1.Efficacy analysis of endoscopic endonasal and craniotomy surgery in the treatment of craniopharyngioma Based on QST Classification

Objective This study aims to explore the efficacy and complication rates of the transcranial approach(TCA)and extended endoscopic endonasal approach(EEEA)for the treatment of craniopharyngiomas based on the QST classification,providing a scientific reference for clinical decision-making on surgical approach.Methods A total of 151 patients who underwent craniopharyngioma surgery at our center from January 2018 to December 2023 were enrolled.The patients were categorized into Q-CP(suprasellar type),S-CP(infundibular type),and T-CP(tuberal type)according to the QST classification.Systematic collection and analysis were performed on the outcomes of TCA and EEEA treatments,respectively.The differences in effectiveness between the two approaches were evaluated based on the QST classification.Results The improvement rate of visual symptoms was overall higher in the EEEA group than in the TCA group(59.1%vs.36.5%,P=0.006),and the visual deterioration rate was lower(3.0%vs.14.1%,P=0.006).However,the incidence of cerebrospinal fluid leakage was higher in the EEEA group(15.2%vs.3.5%,χ2=4.986,P=0.026).The incidence of postoperative seizures(8.2%vs.0,P=0.019),brain contusions(10.6%vs.0,P=0.005),and subdural hematoma(9.4%vs.0%,P=0.01)was higher in the TCA group.For patients with Q-CP type,the EEEA group had a higher rate of total tumor resection(92.9%vs.65.2%,P=0.025)and a lower recurrence rate(3.6%vs.21.7%,P=0.047),with shorter hospital stays and lower postoperative costs.The TCA group had higher intraoperative blood loss in this type(300 mL vs.200 mL,Z=-2.261,P=0.024).For S-CP type patients,the EEEA group showed a higher rate of total tumor resection(91.3%vs.74.2%)and a lower recurrence rate(0 vs.12.9%,P=0.031),with lower postoperative hospital costs.In T-CP type,due to the deeper location,EEEA showed limitations in protecting hypothalamic function and the TCA group had a better postoperative hypothalamic function score(P=0.035).Conclusion Based on QST classification,EEEA has advantages in Q-CP and S-CP types and is recommended as the preferred surgical procedure;In the T-CP type,TCA surgery is more helpful in protecting hypothalamic function.
2.Comparative proteomic analysis of rat adipose-derived mesenchymal stem cells and their exosomes

BACKGROUND:Research on adipose-derived mesenchymal stem cells and their exosomes often uses proteomics to analyze their roles in tissue repair,but comparative proteomic analyses between the two are scarce.OBJECTIVE:To analyze adipose-derived mesenchymal stem cells and their exosomes using proteomic analysis.METHODS:Rat adipose-derived mesenchymal stem cells were isolated,cultured,and identified.Exosomes were then extracted from cell supernatant and identified.Differentially expressed proteins of adipose-derived mesenchymal stem cells and their exosomes were analyzed using DIA proteomics.Gene Ontology and Kyoto Encyclopedia of Genes and Genomes analyses were performed on these differentially expressed proteins.RESULTS AND CONCLUSION:(1) Adipose-derived mesenchymal stem cells displayed a mainly spindle-shaped,fibroblast-like morphology.(2) The exosome suspension protein concentration was 7.66 mg/mL,as determined by the BCA method.(3) Exosomes exhibited a characteristic teacup shape with a visible double-layer membrane vesicle structure.The center presented a low electron density component.The exosomes showed a peak particle size distribution of 112.2 nm,a concentration of 7.5×1011 particles/mL,and an average diameter ranging from 70 to 140 nm.(4) Exosomes expressed high levels of surface marker proteins CD9 and TSG101.(5) Gene Ontology analysis of differentially expressed proteins revealed enrichment in the extracellular matrix and synapses,with functions related to ion binding,ribosome binding,and particularly cell adhesion and translation.(6) Kyoto Encyclopedia of Genes and Genomes pathway analysis indicated that the differentially expressed proteins were primarily involved in extracellular matrix receptor interaction,ribosome,and cytokine receptor interaction,and also associated with various metabolic diseases like cholesterol and thyroid disorders.
3.Factors influencing occupational burnout and career choice among resident physicians during standardized training

Objective:To investigate the prevalence,related factors,and career implications of occupational burnout among resident physicians during standardized training in Inner Mongolia.Methods:This cross-sectional study used convenience sampling to enroll 2 891 resident physicians,assessing them with a self-developed general information questionnaire,socio-psychological scales,and a universal burnout inventory.Latent profile analysis clas-sified participants based on burnout dimensions,while logistic regression models examined the factors influencing burnout and their relationship with career choices.Results:Resident physicians were divided into low(59.0%),moderate(28.9%),and high(12.1%)burnout subgroups.Logistic regression showed that,compared to the low burnout group,significant risk factors included professional master's students(OR=0.44),working>70 hours weekly(OR=0.63),>4 night shifts monthly(OR=0.66),anxiety(mild OR=0.49;moderate OR=0.26;se-vere OR=0.14),depression(mild OR=0.38;moderate OR=0.24;severe OR=0.11),insomnia(mild OR=0.51;moderate OR=0.44;severe OR=0.38),low social support(OR=0.23),and low happiness index(OR=0.42).Male residents(OR=1.25)were less likely to experience burnout.High burnout residents were 5.11 times more likely to quit the medical profession.Conclusion:Training intensity and socio-psychological factors signifi-cantly contribute resident physicians' burnout levels,with severe burnout increase career attrition.
4.Analysis of risk factors and development of a nomogram model for early recurrence following curative resection of resectable pancreatic cancer

