1.Value of deep learning reconstruction combined with black blood technique in carotid CT angiography

Objective:To evaluate the impact of deep learning reconstruction (DLR) on the image quality of carotid CT angiography (CTA), and to explore the visibility of black blood techniques for carotid atherosclerotic plaques and its ability to evaluate the degree of luminal stenosis.Methods:This was a cross-sectional study. A total of 122 carotid atherosclerotic plaques were retrospectively analyzed from 77 patients with soft plaques in the carotid artery CTA of Affiliated Zhongshan Hospital of Dalian University from March 2024 to March 2025. The CTA images were reconstructed using the hybrid iterative reconstruction (HIR) algorithm (HIR group) and DLR algorithm (DLR group). Objective image quality was evaluated by measuring the CT values and image noise (SD) of the carotid, sternocleidomastoid muscle and neck subcutaneous fat, then calculating the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR). Subjective image quality was assessed using a 5-point scale. The reconstruction algorithm with better image quality was selected for black blood image reconstruction. In CTA and black blood images, the CT values and SD of soft plaques, sternocleidomastoid muscle and neck subcutaneous fat were measured, SNR and CNR were calculated, and visibility of plaques were scored using a 5-point scale. The objective indicators between two groups were compared using paired sample t-test or Wilcoxon signed rank test, and the subjective scores were compared using Wilcoxon signed rank test, the degree of luminal stenosis was compared using χ2 test, and the consistency evaluation was evaluated using weighted Kappa test. Results:In the DLR group, the CT values and SD of the aortic arch, brachiocephalic trunk, left common carotid artery origin, left subclavian artery origin, mid-segment of bilateral common carotid arteries, carotid bifurcation, internal carotid artery origin, and siphon were lower than those in the HIR group (all P<0.001). The SNR and CNR were higher than those in the HIR group (all P<0.001). The image quality score of CTA images in the DLR group was higher than that in the HIR group ( Z=7.44, P<0.001). Black blood images were reconstructed from DLR CTA images with higher image quality. The CT values, SNR, and CNR of soft plaques in the black blood images were higher than those in the DLR CTA images, and the SD was lower than that in the DLR CTA images (all P<0.05). Subjective evaluation showed that the visibility score of soft plaques in black blood images was higher than in DLR CTA images ( Z=-8.92, P<0.001). In DLR CTA images, there were 60 mildly narrowed lumens, 38 moderately narrowed lumens, and 24 severely narrowed lumens. In the black blood images, there were 54 mildly narrowed lumens, 39 moderately narrowed lumens, and 29 severely narrowed lumens. There was no statistically significant difference in the diagnosis of luminal stenosis between black blood images and DLR CTA images ( χ2=0.80, P=0.670), and the diagnostic consistency was good ( Kappa=0.893, P<0.001). Conclusion:DLR algorithm can significantly improve the image quality and reduce image noise. DLR algorithm combined with the black blood technique can significantly enhance the visualization of soft plaques.

3.Current status and strategy suggestions of global suicide prevention for China

According to the recent WHO mortality statistics, suicide accounted for approximately 720 000 deaths worldwide in 2021, representing a critical public health challenge.The implementation of effective prevention strategies for suicide is essential for reducing the suicide rate which substantially contributes to the global burden of disease. This article systematically examines core intervention strategies from the WHO's suicide prevention implementation guidelines, alongside national suicide prevention policies and practical experiences from some representative countries. Drawing upon China's current suicide intervention context, tailored recommendations are proposed for a suicide prevention framework in China. Successful implementation requires a whole-of-society approach, characterized by coordinated cross-disciplinary collaboration and integrated community-based interventions. Professional workers in medical, educational, mental health and public health should assume leadership roles by leveraging their clinical expertise, thereby supporting the achievement of the mental health promotion targets outlined in the " Healthy China 2030" national health strategy, as well as reaching the target of reducing global suicide rates by 1/3 from 2015 to 2030, in accordance with the UN Sustainable Development Goals 2030.
4.A study of the effectiveness of an ICU transitional care program in improving relocation stress and caregiving skills of parents of critically ill children

Objective Constructing a transitional care program for critically ill children in the ICU and exploring the effects of its application on the relocation stress and caregiving abilities of parents of children to improve their caregiving experience.Methods Convenience sampling method was used to select 150 children and their families transferred from PICU to the general ward of a tertiary pediatric hospital in Shanghai from January to September 2023 as research subjects.Subjects enrolled from June to September 2023 were selected as an experimental group,and were applied with the ICU transitional care program for critically ill children.Those enrolled from January to April 2023 were selected as a control group,and received routine ICU transitional care.Family Relocation Stress Scale and Family Caregiver Task Inventory were used to evaluate children's families at 24h before,24h after,and 72h after children's transition from ICU.The incidence of unplanned readmission to ICU within 48h and adverse events were compared between 2 groups of children.Results Eventually 74 pairs in the experimental group and 75 pairs in the control group completed the study.The Family Caregiver Task Inventory scores in the experimental group were lower than that in the control group(P=0.046).Generalized estimation equation showed that,there were significant differences on the intervention effect,the time effect,and the interaction effect of the Family Relocation Stress Scale scores in 2 groups(P<0.05).Conclusion The ICU transitional care program for critically ill children constructed in this study can effectively improve the level of migration stress and caregiving ability of parents of critically ill children,and optimize the quality of transitional care for critically ill children.

