1.Biomechanical study on wing shaped titanium plate fixation of acetabular anterior column and posterior hemi-transverse fracture under multiple working conditions.

This article aims to compare and analyze the biomechanical differences between wing-shaped titanium plates and traditional titanium plates in fixing acetabular anterior column and posterior hemi-transverse (ACPHT) fracture under multiple working conditions using the finite element method. Firstly, four sets of internal fixation models for acetabular ACPHT fractures were established, and the hip joint stress under standing, sitting, forward extension, and abduction conditions was calculated through analysis software. Then, the stress of screws and titanium plates, as well as the stress and displacement of the fracture end face, were analyzed. Research has found that when using wing-shaped titanium plates to fix acetabular ACPHT fractures, the peak stress of screws decreases under all working conditions, while the peak stress of wing-shaped titanium plates decreases under standing and sitting conditions and increases under forward and outward extension conditions. The relative displacement and mean stress of the fracture end face decrease under all working conditions, but the values are higher under forward and outward extension conditions. Wing-shaped titanium plates can reduce the probability of screw fatigue failure when fixing acetabular ACPHT fractures and can bear greater loads under forward and outward extension conditions, improving the mechanical stability of the pelvis. Moreover, the stress on the fracture end surface is more conducive to stimulating fracture healing and promoting bone tissue growth. However, premature forward and outward extension rehabilitation exercises should not be performed.

2.Simulation of Potential Suitable Habitats for the Tibetan Medicine"Zhi Da Sa Zeng"and Analysis on Influencing Factors

Objective To investigate the spatial distribution patterns and environmental determinants of Lagotis brachystachya Maxim.,a keystone species in Tibetan medicine("Zhi Da Sa Zeng"),under contemporary climatic conditions in China;To provide references for the sustainable utilization of its resources and the ecological protection of alpine grassland areas.Methods Based on the maximum entropy model,the selected species distribution data and environmental factors were added to MaxEnt 4.3.1 software for modeling.The geographical detector was applied to quantify the factors influencing the spatial differentiation of the suitable areas,employing both factor detection and interaction detection functionalities.Additionally,an overlay analysis with land cover types was conducted to quantify the distribution characteristics of grassland within the potential suitable areas for Lagotis brachystachya Maxim..Results Under the current climatic conditions,dominant environmental drivers included elevation,monthly average temperature difference between day and night,average precipitation in June,wind speed in September,solar radiation in August,average precipitation in May,average precipitation in October,among which elevation demonstrated the strongest explanatory power(q=0.37)in habitat suitability analysis.The model prediction results showed that the potential suitable area for Lagotis brachystachya Maxim.was 5.31×105 km2.Considering the type of surface coverage,the main suitable habitat for Lagotis brachystachya Maxim.was grassland,with an area of 4.34×105 km2.The high suitable grassland of areas were mainly distributed in Qinghai Province(Yushu Tibetan Autonomous Prefecture,Huangnan Tibetan Autonomous Prefecture,Golog Tibetan Autonomous Prefecture,Hainan Tibetan Autonomous Prefecture,Haibei Tibetan Autonomous Prefecture),Sichuan Province(Ganzi Tibetan Autonomous Prefecture,Aba Qiang Autonomous Prefecture),Xizang Autonomous Region(Naqu City,Shigatse City,Changdu City),and Gansu Province(Gannan Tibetan Autonomous Prefecture).Conclusion This study can provide references for the protection of wild resources and the selection of domestication and cultivation areas for Lagotis brachystachya Maxim..
3.Differential analysis of intestinal flora in patients with hepatic blastomycosis based on second-generation sequencing

