Objective:To explore the feasibility and effect of applying spherical fitting technology in CT three-dimensional reconstruction in preoperative planning of the resection range of Cam-type femoral acetabular impingement (FAI).Methods:46 Cam-type FAI patients who underwent arthroscopic femoroplasty in the Department of Sports Medicine in the Third Affiliated Hospital of Southern Medical University from July 2020 to June 2022 were enrolled in the spherical fitting group, including 26 females and 20 males, with an average age of 38.4±15.2 years (range of 24-53 years). The preoperative planning was performed using spherical fitting technology in CT three-dimensional reconstruction. Another 42 Cam-type FAI patients who underwent arthroscopic femoroplasty from July 2018 to June 2020 were enrolled in the observation group, including 25 females and 17 males, with an average age of 43.6±18.4 years (range 24-61 years). The preoperative planning was performed using CT three-dimensional reconstruction observation method. This study evaluated the impact of femoroplasty planned by spherical fitting technique on patients' prognosis though comparing the differences of postoperative α angle, femoral head-neck offset ratio, modified Harris hip score (mHHS), and visual analog score (VAS) at 3, 6, 9, and 12 months between two groups. Results:The average follow-up time was 11.8±0.6 months in spherical fitting group and 11.3±0.8 months in observation group. There was no significant difference regarding α angle, femoral head-neck offset ratio, mHHS, and VAS between two groups preoperatively ( P>0.05). The mHHS in two groups increased gradually, while VAS decreased sequentially at 3, 6, 9, and 12 months postoperatively. The postoperative mHHS and VAS were significantly better than those before surgery ( P<0.05). The mHHS was 83.2±14.8 vs. 70.5±11.2 in spherical fitting group and observation group at the followup of 9 months with significant difference ( t=4.471, P=0.007). It was 85.7±13.3 vs. 73.2±12.5 at the followup of 12 months with significant difference ( t=4.596, P=0.008). No significant difference was found in α angle, femoral head-neck offset ratio or mHHS at 3 and 6 months postoperatively ( P>0.05), and no significant difference was found in VAS at 3, 6, 9, and 12 months postoperatively between two groups. Conclusion:The spherical fitting technology in CT three-dimensional reconstruction could assist surgeons with planning of the resection range of Cam deformity preoperatively, and achieve a smooth progress of arthroscopic femoroplasty intraoperatively; after short term follow-up, we found that patients who underwent femoroplasty using this technology achieved satisfactory outcomes.

Objective:To analyze the clinical characteristics of hospitalized patients with distal deep venous thrombosis (DDVT).Methods:Medical records of patients without DDVT at admission and discharged from January 1,2021 to June 30,2021, were retrospectively reviewed. During the pre-hospitalization and hospitalization period the ultrasonography was performed for detection of DDVT. The venous thromboembolism (VTE) risk scores were evaluated with Caprini scale for surgical patients, and evaluated with Padua scalefor nonsurgical patients. Results:A total of 16 400 patients, 8 827 males and 7 573 females with a mean age of (63.8±14.2) years (14 to 101 years) were enrolled, among whom DDVT occurred in 1 193 patients (DDVT group), including 73 cases developed during pre-hospitalization; meanwhile 124 patients were diagnosed as deep vein thrombosis(DVT), and 15 083 patients without DVT served as non-DDVT group. The detection rate of DDVT in patients with medium/high VTE risk scorewas significantly higher than thatin low-risk patients [12.8% (982/7 644) vs. 2.4% (211/8 756), χ2=659.10, P<0.001]. The detection rate of DDVT for medium/high VTE risk score group was the highest in emergency intensive care unite and intensive care unite(ICU; 34.3%, 68/198), followed by that in departments of rehabilitation (25.7%, 45/175), neurology (19.9%,37/186), neurosurgery(19.4%,83/428), respiratory medicine(19.0%,56/295) and orthopedics (15.4%, 378/2 451).The detection rate of DDVT increased with age.The lowest detection rate was found in the age group 14 to 40 years, for low VTE risk score group it was 0.5%(4/770), for the medium/high VTE risk score group it was 3.0%(11/362, χ2=10.10, P<0.001).In patients over 75 years of age, the detection rate of DDVT was 5.2%(85/1 624) in low VTE risk score group, and 18.0%(389/2 158) in medium/high VTE risk score(χ2=138.82, P<0.001).The mean age of the DDVT group was older than that of non-DDVT group [(71.0±12.1) vs. (63.2±14.2) years, t=21.14, P<0.001]. The abnormality rate of D-dimer level in DDVT patients was significantly higher than that in non-DDVT patients [71.4%(813/1 138) vs. 25.4%(3 492/13 770), χ2=1 086.80, P<0.001]. The median length of hospital stay was significantly longer than that of the non-DDVT group [11.0 (8.0, 19.0) vs. 6.0 (4.0, 10.0)d, Z=-26.67, P<0.001].The risk factors for non-surgical DDVT patients were age, bed rest for ≥3 d, and acute infection; while the risk factors for surgical DDVT patients were age, fracture in hip, pelvis or lower limb, bedridden for ≥3 d and body mass index (BMI)>25 (kg/m 2). Conclusion:The detection rate of DDVT is higher in hospitalized patients, especially for those in ICU and rehabilitation department, and those over 75 years old with high VTE score. Advanced age, fracture or immobilization, and long-term bed rest are the main risk factors for DDVT.