Objective:To identify independent risk factors for early recurrence following curative resection of resectable pancreatic cancer and establish a nomogram prediction model.Methods:Clinical data from 405 patients with resectable pancreatic cancer treated at Renji Hospital, Shanghai Jiao Tong University School of Medicine from February 2010 to December 2020 were retrospectively reviewed. Patients were stratified into a training cohort (265 patients form February 2010 to December 2018) and a validation cohort (140 patients from January 2019 to December 2020) based on surgery dates. Optimal cutoff values for clinical variables were determined using X-tile software. Independent risk factors were identified through univariate and multivariate Cox proportional hazards regression analyses. Kaplan-Meier curves for recurrence-free survival (RFS) were generated across subgroups, and a nomogram was developed to predict early recurrence (within 1 year post-surgery). Time-dependent receiver operating characteristic (tROC) curves was drawn and area under the curve (AUC) metrics were utilized to evaluate predictive accuracy, while model reliability was assessed by calibration curves. Individualized risk scores derived from the nomogram were stratified into high- and low-risk groups using X-tile-derived cutoff values. Survival differences between groups were analyzed via log-rank tests. The clinical application value was judged by decision curve analysis (DCA) compared to TNM staging. Results:In the training cohort, 139 patients (52.45%) experienced early recurrence, with a median RFS of 11.1 months [interquartile range ( IQR): 6.0-26.0]. The validation cohort reported 70 early recurrences (50.00%) and a median RFS of 11.8 months ( IQR: 4.9-21.4). Univariate analysis revealed significant associations between early recurrence and tumor diameter, carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), carbohydrate antigen 125 (CA125), systemic immune-inflammation index (SⅡ), and prognostic nutritional index (PNI). Multivariate analysis identified tumor diameter ≥3.75 cm ( HR=1.718, 95% CI 1.223-2.412, P=0.002), CA19-9≥218 U/ml ( HR=1.567, 95% CI 1.107-2.220, P=0.011), CA125≥20.98 U/ml ( HR=2.501, 95% CI 1.768-3.539, P<0.001), SⅡ≥388.28 ( HR=1.708, 95% CI 1.096-2.662, P=0.018), and PNI<53.18 ( HR=0.596, 95% CI 0.404-0.879, P=0.009) as independent risk factors for early recurrence. The nomogram achieved AUC values of 0.771 and 0.708 in the training and validation cohorts, respectively. Calibration curves demonstrated strong agreement between predicted and observed survival probabilities. Kaplan-Meier analysis revealed significantly lower 1-year RFS rates in high-risk versus low-risk groups for both cohorts (training: HR=3.65, 95% CI 2.45-5.44, P<0.001; validation: HR=2.37, 95% CI 1.39-4.06, P=0.001). DCA indicated superior net benefit of the nomogram over TNM staging across threshold probabilities of 0.2-0.9. Conclusions:The proposed nomogram effectively integrates clinical and serological biomarkers to preoperatively assess early recurrence risk in resectable pancreatic cancer patients, offering enhanced precision for clinical decision-making.
5.Recent advancements of elbow hemiarthroplasty for distal humeral fractures

Open reduction and internal fixation are the preferred methods for treating intra-articular fractures of the distal humerus. However, in cases of comminuted fractures, open reduction and internal fixation may result in loss of reduction. Although total elbow arthroplasty can be used as an alternative treatment option, total elbow arthroplasty has several limitations, including excessive bone removal, strict weight-bearing restrictions, polyethylene liner wear, and alterations in biomechanical properties. Elbow hemiarthroplasty is emerging as a novel surgical technique, only the distal end of the humerus is replaced while the proximal end of the ulna and radius is retained. Particularly for patients with high functional demands, elbow hemiarthroplasty may be a viable treatment option. This technique is particularly suitable for young patients with severely comminuted and non-reconstructive intra-articular fractures. Postoperative complications of elbow hemiarthroplasty include elbow stiffness, proximal wear of the ulna and radius, and prosthesis loosening, etc. In some cases, total elbow prosthesis revision is required. Existing research has initially confirmed that elbow hemiarthroplasty has certain advantages in terms of safety and elbow joint function recovery. Additionally, to better accommodate the anatomical characteristics of the Chinese population, customized 3D-printed prostheses based on the contralateral distal humerus and the ipsilateral proximal ulna and radius can be utilized. Further research and development of elbow hemiarthroplasty and novel technologies tailored to the Chinese population should be encouraged to enhance the precise treatment of complex elbow injuries in China.
6.Diagnosis and treatment of elbow triad