Exploring the variability of the intestinal flora of patients with hepatic blastocysticercosis and searching for members of the intestinal microflora that may play a role in the disease process by means of macro-genome sequencing technology. A case-control study was used to include fecal samples from patients with hepatic vesicular schistosomiasis admitted to Qinghai Provincial People′s Hospital between October 2023 and January 2024 and individuals attending health checkups. The experimental group (AE group) consisted of 10 patients with liver vesicular schistosomiasis and the control group (NC group) consisted of 9 individuals attending health checkups. Macrogenomic sequencing was performed on these two groups of samples using the Illumina Novaseq 6000 sequencing platform, using fastp (v0.20.1) to remove junctions, and bbmap (v38.93-0) to remove the hosted sequences, followed by sequence splicing using MEGAHIT (v1.2.9), and then using prodigal (v2.6.3) to The spliced scaffold was subjected to ORF prediction and translated into amino acid sequences, followed by the construction of a non-redundant gene set using MMSeqs2 (v13.45111), and finally compared with the non-redundant gene set using salmon (v1.8.0). Species were annotated by the non-redundant database, species abundance was calculated in each sample, and the two sets were tested using Wilcoxon rank sum test. Finally, the differences in intestinal flora between the two groups were statistically analyzed using linear discriminant analysis, and the correlation between the differential intestinal flora and clinical indicators was analyzed using redundancy analysis (RDA). The results showed that the effective data volume of each sample was distributed from 10.41 to 12.46 G. The number of ORFs in the de-redundantly constructed gene catalogue (non-redundant gene set) was 4 951 408, and the annotation rate of the non-redundant genes was 97.97% when compared with the NR database. The ages of the study subjects in the two groups were (44.78±4.58) years in the NC group and (42.90±10.44) years in the AE group, and the difference was not statistically significant ( t=0.530, P=0.476). The two groups were matched for body mass index (BMI) ( t=2.368, P=0.142), gender ( χ2=0.200, P=0.655), and dietary habits. There was no statistically significant difference in alpha diversity in the AE group (ACE index, t=0.942; chao1 index, t=0.947; shannon index, t=0.813, the simpson′s index, t=0.613, P>0.05), while beta diversity analysis showed significant differences in the overall structure of the two communities (Stress=0.054 5). A total of 120 species were annotated at the phylum level, of which two differed. While 1 736 species were annotated at the genus level, 69 were different, and 309 were different at the species level. The AE group ranked the top 6 in terms of abundance of Anaplasma, Escherichiaceae, Clostridium, Alternaria, Ruminalia, and Treponema spp. at the genus level; whereas, Segatella, Prevotella, E. faecalis, Rossella, and beneficial rod-shaped bacteria were more abundant in the NC group. There were differences in the abundance and diversity of intestinal flora between the two groups, and the structure of community composition was significantly different. Statistical results by linear discriminant analysis (LDA) showed that LDA scores >2 in the NC group included beneficial bacillus spp. and E. faecalis spp. in young infants, etc. LDA scores >2 in the AE group at the mid-species level included Clostridium polterococcus, unknown microorganisms in the genus Clostridium intestinalis, Hathaway′s Henkett′s bacillus, and Clostridium oryzae in the genus Clostridium refractory to culture and small Clostridium spp. in the AE group. Clostridium intestinalis. The RDA results showed a negative correlation between beneficial rod genera and liver function indices, and a positive correlation between Clostridium intestinalis genera and liver function indices. In conclusion, patients with hepatic blastomycosis have altered intestinal flora abundance and diversity, with significant structural changes in community composition and differences in several genera, including Mycobacterium anisopliae and Clostridium intestinalis, and imbalances in the intestinal flora may affect hepatic function by influencing intestinal metabolites and may have an impact on the development of hepatic blastomycosis, a finding that warrants further in-depth study.
4.The effect of ethyl alcohol extract of moutan cortex on virulence factors of Candida tropicalis

Objective:To investigate the in vitro bacteriostatic effect of ethyl alcohol extract of Moutan Cortex (EAEMC) on Candida tropicalis and its effect on virulence factors, including aspartic protease, hemolysin, phospholipase, esterase, lipase activities and biofilm. Methods:EAEMC powder was obtained by ultrasonic extraction, decompression concentration and lyophilization; the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of EAEMC on 21 clinical strains and one standard strain of Candida tropicalis were determined by microdilution. Five extracellular enzyme activities of Candida tropicalis and the effect of EAEMC on them were detected by the plate assay, and the results were analyzed by ANOVA. The biofilm model of Candida tropicalis was constructed in vitro, and the inhibition rate of EAEMC on Candida tropicalis biofilm was evaluated using the thiazolyl blue (MTT) method. Results:The MIC of EAEMC against Candida tropicalis BNCC335988 was 12.5 g/L and the MBC value was 25 g/L, while for the clinical strains, the MIC was 12.5-25 g/L and the MBC was 25-50 g/L. Aspartic protease, esterase and hemolytic activities of Candida tropicalis were positive, but phospholipase and lipase showed negative activities. At a concentration of 1/2 MIC of EAEMC, the aspartic protease and hemolytic activities of Candida tropicalis were completely inhibited the aspartic protease and hemolytic activities of Candida tropicalis were completely inhibited and the esterase activity was completely inhibited at a concentration of MIC of EAEMC. The inhibition of Candida tropicalis BNCC335988 biofilm by EAEMC reached more than 70% at a concentration of 2MIC, more than 80% at a concentration of 4MIC, and more than 90% at a concentration of 8MIC. Conclusion:EAEMC can achieve bacteriostatic effects by reducing the aspartic protease, esterase and hemolysin activities of Candida tropicalis, as well as inhibiting biofilm formation.

