1.Relationship between serum visfatin, NLR and disease severity in elderly patients (≥90 years old) with coronary heart disease

Objective To study the relationship between serum visfatin, neutrophil-to-lymphocyte ratio (NLR) and disease severity of coronary heart disease (CHD) in elderly patients (≥90 years old). Methods One hundred and two elderly patients (≥90 years old) with CHD who received coronary CT angiography (CTA) were selected from January 2020 to June 2024. In addition, thirty-five elderly patients (≥90 years old) without CHD who underwent coronary CTA during the same period were included in the control group. The CHD patients were divided into mild group, moderate group and severe group by CT-SYNTAX score. The clinical data and levels of serum visfatin and NLR were compared, and the correlation and diagnostic value of the above levels with disease severity of CHD in elderly patients were analyzed. Results Serum visfatin and NLR were manifested as severe group>moderate group>mild group>control group (P<0.05). Pearson correlation analysis found that serum visfatin and NLR were moderately positively correlated with CT-SYNTAX score (r=0.574, 0.482, P<0.001). Receiver operating characteristics (ROC) curve indicated that the area under the curve (AUC) of combination of serum visfatin and NLR in diagnosing severe lesion in elderly patients (≥90 years old) with CHD was 0.882, which was higher than that of visfatin or NLR alone, and its 95%CI was 0.803-0.938. Conclusion There is a certain correlation between serum visfatin, NLR and disease severity of CHD in elderly patients. The combination of the above indicators has a higher predictive value on severe lesion.

Objective To investigate the effects of kaempferol on the oxidative damage and apoptosis of human lens epithelial cells induced by hydrogen peroxide(H2O2),as well as its regulatory role in the Janus kinase 2/signal transducer and activator of transcription 3(JAK2/STAT3)signaling pathway.Methods Human lens epithelial cells HLE-B3 were cultured in vitro and divided into the blank group(no intervention),the model group(400 pmol?L-1 H2O2),the kaempferol group(400 μmol?L-1 H2O2+40 pmol?L-1 kaempferol),the AG490 group(400 pmol?L-1 H2O2+50 μmol?L-1 JAK2/STAT3 signaling pathway inhibitor AG490),the inhibitor group(400 pmol?L-1 H2O2+40 pmol?L-1 kaempferol+50 μmol?L-1 AG490),and the activator group(400 pmol?L-1 H2O2+40 pmol?L-1 kaempferol+0.5 μmol?L-1 JAK2/STAT3 signaling pathway activator C-A1).After 24 h of intervention,Hoechst 33258 staining was used to determine the ap-optosis rate of these cells.Malondialdehyde(MDA)and superoxide dismutase(SOD)kits were used to detect the levels of MDA and SOD.The protein expression levels of Cysteine aspartic protease 3(Caspase-3),B-cell lymphoma-2(Bcl-2),JAK2,STAT3,p-JAK2,and p-STAT3 were detected by Western blot.Results Compared with the blank group,the apop-tosis rate,the MDA level,and the protein expression levels of Caspase-3,p-JAK2,and p-STAT3 increased(all P<0.05)in the model group,while the protein expression levels of SOD and Bcl-2 decreased(all P<0.05).Compared with the model group,the apoptosis rate,the MDA level,and the protein expression levels of Caspase-3,p-JAK2,and p-STAT3 decreased in the kaempferol and AG490 groups(all P<0.05),while the protein expression levels of SOD and Bcl-2 increased(all P<0.05).Compared with the kaempferol group,the apoptosis rate,the MDA level,and the protein expression levels of Caspase-3,p-JAK2,and p-STAT3 decreased in the inhibitor group,while the protein expression levels of SOD and Bcl-2 in-creased(all P<0.05);the changes in the above indicators in the activator group were opposite to those in the inhibitor group(all P<0.05).Conclusion Kaempferol can protect HLE-B3 cells from H2O2-induced oxidative damage and inhibit their apoptosis by inhibiting JAK2/STAT3 signaling pathway.
4.Association of lipid metabolism with gestational diabetes mellitus in obese pregnant women

