Chemotherapeutic drugs, such as cisplatin and phenanthriplatin (PhenPt), as STING agonists to induce DNA damage and activate the cyclic GMP-AMP synthase-stimulator of interferon genes (cGAS-STING) signaling pathway provides a potential strategy for clinical chemo-immunotherapy. However, treatment with Pt-based drugs leads to irreversible ectopia of phosphatidylserine (PS), a major component of the intracellular membrane, to the surface of the cancer cells by enzymes (Xkr8). Exposed PS can bind to immune cell receptors and inhibit the presentation of tumor antigens, leading to immunosuppression and attenuation of chemotherapy. Herein, we report a novel approach to enhance chemo-immunotherapy by constructing siRNA targeted Xkr8 (siXkr8)-mediated tetrahedral framework nucleic acid nanogel structure concurrently loaded with PhenPt (siXkr8-FNG/PhenPt) for co-delivery of siRNA and Pt-based drugs. The results showed that siXkr8-FNG/PhenPt can not only be used as an efficient delivery carrier to deliver siXkr8, block the expression of Xkr8, reduce the exposure of PS on the cancer cells surface, but also act as an immune stimulant to activate cGAS-STING pathway, effectively improve the immunosuppressive microenvironment, produce antitumor immune response, and inhibit tumor growth and metastasis. Overall, this new delivery system is important for improving the effect of Pt-based drug chemotherapy, inducing immune enhancement and nucleic acid drug delivery.

Objective:
To determine the internal relationships pf perceived discrimination and rejection sensitivity with social adjustment among college students with left behind experiences, so as to provide a reference for targeted prevention and intervention strategies for social adjustment issues in this population.
Methods:
A cluster random sampling method was used to select 2 999 college students from 3 universities in Qiqihar and Daqing, Heilongjiang Province. In October, 2024 (T1) and April 2025 (T2), the Discrimination Perception Questionnaire of college students, Rejection Sensitivity Questionnaire, and the Chinese College Student Adjustment Scale (CCSAS) were used to assess students perceptions of discrimination, rejection sensitivity, and social adjustment levels. Independent sample t-test was used for group comparisons. Traditional cross sectional comparisons and cross sectional network analysis were performed using T1 data for the core sample of 1 080 students with left behind experiences (T1). Longitudinal network analysis was performed using paired data from T1 and T2 ( n =1 024) and a preliminary cross lagged model was used to determine the relationships between variables. Sensitivity analysis was performed by including followup data from students without left behind experiences to test the robustness of the results.
Results:
After gender adjustment, students with leftbehind experiences had higher scores on discrimination perception (13.14±0.16) and rejection sensitivity (92.82±0.73) compared to the non left behind group (10.25±0.12) (89.12±0.55) while the social adjustment score (194.94±1.05) was lower than the non left behind group (202.82±0.79). The differences were statistically significant ( F =212.03, 16.52, 36.02, all P <0.01). Cross sectional network analysis showed that the "anticipated rejection likelihood" was the core node ( EI=-2.27 , BEI =-2.37), playing a key role in inter network connections and overall dynamic regulation. Longitudinal network comparison revealed statistically significant changes in the intensity centrality of the nodes "individual discrimination perception" and "anticipated rejection likelihood" (both P <0.05), while other nodes remained relatively stable. Cross lagged network analysis showed that the predictive effects of T1 on the variables at T2 were not statistically significant (maximum | r |=0.05, FDR corrected P >0.50), but the binomial test indicated a systematic positive correlation trend between the variables (66.7% positive correlation, P =0.04). Sensitivity analysis showed that the bridge effect of the "anticipated rejection likelihood" and "learning adjustment" nodes at T2 was more significant in the left behind group compared to the non left behind group.
Conclusion
The social adjustment issues of college students with left behind experiences require focused attention on the negative regulatory effect of "rejection cognition" and the positive regulatory effect of "learning adaptation".

Atherosclerosis is a multifocal, smoldering, immunoinflammatory disease caused by lipid accumulation. Acute cardio-cerebrovascular disease caused by AS is one of the most serious life threats in the world. Endothelial cell injury, vascular inflammatory stimulation, abnormal lipid metabolism and coagulation disorder are the main pathological mechanisms of AS. Thrombomodulin (TM) is a transmembrane glycoprotein mainly expressed on the surface of endothelium. It plays a key role in maintaining the dynamic equilibrium of the vascular system through its functions of anti-coagulation, anti-inflammation and cell protection. Recombinant human soluble thrombomodulin (rhsTM), a soluble form of human TM containing the extracellular domain of TM, might be effective in the treatment of AS. This review summarizes the structure and function of TM and the mechanism of rhsTM in the treatment of AS. Aiming to provide new ideas for the prevention and treatment of AS.

Objective:To explore the effect of probiotics on reducing ventilator-associated pneumonia (VAP) in sepsis patients. Methods:A total of 94 cases were randomly (random number) divided into the probiotic group ( n = 46) and the control group ( n = 48). All of the patients were given enteral nutrition therapy by nasogastric tube within 24-72 h after admission. And patients in the probiotic group were given live combined bifidobacterium, lactobacillus and enterococcus powder besides the regular therapy. The incidence of VAP, bacteremia, mortality, mechanical ventilation time and length of ICU stay were compared between the two groups. Results:Compared with the control group, the incidences of VAP and bacteremia in the probiotics group were significantly lower (χ 2=4.763, P=0.029; χ 2=4.438, P=0.035). There were no significant differences in 28-day mortality and the length of hospital stay between the two groups (χ 2=2.02, P=0.167; t=1.29, P=0.208). Mechanical ventilation time in the probiotics group was significantly shorter than that in the control group ( t=2.16, P=0.038). The Log-Rank test showed that the time of VAP-free in the probiotics group was significantly longer than that in the control group ( P < 0.05). After adjusting for APACHEⅡ score and age, COX proportional risk model analysis showed that the RR values of the probiotics group and the control group for 28-day VAP were 0.18 (95% CI: 0.12-0.74, P=0.025) and 0.21 (95% CI: 0.19-0.95, P=0.042), respectively. Conclusions:Probiotics treatment can reduce the incidence of VAP in sepsis patients.