8.An analysis of occupational health examination results of 450 radiation workers in a steel mill

Objective To investigate the occupational health status of 450 radiation workers in a steel mill when they resigned, and to provide references and suggestions for the occupational health monitoring of the enterprise employees and for the workers’ proper arrangement after their resignations. Methods Occupational health examinations were performedon 450 steel workers who were about to resign in accordance with the applicable laws and regulations. And the examination results were statistically analyzed. Results Among the 450 radiation workers, 82% had abnormal examination results which included chromosome aberration in peripheral blood lymphocytes (4.89%), lens opacity (44.44%), pinguecula (25.33%), hypertension (28.22%), thyroid dysfunction (9.59%), and abnormal liver ultrasound findings (30.89%). Compared with the normal population, these steel workers had a significantly higher chromosome aberration rate in peripheral blood lymphocytes and a significantly higher incidence rate of ocular disorders. Conclusion It is important to effectively manage the occupational health of radiation employees in steel mills, and to regularly provide training on occupational health protection. To avoid radiation damage, workers should pay close attention to radiation protection at work, especially eye protection, to avoid eye disorders.
9.Clinical value of activin A in early identification of moderate and severe acute pancreatitis: a prospective study

Objective:To explore the value of serum activin A (ACT-A) level in early identification of moderate and severe acute pancreatitis (AP).Methods:A prospective case control study was conducted. A total of 120 patients with AP admitted to department of hepatobiliary surgery of Affiliated Nanhua Hospital of Hengyang Medical College of University of South China between October 2020 and April 2022 were recruited. According to the revised Atlanta classification, all patients were classified into mild AP group and moderate-to-severe AP group. The blood samples within 24 hours of onset were drawn, and the serum ACT-A and C-reactive protein (CRP) levels were detected by enzyme-linked immunosorbent assay (ELISA). The Ranson score and the modified CT severity index (MCTSI) were performed. Pearson correlation method was used to analyze the correlation of various parameters. The receiver operator characteristic curve (ROC curve) was plotted to analyze the predictive value of ACT-A and CRP for moderate-to-severe AP.Results:A total of 120 patients with AP were enrolled, including 83 patients with mild AP and 37 patients with moderate-to-severe AP. Serum ACT-A and CRP levels within 24 hours of onset in the moderate-to-severe AP group were significantly higher than those in the mild AP group [ACT-A (ng/L): 140.4±37.7 vs. 53.9±30.5, lg CRP: 1.42±0.91 vs. 0.77±0.70, both P < 0.01], and the Ranson score and MCTSI score were also significantly higher than those in the mild AP group (Ranson score: 5.3±1.3 vs. 1.8±1.6, MCTSI score: 5.5±1.0 vs. 2.7±1.2, both P < 0.01). Correlation analysis showed that the serum ACT-A level was positively correlated with serum CRP level, Ranson score and MCTSI score ( R2 value was 0.272, 0.841, 0.616, respectively, all P < 0.05). ROC curve analysis showed that the serum ACT-A, CRP and Ranson score had predictive value for moderate-to-severe AP. The area under the ROC curve (AUC) was 0.948 [95% confidence interval (95% CI) was 0.909-0.986], 0.711 (95% CI was 0.606-0.815), 0.946 (95% CI was 0.910-0.982), respectively. When serum ACT-A > 112.6 ng/L, the sensitivity and specificity of predicting moderate-to-severe AP were 78.38% and 96.39%, respectively, which was better than serum CRP with sensitivity and specificity of 72.92% and 66.27%, respectively, and the specificity was better than Ranson score (71.08%). Conclusion:ACT-A can be detected in the early stage of AP, and it is positively correlated with the disease severity, which can early identify moderate-to-severe AP.
10.Merkel cell carcinoma: a clinicopathological study of 10 cases

Objective:To investigate the clinicopathological features, differential diagnosis and prognosis of Merkel cell carcinoma (MCC).Methods:The clinical and pathological data of 10 patients with MCC were collected at the 940th Hospital of PLA. The histological characteristics were examined. Immunohistochemical EnVision method was used to detect thyroid transcription factor-1 (TTF1), broad-spectrum cytokeratin (CKpan), CK20, S-100, Ki-67, CD56, chromogranin A, synaptophysin and other markers in the 10 cases.Results:Intradermal MCC of the skin showed a nested, cord-like, cribriform distribution, polygonal cells, uniform size, and lack of cytoplasm. Tumor cell nuclei were large and round, with clear nuclear membranes, fine and scattered chromatin, absence of nucleoli, and mitotic figures of 10 per 50 high power fields. Among them, one patient had sarcoma and squamous cell carcinoma in situ, one patient had squamous cell carcinoma in situ, and one patient had unique cell morphology. Immunohistochemical staining showed that all cancer cells expressed CKpan, synaptophysin and CD56. There were seven cases with perinuclear dot-like positivity of CK20. Six MCCs expressed chromogranin A to varying degrees, while 2 MCCs were weakly positive for p63. The nuclear positive index in the Ki-67 hotspot area was 60%.Conclusion:The histology of MCC varies. Rendering a correct diagnosis of MCC requires adequate sampling, close correlation with clinical history and rational use of immunohistochemical staining. The treatment requires standardized surgery, postoperative radiotherapy and multimodal chemotherapy. Immunotherapy may replace the traditional treatment in the future.