Objective:To investigate risk factors for gestational diabetes mellitus (GDM) and establish early lipid management thresholds in obese pregnant women.Methods:This prospective cohort study enrolled obese women [pre-pregnancy body mass index (BMI)≥28 kg/m2] during their first prenatal visit (6-12 weeks of gestation) at Beijing Obstetrics and Gynecology Hospital, Capital Medical University from October 2021 to October 2022. Serum lipid profiles [total cholesterol, triglycerides (TG), high density lipoprotein-cholesterol (HDL-C), low density lipoprotein-cholesterol] were measured in the first (6-12 weeks), second (24-28 weeks), and third trimesters (34-37 weeks), with body weight tracked at 16, 20, and 24 weeks of gestation. Participants were stratified by pre-pregnancy BMI into mild (≥28-<30 kg/m2), moderate (≥30-<35 kg/m2), and severe obesity (≥35 kg/m2) groups; meanwhile, they were also stratified into GDM and non-GDM groups. Intergroup differences were analyzed using independent t-tests, Wilcoxon and Kruskal-Wallis rank-sum tests, one-way analysis of variance, and Chi-square tests. Binary logistic regression identified independent risk factors for GDM, while a decision tree model incorporating maternal age, pre-pregnancy BMI, first-trimester FBG, lipid levels, and weight gain at 16/20/24 weeks was constructed to delineate GDM risk stratification. Receiver operating characteristic curves established first-trimester lipid thresholds for GDM prevention in mild and moderate obesity subgroups. Results:After excluding 88 cases, 487 women were analyzed, with 202 (41.5%) developing GDM. (1) Weight gain disparities: There were statistically significant differences in weight gain at 16, 20, and 24 weeks of gestation among pregnant women with mild, moderate, and severe pre-pregnancy obesity [at 16 weeks: 1.0 (0.0-3.0), 0.9 (-1.0-2.0) vs.-1.4 (-3.0-1.1) kg, χ 2=16.92; at 20 weeks: 3.0 (1.0-5.0), 2.0 (0.0-4.0) vs.-0.5 (-2.3-1.3) kg, χ 2=23.94; at 24 weeks of gestation: 4.0 (2.0-7.0), 3.0 (1.0-5.0) vs. 0.0 (-2.0-3.1) kg, χ 2=28.27; all P<0.001]. GDM incidence escalated with obesity severity: 36.9% (93/252) in mild, 45.0% (95/211) in moderate, and 58.3% (14/24) in severe obesity groups ( χ2=6.07, P=0.048). No significart difference was found in serum lipid profiles during the first, second, and third trimesters (all P>0.05). (2) Risk factors: First-trimester FBG was the risk factor of GDM in pre-pregnancy obese pregnant women ( OR=5.196, 95% CI: 3.025-8.926, P<0.001). (3) The decision tree model revealed that the key nodes for GDM development in obese pregnant women were first-trimester FBG, first-trimester TG levels, maternal age, first-trimester HDL-C, and pre-pregnancy BMI. When obese pregnant women had FBG≥5.1 mmol/L and age≥35 years, the incidence of GDM reached 88%. Conversely, when first-trimester TG<1.5 mmol/L, first-trimester FBG<4.9 mmol/L, and first trimester HDL-C≥1.3 mmol/L, the GDM incidence dropped to 15%. (4) First-trimester TG thresholds of 1.5 mmol/L for mild obesity (area under the curve was 0.626, 95% CI: 0.556-0.697) and 1.4 mmol/L for moderate obesity (area under the curve was 0.636, 95% CI:0.560-0.713) to guide lipid management. Conclusions:First-trimester FBG is closely associated with GDM development in pre-pregnancy obese women, with varying incidences across different obesity severity levels. Clinical management should prioritize both glycemic control and early-pregnancy lipid monitoring in mild-to-moderately obese populations.