Elbow Triad refers to an elbow dislocation combined with fractures of the radial head and the coronoid process, representing a complex elbow injury. Management of the elbow triad is challenging and often yields poor outcomes, frequently leading to complications such as elbow instability, ulnar nerve injury, and elbow stiffness. The injury mechanism of the elbow triad typically involves a fall on an outstretched arm, subjecting the forearm to supination, axial loading, and valgus stress. This force sequence results in progressive disruption of the elbow's anatomical structures from lateral to medial. Diagnosis requires particular attention to imaging assessment, and special attention should be paid to differentiating coronoid process fractures from elbow dislocation. Currently, there is no independent classification system, and diagnosis often relies on the modified Mason classification for radial head fractures and the Regan-Morrey and O'Driscoll classifications for coronoid process fractures. Non-surgical indications must be strictly defined and include: congruent joint reduction, absence of bony block to motion, a small or non-displaced coronoid fracture fragment, and inherent elbow stability. For the majority of cases requiring surgical treatment, a lateral approach is recommended, with a combined over-the-top approach when necessary. During surgery, the coronoid process fracture, radial head fracture, and lateral soft tissues should be addressed sequentially, with restoration of stability being crucial. Postoperative care should include early functional exercises and pay attention to preventing complications such as elbow instability, traumatic elbow stiffness, and ulnar nerve injury.
7.A three-party evolutionary game analysis of patient privacy protection in live surgery

With the rapid development of network technology, live surgery has become the new way of surgery teaching. However, the issue of patient privacy protection caused by live surgery has received widespread attention. Based on the evolutionary game theory, this paper constructed an evolutionary game model from the three-party perspectives of doctors, patients, and government and analyzed the game behaviors of the three parties in the process of live surgery. Matlab software was utilized to conduct dynamic simulation and numerical simulation analysis. It was found that the factors affecting the choice of doctors’ strategies included protection costs, the cost of privacy leakage, the benefits of protection, high-traffic benefits, and other aspects; the factors affecting the choice of patient strategies encompassed surgical costs, the risk of privacy leakage, additional benefits, and other aspects; the factors affecting the choice of government strategies embodied regulatory costs and the improvement of credibility. To realize a win-win situation among doctors, patients, and the government, the three parties need to work together to ensure that patient privacy is not violated and find a balance between expanding the influence of medical education and protecting patient privacy.

9.ECG-less technology and deep learning image reconstruction algorithm for coronary CT angiography

Objective To explore the value of ECG-less technology and deep learning image reconstruction(DLIR)algorithm for coronary CT angiography(CCTA).Methods Seventy-one patients with suspected coronary artery disease(CAD)were prospectively enrolled and randomly divided into group A(n=35,underwent CCTA using ECG-less technology)and B(n=36,underwent CCTA using physical electrocardiographic gating).CCTA images of group A were reconstructed using filtered back projection(FBP),adaptive statistical iterative reconstruction V(ASiR-V)(50%blending)and high-strength DLIR(DLIR-H),respectively,while those of group B were reconstructed using conventional ASiR-V(50%blending).The radiatio n doses and examination durations of CCTA were compared between groups.Then subjective and objective evaluation of imaging quality were compared between groups and within group A among 3 reconstruction algorithms.Results No significant difference of radiation dose of CCTA was found between groups(P>0.05).The examination duration in group A was reduced by approximately 29.37%compared to group B(P<0.05).No significant difference of subjective image quality scores nor objective evaluation of ASiR-V reconstructed CCTA was detected between groups(all P>0.05).In group A,compared to FBP algorithm,subjective image quality scores of DLIR-H and ASiR-V upgraded,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)increased and standard deviation(SD)values decreased(all P<0.05),but no significant difference of CT values was found(all P>0.05).Conclusion ECG-less technology could significantly shorten CCTA examination duration,while DLIR could reduce image noise and improve image quality of CCTA.
10.Study on medical damage liability dispute cases involving breach of the duty to inform

ObjectiveTo understand the current situation of medical damage liability disputes involving breach of the duty to inform in China, analyze the factors influencing the types of medical staff’s breach of the duty to inform, and explore the notification problems of medical staff in clinical practice. On these foundations, suggestions were proposed to improve the performance of the duty to inform and reduce medical disputes. MethodsUsing public cases from the China Judgements Online as the data source, the relationship between risk points and types of breach of duty to inform was analyzed using the Chi-square test. Logistic regression analysis was performed to explore the influencing factors of the types of breach of the duty to inform, and qualitative research summarized the specific contents of breach of the duty to inform. ResultsThere were differences in the effects of factors, including whether the patient was hospitalized, whether surgery was performed, whether Intensive Care Unit (ICU) treatment was required, the level of the medical institution, and whether a consultation occurred, on the types of breach of the duty to inform (P<0.05). Whether surgery was performed was an influencing factor for the types of breach of the duty to inform. Qualitative research showed that the contents of breach of duty to inform primarily involve risk, treatment plan, and deficiencies in disease notification. ConclusionStrengthening the performance of the medical staff’s duty to inform should mainly focus on the medical side, coordinating with multiple parties and taking measures to improve the performance of the duty to inform, to reduce unnecessary medical disputes.
