile.

10.The effects of cannabidiol on the expression of NLRP3 inflammasome following multiple cerebral concussions in rats

Objective:To investigate the effects of cannabidiol(CBD)on the NOD-like receptor protein 3(NLRP3)inflammasome in the brains of rats with multiple cerebral concussions(MCC).Methods:Rats were subjec-ted to the MCC model and divided into Sham,MCC,vehicle(MCC+TW),CBD-L(10 mg/kg),and CBD-H(40 mg/kg)groups.Immunofluorescence double staining was used to observe changes in NLRP3 and microglial cells in the brain,and Western Blot was performed to detect the expression changes of the NLRP3 inflammasome.Results:A sig-nificant increase in lectin-positive microglial cells of the cortex with enlarged cell bodies and elevated immunofluores-cence intensity of NLRP3 in the activated microglial cells was revealed by immunofluorescence double staining following MCC(P<0.05).The immunofluorescence intensity of NLRP3 in the activated microglial cells was downregulated by the administration of CBD,with a more pronounced effect observed in the CBD-H group compared to the CBD-L group(P<0.05).The expression of NLRP3,caspase-1,and apoptosis-associated speck-like protein(ASC)in the cortex,hippocampus,and basal ganglia of rats following MCC was significantly increased,as shown by Western Blot analysis(P<0.05),and cortical areas are more elevated.The expression of these proteins in different brain regions was reduced by CBD-10 and CBD-40 intervention(P<0.05).Conclusion:Cannabidiol can reduce the inflammatory response of multiple cerebral concussions rats through NLRP3 inflammasome and protect nerve tissue.