Objective To assess the effectiveness of the Gasdermin D C-terminal fragment(GSDMD-CT)as a novel plasma biomarker for the clinical diagnosis of sepsis.Methods Between July 2021 and November 2024,245 patients from Tangshan Gongren Hospital were enrolled in this study.In accordance with the diagnostic criteria for sepsis and the systemic inflammatory response syndrome(SIRS),patient samples were classified into the sepsis group and the SIRS group.Meanwhile,healthy individuals were selected as the healthy control(HC)group.Sepsis patients were further categorized into the Gram-positive bacterial group,the Gram-negative bacterial group,and the fungal group based on the type of pathogen infection.The levels of GSDMD-CT,C-reactive protein(CRP),and procalcitonin(PCT)were measured in all subjects.Nonparametric tests were employed to compare the differences in various indices among different groups.The diagnostic value of GSDMD-CT in sepsis was evalu-ated by constructing the receiver operating characteristic(ROC)curve.Spearman's correlation analysis was used to examine the relationships among GSDMD-CT,CRP,and PCT.Results The plasma GSDMD-CT levels in the sepsis group 23.02(16.71,33.01)pg/mL and in the SIRS group 16.52(11.26,22.22)pg/mL were significantly higher than those in the healthy control group 7.02(4.42,11.43)pg/mL(U=-10.175,-7.890,P<0.001).Moreover,the plasma GSDMD-CT levels in the sepsis group were significantly higher than those in the SIRS group(U=-2.941,P<0.05).In the Gram-positive bacterial group,the Gram-negative bacterial group,and the fungal group,the GSDMD-CT levels were 23.01(17.16,27.51)pg/mL,23.41(16.78,35.50)pg/mL,and 16.29(14.53,56.27)pg/mL,respectively.When compared with the healthy control group,the GSDMD-CT levels in these three groups were all significantly higher(P<0.05).However,there were no significant differences in GSDMD-CT levels among these three groups(P>0.05).The area under the curve(AUC)of plasma GSDMD-CT for diagnosing sepsis was 0.881(95%confidence interval:0.833~0.929),with a Youden index(YI)of 0.695,a sensitivity of 85.0%,and a specificity of 84.5%.Spearman correlation analysis indicated a weak correlation between GSDMD-CT and C-reactive protein(CRP)(r=0.32,P<0.001)and a positive correlation between GSDMD-CT and procalci-tonin(PCT)(r=0.65,P<0.001).Conclusion GSDMD-CT exhibits significant clinical value in the diagnosis of sepsis and holds great potential as a biomarker in the diagnostic process of sepsis.

Objective To investigate the effects of digital platform construction in departmental Party branches on the or-ganizational stronghold role of primary-level Party organizations.Methods Under the leadership of the hospital Party committee,the Radiology Department Party Branch launched the development of a"Party building+Services"digital platform in July 2021,facilitating a comprehensive integration of political and operational activities.This study retrospectively compared the differences in the indicators of party-building and the disciplinary development,mainly including Party-building effectiveness,team cohe-sion,management efficiency,clinical services,scientific research outputs,and educational performances between three-years pre-and post-implementation.Additionally,it also summed up the impact of an electronic platform development on the role of primary-level Party organizations as a combat bastion.Results Post-implementation metrics demonstrated statistically significant improvements in such indicators of Party organization building and disciplinary development as primary-level Party building effec-tiveness,team cohesion,management efficiency,scientific research outputs,and educational performances(all P<0.05).Conclusion The establishment of an electronic platform within the department fosters deep integration and intelligent empower-ment of Party building and operational activities,enhances the quality of Party building,and promotes the advancement of disci-plines,thereby reinforcing the role of primary-level Party organizations as effective combat bastions.

Objective:To retrieve, evaluate, and summarize the evidence on prevention and management of chronic venous disease of the lower extremities, so as to provide reference for clinical practice.Methods:Clinical decisions, guidelines, evidence summaries, and expert consensus on chronic venous diseases of the lower extremities were systematically retrieved from UpToDate, Joanna Briggs Institute Evidence-Based Health Care Center Database, National Institute for Health and Clinical Excellence, Registered Nurses' Association of Ontario, New Zealand Guidelines Group, National Health and Medical Research Council, Guidelines International Network, Medlive, PubMed, Embase, China Biology Medicine disc, China National Knowledge Infrastructure, Wanfang Data, and specialized collaborative websites including the Society for Vascular Surgery, American Vein and Lymphatic Society, American Venous Forum, and European Society for Vascular Surgery. The search period was from the establishment of the database to March 31, 2025.Results:A total of 18 articles were included, including three guidelines, five clinical decisions, four evidence summaries, and six expert consensus. Ultimately, 19 pieces of evidence were integrated from six aspects of diagnosis, clinical-etiology-anatomy-pathophysiology (CEAP) classification and severity assessment, risk factor assessment, prevention, lifestyle modification, stress management, and patient education and follow-up.Conclusions:Evidence included in this study indicates that the diagnosis of chronic venous disease of the lower extremities, along with CEAP classification and severity assessment, has been widely adopted.