7.Visualization analysis of research status and hot spots of Yinchenhao decoction based on CiteSpace

Objective By using the bibliometrics method,the literature about Yinchenhao decoction was visualized and analyzed to explore its research hot spot and frontier.Methods The related literatures on Yinchenhao decoction were retrieved from CNKI database,and CiteSpace 6.1.R6 software was used to analyze the authors,institutions and key words,and the key words were clustered to draw a visual atlas.Results A total of 856 articles were included,and the number of articles showed a fluctuating upward trend.There were 764 authors and 513 related key words,including clinical application,experimental study and signaling pathway of Yinchenhao decoction.Conclusion Although Yinchenhao decoction is a commonly used clinical prescription for liver disease,there is a lack of studies on the mechanism of action and molecular level.Future research hot spot should focus more on large sample randomized controlled clinical studies and experimental studies,so as to provide new directions for researchers.
8.Conceptual analysis of health literacy in children and adolescents with unintentional injury

【Objective】 To clarify the concept of health literacy among children and adolescents with accidental injuries through literature review and analysis. 【Methods】 A systematic search was conducted across multiple databases, including China National Knowledge Infrastructure, Wanfang database, VIP database, China Biomedical literature database, PubMed, CINAHL, and PscyINFO database. The literature was analyzed using Rodgers′ evolutionary concept analysis method. 【Results】 A total of 56 articles were included. The health literacy of children and adolescents with accidental injuries comprised three conceptual attributes:cognition, emotion and behavior. The influencing factors included children′s demographic factors, family and school related factors and social related factors. The result can be beneficial to reduce the occurrence of injury events, save family economic expenditure and alleviate the burden on social medical resources. However, there is currently a lack of specific measurement tools for assessing health literacy in this population, as the existing evaluation items are drawn from general health literacy scales. 【Conclusions】 The concept of health literacy among children and adolescents with accidental injuries is multifaceted and evolving. Future research should focus on exploring the characteristics of health literacy among children and adolescents of different ages and regions from their own perspectives. Additionally, efforts should be made to refine the concept and develop specialized measurement tools to facilitate further studies in this area.
9.Application of behavioral lifestyle intervention in prevention and treatment of emotional disorders

" Behavior determines health" is the theoretical and practical basis of behavioral medicine. Maladpative lifestyle is not only an important risk factor for many non-communicable diseases, but also an important influencing factor for recurrence and prognosis of emotional disorders. More and more meta-analyses have confirmed the role of behavioral lifestyle in the treatment and prevention of emotional disorders such as depression. Some authoritative treatment guidelines now recommend lifestyle changes as one of the firstline treatment recommendations and given some specific modalities in detail. There is relatively less evidence for treatment of anxiety disorders and bipolar disorder, but there are several well-designed and high-quality studies that confirm the therapeutic and preventive role of lifestyles, particularly circadian rhythms, life regurality and exercises, as part of routine management measures for maintaining stable mood and alleviating anxiety symptoms.
10.Survey and analysis of the shared decision-making concept and the influencing factors for depression in outpatient clinics of tertiary hospitals

Objectives:To investigate the current willingness of depressive outpatients and their doctors in China to engage in shared decision-making (SDM), and to analyze the factors influencing this willingness.Methods:A questionnaire survey was conducted among doctors and patients with depression in 12 tertiary psychiatric hospitals and general hospitals by scanning two-dimensional code and filling in the questionnaire on the mobile terminal. The questionnaire covered patient demographics, emotional state scores, initial diagnosis and treatment, treatment expectations and concerns, symptom improvement needs, medication safety requirements, and diagnosis details (completed by the attending physician). Doctors provided basic information, current depression diagnosis and treatment status, and concerns regarding medications. Logistic regression analysis (univariate and multivariate) was used to identify factors influencing patients′ and doctors′ willingness to engage in SDM.Results:A total of 622 valid patient questionnaires and 45 valid physician questionnaires were collected. Both patients and doctors had a strong willingness to make shared decisions (80.39% (500/622) vs. 60.00% (27/45)). Multivariate binary logistic regression analysis showed that residential location (town versus rural areas: OR (95% CI)=1.895 (1.087-3.305)), acceptable monthly medical expenses (≥1 000-<2 000 CNY vs.<300 CNY: OR (95% CI)=0.194 (0.088-0.427);≥2 000 CNY vs.<300 CNY: OR (95% CI)=0.267 (0.094-0.754)), acceptance of online treatment and consultation (accept versus not accept: OR (95% CI)=3.196 (2.024-5.046)), and knowing about psychotherapy (yes versus no: OR (95% CI)=1.711 (1.003-2.921)) were the factors influencing the willingness of shared decision-making in patients (all P<0.05). For the doctors, the time spent on initial consultation was the factor influencing the willingness to engage in SDM ( OR (95% CI)=1.090 (1.004-1.184), P=0.040). Conclusions:Both depression patients and doctors in tertiary outpatient clinics in China show a strong willingness to engage in SDM, providing a solid foundation for clinical application. However, it is necessary to pay attention to the influence of residential location, acceptable monthly medical expenses, acceptance of online treatment and consultation, and knowledge of psychotherapy should be considered for patients, while the time spent on initial consultations should be considered for doctors.