Pharmaceutical cocrystals are a promising technology that can be used to improve the solubility of poor aqueous compounds. The objective of this study was to systematically investigate the solubility of myricetin (MYR) cocrystals, including their kinetic solubility, thermodynamic solubility, and intrinsic dissolution rate (IDR). The effects of pH, surfactant, ion concentration, and coformers on the cocrystal solubility were evaluated. Furthermore, single crystal structures of MYR, myricetin-isonicotinamide (MYR-INM) and myricetin-caffeine (MYR-CAF) cocrystals were analyzed to discuss the possible reasons for the enhancement of cocrystal solubility from the perspective of the spatial structure. The results indicated that the kinetic solubility of MYR cocrystals was modulated by pH and cocrystal coformer (CCF) ionization in buffer solution, while it primarily depended on the CCF solubility in pure water. In addition, the solubility of MYR cocrystals was increased in a concentration dependent fashion by the surfactant or ion concentration. The thermodynamic solubility of MYR-INM (1:3) cocrystals decreased with the increases of the pH value of the dissolution media. The IDR of MYR cocrystals was faster than that of MYR in the same medium and extremely fast in pH 4.5 buffer. The improved solubility of MYR cocrystals was probably related to the alternate arrangements of MYR and INM/CAF molecules and increased intermolecular distance. The present study provides some references to investigate the solubility behavior of pharmaceutical cocrystals.

Objective To evaluate the strategy of chemoradiotherapy following endoscopic R0 resection for esophageal cancer in M3-T1b stage. Methods There were 45 esophageal cancer patients with M3-T1b stage with endoscopic R0 resection followed by additional chemoradiotherapy from ECETC (Esophageal Cancer Endoscopic Therapy Consortium) as a trial group with 34 males and 11 females at age of 61.37±7.14 years. There were 90 patients with esophagectomy from Fudan University Shanghai Cancer Center as a control group with 63 males and 27 females at age of 61.04±8.17 years. Propensity score match (1:2) was used to balance the factors: gender, age, position, depth of invasion and lymphovascular invasion (LVI), which may influence the outcomes. Overall survival (OS) rate, relapse free survival (RFS) rate, and local recurrence rate were compared between the two groups. Result There was no statistical difference (HR=2.66 with 95%CI 0.87 to 8.11, P=0.179) in terms of OS rate between the two groups. One, two and three years overall survival rate of patients in the control group was 93%, 86%, and 84%, respectively. Nobody died in the trial group within 3 years after surgery. The RFS rate between the two groups didn’t significantly differ (HR=1.48, 95% CI 0.66 to 3.33, P=0.389). One, two and three years RFS rate of patients in the contorl group was 87%, 78%, and 76%, respectively, while 97%, 93%, and 73% in the trial group, respectively. The local recurrence rates between the two groups didn’t significantly differ either ( HR=0.53,95%CI 0.13 to 2.18, P=0.314). One, two and three years local recurrence rate of patients in the control group was 5%, 6% and 6%, respectively, while 0%, 0% and 21% in the trial group, respectively. Conclusion Similar outcomes are found regarding OS, RFS and local recurrence rates between the two groups. The strategy of endoscopic R0 resection followed by additional chemoradiotherapy has prospect for the treatment of esophageal cancer in M3-T1b stage. And this kind of therapy may be provided for those with risk factors or can not tolerate surgery.
